1.Development process of family resilience in children with congenital pseudarthrosis of the tibia based on grounded theory

Objective:To explore the development process of family resilience in children with congenital pseudarthrosis of the tibia (CPT) and to understand the long-term challenges and coping strategies that CPT imposes on the families of affected children.Methods:This study combined purposive sampling and theoretical sampling. It selected 15 caregivers of CPT children from Hunan Children's Hospital for semi-structured interviews. Grounded theory was used to analyze the interview results.Results:The development of family resilience in CPT children's families occurred in three stages: pre-formation stage, formation stage, and maintenance stage. In facing negative emotions and family challenges, caregivers first needed to rebuild their inner beliefs. They then adjusted the family organizational model, adopted open and inclusive communication, and actively sought external support to foster the development of family resilience. Ultimately, caregivers were able to self-regulate their emotions, accumulate caregiving experience, and begin to shift their life perspective.Conclusions:The development of family resilience in CPT children's families is a dynamic, multi-stage process with interactions of multiple factors. Healthcare providers should offer professional health guidance according to the different stages of family development. Moreover, the government and schools should increase their attention and support, working together with families and healthcare providers to enhance family resilience for children with CPT.

3.Application of early respiratory training program based on 4E model in children with bronchiolitis obliterans

Objective:To explore application effect of early respiratory training schemes based on 4E mode (Engage, Educate, Execute, Evaluate) in children with bronchiolitis obliterans (BO) and provide evidence for the clinical implementation of early respiratory rehabilitation in children with BO.Methods:This was a quasi-experimental study. The children with BO who were admitted to 2 wards of the Department of Respiratory Medicine of Hunan Children's Hospital from January 1 to December 31, 2021 were selected as the research objects. They were randomly divided into the control group and the experimental group, with 23 cases in each group. The control group received routine treatment, nursing and rehabilitation guidance. The experimental group established a multidisciplinary team based on the control group, and used the early respiratory training program based on the 4E model to implement intervention. The clinical symptom severity scale was used to evaluate the improvement of the clinical symptoms of the children within 24 hours of being diagnosed as BO, the day of discharge, and 1 and 3 months after discharge for re-examination, and the hospitalization time of the children and the incidence of adverse events related to respiratory training were counted by using medical records and questionnaires.Results:The clinical symptom severity scores of the experimental group within 24 hours of admission diagnosis and the day of discharge were (20.00 ± 2.51) and (11.30 ± 2.46)points respectively, while those of the control group were (20.57 ± 2.21) and (11.70 ± 2.42) points respectively, with no statistically significant difference ( t=0.81, 0.54, both P>0.05). The clinical symptom severity scores of the experimental group were(10.52 ± 2.31) and (8.55 ± 1.06) points, lower than (12.32 ± 1.39) and (12.45 ± 2.19) points of the control group when they returned to the hospital for re-examination 1 and 3 months after discharge, with a statistically significant difference ( t=3.14, 7.25, both P<0.05). The experimental group was hospitalized for (11.78 ± 1.17) days, which was showter than (13.74 ± 1.63) days in the control group, with a statistically significant difference ( t=4.68, P<0.05). No respiratory training-related adverse events occurred in both groups of children during hospitalization. During home respiratory training after discharge, 1 and 2 respiratory training-related adverse events occurred in the experimental group 1 and 3 months after discharge, respectively, compared with 6 and 9 in the control group. The difference was statistically significant ( χ2=4.64, 5.94, both P<0.05). Conclusions:Early respiratory training solutions based on the 4E mode can improve the clinical symptoms of BO children, shorten the hospitalization time, reduce the number of adverse events related to respiratory training, and promote the recovery of children.
4.Clinical effect observation of esketamine combined with dexmedetomidine for drug-induced sleep endoscopy

