1.Clinical efficacy of arthroscopic medial patellofemoral complex reconstruction for recurrent patellar dislocation with high-grade trochlear dysplasia.

OBJECTIVE:
To investigate the midterm clinical efficacy of medial patellofemoral complex (MPFC) reconstruction for recurrent patellar dislocation with high-grade trochlear dysplasia.
METHODS:
A retrospective analysis was carried out among adult patients who underwent arthroscopically assisted MPFC reconstruction between January 2014 and December 2020. Dejour classification was evaluated to grade trochlear dysplasia; tibial tubercle-trochlear groove (TT-TG) distance and Insall-Salvati index were measured. Preoperative and postoperative patient-reported outcome measures (PROMs) were compared, including International Knee Documentation Committee (IKDC) score, Kujala score, Lysholm score and Tegner score. Information regarding returning-to-sport rate, re-instability events and complications was collected. Patellar tilt (PT), lateral patellar displacement (LPD) and bisect offset (BSO) ratio were measured based on axial computed tomography before and after surgery to assess the patellofemoral congruence.
RESULTS:
A total of 46 MPFC reconstructions in 43 patients were enrolled, including 16 male and 27 female. Mean age at surgery was (22.2±7.6) years (range: 14-44 years). Mean follow-up was (49.9±22.6) months (range: 18-102 months). The percentages of Dejour B, C and D dysplasia were 37.0% (17/46), 43.5% (20/46), and 19.6% (9/46), respectively. Mean Insall-Salvati index was 1.2±0.2 (range: 0.85-1.44), and mean TT-TG distance was (19.6±3.5) mm (range: 10.6-28.7 mm). At latest follow-up, there were significant improvements in all PROMs (P < 0.001): IKDC score, from 56.3±15.1 to 86.2±8.1; Kujala score, from 58.9±15.6 to 92.6±5.4; Lysholm score, from 63.7±15.0 to 94.0±5.7; Tegner score, from 3.1±1.4 to 4.7±1.4, and there were no significant differences in the improvements of the scores between the patients with Dejour B, C and D dysplasia. Overall, ninety percent of the patients returned to their preoperative sports level. One patient reported a postoperative subluxation, while no cases of infection, limited range of motion or patella fracture were observed. PT, LPD and BSO ratio were all significant altered (P < 0.001) after MPFC reconstruction.
CONCLUSION
Arthroscopically assisted MPFC reconstruction yielded satisfactory midterm clinical results for recurrent patellar dislocation with high-grade trochlear dysplasia. No significant differences of improvements in knee function were observed among the three types of high-grade trochlear dysplasia.

2.Tongue squamous cell carcinoma-targeting Au-HN-1 nanosystem for CT imaging and photothermal therapy.

Tongue squamous cell carcinoma (TSCC) is a prevalent malignancy that afflicts the head and neck area and presents a high incidence of metastasis and invasion. Accurate diagnosis and effective treatment are essential for enhancing the quality of life and the survival rates of TSCC patients. The current treatment modalities for TSCC frequently suffer from a lack of specificity and efficacy. Nanoparticles with diagnostic and photothermal therapeutic properties may offer a new approach for the targeted therapy of TSCC. However, inadequate accumulation of photosensitizers at the tumor site diminishes the efficacy of photothermal therapy (PTT). This study modified gold nanodots (AuNDs) with the TSCC-targeting peptide HN-1 to improve the selectivity and therapeutic effects of PTT. The Au-HN-1 nanosystem effectively targeted the TSCC cells and was rapidly delivered to the tumor tissues compared to the AuNDs. The enhanced accumulation of photosensitizing agents at tumor sites achieved significant PTT effects in a mouse model of TSCC. Moreover, owing to its stable long-term fluorescence and high X-ray attenuation coefficient, the Au-HN-1 nanosystem can be used for fluorescence and computed tomography imaging of TSCC, rendering it useful for early tumor detection and accurate delineation of surgical margins. In conclusion, Au-HN-1 represents a promising nanomedicine for imaging-based diagnosis and targeted PTT of TSCC.

