1.Changes in hemoglobin and related influencing factors in patients with liver failure undergoing artificial liver support therapy

ObjectiveTo investigate the changing trend of hemoglobin (Hb) and related influencing factors in patients with liver failure after artificial liver support system (ALSS) therapy. MethodsA total of 106 patients with liver failure who were hospitalized and received ALSS therapy in our hospital from January to December 2018 were enrolled and analyzed in terms of clinical data and red blood cell parameters such as Hb, mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), and red blood cell distribution width-coefficient of variation (RDW-CV). A one-way repeated-measures analysis of variance was used for comparison of continuous data with repeated measurement between groups, and the paired t-test was used for comparison between two groups. The Kruskal-Wallis H test was used for comparison of continuous data with skewed distribution between multiple groups, the Mann-Whitney U test was used for further comparison between two groups. Univariate and multivariate linear regression analyses were used to identify the influencing factors for the reduction in Hb after ALSS therapy. ResultsThe 106 patients with liver failure received 606 sessions of ALSS therapy, and Hb was measured for 402 sessions before and after treatment. There was a significant reduction in Hb after ALSS therapy in the patients with liver failure (97.49±20.51 g/L vs 109.38±20.22 g/L, t=32.764, P<0.001). Longitudinal observation was further performed for 14 patients with liver failure, and the results showed that the level of Hb was 108.50±21.61 g/L before the last session of ALSS therapy, with certain recovery compared with the level of Hb (103.14±19.15 g/L) on the second day after ALSS, and there was an increase in Hb on day 3 (102.57±21.73 g/L) and day 7 (105.57±22.04 g/L) after surgery. The level of Hb in patients with liver failure on the second day after ALSS decreased with the increase in the number of ALSS sessions (F=8.996, P<0.001), while MCV and MCH gradually increased with the increase in the number of ALSS sessions (F=9.154 and 13.460, P=0.004 and P<0.001), and RDW-CV first gradually increased and then gradually decreased (F=4.520, P=0.032); MCHC showed fluctuations with no clear trend (F=0.811, P=0.494). The multivariate linear regression analysis showed that the duration of ALSS therapy, the mode of ALSS therapy, and initial treatment were independent risk factors for the reduction in Hb after ALSS therapy. ConclusionALSS therapy can influence the level of peripheral blood Hb in patients with liver failure, and patient blood management should be strengthened for patients with liver failure who are receiving ALSS therapy.
2.Acute effects of air pollution on pulmonary function and exhaled nitric oxide in children in Shanghai

ObjectiveTo investigate the acute effects of compound air pollution on children’s respiratory function. MethodsUsing panel group study design, 223 students in five classes of grade 4 from two primary schools (a, b) in Xuhui and Hongkou districts of Shanghai were randomly selected to measure pulmonary function and exhaled nitric oxide (FeNO). The first three tests were carried out from May to June in 2020, and the fourth test was carried out from September to December in 2021. At the same time, the daily and hourly mean values of PM2.5, PM10, SO2, NO2, O3 and CO was collected from the nearby air quality monitoring points of the two schools during the same period , as well as meteorological monitoring data (temperature, humidity, wind speed and atmospheric pressure). The linear mixed effect model was used to analyze the effects of air pollution on pulmonary function and respiratory inflammation in the summer. ResultsThe results of single pollutant model showed that PM2.5, PM10, SO2 and NO2 were positively correlated with FeNO, and the effect was reflected in lag0, lag1 and lag3 (P<0.05). PM2.5, PM10 and NO2 were negatively correlated with the changes of lung function FEF25%, FEF50%, FEF75%, FeF25%-75%, PEF, FVC, FEV1 and FEV1/FVC, and the effect was reflected in lag0 to lag3 days (P<0.05). The results of the dual pollutant model showed that the concentration changes of SO2 and NO2 were significantly correlated with the decrease of FEV1 when combined with O3 or PM2.5 (P<0.01), and the concentration changes of PM2.5 was significantly correlated with the increase of FeNO when O3, SO2 and NO2 were combined respectively (P<0.01). The effects of the dual pollutant model were greater than the effect of PM2.5 single pollutant model. ConclusionThe health effects of different air pollutants on children’s respiratory tract function indexes in summer are different. The combined effects of two pollutants on the lung function of children increased to different degrees. Although air pollution is light in summer, it still has an impact on children’s respiratory tract function index and inflammation index, and the combined effect of dual pollutants is more significant than that of single pollutant.
3.Factors influencing abnormal posterior corneal surface elevation with normal corneal biomechanical parameters

