1.Prediction of neurological function rehabilitation outcome for stroke patients using interpretable machine learning models

ObjectiveTo develop a machine learning (ML)-based prediction model for neurological rehabilitation outcomes of stroke patients. MethodsA total of 420 stroke patients admitted to the Fuzhou First People's Hospital from October, 2022 to October, 2024 were enrolled as the training set. According to the modified Rankin Scale (mRS) scores three months after discharge, the patients were divided into prognosis group (n = 289) and poor prognosis group (n = 131). An additional 180 stroke patients hospitalized in the same hospital from November, 2024 to April, 2025 were selected as the validation set. Univariate analysis, least absolute shrinkage and selection operator regression, and multivariate logistic regression were performed to identify independent influencing factors for the prognosis of neurological function recovery. Using the screened independent influencing factors as feature variables, six ML models were established, including logistic regression, linear discriminant analysis, naive Bayes, support vector machine, random forest and extreme gradient boosting (XGBoost). The area under the receiver operating characteristic curve (AUC), confusion matrix indicators (accuracy, precision, recall and F1-score), calibration curve and decision curve analysis were adopted to evaluate the predictive efficacy, calibration degree and clinical net benefit of each model, with external validation conducted in the validation set. The SHapley Additive exPlanations framework was used to interpret the optimal model, and bar charts were applied to visualize the feature importance of the best model. ResultsAge, National Institutes of Health Stroke Scale (NIHSS) score, collateral circulation grading, fasting plasma glucose (FPG), lymphocyte percentage (LYMPH%), and homocysteine (Hcy) were independent risk factors for poor neurological rehabilitation prognosis (P < 0.05). For the XGBoost model, the AUC of the training and validation sets were 0.963 (95%CI 0.947 to 0.979) and 0.825 (95%CI 0.764 to 0.885), respectively, while the accuracy was 88.81% and 77.22%, the precision was 92.86% and 68.42%, the recall was 69.47% and 47.27%, and the F1-score was 79.48% and 55.91%, optimal in both calibration and clinical net benefit. The feature importance ranking for the XGBoost model from high to low was NIHSS score, age, collateral circulation grading, FPG, Hcy and LYMPH%. ConclusionThe interpretable XGBoost ML model exhibits excellent predictive efficacy and favorable clinical applicability in predicting neurological rehabilitation outcomes for stroke patients.
2.Efficacy and safety of surgery-assisted transjugular intrahepatic portosystemic shunt in treatment of portal hypertension comorbid with complex portal vein thrombosis

ObjectiveTo investigate the feasibility, safety, and efficacy of surgery-assisted transjugular intrahepatic portosystemic shunt (SA-TIPS) in the treatment of portal hypertension comorbid with complex portal vein thrombosis, including cavernous transformation of the portal vein (CTPV). MethodsAn analysis was performed for the data of 36 patients with portal hypertension and complex portal vein thrombosis who underwent SA-TIPS in Beijing Shijitan Hospital, Capital Medical University, from November 2023 to January 2025, including general status, technical data of the surgical process (surgical success rate, puncture times, time of operation, the number of stents used, and the length of shunt), perioperative complications, and surgical recovery. The change in portal pressure gradient (PPG) after shunt was compared, and the rate of reaching the standard for PPG reduction was calculated, as well as stent patency rate within 1 week after surgery. The paired samples t-test was used for comparison of continuous data between two groups. ResultsAmong the 36 patients, 34 (94.4%) underwent SA-TIPS successfully. The incidence rate of perioperative complications was 16.7% (6/36), including 3 cases of thoraco-abdominal hemorrhage, 2 cases of intraoperative arrhythmia, and 1 case of incision infection. There was a significant reduction in PPG after SA-TIPS (t=19.85, P<0.01), and the patients achieving a ≥50% reduction in PPG accounted for 76.5% (26/34). Imaging reexamination within 1 week showed a shunt patency rate of 100%. ConclusionSA-TIPS has a high technical success rate, a favorable safety profile, and good efficacy in the treatment of portal hypertension comorbid with complex portal vein thrombosis (including CTPV), and therefore, it holds promise for clinical application.
3.Chaihu Guizhi Ganjiangtang and Its Single Active Ingredient in Treatment of Dyspepsia Caused by Chronic Cholecystitis: A Review