3.Analysis of Cultivating Interpersonal Communication Competence Effect of Physician in Standardized Training of Specialist Physician

Objective To evaluate the interpersonal communication competence of doctors in 10 dimensions of standardized training for specialized doctors and their communication differences with various groups,and to analyze the existing problems and solutions.Methods The 10-item short-form version of the Interpersonal Communication Competence Scale(ICCS-SF)was used to assess the communication abilities of 395 physicians undergoing training at six specialized physician training bases in Shanghai.Descriptive statistics,chi-square tests,non-parametric tests,and multiple linear regression of methods were employed to analyze the communication status and training status of specialist physicians.Results The maximum score for interpersonal communication competence is 50 points,and the average score for the specialist physicians in this study was 38.0(35.0~41.0)points.The impact of the training on the scores is reflected in the total score and the two sub-dimensions of social relaxation and self-affirmation,with statistically significant differences(P<0.05).Compared to patients and colleagues,specialized training physicians reported the greatest difficulty in communicating with interdisciplinary teams.Physicians who recognized the importance of communication and those whose training included a greater variety of skills achieved higher ICCS-SF scores(P<0.001).Conclusion It is necessary to further strengthen the coverage and standardization of training,and pay attention to the cultivation of communication competence between interdisciplinary teams on the basis of attaching importance to the communication competence between doctors and patients.
4.Mechanism prediction and experimental verification of Maxing Shigan Decoction against influenza A virus infection based on UPLC-MS/MS and network pharmacology

Objective:
To investigate the chemical compositions of Maxing Shigan Decoction (麻杏石甘汤, MXSGD) and elucidate its anti-influenza A virus (IAV) mechanism from prediction to validation.
Methods:
Ultra high-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) was employed to analyze the chemical compositions of MXSGD. Network pharmacology theories were used to screen and identify shared targets of both the potential targets of active ingredients of MXSGD and IAV. A protein-protein interaction (PPI) network was then constructed, followed by Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses. The binding stability between core bioactive compounds and key targets was validated by molecular docking and dynamic simulations. A total of 24 BALB/c mice were infected with IAV to build IAV mouse models. After successful modelling, the mouse models were randomly divided into model, MXSGD high-dose (2.8 g/kg), MXSGD low-dose (1.4 g/kg), and oseltamivir (20.14 mg/kg) groups, with an additional normal mice as control group (n = 6 per group). The treatments were administered by gavage daily between 8:00 a.m. and 10:00 a.m. for five consecutive days. Upon completion of the administration, the body weight ratio, lung index, protein content in the bronchoalveolar lavage fluid (BALF), and the levels of inflammatory factors including interleukin (IL)-6 and tumor necrosis factor (TNF)-α in mice were measured to preliminarily analyze the therapeutic efficacy of MXSGD against IAV infection. Furthermore, the expression levels of mechanistic target of rapamycin (mTOR), hypoxia inducible factor (HIF)-1α, and vascular endothelial growth factor (VEGF) proteins in the HIF-1 signaling pathway, which was enriched by network pharmacology, were detected by Western blot.
Results:
A total of 212 chemical components in MXSGD were identified by the UPLC-MS/MS method. These chemical components can be classified into 9 primary categories and 31 secondary categories. After intersecting the chemical component targets with IAV-related targets, a total of 567 potential MXSGD components targeting IAV were identified. The construction of PPI network and the results of both GO and KEGG enrichment analyses revealed that the anti-IAV effects of MXSGD were associated with multiple pathways, including apoptosis, TNF, HIF-1, and IL-17 signaling pathways. The results of molecular docking demonstrated that the binding energies between the core compound 1-methoxyphaseollin and key targets including HIF-1α, mTOR, and VEGF were all lower than – 5.0 kcal/mol. Furthermore, molecular dynamics simulations confirmed the structural stability of the resulting complexes. Animal experiments showed that compared with the normal controls, IAV-infected mice showed significantly reduced body weight ratio, markedly increased lung index, protein content in BALF, and the levels of inflammatory factors such as IL-6 and TNF-α (P < 0.01), thereby causing damage to the lung tissue; consequently, the expression levels of mTOR, HIF-1α, and VEGF proteins in the lung tissues of these mice were significantly elevated (P < 0.01). However, after MXSGD treatment, the mouse models presented a significant increase in body weight ratio, as well as marked decreases in lung index, protein content in BALF, and the levels of inflammatory factors including IL-6 and TNF-α (P < 0.01). Furthermore, the therapy alleviated IAV-induced injuries and significantly downregulated the expression levels of mTOR, HIF-1α, and VEGF proteins in lung tissues (P < 0.01 or P < 0.05).
Conclusion
MXSGD exerts anti-IAV effects through multi-component, multi-target, and multi-pathway synergism. Among them, 1-methoxyphaseollin is identified as a potential key component, which alleviates virus-induced lung injury and inflammatory response via the regulation of HIF-1 signaling pathway, providing experimental evidence for the clinical application of MXSGD.
5.Progress and technical points of transoral endoscopic thyroidectomy vestibular approach