Objective To investigate the influence factors of abnormal posterior corneal surface elevation of myopia patients under normal corneal biomechanical state.Methods A retrospective case-control study was conducted.A total of 99 myopia patients(99 right eyes)who were going to undergo refractive surgery in Nanjing Aier Eye Hospital from April 2022 to December 2022 were included.All patients received routine preoperative examinations,and the corneal morphologi-cal and biomechanical examinations were performed by Pentacam combined with Corvis ST.For patients with normal Cor-vis biomechanical index(CBI,corneal biomechanical indexes measured by Corvis ST),corneal back difference(BD,difference between the enhanced posterior surface elevation and the normal posterior surface elevation)obtained by BADⅢ-Belin/Ambrósio Enhanced Ectasia Display(BAD)of the Pentacam 3D anterior segment analysis system was used to di-vide the posterior corneal surface elevation into group A(BD<12 μm,33 patients,33 eyes,control group),group B(12μm≤BD≤16 μm,32 patients,32 eyes,suspicious corneal abnormality group),and group C(BD>16 μm,34 patients,34 eyes,corneal abnormality group).Basic information about the patients,such as gender and age,was collected;ocular pa-rameters,such as intraocular pressure,spherical equivalent(SE),thinnest pachymetry(THP)and anterior chamber depth(ACD),were included;corneal morphological parameters obtained by Pentacam,such as corneal horizontal diameter(HWTW),BD,anterior Sim Ks(ASK),posterior Sim Ks(PSK),BAD system-related indicators[including deviation of normality of the front elevation(Df),deviation of normality of pachymetric progression(Dp),deviation of normality of corneal thinnest point(Dt),and overall deviation of normality(Do)]and pachymetric progression index(PPI),were en-rolled;corneal biomechanical integration parameters obtained by Corvis ST,such as stiffness parameters,integrated radius(IR,area under the inverse concave radius curve),Ambrósio relational thickness to the horizontal profile(ARTh,THP/thickness change rate)and deformation amplitude ratio,were included.The comparison between groups was conducted with one-way ANOVA test and H test,Spearman Correlation was used to analyze the correlation between the BD and ocular parameters in each group,and a multivariate logistic regression model was used to explore the main factors affecting the classification of BD values under normal corneal biomechanical state.The probability distribution of abnormal posterior corneal surface elevation under normal corneal biomechanical state was plotted according to the logistic regression equa-tion,and then the receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficiency of the logistic regression equation.Results There were statistically significant differences in THP,HWTW,PSK,Df,Dp,Dt,Do,PPImin,PPIavg,ACD,Db and ARTh among the three groups(all P<0.05);in pairwise comparisons,only PSK(F=11.385),Db(H=87.107),Do(F=50.051)and PPImin(F=16.556)showed statistically significant differences(allP<0.01),and the other parameters only showed statistically significant differences between Group A and Group B,Group A and Group C(all P<0.05).The BD of the three groups were highly positively correlated with Db(r=0.957,0.914 and 0.952,all P<0.05).In addition,the BD in Group A was slightly negatively correlated with PSK(r=-0.437,P<0.05)and slightly positively correlated with Do(r=0.432,P<0.05);the BD in Group C showed a low degree of negative corre-lation with SE(r=-0.399,P<0.05)and a moderate negative correlation with HWTW(r=-0.482,P<0.05).Using BD as the dependent variable and other parameters as covariates for multivariate logistic regression,the results showed that the influencing factors of BD in Group B were THP,HWTW and ARTh,the influencing factors of BD in Group C were THP,HWTW,ARTh and IR,and the overall percentage of model classification was 72.7%.According to ROC curve analysis,the new parameter Logistic(C)in multivariate logistic regression could improve the diagnostic efficiency of normal corneal bio-mechanical parameters with abnormal corneal posterior surface elevation,and its area under the curve(sensitivity,speci-ficity)was 0.975(100.0%,90.9%).Conclusion THP,HWTW,ARTh and IR are the main influencing factors of ab-normal posterior corneal surface elevation in myopic patients under normal corneal biomechanical state;the influence of corneal morphological and biomechanical parameters should be considered when diagnosing the abnormal posterior corneal surface elevation.

