1.Study on residents’willingness to pay for community pharmacies’medication guidance services and its influencing factors

OBJECTIVE To explore the willingness to pay (WTP) of Urumqi residents for community pharmacies’ medication guidance services and its influencing factors, so as to provide data support for the optimization of community pharmacy services and the establishment of a fee structure for medication guidance services. METHODS A stratified quota sampling method was employed to select 14 communities in Urumqi City. From April to June 2025, a combined offline and online questionnaire survey was conducted among adult residents in these communities. The contingent valuation method was used to construct three hypothetical scenarios (namely, basic, enhanced and extended services) of medication counselling in community pharmacies to assess residents’ WTP for these services. Binary Logistic regression was employed to analyze the influencing factors of WTP. RESULTS A total of 576 valid questionnaires were obtained. Under the scenarios of basic, enhanced and extended services, 38.54%, 49.65% and 67.19% of the respondents expressed WTP for the services, respectively. Occupational type, type of basic medical insurance, annual income, perception of pharmacists’ profession, and acceptance level of the service were identified as major influencing factors for WTP (P<0.05). CONCLUSIONS The willingness of residents in Urumqi to pay for medication counseling services provided by pharmacists in community pharmacies significantly increases with the enrichment of service content. It is recommended to incorporate basic medication counselling services provided by pharmacists in community pharmacies into medical insurance payment, while value-added services should be partially or fully self-paid by residents. Additionally, efforts should be made to strengthen the promotion of the professional and service value of licensed pharmacists, so as to facilitate the high-quality development of pharmaceutical care.
2.Soil conditioners affect rhizospheric bacterial communities of Cabernet Sauvignon.

Three soil conditioners were prepared from granulated food waste and decomposed cattle manure combined with desulfurization gypsum, coal gangue, and maifanite, respectively. Field trials were conducted in the saline field growing Cabernet Sauvignon. The effects of soil conditioners on rhizospheric bacterial communities were studied, with the aim of providing a scientific basis for soil amelioration and restoration. Five treatments were designed, including the control (T1), conventional fertilization (T2), reduced chemical fertilization+organic matter-based soil conditioner with calcium additives (T3), reduced chemical fertilization+organic matter-based soil conditioner with silica additives (T4), and reduced chemical fertilization+organic matter-based soil conditioner with magnesium additives (T5), each with three replications. The results indicated that soil conditioners improved the rhizospheric nutrients, yield, and quality of grape (P<0.05), increased relative abundance of Proteobacteria by 17.32%-23.37%, decreased relative abundance of unidentified_Bacteria and Acidobacteriota by 4.22%-28.42% and 20.88%-35.81%, respectively. The bacterial community composition and diversity were different between treatments. Function analysis showed that the expression levels of the genes involved in chromosome and protein synthesis, mRNA biosynthesis, and glyoxylate and dicarboxylate metabolism were up-regulated in the treatments with soil conditioners. The correlation analysis revealed that multiple environmental factors affected the alpha diversity of rhizospheric bacterial communities, and some bacterial taxa were closely related to the grape yield and quality. It is concluded that soil conditioners can effectively alter rhizosphere nutrient levels and bacterial community structures and functions. T5 treatment outperforms other treatments in improving the physico-chemical and biological characteristics of rhizosphere, and the yield, and quality of grape. It has potential for application, and provides an important basis for development of new-type soil conditioners.

3.Relationship of 25- (OH) D and MCP-1 levels with bone mineral density and bone metabolism indexes in preschool children after fractures