TOETVA has both advantages of beauty and curative effect. Based on the clinical experience of this technique, the author reviewed the development, indications, complications and surgical skills of TOETVA in recent years, and looked forward to the development trend of this technique.
6.Prokaryotic expression,purification of coxsackievirus A16 Vp1 protein and preparation of rabbit polyclonal antibodies

Objective:To construct the prokaryotic expression vector of Coxsackievirus A16(CA16)viral protein 1(Vp1),express it in Escherichia coli(E.coli)BL21,purify the protein,prepare rabbit polyclonal antibodies,and identify the immunoreactivity of the antibodies.Methods:Bioinformatics online tools were used to predict the amino acid composition,conserved domains,secondary and tertiary structures of the Vp1 protein.The Vp1 gene of CA16 was amplified and cloned into the prokaryotic expression vector pET28a(+).The pET28a-Vp1 was transformed into E.coli BL21,and the expression was induced using isopropyl β-D-thiogalactopyranoside(IPTG).Western blotting method was used to identify the induced expression of Vp1 protein,and the induction time and temperature were optimized to improve the expression efficiency.Two female SPF New Zealand white rabbits were taken,and the purified recombinant protein Vp1 was used to immunize the rabbits to prepare rabbit anti-Vp1 protein polyclonal antibodies.The antibody titer was determined by ELISA method,and the immunoreactivity of the antibodies was identified by Western blotting method.Results:The bioinformatics analysis results showed that the Vp1 gene encoded 297 amino acids,with a relative molecular mass of 33 046.39 and an isoelectric point of 8.32,belonging to a hydrophilic protein;in the secondary structure,α-helix accounted for 15.15%,random coil accounted for 67.68%,and extended strand accounted for 17.17%.Sequencing of the recombinant plasmid pET28a-Vp1 confirmed that the pET28a-Vp1 plasmid was correctly constructed.The Western blotting results showed that the target protein was expressed in the IPTG induction group at a relative molecular mass of 33 000,and the target protein was mainly expressed in inclusion bodies.The optimal induction conditions for protein expression were IPTG concentration of 0.4 mmol·L-1,temperature of 16℃,and induction time of 20 h.The ELISA assay results showed that the titer of the rabbit polyclonal antibody was 1:1 024 000.The Western blotting results showed that the Vp1 rabbit polyclonal antibody could bind to the viral Vp1 protein in the CA16-infected cells.Conclusion:The polyclonal antibody against CA16 Vp1 is successfully prepared,and this antibody has significant binding characteristics to CA16 Vp1,which can be used for the diagnosis of enterovirus infection and the development of treatment methods.
7.A Case of Multidisciplinary Treatment for Acute Hemorrhage Caused by Giant Bladder Paraganglioma

