Mitochondrial diabetes mellitus(MDM)is a genetically heterogeneous disorder caused by mitochondrial DNA(mtDNA)or nuclear DNA mutations,characterized by multi-system involvement and diverse clinical phenotypes.We report a pediatric case presenting with growth retardation followed by subsequent development of diabetes mellitus.Systematic evaluation revealed concurrent bilateral sensorineural hearing loss,bilateral basal ganglia calcification,and electroencephalographic abnormalities.A post-exercise lactate test demonstrated significant elevation of serum lactate levels immediately after physical exertion.Genetic analysis identified a large-scale mitochondrial DNA deletion spanning from m.8649 to m.16084.This case re-port is complemented by a literature review focusing on the pathogenesis,genetic characteristics,and therapeu-tic approaches of mitochondrial diabetes,with particular emphasis on mitochondrial disorders exhibiting large-scale mtDNA deletions alongside diabetic manifestations.Our comprehensive analysis aims to enhance clinical understanding and inform diagnostic strategies for this complex disease entity.

Objective:To compare the clinical efficacy of unilateral puncture and bilateral puncture vertebroplasty in osteoporotic compression fracture patients.Methods:A regression comparative analysis was performed using case-control study to collect clinical data of 372 patients with osteoporosis compression fractures admitted to Juxian People′s Hospital from January 2022 to July 2023, including 213 males and 159 females, and the age was (59.8±6.2) years old, aged range of 50 to 70 years old. 194 patients treated with unilateral puncture vertebroplasty were included in the unilateral group, and 178 patients treated with bilateral puncture vertebroplasty were included in the bilateral group. The intraoperative indicators, vertebral changes before surgery and 6 months after surgery, and the occurrence of adverse reactions after surgery of the two groups of patients were compared. The visual analogue scale (VAS), activities of daily living (ADL) score, oswestry disability index (ODI), and quality of life (QOL) scale were used to compare the pain degree, activity ability, improvement of functional disability, and changes in quality of life and clinical efficacy of the two groups of patients before the operation and at 3 and 6 months after the operation. SPSS 21.0 software was conducted to analyze data. Measurement data with normal distribution were represented as mean±standard deviation( ± s), and the comparison between groups was conducted using the t-test. The comparison of count data were represented as n(%) and was conducted by χ2 test or Fisher exact probability. Between-group comparisons of different time intervals were performed with repeated-measures analysis of variance test. Results:The surgical time, cement dosage, bleeding amount, and fluoroscopy of patients in the unilateral group were (29.86±5.87) min, (2.58±0.37) mL, (22.65±4.83) mL, and 12 times, respectively, and (42.63±4.55) min, (7.12±0.61) mL, (50.31±5.84) mL, 25 times in the bilateral group, respectively. The difference between the two groups was statistically significant ( P<0.05). Preoperatively, the anterior height ratio of the vertebral body, sagittal concavity angle, and kyphosis angle of the affected vertebra in the unilateral group were (59.83±5.55)%, 13.04°±1.14°, and 18.93°±2.56°, respectively, while in the bilateral group, there were (60.28±5.79)%, 13.08°±1.13°, and 18.67°±2.69°, respectively. After the operation, the unilateral group were (90.20±5.52), 3.95°±0.57°, and 11.03°±1.21°, respectively. The bilateral group were (90.58±6.00), 3.99°±0.59°, and 10.91°±1.14°, respectively. Comparison of data before and after surgery between the two groups, the difference was statistically significant ( P<0.05). The overall complication rate of unilateral group was 13 cases (6.70%), and the overall complication rate of bilateral group was 26 cases (14.61%). The difference between the two groups was statistically significant ( P=0.013). The VAS, ADL, ODI, and QOL of the unilateral group were (3.49±1.10), (66.10±13.02), (18.22±4.81) and (62.10±10.00) points, the two-sided groups were (5.49±1.13), (56.19±10.27), (24.76±5.81) and (52.13±9.56) points, respectively. The differences between the two groups were statistical significance ( P<0.05), the overall treatment efficiency of patients in the unilateral group was 90.72%, and the overall treatment efficiency of patients in the bilateral group was 83.15%, and the difference between the two groups was statistically significant( P<0.05). Conclusions:Unilateral puncture vertebroplasty has the advantages of short operation time, less bone cement usage, low blood loss, fewer fluoroscopy times, low complication rate, better postoperative pain relief and functional recovery, and high overall treatment efficiency in the treatment of patients with osteoporotic compression fractures. It has obvious advantages over bilateral puncture vertebroplasty.

