1.Efficacy and safety of surgery-assisted transjugular intrahepatic portosystemic shunt in treatment of portal hypertension comorbid with complex portal vein thrombosis

ObjectiveTo investigate the feasibility, safety, and efficacy of surgery-assisted transjugular intrahepatic portosystemic shunt (SA-TIPS) in the treatment of portal hypertension comorbid with complex portal vein thrombosis, including cavernous transformation of the portal vein (CTPV). MethodsAn analysis was performed for the data of 36 patients with portal hypertension and complex portal vein thrombosis who underwent SA-TIPS in Beijing Shijitan Hospital, Capital Medical University, from November 2023 to January 2025, including general status, technical data of the surgical process (surgical success rate, puncture times, time of operation, the number of stents used, and the length of shunt), perioperative complications, and surgical recovery. The change in portal pressure gradient (PPG) after shunt was compared, and the rate of reaching the standard for PPG reduction was calculated, as well as stent patency rate within 1 week after surgery. The paired samples t-test was used for comparison of continuous data between two groups. ResultsAmong the 36 patients, 34 (94.4%) underwent SA-TIPS successfully. The incidence rate of perioperative complications was 16.7% (6/36), including 3 cases of thoraco-abdominal hemorrhage, 2 cases of intraoperative arrhythmia, and 1 case of incision infection. There was a significant reduction in PPG after SA-TIPS (t=19.85, P<0.01), and the patients achieving a ≥50% reduction in PPG accounted for 76.5% (26/34). Imaging reexamination within 1 week showed a shunt patency rate of 100%. ConclusionSA-TIPS has a high technical success rate, a favorable safety profile, and good efficacy in the treatment of portal hypertension comorbid with complex portal vein thrombosis (including CTPV), and therefore, it holds promise for clinical application.

Objective Exploring the behavioral changes induced by quinpirole in obsessive-compulsive disorder(OCD)mouse,investigating the activation of neurons in different brain regions,and identifying differentially expressed genes(DEGs)and enriched biological pathways through transcriptome sequencing technology to elucidate the pathogenesis of OCD.Methods Randomly assign 32 male C57BL/6J mice,aged two months,to an OCD group and a control group(n=16).Administering quinpirole(0.75 mg/kg)via subcutaneous injection to the OCD group mice every other day for a total of 19 injections,while the control group mice received an equivalent volume of saline solution.Following the completion of the model construction,open field testing,elevated plus maze testing,and marble burying tests were conducted.After the completion of behavioral studies,tissue samples were collected.Neuronal damage was assessed using Nissl staining,while the expression of c-Fos and Iba1 proteins was examined through immunofluorescence staining.Transcriptome sequencing technology was utilized to screen for differentially expressed genes and to enrich relevant signaling pathways.The expression of inflammatory cytokines,including TNF-α,NF-κB p65,phosphorylated NF-κB p65(p-NF-κB p65),and IL-6,was detected using Western Blot analysis.Results Mouse induced with OCD by quinpirole exhibit anxiety-like behaviors and compulsive-like behaviors.Neurons in the hippocampal and hypothalamic regions exhibit signs of damage.The expression of c-Fos and Iba1 proteins is increased in the cortex,striatum,hypothalamus,and other brain regions.Western Blot result indicate a significant increase in the expression of pro-inflammatory cytokines such as TNF-α,p-NF-κB p65,and IL-6.Conclusions In OCD mouse,neurons in multiple brain regions are abnormally activated,microglia exhibit dysfunction,and neuroinflammation induced by the activation of the NF-κB signaling pathway accompanies the development of OCD.

4.Study on the predictive value of ICH-LR2S2 score and ICH-APS score for stroke-associated pneumonia in patients with spontaneous intracerebral hemorrhage

