Objective To explore the value of MRI subtraction technique(ST)for evaluating the efficacy of systemic therapy for advanced hepatocellular carcinoma(HCC)and predicting prognosis after combining with surgery. Methods Totally 35 patients with 39 HCC lesions who received systemic therapy+radical resection were retrospectively collected.Based on preoperative MRI,tumor activity ratio(recorded as tumor activityST)was obtained with ST,while tumor activity value(recorded as tumor activitypathology)was obtained through postoperative pathology, and their correlation was analyzed. The patients were regularly followed up after surgery, and the survival data were recorded.Receiver operating characteristic(ROC)curve was drawn to evaluate the efficacy of tumor activityST for predicting patients'survival status. Then the patients were divided into survival benefit group and no survival benefit group according to the cut-off value, and survival analysis was conducted. Results Tumor activityST was positively correlated with tumor activitypathology(r=0.900,P<0.001).The median follow-up time was 32.93 months, during which 8 patients died, and the median survival time was 29.9 months. The area under the curve(AUC)of tumor activityST for predicting patients'survival status was 0.67,and the cut-off value was 0.36.Thirty patients with tumor activityST<0.36 were enrolled in survival benefit group, while 5 patients≥0.36 were collected in no survival benefit group. The overall survival in survival benefit group was longer than that in no survival benefit group(P<0.001).Conclusion MRI ST could be used to non-invasively evaluate the efficacy of systemic therapy for advanced HCC and predict prognosis after combining with surgery.

Objective To explore the impact of suture configuration and fixation type on the biome-chanical strength of rotator cuff repair, using a factorial design study. Methods Sixteen fresh-frozen porcine shoulder samples were randomized into an anchorless double-row suture bridge transosseous su-tures(DS)group,an anchored double-row suture bridge transosseous-equivalent(DE)group,an an-chorless X-BOX construct transosseous sutures(XS)group, and an anchored X-BOX construct transos-seous-equivalent(XE)group,each of four, according to suture configuration(double-row suture bridge, traditional X-BOX construct)and fixation type(suture anchors, transosseous sutures). Then, their fatigue resistance(first-cycle excursion, gap length difference ratio, and the percentage of ex-posed footprints)and the failure strength(the maximum failure load and the re-tear type)were mea-sured using a biomechanical material testing machine. Results Different suture configurations affected failure strength(F=39.559,P<0.001),with the double-row suture bridge groups(693.07±58.35 N,746.76±138.57 N)showing significantly higher failure strength, compared to the traditional X-BOX groups(462.90±18.91 N,421.43±90.76 N).However, the fixation type did not significantly im-pact failure strength(F=1.161,P=0.302).Moreover,the suture configuration influenced the gap differ-ence ratio(F=7.781,P=0.016),but had no significant correlation with other fatigue resistance indica-tors(P>0.05).Meanwhile,failure strength and fatigue resistance were not correlated with fixation type, and the interaction between suture and fixation type(P>0.05).The incidence of failure types for the four suture configurations was as follows:Type I tendon tear:XS>XE>DS=DE;type II tendon tear:DS>XE>XS=DE;fixing material-related failure:DE>DS=XE=XS.Conclusion The failure strength and gap formation ratio in rotator cuff repair under fatigue loading are influenced by suture configuration, whereas no significant association has been observed with respect to fixation method, whether using transosseous sutures or suture anchors.

OBJECTIVE To evaluate the immunoprotection effect of a novel inactivated whole cell vaccine against Acinetobacter baumannii based on ultrasonic microbubble physical damage technique(IWC)and explore its poten-tial of clinical transformation.METHODS Totally 48 C57BL/6 mice were randomly assigned to divide into three groups and receive the nasal inoculation of corresponding preparations, the IWC group and the paraformalde-hyde inactivated vaccine group were inoculated with 20 μl of 1× 107 CFU vaccine, the control group was treated with 20 μl phosphate buffered salt solution. The infection models were established 7 days after intraperitoneal in-jection of a lethal dose of A.baumannii.The 7-day mortality rates of the mice were statistically analyzed after tox-in attack. The counts of colonized bacterial colonies on lung and spleen tissues were determined by plate count method after toxin attack for 24 hours. The levels of inflammatory factors interleukin(IL)-6,tumor necro-sis factor α(TNF-α)and IL-1β in the lung tissues were detected by enzyme-linked immunosorbent assay(ELISA),and the pathological damage was observed.RESULTS The survival rate of the IWC group was higher than that of the control group, and the counts of colonized bacterial colonies on lung and spleen tissues were less in the IWC group than those in the control group(P<0.05).As compared the paraformaldehyde inactivated vaccine group, the survival rate of the IWC group increased by 10.00%,and the counts of colonized bacterial colonies on the lung tissues were slightly less in the IWC group than those in the paraformaldehyde inactivated vaccine group(P<0.05),and the counts of colonized bacterial colonies on spleens were basically the same. The levels of lung tis-sue inflammatory factors of the IWC group were lower than those of the other two groups(P<0.05).The patho-logical damage was alleviated, and the IWC group was superior to the control group in the integrity of alveolar structure.CONCLUSIONS IWC can maintain the immunogenicity of pathogens through physical damage technique, effectively activate the immune response of the hose, and reduce the bacterial load and inflammatory injury,show-ing better immunoprotection effect than the traditional chemical inactivation method. The study has provided ex-perimental bases for development of novel, specific, safe and highly efficient vaccine as well as new ideas and strategies for clinical prevention and treatment of A.baumannii infection.