5.Single-center clinical analysis of 9 049 children hospitalized for accidental injury

Objective:To analyze the characteristics of child cases admitted to accidental injury and provide reliable basis for the prevention of accidental injury.Methods:The clinical data of children admitted to Hunan Children′s Hospital aged 0-18 due to accidental injuries from January 1, 2018 to December 31, 2021 were retrospectively analyzed. The type of accidental injury and characteristic factors such as prognosis, gender, age, time, and location of the child were analyzed.Results:A total of 9 049 children with accidental injury were admitted, accounting for 3.72%(9 049/33 697) of the total number of hospitalized children. The top three types of accidental injuries were falls/drop (3 695 cases), foreign bodies/suffocation (2 639 cases) and traffic accidents (1 165 cases), accounting for 82.87%(7 499/9 049). There were 8 760 cases (96.81%) of improvement and recovery, 178 cases (1.97%) of disability, and 111 cases (1.23%) of unhealed/dead. Among the accidental injuries, 5 833 cases (64.46%) were boys and 3 216 cases (35.54%) were girls, and the incidence ratio was 1.81∶1. There was significant difference between boys and girls in the composition ratio of the type of accidental injury such as falls/falls, foreign bodies/suffocation, poisoning, sharp object injury, drowning ( χ2 values were 3.90-20.56, all P<0.05). Among the accidental injuries, the children aged 1 to<3 years had higher accidental injuries than the other age groups (3 263 cases, accounting for 36.06%), and the composition ratio of accidental injuries in different age groups was different ( χ2 values were 12.98-573.97, all P<0.05). Among the accidental injuries, the accidental injuries occurred in the second quarter and the third quarter were higher than those in the other two quarters (4 892 cases, accounting for 54.06%), and the composition ratio of accidental injuries such as falls/falls, foreign bodies/suffocation, burn and scald, drowning occurred in different quarters was different ( χ2 values were 10.79-18.88, all P<0.05). In the case of accidental injuries, the family was the most likely place of accidental injury, with different types of accidental injuries occurring in different places ( χ2 values were 10.08-2 186.54, all P<0.05). Conclusions:Children′s unintentional injuries are most likely to occur in boys aged 1-<3 years, and fall/fall is the main injury type. Traffic accidents are the most important unintentional injury type leading to children′s unhealed/dead. Different injury types were related to child gender, age, quarter, and place of occurrence.Due to the differences in the occurrence mechanism and injury mode of accidental injuries in different countries and regions, and the majority of accidental injuries can be prevented, targeted preventive measures should be taken according to the characteristics of children′s accidental injuries in different regions, and a comprehensive prevention system for children′s accidental injuries should be constructed to ensure children′s safety.
6.Non-contact anterior cruciate ligament injury and functional ankle instability: a correlation study in young adults