Chaihu Guizhi Ganjiangtang was first recorded in the Treatise on Cold Damage (Shang Han Lun). This prescription is composed of Bupleuri Radix, Scutellariae Radix, Cinnamomi Ramulus, Zingiberis Rhizoma, Trichosanthis Radix, Ostreae Concha, and Glycyrrhizae Radix et Rhizoma. It has the effects of soothing Lesser Yang, warming the spleen, and stimulating the generation of body fluid. It is mainly used to treat digestive tract diseases such as chronic cholecystitis (CC), irritable bowel syndrome, and non-alcoholic fatty liver disease. Dyspepsia caused by CC presents a variety of gastrointestinal symptoms such as abdominal pain, poor appetite, postprandial fullness, aversion to greasy food, soft stool, and bitter mouth, being a type of biliary dyspepsia. In modern medicine, dyspepsia caused by CC is mainly managed by medical treatment and surgical treatment. Internal medicine mainly focuses on reducing inflammation, promoting the function of gallbladder, resolving stones, alleviating spasms, and relieving the pain for CC, demonstrating definite short-term efficacy but suffering from single effects, high recurrence rate, and poor compliance. Although surgical treatment can cure cholecystitis, it is accompanied by the increased incidence of adverse events such as abdominal pain, diarrhea, and dyspepsia. Modern clinical studies have confirmed that Chaihu Guizhi Ganjiangtang can significantly alleviate the symptoms such as abdominal pain and dyspepsia of CC patients. Pharmacological studies have found that Chaihu Guizhi Ganjiangtang mainly contains active ingredients such as Bupleuri Radix saponins, baicalin, cinnamaldehyde, gingerol, Trichosanthis Radix polysaccharide, Ostreae Concha polysaccharide, and Glycyrrhizae Radix et Rhizoma total flavonoids. Chaihu Guizhi Ganjiangtang can ameliorate the symptoms of dyspepsia caused by CC by inhibiting inflammatory responses, improving gallbladder contraction and gastrointestinal motility, regulating the bile acid-intestinal flora axis and the brain-gut axis, and modulating blood lipids through multiple targets. By reviewing the previous literature, this article summarizes the research progress in the treatment of dyspepsia caused by CC with Chaihu Guizhi Ganjiangtang and its main active ingredients as well as the pathogenesis of this disease and puts forward the shortcomings and improvement strategies for the current research. The review aims to provide a reference for the further research on Chaihu Guizhi Ganjiangtang in the treatment of dyspepsia caused by CC.
4.Research progress on the mechanisms linking and intervention of adolescent depression and lipid metabolism disorders

In recent years, the incidence of depression among adolescents has increased significantly, becoming a serious global public health problem. There is a close relationship between adolescent depression and lipid metabolism abnormalities. However, the mechanisms underlying this association have not been fully elucidated. This article provides a review of the mechanisms and interventions linking adolescent depression and lipid metabolism abnormalities, aiming to offer new ideas for interventions in adolescent depression. Using computerized searches of the China National Knowledge Infrastructure (CNKI), Wanfang Data, PubMed, and Web of Science, studies on the mechanisms and interventions related to adolescent depression and lipid metabolism were collected, with the search period extending up to October 10, 2025. Two researchers independently conducted literature screening and extracted basic information from the included studies. A total of 12 original studies were included. The results indicate that adolescent depression is associated with certain lipid metabolites. Neurobiological factors, immune inflammation, oxidative stress, and gut microbiota imbalance play important roles in this association. In addition, lipid metabolites such as omega-3 polyunsaturated fatty acids (ω-3 PUFAs) have a significant ameliorating effect on adolescent depression. 5.Clinical features and prognostic analysis of primary bladder adenocarcinoma