Objective:To investigate the relationship of the levels of 25-hydroxyvitamin (OH) D, monocyte chemoattractant protein-1 (MCP-1) and bone mineral density (BMD) and bone metabolism in preschool children after fractures.Methods:General data of 200 preschool children with fractures admitted to the Department of Orthopedics of Shanxi Children’s Hospital from Apr. 2021 to Jun. 2024 were retrospectively analyzed. The 25- (OH) D level of the children after fracture was determined by electrochemical luminescence method. According to the 25- (OH) D level, the children were divided into VitD deficiency group, VitD insufficient group and VitD sufficient group. The MCP-1 level, BMD and bone metabolism indexes of children among groups were compared and analyzed. SPSS 26.0 statistical software was used to analyze the data in the paper. According to the data type, t test or χ2 test were used to compare among the groups, and the correlation of 25- (OH) D and MCP-1 levels with BMD and bone metabolism indexes was analyzed by Spearman and Pearson methods. Results:The difference of 25- (OH) D levels in different age childeren was statistically significant ( t = 145.26, P<0.05) , and the analysis showed that 26 cases were VitD deficient, 64 cases were VitD insufficient, and 110 cases were VitD sufficient; The mean BMD in VitD patients with different levels was significantly different ( F=783.25, P<0.05) ; With the increase of MCP-1, PTH and TPINP levels decreased ( F=78.98, 703.57, 243.27, P<0.05) , while the levels of PICP and BGP increased ( F=122.97, 340.32, P<0.05) ; 25- (OH) D was positively correlated with BMD, PICP and BGP ( r=0.93, 0.76, 0.87, P<0.05) , and negatively correlated with PTH and TPINP ( r=-0.94, -0.81, P<0.05) . MCP-1 was negatively correlated with BMD, PICP and BGP ( r=-0.54, -0.51, -0.56, P<0.05) , and positively correlated with PTH and TPINP ( r=0.57, 0.55, P<0.05) . Conclusions:The 25- (OH) D level is significantly correlated with BMD and bone metabolism indexes in preschool children after fracture, and the lack and insufficiency of VitD significantly affect BMD and bone metabolism status. At the same time, MCP-1 may also play an important role in metabolic changes after fracture. Therefore, in clinical treatment, it is necessary to strengthen the monitoring and management of 25- (OH) D and MCP-1 levels after fracture in children, which is of great significance to promote bone healing and improve bone density.
4.The challenges and strategies of conversion therapy for intrahepatic cholangiocarcinoma with middle hepatic vein invasion

Intrahepatic cholangiocarcinoma(ICC)is a malignancy with an insidious onset and extremely high mortality,often resulting in the inability to perform radical surgery due to vascular invasion and metastasis.The middle hepatic vein(MHV),located adjacent to the left and right hepatic veins and the second hepatic hilum,is an essential component of the hepatic vascular system.Patients with ICC involving the MHV face significant treatment challenges,and there is considerable debate regarding the optimal treatment decisions.Conversion therapy has provided new hope for patients with initially unresectable ICC,particularly for those with MHV invasion,who have an urgent need for such treatment.This article combines relevant research findings and clinical experience of the authors'team to discuss several challenges in conversion therapy for ICC with MHV invasion,aiming to provide reference for the clinical management of these patients.
5.Anthocyanins alleviate pentatetrazene-induced epileptic seizures in rats by inhibiting cuproptosis

Objective:To study the effect of anthocyanins(C3G)on cuproptosis in chronic epileptic rats.Methods:Chronic epileptic rat model was induced by pentatetrazol(PTZ),and 90 SD rats were randomly divided into control group,PTZ group,elesclomol(ELC)group,tetrathiomolybdate(TTM)group,C3G group and ELC+C3G group.The grade,latency and frequency of seizures were recorded in each group.electroencephalogram(EEG)was used to detect abnormal electrical discharge in the brain.The action potential of hippocampal neurons was measured by patch-clamp technique.The contents of glutathione(GSH)and cuprous ions(Cu+)in hippocampus were determined by kit.Neuron damage in hippocampus was evaluated by Nissl staining.The expression of ferredoxin1(FDX1)and lipoic acid synthase(LIAS)in hippocampus was analyzed by immunohistochemistry and Western blot.Results:Compared with the control group,the rats in the PTZ group exhibited epileptic-like seizures,suggesting that the modeling was successful.Com-pared with other epileptic groups,the ELC group showed increased seizure grade,more abnormal discharges,shortened latency period,enhanced neuronal excitability,decreased Nissl particles,elevated Cu+levels,decreased GSH levels,and increased expressions of FDX1 and LIAS.The reverse was observed in C3G group(P<0.05).Neuron damage in ELC+C3G group was less severe than that in ELC group,but more than that in PTZ group(P<0.05).Neuron dam-age in TTM group was less severe than that in PTZ group,but more severe than that in C3G group(P<0.05).Conclusion:cuproptosis exists in hippocampus of rats with chronic epilepsy,and the C3G can significantly inhibit cu-proptosis and alleviate the occurrence and development of chronic epilepsy.
6.The cutting-edge progress of novel biomedicines in ovulatory dysfunction therapy.