Objective:To study the correlation between non-contact anterior cruciate ligament (ACL) injury and functional ankle instability (FAI) in young patients.Methods:A retrospective analysis was conducted of the 102 patients with non-contact ACL injury[61 males and 41 females, with an age of (31.9±6.1) years and a Tegner activity score of (6.1±1.9) points] who had been treated at Department of Orthopedics, Sun Yat-sen Memorial Hospital from January 2017 to March 2020 (injury group). Another 102 citizens without ACL injury from Guangzhou [56 males and 46 females, with an age of (30.3±7.2) years and a Tegner activity score of (6.0±2.1) points] were recruited as a control group. The Cumberland ankle instability tool (CAIT) and the Ankle Joint Functional Assessment Tool (AJFAT) were used to assess whether the subjects had self-conscious FAI or not. A correlation analysis was conducted using the data collected.Results:The 2 groups were comparable because there were no significant differences between them in general data ( P>0.05). By the CAIT score, the incidence of FAI in the injury group [52.9% (54/102)] was significantly higher than that in the control group [32.4% (33/102)] ( P<0.05); by the AJFAT score, the incidence of FAI in the injury group [59.8% (61/102) ] was significantly higher than that in the control group [39.2% (40/102)] ( P<0.05). Pearson correlation analysis showed that diagnoses of FAI by CAIT and by AJFAT were respectively correlated with ACL injury ( r=-0.159, P=0.023; r=-0.215, P=0.002). Conclusions:The incidence of FAI may be high in patients with ACL injury and there is a correlation between FAI and ACL injury.
7.Effect of splenectomy on the risk of hepatocellular carcinoma development among patients with liver cirrhosis and portal hypertension: a multi-institutional cohort study

Objective:To identify whether splenectomy for treatment of hypersplenism has any impact on development of hepatocellular carcinoma(HCC) among patients with liver cirrhosis and hepatitis.Methods:Patients who underwent splenectomy for hypersplenism secondary to liver cirrhosis and portal hypertension between January 2008 and December 2012 were included from seven hospitals in China, whereas patients receiving medication treatments for liver cirrhosis and portal hypertension (non-splenectomy) at the same time period among the seven hospitals were included as control groups. In the splenectomy group, all the patients received open or laparoscopic splenectomy with or without pericardial devascularization. In contrast, patients in the control group were treated conservatively for liver cirrhosis and portal hypertension with medicines (non-splenectomy) with no invasive treatments, such as transjugular intrahepatic portosystemic shunt, splenectomy or liver transplantation before HCC development. All the patients were routinely screened for HCC development with abdominal ultrasound, liver function and alpha-fetoprotein every 3 to 6 months. To minimize the selection bias, propensity score matching (PSM) was used to match the baseline data of patients among splenectomy versus non-splenectomy groups. The Kaplan-Meier method was used to calculate the overall survival and cumulative incidence of HCC development, and the Log-rank test was used to compare the survival or disease rates between the two groups. Univariate and Cox proportional hazard regression models were used to analyze the potential risk factors associated with development of HCC.Results:A total of 871 patients with liver cirrhosis and hypertension were included synchronously from 7 tertiary hospitals. Among them, 407 patients had a history of splenectomy for hypersplenism (splenectomy group), whereas 464 patients who received medical treatment but not splenectomy (non-splenectomy group). After PSM,233 pairs of patients were matched in adjusted cohorts. The cumulative incidence of HCC diagnosis at 1,3,5 and 7 years were 1%,6%,7% and 15% in the splenectomy group, which was significantly lower than 1%,6%,15% and 23% in the non-splenectomy group ( HR=0.53,95% CI:0.31 to 0.91, P=0.028). On multivariable analysis, splenectomy was independently associated with decreased risk of HCC development ( HR=0.55, 95%CI:0.32 to 0.95, P=0.031). The cumulative survival rates of all the patients at 1,3,5,and 7 years were 100%,97%,91%,86% in the splenectomy group,which was similar with that of 100%,97%,92%,84% in the non-splenectomy group ( P=0.899). In total,49 patients (12.0%) among splenectomy group and 75 patients (16.2%) in non-splenectomy group developed HCC during the study period, respectively. Compared to patients in non-splenectomy group, patients who developed HCC after splenectomy were unlikely to receive curative resection for HCC (12.2% vs. 33.3%,χ2=7.029, P=0.008). Conclusion:Splenectomy for treatment of hypersplenism may decrease the risk of HCC development among patients with liver cirrhosis and portal hypertension.