OBJECTIVE To investigate the effects of esketamine combined with dexmedetomidine used for drug-induced sleep endoscopy in patients with obstructive sleep apnea syndrome. METHODS Totally 60 patients with obstructive sleep apnea syndrome were randomly divided into control group and observation group ,with 30 cases in each group. Anesthesia induction scheme of control group included loading dose of dexmedetomidine 1 μg/kg,pumped for 10 min,maintained at 1 μg(/ kg·h),and intravenous administration of propofol at a constant rate of 3 mg(/ kg·h)until the patient snored. That of observation group included same administration route and dose of dexmedetomidine as control group ,intravenous administration of esketamine 1 mg/kg at a constant rate of 0.5 mg/(kg·h)until the patient snored. Sleep endoscopy was performed when the bispectral index (BIS)was lower than 75 and the alert /sedation(OAA/S)score was higher than 1. The vital signs ,BIS and OAA/S scores of patients in awake state (T0),10 min load dose of dexmedetomidine infusion (T1),at the time of examination (T2)and at the end of examination (T3) were recorded ,as well as the patient ’s medication (including the one-time success rate of examination ,the number of additional drugs due to physical movement during examination ),and the occurrence of adverse events after medication. RESULTS In the comparison between groups ,the pulse oxygen saturation (SpO2)and BIS of patients in the observation group were significantly higher than those in the control group at T1,T2 and T3;the heart rates and mean arterial pressures (MAP)at T2 and T3 were significantly lower than those in the control group ;and the OAA/S score at T1 and T2 were significantly higher than those in the control group (P<0.05). Comparison within the group ,the heart rates at T1,T2 and T3 in the observation group were significantly lower than those at T0,and the MAP at T2 and T3 were significantly lower than that at T1(P<0.05);in control group ,compared with T0,SpO2 decreased significantly at T1,T2 and T3,heart rate decreased significantly at T1,and MAP increased significantly at T2(P<0.05). Sixty patients successfully completed drug-induced sleep endoscopy after medication. The one-time success rate of examination in the observation group was significantly higher (No.191460443) than control group (P<0.05),and the number of additional drugs due to physical movement during examination was significantly less than control group (P<0.05). There was no significant difference in the incidence of adverse events between 2 groups(P>0.05). CONCLUSIONS Esketamine combined with dexmedetomidine has less respiratory inhibition and less effect on hypoxia hemodynamics in patients with obstructive sleep apnea syndrome. It has less intervention times in the process of sleep endoscopy ,and has more advantages than propofol.
5.Research on the evaluation index system of hospital culture construction led by Party building and the mediating effect

Objective:To establish an evaluation index system of hospital culture construction led by Party building in public hospitals, and to explore the relationships among Party building, hospital culture and patient satisfaction, and explain the mediating effect of hospital culture between Party building and patient satisfaction.Methods:Literature review and two rounds of Delphi method were used to create an evaluation index system of hospital culture construction led by Party building in public hospitals. On such basis, a questionnaire was designed and 429 staff from 33 public hospitals in Shanghai were surveyed through April and June 2021. Cronbach α coefficient method and exploratory factor analysis were used to test the reliability and validity of the indexes, and the regression analysis method was used to verify the mediating effects. Results:Following two rounds of Delphi survey run by 14 experts, a performance evaluation index system for the hospital culture construction was constructed, comprising 5 level-1, 13 level-2 and 29 level-3 indexes. The Cronbach α coefficients of overall indexes and 5 level-1 indexes fell in the range of 0.860-0.901( P<0.01), and the retest reliability of overall index and 5 level-1 indexes fell between 0.91-0.93( P<0.01). Based on the exploratory factor analysis, all of the characteristic roots of the factors generated by orthogonal rotation were>1, and the total variance explained contribution rate was 56.84%, proving the good validity of the indexes. The effect of Party building on patient satisfaction was significant( β1=0.139, P<0.01), and that on hospital culture effect significant as well( β2=0.168, P<0.01), while that on patient satisfaction was also significant( β3=0.115, δ=0.448, P<0.01)with hospital culture effect introduced as the mediating effect. Conclusions:The evaluation index system of hospital culture construction led by Party building proves highly scientific and effective, and hospital culture plays to some extent a mediating role between Party building and patient satisfaction. This proves that Party building presents a significant transmission effect on patient satisfaction through hospital culture building. The study provides a reference for strengthening Party building and hospital culture construction and improving patient satisfaction in public hospitals.
6.Surgical treatment of pilonidal disease

Objective To study the surgical treatment of the pilonidal disease.Methods The clinical data of 33 cases of the pilonidal disease were retrospectively analyzed from Jul 2007 to Feb 2014.18 cases were treated with Excision and Marsupialization,and 15 cases were treated with Rhomboid excision and Limberg flap.Results All 18 cases in the excision and marsupialization group,were cured by surgery.all 15 cases in the rhomboid excision and Limberg flap group were cured,five of these cases were delayed healing dehiscence or necrosis,all this cases were healed after dressing drainage.The average healing time of the Limberg flap group was shorter than that of the Marsupialization group[(19 ±7) d vs.(37 ± 12) d,t =6.556,P < 0.01].Postoperative recurrence of the Marsupialization group was 1 case,the recurrence rate was 5.6%,and there was no recurrence after Limberg flap transfer.The recurrence rate of the 2 groups was statistically insignificant (P > 0.05).Conclusion The excision and marsupialization and the rhomboid excision and Limberg flap are effective in the treatment of the pilonidal disease,and the Limberg flap transfer is recommended in complicated and recurrence cases.
7. KRAS mutations analysis in mucinous epithelial lesions of the endometrium