Objective To study the predictive value of the Prognostic Assessment Scale for Patients with Spontaneous Intracerebral Hemorrhage(ICH-LR2S2)and the Prehospital Risk Assessment Scale for Prehospital Deterioration Risk Assessment Scale(ICH-APS)for the development of stroke-associated pneu-monia(SAP)in patients with spontaneous intracerebral hemorrhage.Methods A total of 349 patients with spontaneous intracerebral hemorrhage who were hospitalized for the first time in this hospital from July 2023 to July 2024 were selected as the research subjects.The general demographic data and medical documentations of the patients were collected,and ICH-LR2S2 score and ICH-APS score were carried out within 48 hours after admission.According to whether pneumonia occurred within 7 days after admission,the patients were divided into the SAP group and the non-SAP group,and the diagnostic efficiency of the ICH-LR2S2 score and ICH-APS score for SAP in patients with spontaneous intracerebral hemorrhage was evaluated.Results Among the 349 patients with spontaneous intracerebral hemorrhage,98 patients(28.08%)had pneumonia.The results of multivariate logistic regression analysis showed that age,history of chronic obstructive pulmonary disease(COPD),nasogastric tube,tracheal intubation,National Institutes of Health Stroke Scale(NIHSS)score,Glasgow Coma Scale(GCS)score,C-reactive protein,fasting blood glucose,dysphagia,ICH-LR2S2 score,ICH-APS-A score,and ICH-APS-B score were independent influencing factors for SAP in patients with spontane-ous intracerebral hemorrhage(P<0.05).The results of the receiver operating characteristic(ROC)curve showed that the ICH-LR2S2 score had the highest diagnostic efficiency for SAP in patients with spontaneous intracerebral hemorrhage,with an area under the curve(AUC)of 0.837,a sensitivity of 0.827,a specificity of 0.783,and a Youden index of 0.610.Conclusion ICH-LR2S2 score has a high predictive value for the occur-rence of SAP in patients with spontaneous intracerebral hemorrhage.
5.Probucol combined with tirofiban for angina pectoris in patients with coronary heart disease after interventional surgery

Objective To discuss the effect of probucol combined with tirofiban treatment on serum lipid peroxide levels and vascular endothelial function in patients with angina pectoris after interventional surgery of coronary heart disease(CHD).Methods A total of 120 patients with angina pectoris occurring after interventional surgery from January 2018 to December 2022 at the People's Hospital of Inner Mongolia Autonomous Region of China were enrolled in this study.Using random digital table method,the patients were divided into observation group(n=60)and control group(n=60).The patients of the control group received tirofiban,while the patients of the observation group received tirofiban plus probucol.All the preoperative and postoperative 7-day relevant indicators were recorded.The main observation indicator was the overall effective rate,and the secondary observation indicators included the frequency and duration of angina attacks,Seattle Angina Questionnaire(SAQ)score,cardiac function,myocardial injury markers,serum lipid peroxide levels,endothelial function and adverse reactions.Results The total effective rate in the observation group was 91.67%(55/60),which was higher than 78.33%(47/60)in the control group,the difference was statistically significant(P<0.05).After treatment,the improvement degree of angina pectoris,SAQ score,LVEF,serum superoxide dismutase(SOD),nitric oxide(NO)in the observation group were higher than those in the control group,while the LVESD,LVEDD,serum creatine kinase isozyme(CK-MB),lactate dehydrogenase(LDH),creatine kinase(CK),malondialdehyde(MDA),endothelin-1(ET-1)in the observation group were lower than those in the control group,the differences in the above indexes were statistically significant(all P<0.05).Conclusion For patients with angina pectoris occurring after interventional surgery of CHD,probucol combined with tirofiban has exact efficacy,which can promote the recovery of cardiac function,reduce myocardial injury,regulate serum lipid peroxide levels,and improve vascular endothelial function.
6.Research progress on the role of common vitamins and iron,copper,and zinc in the treatment of Alzheimer's disease

Alzheimer's disease(AD)is one of the most prevalent neurodegenerative disorders,and is increasingly becoming a global health challenge,in line with the intensification of population aging.The pathogenesis of AD is currently unclear and treatment options are limited.Currently available drugs can only alleviate the symptoms of AD to a certain extent,and cannot delay its progression.Numerous studies,however,have shown that a variety of micronutrients may play a positive role in the treatment of AD.Vitamins and trace metal elements have many functions,including anti-inflammation,anti-oxidation,and protection of mitochondria.Micronutrients can effectively reduce β-amyloid protein plaque deposition and Tau protein hyperphosphorylation to improve brain energy metabolism,thereby helping to prevent and treat AD.This article reviews and summarizes the roles of common vitamins and trace metal elements in the prevention and treatment of AD,with the goal of offering new perspectives for the clinical management and prevention of this disease.