Bladder paraganglioma is a rare neuroendocrine tumor, accounting for only 0.05% of all bladder tumors. This article reports the diagnosis and treatment of a 16-year-old male patient with acute hemorrhage secondary to a giant bladder paraganglioma(approximately 10 cm in maximum diameter). Preoperative imaging evaluation suggested locally advanced disease, and the patient subsequently received three cycles of neoadjuvant chemotherapy with temozolomide. He patient's blood tests revealed hemorrhagic shock. During the waiting period of surgery, hemostasis was successfully achieved through emergency transarterial embolization. Subsequently, the patient underwent emergency radical cystectomy combined with orthotopic ileal neobladder reconstruction under general anesthesia and recovered well after surgery. Postoperative pathological examination confirmed the diagnosis of bladder paraganglioma, with positive immunohistochemical staining for succinate dehydrogenase subunit B and a Ki-67 proliferation index of 3%. At the 6-week follow-up, the patient's blood pressure and endocrine parameters had returned to normal, with a satisfactory quality of life. This case demonstrates that a multidisciplinary team approach can effectively integrate expertise from various specialties to formulate comprehensive and systematic treatment plans, which is crucial for ensuring successful management of such complex cases.

9.Predictive value of labor progression angle, fetal head descent distance, and their change rate in the outcome of vaginal trial delivery of scarred uterus after cesarean section

Objective To investigate the predictive value of labor progress angle (AOP), fetal head descent distance (HPD) and their change rates in the outcome of vaginal trial of cesarean scar uterus. Methods A total of 170 pregnant women who underwent vaginal trial production of scar uterus after cesarean section were selected as study subjects, and were divided into successful group and failed group based on the trial production outcomes. Advanced oxidation processes (AOP) and head-perineum distance (HPD) were measured by ultrasound during the active phase of the first stage of labor when the cervix dilated to 4 cm and at 1 hour after the cervix dilated to 4 cm, respectively. The AOP change rate and HPD change rate after 1 hour of progress were calculated. The receiver operating characteristic (ROC) curve was used to analyze the predictive efficacy of AOP, HPD and their change rates in the outcome of vaginal trial production of scar uterus after cesarean section. Delong test was used to compare the differences in area under curves (AUCs). Results Among 170 pregnant women with scarred uterus after cesarean section who were pregnant again, 139 cases (success group) were succeed in transvaginal delivery, while 31 cases failed trial delivery, and transferred to cesarean section (failure group). The AOP of the successful group was significantly larger than that of the failed group when the cervix was opened to 4 cm, and the HPD was significantly shorter than that of the failure group (
10.Comparative study of light,caffeine,and combined methods for inducing a zebrafish model of insomnia

Objective To establish different sleep deprivation models in zebrafish to provide reproducible and practical modeling reference solutions for basic research on insomnia.Methods Zebrafish insomnia models were induced by two interventions:continuous light(150 Lux)and light plus caffeine.The zebrafish were divided randomly into control,light,caffeine(100 μmol/L),and combined light and caffeine groups.The locomotor ability of zebrafish in each group was observed using open field and circadian rhythm behavioral experiments.The expression and secretion of related sleep genes and the neurotransmitter 5-hydroxytryptamine(serotonin;5-HT)were detected using quantitative polymerase chain reaction and enzyme-linked immunosorbent assay,respectively.Results Sleep time,resting time(during the day),and sleep rounds were significantly reduced(P<0.01 or P=0.01)and the distance traveled was significantly increased in the light group compared with the control group(P<0.01).The resting time(daytime)and sleep rounds were increased in the combined and caffeine groups(P<0.01)compared with the control group.There was no significant difference in the activity distance between the combined and caffeine groups(P>0.05).The percentages of swimming distance and swimming time in the central area were decreased in the light group compared with the control group(P<0.05),and were both decreased in the caffeine group compared with the light group(P<0.01).HT receptor 1Aa(HTR1aa)mRNA expression at 6:00.and 12:00 was up-regulated in the light group compared with the control group(P<0.05),but there was no significant difference in HTR1ab mRNA levels between the light group and the combined group(P>0.05).5-HT secretion was decreased in the light group at 6:00(P<0.01)and at 12:00 compared with the control group.5-HT levels were reduced in both the light and combined groups(P<0.01),and secretion levels in the light and combined groups were still lower than in the control group at 18:00.(P<0.01).Conclusions Light alone is the best intervention for modeling long-lasting insomnia in zebrafish larvae.The responsible mechanisms may be related to the HTR1aa gene as well as biological factors such as 5-HT.