Objective:To investigate the liver-protective effect of Peipi Shugan Decoction (PSD) in rats with liver fibrosis through network pharmacology and experimental animal models.Methods:TCMSP was used to retrieve the active components of Peipi Shugan Decoction, GeneCards database was used to obtain the liver fibrosis related targets, and Venny 2.1 was used to obtain the intersection targets. A protein-protein interaction (PPI) network was constructed using the STRING 12.0 database, and Go function and KEGG pathway enrichment analysis were performed through David database. A total of 60 male Sprague-Dawley (SD) rats were divided into two groups: a blank control group ( n=10) and a model establishment group ( n=50) with random number table method. Liver fibrosis models were induced in the model group by intraperitoneal injection of a 40% CCl?- olive oil solution. After successful modeling, the rats were randomly assigned to the following groups ( n=10 per group): model group, colchicine group, and Peipi Shugan Decoction low-, medium-, and high-dosage groups. The colchicine group received colchicine suspension at 0.2 ml/kg via intragastric administration. Peipi Shugan Decoction low-, medium-, and high-dosage groups were administered the decoction at dosages of 2.81, 5.63, and 11.25 g/kg, respectively. The blank control and model groups received an equal volume of normal saline. All treatments were administered once daily for 8 consecutive weeks. HE staining and Masson staining were used to observe the morphological changes of liver tissue and the deposition of collagen fibers. he levels of GPT, GOT and ALP were detected by automatic biochemical analyzer. The expressions of JAK2/STAT1 signaling pathway related proteins, α - smooth muscle actin (α-SMA) and Collagen Ⅰ in rat liver were detected by Western blot. The expression of α-SMA protein in liver tissue was detected by immunohistochemistry. The number and ratio of CD4 + T cells and CD8 + T cells in liver tissue were analyzed by flow cytometry. The levels of IL-6, IL-1β and TNF-α in serum were detected by ELISA. Results:A total of 94 active components were screened out from Peipi Shugan Decoction; the prediction analysis results revealed that this compound formula shared 122 common targets with liver fibrosis diseases; The top five core targets with degree values in the PPI network are AKT1, TNF, IL-6, TP53, and IL-1β. GO enrichment analysis further indicated that Peipi Shugan Decoction mainly achieved anti-liver fibrosis effects by regulating JAK2/STAT1 signaling pathway and other mechanisms. Compared with the model group, the colchicine group and Peipi Shugan Decoction low-, medium-, and high-dosage groups exhibited decreased serum levels of GPT, GOT, ALP, IL-6, TNF-α, and IL-1β ( P<0.05), as well as reduced expression of α-SMA in liver tissue ( P<0.05). Peipi Shugan Decoction medium- and high-dosage groups showed increased protein expression of p-STAT1/STAT1 in liver tissue ( P<0.05), while Peipi Shugan Decoction high-dosage group demonstrated decreased α-SMA protein expression ( P<0.05). Additionally, Peipi Shugan Decoction medium- and high-dosage groups exhibited reduced expressions of CD4 + and CD8 + ( P<0.05). Conclusions:Peipi Shugan Decoction has the characteristics of multi-component, multi-target and multi-pathway in the treatment of liver fibrosis. Its mechanism is mainly related to activating the JAK2/STAT1 signaling pathway, inhibiting cellular inflammatory responses and hindering the activation of hepatic stellate cells (HSC).