Ovulatory dysfunction (OD) is one of the main causes of infertility in women of childbearing age, which not only affects their reproductive ability, but also physical and mental health. Traditional treatment strategies have limited efficacies, and the emergence of biomedicines provides a promising alternative solution via the strategies of combining engineered design with modern advanced technology. This review explores the pathophysiological characteristics and related induction mechanisms of OD, and evaluates the current cutting-edge advances in its treatments. It emphasizes the potentials of biomedicines strategies such as hydrogels, nanoparticles and extracellular vesicles in improving therapeutic precision and efficacy. By mimicking natural physiological processes, and achieving controlled drug release, these advanced drug carriers are expected to address the challenges in ovarian microenvironment reprogramming, tissue repair, and metabolic and immune regulation. Despite the promising progress, there are still challenges in terms of biomedical complexity, differences between animal models and human physiology, and the demand for intelligent drug carriers in the therapy of OD. Future researches are mainly dedicated to developing precise personalized biomedicines in OD therapy through interdisciplinary collaboration, promoting the development of reproductive regenerative medicine.
7.Empagliflozin alleviates ox-LDL-induced injury of human umbilical vein endothelial cells by inhibiting the EGFR signaling pathway

Aim To investigate the protective and reparative effect of empagliflozin on oxidized-low density lipo-protein(ox-LDL)-induced injury in human umbilical vein endothelial cells(HUVEC)and its mechanism of action.Methods Primary HUVEC were cultured in vitro.ox-LDL was used to induce HUVEC injury model,and the cell sur-vival rate was measured by CCK-8 assay.EDU method was used to detect cell proliferation.Western blot was used to detect the protein levels of Ki-67,Bcl-2,cleaved Caspase-3,Bax,endothelial nitric oxide synthase(eNOS),intercellular adhesion molecule-1(ICAM-1),vascular cell adhesion molecule 1(VCAM-1)and epidermal growth factor receptor(EGFR)in HUVEC.RT-qPCR was used to detect the mRNA expression levels of interleukin-6(IL-6)and tumor necro-sis factor-α(TNF-α)in HUVEC.Using Swiss targets,GeneCards databases,gene ontology(GO)and Kyoto encyclope-dia of genes and genomes(KEGG),protein-protein interaction(PPI)network analysis,and ClickDocking(https://mcule.com/apps/1-click-docking/)to predict the target of empagliflozin.Results CCK-8 results showed that 0.025 μmol/L empagliflozin significantly alleviated ox-LDL-induced HUVEC injury(P<0.01).EDU results showed that ox-LDL treatment for 24 h significantly inhibited the proliferation of HUVEC(P<0.01),while empagliflozin treatment sig-nificantly alleviated the inhibition of cell proliferation(P<0.01).The results of Western blot showed ox-LDL treatment significantly decreased the protein expression levels of Ki-67,Bcl-2,and eNOS,and increased the protein expression levels ofcleaved Caspase-3,Bax,ICAM-1,and VCAM-1 in HUVEC(all P<0.05).However,empagliflozin treatment reversed these changes(all P<0.05).RT-qPCR results showed that ox-LDL treatment increased the mRNA expression levels of IL-6 and TNF-α in HUVEC(P<0.01),while empagliflozin treatment decreased their expression levels(P<0.05).However,after adding EGFR agonist NSC 228155,the protective effect of empagliflozin against ox-LDL-mediated HUVEC injury was significantly reversed(P<0.05).Conclusion Empagliflozin can significantly reduce ox-LDL-in-duced HUVEC injury,which may be related to EGFR signaling pathway.
8.Nebulization Characteristics Study of Human Interferon α1b for Injection Based on ELISA