9.Clinical investigation on the related factors for the application of systemic glucocorticoids in patients with acute exacerbation of chronic obstructive pulmonary disease with carbon dioxide retention

Objective:To investigate the factors affecting the application of systemic glucocorticoids in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with carbon dioxide (CO 2) retention, and to guide the formulation of a strategy to reduce systemic glucocorticoid exposure. Methods:The AECOPD patients with CO 2 retention admitted to the Ningde Municipal Hospital of Fujian Medical University from January 2017 to December 2019 were enrolled. The general information, past history, times of acute exacerbations within 1 year, pneumonia on admission, causes of COPD, heart failure, blood gas analysis, eosinophil count (EOS), albumin (Alb) and apolipoprotein E (ApoE) levels, exhaled nitric oxide (FeNO) level, inhaled glucocorticoid and non-invasive mechanical ventilation treatment at acute exacerbation were collected. The patients were divided into recommended dosage group (exposure levels in the recommended dosage range, cumulative prednisone dosage ≤ 200 mg) and exceeded group (exposure levels exceeded the recommended dose, cumulative prednisone dosage > 200 mg) according to cumulative systemic glucocorticoid exposure dosage of the patients during hospitalization. The clinical data of patients between the two groups were compared, and possible factors with P < 0.1 in univariate analysis were included in multivariate Logistic regression analysis to screen the related factors of systemic glucocorticoid exposure level in AECOPD patients with CO 2 retention. Results:According to the order of hospitalization, 151 AECOPD patients with CO 2 retention were enrolled, 8 patients were excluded, and 143 patients were enrolled in the analysis. Of the 143 patients, 68 received the recommended dose of systemic glucocorticoid, and 75 received excessive systemic glucocorticoid. Age, percentage of forced expiratory volume in 1 second (FEV1%) at stable phase, frequency of acute exacerbation within 1 year, heart failure ratio, oxygen index (PaO 2/FiO 2), arterial partial pressure of carbon dioxide (PaCO 2), serum EOS and ApoE levels at admission, the ratio of aerosolized inhaled glucocorticoids and non-invasive mechanical ventilation showed statistical differences between the two groups. Multivariate Logistic regression analysis showed that related factors affecting systemic glucocorticoid exposure levels of AECOPD patients with CO 2 retention were FEV1% at stable phase [odds ratio ( OR) = 0.957, 95% confidence interval (95% CI) was 0.921-0.994, P = 0.023], acute exacerbation frequency within 1 year ( OR = 1.530, 95% CI was 1.121-2.088, P = 0.007), heart failure ( OR = 3.022, 95% CI was 1.263-7.231, P = 0.013), PaCO 2 ( OR = 1.062, 95% CI was 1.010-1.115, P = 0.018) and EOS at admission ( OR = 0.103, 95% CI was 0.016-0.684, P = 0.019), aerosolized inhaled glucocorticoids ( OR = 0.337, 95% CI was 0.145-0.783, P = 0.011) and non-invasive mechanical ventilation at acute exacerbation ( OR = 0.422, 95% CI was 0.188-0.948, P = 0.037), of which high FEV1% at stable phase, high EOS at admission, aerosolized inhaled glucocorticoid and non-invasive mechanical ventilation at acute exacerbation were protective factors, while high frequency of acute exacerbation within 1 year, heart failure and high PaCO 2 were risk factors. Conclusions:For AECOPD patients with CO 2 retention, high FEV1% at stable phase, high EOS level at admission, aerosolized inhaled glucocorticoid and non-invasive mechanical ventilation at acute exacerbation can reduce systemic glucocorticoid exposure. 10. Application of virtual medical experimental platform on formative evaluation of functional experimental teaching

Based on the combination of formative evaluation and summative evaluation, the newly-built virtual experimental platform was applied to the functional experimental assessment of clinical students. Its effects were observed by the process evaluation of "daily performance-virtual and real operation + written exam" and feedback from teachers and students. The results showed that daily performance, experimental operation, experimental report and final score of students in the experimental group who received the teaching by virtual experimental platform were significantly better than those in the control group; the students' interests in learning and self-learning ability in the experimental group were also significantly improved. This research showed that the introduction of virtual experimental platform into the teaching evaluation of diversified functional experiment enriches the contents and ways of formative evaluation, so as to improves students' experimental skills, and further enhance the effects of experimental teaching.