Objective To compare the effectiveness between ultrasound-guided paravertebral block(PVB)and general anesthesia in percutaneous nephrolithotomy(PCNL).Methods Clinical data of 284 patients who underwent PCNL in our hospital from January 2023 to March 2024 were collected.These were 78 cases of PVB(PVB group)and 206 cases of general anesthesia(general anesthesia group).A total of 61 cases in each of the two groups were matched with propensity score,and the operative time,intraoperative fluid infusion volume,postoperative haemoglobin changes,postoperative visual analogue scale(VAS),hospital stay,hospital costs,and incidence of postoperative complications were compared.Results The operations were completed in both groups of patients successfully,with stable vital signs during the operation and no intraoperative anesthetic complications such as bradycardia,respiratory depression,or hypotension.There were significant differences between the two groups in heart rate(HR)before the start of surgery[(66.5±10.4)beats/min vs.(77.5±11.7)beats/min,t=-5.471,P=0.000],HR at the end of surgery[(60.5±8.4)beats/min vs.(71.0±12.3)beats/min,t=-5.472,P=0.000],mean arterial pressure(MAP)before the start of surgery[(97.2±11.9)mm Hg vs.(103.1±12.3)mm Hg,t=-2.694,P=0.008],MAP at the end of surgery[(85.2±8.8)mm Hg vs.(94.3±11.6)mm Hg,t=-4.892,P=0.000],pulse oxygen saturation(SpO2)before the start of surgery[(99.0±1.1)％vs.(99.6±0.6)％,t=-3.347,P=0.001]and SpO2 at the end of surgery[(99.2±1.1)％vs.(99.8±0.4)％,t=-4.122,P=0.000].The operative time was significantly shorter in the PVB group than that in the general anesthesia group[55.0(41.5,75.5)min vs.95.0(65.0,130.0)min,Z=-5.173,P=0.000].The postoperative haemoglobin changes[5(2,11)g/L vs.8(4,11)g/L,Z=-2.099,P=0.036],postoperative VAS score[1(1,1)points vs.1(1,2)points,Z=-3.342,P=0.001],postoperative hospital stay[3(2,3)dvs.4(3,6)d,Z=-6.016,P=0.000]and hospital costs[(14 499.4±2141.0)yuan vs.(19 634.2±3846.8)yuan,t=-9.109,P=0.000]were statistically lower in the PVB group than those in the general anesthesia group.Comparison of intraoperative fluid infusion volume[600(600,600)ml vs.600(600,1100)ml,Z=-1.800,P=0.072],postoperative venting time[1(1,1)d vs.1(1,1)d,Z=-1.045,P=0.296],and phase Ⅰ stone removal rate[88.5％(54/61)vs.82.0％(50/61),x2=1.043,P=0.307]showed no significant differences between the two groups.The incidence of postoperative complications in the PVB group was significantly lower than that in the general anesthesia group[6.6％(4/61)vs.19.7％(12/61),x2=4.604,P=0.032].Conclusion Ultrasound-guided PVB in PCNL maintains stable intraoperative vital signs,reduces operative time and bleeding,alleviates postoperative pain,reduces postoperative complications and postoperative hospital stay,and saves medical costs,having a good safety and efficacy.