Objective:
To investigate the frequency of KRAS mutation in mucinous epithelial lesions of the endometrium, and analyze the correlation between KRAS mutation and the clinicopathologic features.
Methods:
The cohort included forty-three cases of mucinous epithelial lesions of the endometrium selected from July 2015 to October 2017 from Beijing Obstetrics and Gynecology Hospital, and 22 control cases. Genomic DNA was extracted from formalin-fixed paraffin-embedded tissue sections. Polymerase chain reaction amplification for KRAS exons 2 and 3 was performed, followed by sequencing using capillary electrophoresis. The Fisher exact test was used to compare the prevalence of KRAS mutation among the different groups.
Results:
The patients′age ranged from 33 to 77 years [mean (55.12±9.34) years, median 55 years]. None of the eight cases of endometrial hyperplasia with mucinous differentiation without atypia showed KRAS mutation. The frequency of KRAS mutations was 1/10 in endometrial atypical hyperplasia, 1/12 in endometrioid carcinoma, 4/11 in endometrial atypical hyperplasia with mucinous differentiation (EAHMD), 6/15 in endometrioid carcinoma with mucinous differentiation (ECMD) and 8/9 in mucinous carcinoma (MC), respectively. 8.Clinical efficacy and safety of flexible ureteroscope combined with holmium laser in the treatment of upper urinary tract calculi(≥2.0cm)

Objective To investigate the clinical efficacy and safety of flexible ureteroscope combined with holmium laser in the treatment of upper urinary tract calculi(≥2.0cm).Methods One hundred patients with upper urinary tract calculi (≥2.0cm) were divided into control group and observation group according to random digits table method with 50 cases each.The control group was treated with percutaneous nephrolithotomy combined with holmium laser,and the observation group was treated with flexible ureteroscope combined with holmium laser.The operation related index, stone clearance rate and postoperative complications were compared between 2 groups.Results The operation time, two stage stone removal rate and length of hospital stay in observation group were significantly lower than those in control group:(45.76 ± 9.24)min vs.(52.12 ± 10.68)min,2.00%(1/50) vs.16.00%(8/50)and(4.46 ± 1.22)d vs.(5.73 ± 1.91)d,and there were statistical differences(P<0.01 or <0.05).The stone clearance rate in observation group was significantly higher than that in control group:94.59%(70/74)vs.81.69%(58/71),and there was statistical difference(P<0.05).There was no statistical difference in incidence of postoperative complications between 2 groups (P>0.05).Conclusions Flexible ureteroscope combined with holmium laser in the treatment of upper urinary tract calculi (≥2.0cm) can effectively reduce the reoperation rate, shorten the operation time and hospitalization time,and improve the stone clearance rate.The treatment is safe and effective.
9.Effect of transcutaneous electrical acupoint stimulation on postoperative analgesia of ureteroscopic holmium laser lithotripsy.

OBJECTIVETo explore the effect of transcutaneous electrical acupoint stimulation (TEAS) on postoperative analgesia of ureteroscopic holmium laser lithotripsy.
METHODSOne hundred and twenty adult patients, American Association of Anesthesiologists (ASA) Class Ⅰ or Ⅱ, scheduled to ureteroscopic holmium laser lithotripsy, were randomly assigned into an observation group and a control group, 60 cases in each one. The patients in the observation group were treated with TEAS for postoperative analgesia. TEAS was implemented at bilateral Shenshu (BL 23) and Yinlingquan (SP 9) at the time of back ward and postoperative 4 h, 8 h, 12 h. TEAS at 7:00, 11:00 and 15:00 at the above acupoints were used on the second and third days; while placebo (twice a day, 100 mg a time) was used. Tramadol hydrochloride tablets for postoperative analgesia were applied in the contnol group, twice a day, 100 mg a time, and electrode sheets without stimulation were put on Shenshu (BL 23) and Yinlingquan (SP 9). When analgesia was insufficient with the score of visual analogue scale (VAS)≥3, the patients were treated with tramadol tablets for remedy analgesia. The VAS score, the concentrations of serotonin (5-HT) and substance P (SP) in 3 mL venous blood at the time of back ward (T), postoperative 4 h (T), 12 h (T), 24 h (T), and 48 h (T) were detected respectively. The total amount of medication for remedy analgesia and the incidence of adverse reactions, such as nausea and vomiting within postoperative 48 h were compared between the two groups.
RESULTSThe VAS scores at T through T were lower than those at T in the two groups (all <0.05). Compared with the control group, the VAS scores at T through T in the observation group were lower (all <0.05). The total dose of remedy analgesic medicine within 48 h after operation in the observation group was lower than that in the control group (<0.05). Compared with the control group, the concentrations of 5-HT at T, T, T and SP at T through T were lower (all <0.05). The numbers of constipation, nausea and vomiting in the observation group were less than those in the control group (both <0.05).
CONCLUSIONTEAS can relieve the pain and reduce the total amount of analgesic medicine, the levels of substance causing pain and the incidence of adverse reactions after ureteroscopic holmium laser lithotripsy.
10.Review of application of distress thermometer and problem list in cancer patients

Distress thermometer (DT) and problem list (PL) are rapid screening tools recommended by National Comprehensive Cancer Network for cancer patients' levels of distress. DT has good validation and reliability and widely used around the world. In this paper, we reviewed the cutoff score of DT, grading disputes of PL, and application situations of them to provide a reference for the tools selection and applying of psychological distress screening of cancer patients in China.