Objective:To explore the clinical characteristics and prognostic factors of primary bladder adenocarcinoma(ACB).Methods:The clinical data of 33 patients with primary ACB who underwent surgical treatment in the Department of Urology of Peking University Third Hospital from July 2003 to January 2024 were retrospectively analyzed. There were 5 females and 28 males,with an average age of(61.3 ± 11.5)years. Twelve patients had comorbidities(6 hypertension,2 coronary heart disease,5 diabetes mellitus,and 3 cerebrovascular diseases)and 12 had a smoking history. The mean body mass index(BMI)was(24.8 ± 3.2)kg/m 2. The maximum tumor diameter measured by enhanced computed tomography(CT)was(29.7 ± 12.7)mm. The preoperative neutrophil-to-lymphocyte ratio(NLR)was 3.4 ± 3.2,and the systemic immune-inflammation index(SII)was(582 ± 496)× 10 9/L.Patients were divided into two groups according to the surgical approach:the radical cystectomy group( n = 23)and the bladder-sparing group( n = 10). For the radical cystectomy group,there were 19 males and 4 females,with a mean age of(59.9 ± 12.6)years. Five patients had comorbidities(3 hypertension,1 coronary heart disease,1 diabetes mellitus,and 2 cerebrovascular diseases). Eight patients had a smoking history,with a mean BMI of(25.2 ± 3.5)kg/m 2. The maximum tumor diameter was(33.6 ± 10.9)mm,the preoperative NLR was 3.5 ± 3.5,and the SII was(618 ± 558)× 10 9/L. For the bladder-sparing group,there were 9 males and 1 female,with a mean age of(64.5 ± 8.2)years. Seven patients had comorbidities(3 hypertension,1 coronary heart disease,4 diabetes mellitus,and 1 cerebrovascular diseases). Four patients had a smoking history,with a mean BMI of(23.5 ± 2.3)kg/m 2. The maximum tumor diameter was(20.7 ± 12.5)mm,the preoperative NLR was 3.1 ± 2.2,and the SII was(501 ± 323)× 10 9/L. Statistically significant differences were observed between the two groups in terms of comorbidities( P = 0.008)and maximum tumor diameter( P = 0.006),while no significant differences were found in other data( P > 0.05). The Kaplan-Meier survival curve was drawn,and Cox regression was used to analyze the prognostic factors of progression-free survival(PFS)and overall survival(OS)of patients. Results:Among the 33 patients,low-grade adenocarcinoma and high-grade adenocarcinoma accounted for 60.6% and 39.4% respectively according to the postoperative pathology,and 3 patients had positive surgical margins. There were 22 cases of muscle-invasive bladder adenocarcinoma,5 cases of lymph node metastasis,and 1 case of distant metastasis. The patients in tumor stages Ⅰ—Ⅳ were 9 cases(27.3%),8 cases(24.2%),7 cases(21.2%),and 9 cases(27.3%)respectively. Nine patients received postoperative adjuvant therapy,including 6 with adjuvant chemotherapy,2 with adjuvant chemotherapy combined with radiotherapy,and 1 with adjuvant immunotherapy. In the radical cystectomy group( n = 23),there were 13 cases of low-grade and 10 cases of high-grade pathological grading,2 cases with positive margins,19 cases of muscle-invasive bladder adenocarcinoma,5 cases of lymph node metastasis,1 case of distant metastasis,and 5 patients received adjuvant therapy(4 cases of adjuvant chemotherapy,and 1 case of adjuvant immunotherapy). In the bladder-sparing group( n = 10),there were 7 cases of low-grade,3 cases of high-grade pathological grading,1 case with positive margins,3 cases of muscle-invasive bladder adenocarcinoma,zero lymph node or distant metastasis,and 4 patients received adjuvant therapy(2 cases of adjuvant chemotherapy,and 2 cases of combined adjuvant chemotherapy and radiotherapy). A statistically significant difference was found in the proportion of muscle-invasive bladder adenocarcinoma between the two groups( P = 0.006),while no significant differences were observed in other data( P > 0.05).The median follow-up duration of the patients was 28.0 months,the median PFS was 86.0 months,and the median OS was 90.0 months. The 2-year PFS and OS were 65.4% and 73.1% respectively. The 5-year PFS and OS were 54.2% and 56.5% respectively. The Kaplan-Meier survival analysis showed that there were no significant differences in PFS( P = 0.777)and OS( P = 0.585)between the radical cystectomy group and the bladder-preserving group. Female( P = 0.011),BMI < 25 kg/m2( P = 0.038),and positive surgical margins( P < 0.01)were associated with poorer PFS. Aged ≥ 70 years( P = 0.003),lymph node metastasis( P = 0.041),and positive surgical margins( P = 0.025)were associated with poorer OS,and patients in the adjuvant therapy group had better OS( P = 0.005). Multivariate Cox regression analysis indicated that positive surgical margins(HR 10.2, P = 0.012)were an independent impact factor for PFS,and positive surgical margins(HR 39.8, P = 0.001)and adjuvant therapy(HR 0.12, P = 0.021)were independent impact factors for OS. Conclusions:Positive surgical margins and adjuvant therapy are independent impact factors for the prognosis of patients with primary ACB.
6.DEB-BACE versus BACE for the treatment of advanced lung squamous cell carcinoma:a retrospective clinical study