Objective:To investigate the application value of antegrade splenic superior region dissection first in laparoscopic total gastrectomy of obesity gastric cancer.Methods:The retrospective and descriptive study was conducted. The clinicopathological data of 21 obesity patients with gastric cancer who underwent laparoscopic total gastrectomy in Renji Hospital of Shanghai Jiaotong University School of Medicine from July 2018 to October 2023 were collected. There were 16 males and 5 females, aged (58±13)years. All 21 patients underwent laparoscopic total gastrec-tomy with antegrade splenic superior region dissection first. Observation indicators: operation time, volume of intraoperative blood loss, laparotomy conversion, intraoperative splenic hemorrhage or gastric hemorrhage, lymph node dissection, time to postoperative first flatus, time to postoperative initial liquid food intake, duration of postoperative hospital stay, postoperative complication. Measure-ment data with normal distribution were represented as Mean± SD, and count data were expressed as absolute numbers. Results:All 21 patients underwent laparoscopic total gastrectomy success-fully, with the operation time of (283±47)minutes, time for splenogastric ligament and vascular manage-ment of (34±12)minutes, volume of intraoperative blood loss of (143±86)mL, and no laparotomy conversion. There was no intraoperative splenic hemorrhage or gastric haemorrhage. The total number of lymph node dissected in 21 patients was 375, with the number of lymph node dissected as (21±9)per case. Time to postoperative first flatus, time to postoperative initial liquid food intake and duration of postoperative hospital stay in 21 patients were (3.1±0.7)days, (4.0±0.8)days and (10.1±3.0)days, respectively. There were 2 patients with postoperative complications, including 1 case of incision infection and 1 case of lung infection. The 2 patients with postoperative com-plications were recovered and discharged after conservative treatment. There was no death during the postoperative 30 days.Conclusion:The application of antegrade splenic superior region dissec-tion first in laparoscopic total gastrectomy is safe and feasible, which can reduce surgical difficulty.

Objective:To explore the application effect of objectives and key results (OKR) management method in the clinical intensive training of emergency department nurses.Methods:A total of 90 nurses from the emergency department of The First Affiliated Hospital of Hebei North University who participated in clinical intensive training from September to December 2022 were selected. They were randomly divided into a control group and an observation group (45 in each group) using a random number table. The observation group was managed using OKR method, while the control group was managed using the conventional management method. The academic performance (theoretical knowledge and practical operation), clinical comprehensive ability (nurse core competency evaluation form), self-learning ability (Self-Rating Scale of Self-Directed Learning), and training satisfaction were compared between two groups of nurses at the end of each month (for a total of 3 months).Results:The observation group had significantly higher theoretical knowledge scores [(65.78±5.11), (77.89±4.22), (85.10±4.70)] and practical operation scores [(62.33±4.01), (72.45±4.29), (83.20±5.10)] than the control group [(61.99±6.09), (74.18±4.09), (82.60±5.40); (60.09±4.56), (70.24±4.11), (80.70±4.60)]. The clinical comprehensive ability (nursing ability and critical clinical thinking), self-learning ability (learning behavior, learning awareness, learning strategies, and learning evaluation), and training satisfaction (theoretical knowledge training, clinical skills training, and work plan) of nurses in the observation group were also higher than those of nurses in the control group.Conclusions:The application of OKR management method in clinical intensive training of emergency department nurses can significantly improve their clinical abilities and knowledge level, mobilize their self-learning ability, and achieve higher satisfaction.

Objective To explore the value of artificial intelligence federated learning system based on chest X-ray films for pathogen diagnosis of community-acquired pneumonia(CAP)in children.Methods Totally 900 cases of CAP children from 2 hospitals were retrospectively enrolled,including bacterial,viral and mycoplasma CAP(each n=300),and chest posteroanterior X-ray films were collected.Meanwhile,chest posteroanterior X-ray films of 5856 children from the publicly available dataset GWCMCx were collected,including 4273 CAP images and 1583 healthy chest images.All above 6756 images were divided into training set(n=5359)and validation set(n=1397)at the ratio of 8∶2.Then a pathogen diagnosis model of children CAP was established based on attention mechanism.Binary and ternary diagnostic algorithms were designed,and federated deployment training was performed.The efficacy of this system for pathogen diagnosis of children CAP was analyzed and compared with DenseNet model.Results Based on all data,the accuracy of the obtained artificial intelligence federated learning system model for diagnosing children CAP was 97.00%,with the area under the curve(AUC)of 0.990.Based on hospital data,the AUC of this system using single imaging data and clinical-imaging data for pathogen diagnosis of children CAP was 0.858 and 0.836,respectively,both better than that of DenseNet model(0.740,both P＜0.05).Conclusion The artificial intelligence federated learning system based on chest X-ray films could be used for pathogen diagnosis of children CAP.