Objective To investigate the nebulization characteristics of human interferon α1b for injection.To characterize and compare the delivery rate,total drug substance and the aerodynamic characteristics between the two types of nebulizer.Methods Connect two types of nebulizers with a breathing simulator,respectively,and simulate the breathing patterns of an infant and child.Measure the delivery rate,total delivered dose,and delivery uniformity.The aerodynamic properties of human interferon α1b for injection were evaluated by the next generation impactor(NGI).The content of human interferon α1b was quantified by double antibody sandwich ELISA.Results In the three batches of samples in infant mode,the delivery rate and total delivered dose determind by A nebulizer were 0.45,0.49,0.44 μg·min-1,3.06,3.21,3.81 μg,respectcively;and 0.12,0.14,0.16 μg·min-1,0.73,0.73,0.75 μg,respectcively by B nebulizer.In child mode,the delivery rate and total delivered doses determined by A nebulizer were 1.36,1.49,1.20 μg·min-1,7.44,7.17,and 6.54 μg,respectcively;and 0.37,0.36,0.43 μg·min-1,1.66,1.59,and 1.41 μg,respectcively by B nebulizer.In child and infant mode,the ten results of the total drug substance delivered determined by nebulizer A were both between 65%to 135%of the average.The FPD,FPF,MMAD,and GSD determined by A neublizer of three batch samples were 2.48,2.92,2.35 μg,59.0%,57.4%,59.1%,4.18,4.34,4.15 μm,1.94,1.98,2.01,respectively.The FPD,FPF,MMAD and GSD determined by B neublizer of three batches samples were 2.70,3.38,3.06 μg,67.6%,66.4%,66.3%,3.55,3.65,3.68 μm,2.03,2.04,2.06,respectively.Conclusions The data obtained in this research characterized the in vitro nebulization characteristics of human interferon α1b for injection and provided a theoretical basis and reference for in vitro study and clinical practice.The influence of different types of nebulizers on nebulization characteristics was evaluated as well.It is suggested that the quality standard of nebulizers be strictly formulated and the use of nebulizers be standardized.
9.Advantages,challenges,and optimization strategies of robotic radical resection for perihilar cholangiocarcinoma

Perihilar cholangiocarcinoma(PHCC)has an insidious onset,is highly aggressive,and carries a poor prognosis.Radical surgical resection is crucial for improving patient survival.Due to the complex anatomy of the hepatic hilum,conventional laparoscopic surgery faces numerous challenges.In recent years,robotic surgery has shown significant potential in PHCC procedures,owing to its high-definition naked-eye 3D visualization,precise maneuvers,and superior dexterity.However,its widespread adoption remains limited by high costs,restricted intraoperative working space,and the absence of tactile feedback.Drawing on international research progress and our own clinical experience,this article analyzes optimization strategies including assessment of local tumor involvement,rational trocar placement,lymphatic and neural plexus dissection,determination of the extent of hepatectomy,vascular resection and reconstruction,bile duct reconstruction,and choledochojejunostomy.The aim is to explore approaches to overcoming these challenges and breaking through existing limitations,thereby providing new strategies and technical pathways for precise minimally invasive treatment of PHCC.
10.Impact of suture configuration and fixation type on biomechanical strength of rotator cuff repair:A factorial design study

Objective To explore the impact of suture configuration and fixation type on the biome-chanical strength of rotator cuff repair,using a factorial design study.Methods Sixteen fresh-frozen porcine shoulder samples were randomized into an anchorless double-row suture bridge transosseous su-tures(DS)group,an anchored double-row suture bridge transosseous-equivalent(DE)group,an an-chorless X-BOX construct transosseous sutures(XS)group,and an anchored X-BOX construct transos-seous-equivalent(XE)group,each of four,according to suture configuration(double-row suture bridge,traditional X-BOX construct)and fixation type(suture anchors,transosseous sutures).Then,their fatigue resistance(first-cycle excursion,gap length difference ratio,and the percentage of ex-posed footprints)and the failure strength(the maximum failure load and the re-tear type)were mea-sured using a biomechanical material testing machine.Results Different suture configurations affected failure strength(F=39.559,P<0.001),with the double-row suture bridge groups(693.07±58.35 N,746.76±138.57 N)showing significantly higher failure strength,compared to the traditional X-BOX groups(462.90±18.91 N,421.43±90.76 N).However,the fixation type did not significantly im-pact failure strength(F=1.161,P=0.302).Moreover,the suture configuration influenced the gap differ-ence ratio(F=7.781,P=0.016),but had no significant correlation with other fatigue resistance indica-tors(P>0.05).Meanwhile,failure strength and fatigue resistance were not correlated with fixation type,and the interaction between suture and fixation type(P>0.05).The incidence of failure types for the four suture configurations was as follows:Type I tendon tear:XS>XE>DS=DE;type II tendon tear:DS>XE>XS=DE;fixing material-related failure:DE>DS=XE=XS.Conclusion The failure strength and gap formation ratio in rotator cuff repair under fatigue loading are influenced by suture configuration,whereas no significant association has been observed with respect to fixation method,whether using transosseous sutures or suture anchors.