Objective To investigate the effectiveness and safety of drug-eluting beads bronchial arterial chemoembolization(DEB-BACE)versus BACE for the treatment of stage Ⅲ-Ⅳ lung squamous cell carcinoma.Methods A total of 104 patients with stage Ⅲ-Ⅳ lung squamous cell carcinoma,who were admitted to the Lishui Municipal Central Hospital of China between January 2013 and August 2021,were enrolled in this study.According to the therapeutic scheme,the patients were divided into DEB-BACE group(n=41)and BACE group(n=63).For patients of DEB-BACE group,Cisplatin at 75 mg/m2 dose and gemcitabine at 1 000 mg/m2 dose(400 mg was used as loaded-drug dose)were injected through a microcatheter,which was followed by embolization with CalliSpheres microspheres loaded with 400 mg of gemcitabine.For patients of BACE group,Cisplatin at 75 mg/m2 and gemcitabatin at 1 000 mg/m2 were injected through a microcatheter,which was followed by arterial embolization with blank microspheres.Three weeks after DEB-BACE or BACE,the patients of both groups were started on intravenous chemotherapy.The primary study endpoint was overall survival(OS).The secondary study endpoints included progression-free survival(PFS),objective response rate(ORR),disease control rate(DCR),adverse reactions,and the remission rate of dyspnea.Results Of the 104 patients,63 received BACE sequential intravenous chemotherapy and 41 received DEB-BACE sequential intravenous chemotherapy.The median OS in DEB-BACE group was 23.0 moths,which was obviously longer than 12.0 months in BACE group(P=0.009).Multivariate Cox regression analysis showed that DEB-BACE treatment was an independent risk factor for OS(HR=0.59,95% CI:0.38-0.91,Log-rank test P=0.018).Meanwhile,the remission rate of dyspnea in DEB-BACE group was significantly higher than that in BACE group(57.1% vs 30.6%,P<0.043).Conclusion Compared with BACE sequential intravenous chemotherapy,DEB-BACE sequential intravenous chemotherapy can significantly prolong the survival time of patients with stage Ⅲ-Ⅳ lung squamous cell carcinoma and significantly improve the symptoms of dyspnea,which has important applications in the treatment of patients with advanced lung squamous cell carcinoma.
7.Construction and validation of a risk prediction model for clinical characteristics of patients with chronic non-bacterial prostatitis

Objective To investigate the clinical characteristics of patients with chronic abacterial pros-tafitis(CAP),the CAP related factors were analyzed,and a risk prediction model for CAP were constructed and validated.Methods The clinical dataes of 252 suspected CAP patients admitted to the hospital from June 2022 to December 2024 were collected,the patients were divided into modeling set(ni=177)and validation set(n=75)by 7∶3 ratio.Based on the modeling set dataes,the Lasso was used to screen CAP related predictive factors,a logistic multiple factor model was used to analyze the independent influence factors of CAP and a risk prediction model was constructed.The validation set patient dataes were used to plot ROC and DCA and validate the predic-tion model.Results There were 86 cases of CAP in the modeling set,accounting for 48.59%;32 cases of CAP in the validation set,accounting for 42.67%.The Logistic multiple regression analysis showed that BMI,waist to hip ratio,abnormal elevation of IL-8,COX-2,and PGE2 in prostate fluid were independent influence factors of CAP(P<0.05),a Nomogram column chart based on this was established.The ROC analysis showed that the sensitivity of the model for detecting CAP in the modeling and validation sets were 0.814 and 0.802,respectively,and the specificity were 0.673 and 0.703,respectively.The DCA analysis showed that the net benefit thresholds for model-ing and validation sets by column charts are 0.1~0.9 and 0.2~1.0,respectively.Conclusions The occurrence of CAP is related to the patient's BMI,waist to hip ratio,the levels of IL-8,COX-2,and PGE2 in prostate fluid.The predictive model established based on this is highly accurate and it can help for CAP screening.
8.A single-blind randomized controlled trial of effects of exergaming on cognitive function and negative symptoms in schizophrenia

Objective:To investigate the effects of exergaming on the cognitive functions and negative symp-toms in patients with schizophrenia.Methods:Sixty hospitalized patients meeting ICD-10 diagnostic criteria for schizophrenia were randomly assigned to the exergaming group(n=30)and the conventional treatment group(n=30).The exergaming group received structured exergaming therapy in addition to conventional treatment.Cognitive function was assessed with the MATRICS Consensus Cognitive Battery(MCCB)and Mini-Mental State Examina-tion(MMSE),while positive and negative symptoms were evaluated with the Scale for the Assessment of Positive Symptoms(SAPS)and Scale for the Assessment of Negative Symptoms(SANS).Results:Both groups showed significant improvements in the MCCB scores(Trail Making Test,Symbol Coding,Animal Naming,Maze,and Learning Tests)and MMSE scores post-intervention(time main effect,group main effect,and time × group inter-action,Ps<0.05).The exergaming group showed significantly greater improvements in most cognitive domains compared to the control group.The SANS scores decreased significantly in both groups,with a greater reduction in the exergaming group(P<0.05).The SAPS scores also declined,but no significant between-group differences were observed(P>0.05).Conclusion:Exergaming could effectively improve the cognitive deficits and negative symptoms in patients with schizophrenia.
9.Partial nephrectomy for renal cell carcinoma in a transplanted kidney

Renal cell carcinoma in a transplanted kidney is rare, and partial nephrectomy in transplanted kidney is even more uncommon. A patient with papillary transplanted renal cell carcinoma who underwent partial nephrectomy was reported. The patient was admitted to the hospital due to the discovery of transplanted kidney mass, and the enhanced examination of CTU found a lesion in the upper pole of the transplanted kidney in the right iliac fossa, which may be renal cancer, with a diameter of about 2 cm. After open partial nephrectomy of the transplanted kidney, the postoperative pathology showed that the tumor was papillary renal cell carcinoma. The antirejection drugs were used after surgery. During the follow-up period of 16 months, there was no recurrence of transplanted kidney tumor.

In the bladder-sparing group( n = 10),there were 7 cases of low-grade,3 cases of high-grade pathological grading,1 case with positive margins,3 cases of muscle-invasive bladder adenocarcinoma,zero lymph node or distant metastasis,and 4 patients received adjuvant therapy(2 cases of adjuvant chemotherapy,and 2 cases of combined adjuvant chemotherapy and radiotherapy). A statistically significant difference was found in the proportion of muscle-invasive bladder adenocarcinoma between the two groups( P = 0.006),while no significant differences were observed in other data( P > 0.05).The median follow-up duration of the patients was 28.0 months,the median PFS was 86.0 months,and the median OS was 90.0 months. The 2-year PFS and OS were 65.4% and 73.1% respectively. The 5-year PFS and OS were 54.2% and 56.5% respectively. The Kaplan-Meier survival analysis showed that there were no significant differences in PFS( P = 0.777)and OS( P = 0.585)between the radical cystectomy group and the bladder-preserving group. Female( P = 0.011),BMI < 25 kg/m2( P = 0.038),and positive surgical margins( P < 0.01)were associated with poorer PFS. Aged ≥ 70 years( P = 0.003),lymph node metastasis( P = 0.041),and positive surgical margins( P = 0.025)were associated with poorer OS,and patients in the adjuvant therapy group had better OS( P = 0.005). Multivariate Cox regression analysis indicated that positive surgical margins(HR 10.2, P = 0.012)were an independent impact factor for PFS,and positive surgical margins(HR 39.8, P = 0.001)and adjuvant therapy(HR 0.12, P = 0.021)were independent impact factors for OS. Conclusions:Positive surgical margins and adjuvant therapy are independent impact factors for the prognosis of patients with primary ACB.