8.Serum myostatin and follistatin as biomarkers of sarcopenia in elderly women

Objective:This study aimed to explore the clinical value of myostatin(MST) and follistatin(FST) as biological biomarkers in evaluating sarcopenia in elderly women.Methods:This was a retrospective cross-sectional study that enrolled 350 females aged 20-89 years who underwent physical examinations in Shanghai Huadong Hospital in 2021. Demographic characteristics, muscle mass, fat mass, bone mineral density, hand grip strength, gait speed, and serum indices of MST and FST were collected.Results:The serum levels of MST did not change significantly with age. However, the serum levels of FST increased with age. In women aged≥60 years, MST was positively correlated with total lean mass and appendicular skeletal muscle index(ASMI; r=0.236, P=0.041; r=0.289, P=0.014), while FST was negatively correlated with ASMI( r=-0.265, P=0.030). In multivariate stepwise regression analysis, after adjusting for age, body mass index, hip bone mineral density, and total fat mass, only FST was independently correlated with ASMI( β=-0.238, P=0.006), while MST was not correlated with ASMI. The receiver operating characteristic curve was plotted using muscle mass reduction as the state variable and serum FST level as the test variable. The area under the curve was 0.753. And when the FST cutoff value was 17.49 ng/mL, the maximum Jordan index was 0.46, with a sensitivity of 77.3% and a specificity of 68.7%. Women aged ≥60 years were divided into three groups based on serum FST levels. Compared to the upper third of the serum FST level group, the low third of the FST level group had a significantly reduced risk of suffering from sarcopenia( OR=0.098, P =0.036). Conclusions:Serum FST lever has a better correlation with muscle mass among elderly women, making it a promising biomarker for evaluating muscle mass.
9.Clinicopathological characteristics and prognosis of multilocular cystic renal neoplasm of low malignant potential

Objective:To analyze the clinicopathological characteristics and prognosis of patients with multilocular cystic renal neoplasm of low malignant potential and compare the clinicopathological charac-teristics of patients with multilocular cystic renal neoplasm of low malignant potential who underwent different surgical methods.Methods:Clinicopathological data and prognosis of patients admitted to Peking University Third Hospital from January 2010 to September 2023 were collected.Patients who underwent radical nephrectomy or nephron-sparing surgery and were pathologically diagnosed with multilocular cystic renal neoplasm of low malignant potential were identified.Based on the surgical methods,the patients were divided into radical nephrectomy group and nephron-sparing surgery group.The clinicopathological characteristics of the two groups were compared.Results:A total of 35 patients were enrolled in this study.The median age at diagnosis was 53.0(39.0-62.0)years.Among the 35 patients,23 were males(65.7%)and 12 were females(34.3%).Nine patients underwent radical nephrectomy(25.7%),while 26 patients underwent nephron-sparing surgery(74.3%).The clinical T-stage of 35 patients did not exceed T2a stage.The median operation time was 145.0 min,and the median estimated intraoperative blood loss was 20.0 mL.The median postoperative hospitalization days was 6.0 d.The postoperative pathological results did not indicate renal sinus invasion,sarcomatous change,adrenal invasion or lymph node invasion.Based on the surgical methods,the patients were divided into a radical nephrectomy group and a nephron-sparing surgery group.There was no significant difference in clinicopathological charac-teristics between the two groups.Except for one patient who was lost to the follow-up,all the other patients were followed up for 8-111 months,with a median follow-up time of 70.5 months.Only one patient died from non-cancer-specific reasons,other patients had no tumor metastasis or recurrence.Conclusion:Patients with multilocular cystic renal neoplasm of low malignant potential have a good prog-nosis.There is no significant difference in clinicopathological characteristics of patients between nephron-sparing surgery group and radical nephrectomy group for multilocular cystic renal neoplasm of low malig-nant potential.
10.Value of Sonazoid contrast-enhanced ultrasound for preoperatively evaluating pathological grade of hepatocellular carcinoma

Objective To analyze the correlation between the characteristics of Sonazoid contrast-enhanced ultrasound(CEUS)and pathological differentiation in hepatocellular carcinoma(HCC).Methods A total of 64 patients with HCC diagnosed pathologically by CEUS examination were included,and a total of 64 lesions were divided into the high,medium and low differentiation groups(6,48 and 10 cases,respectively)according to the degree of pathological differentiation.The enhancement pattern,enhancement level and enhancement pattern of CEUS arterial stage in HCC with different pathological differentiation were compared.Results The enhancement pattern of arterial phase was divided into the uniform enhancement and the uneven enhancement.All lesions in the low differentiated group and 58.3%in the middle differentiated group showed uneven and high enhancement.In the highly differentiated group,lesions showed homogenous hyperintensification,homogenous isointensification and non-homogenous hyperintensification.At arterial stage,all lesions in the middle and low differentiated groups and 66.7%lesions in the highly differentiated group showed high enhancement,and the enhancement levels of HCC with different differentiation degrees were significantly different(P<0.01).At the portal stage,16.7%,25.0%and 70.0%lesions in the high,medium and low differentiated HCC groups subsided to low enhancement,and the enhancement levels of HCC with different differentiation degrees were significantly different(P<0.05).In the delayed stage,75%lesions in the medium-differentiated group and all lesions in the low-differentiated group showed low enhancement,and 66.7%lesions in the highly differentiated group showed equal enhancement.Enhancement levels of HCC with different differentiation degrees were significantly different(P<0.01).At the Kupffer stage,all lesions in the low differentiated group and 95.8%of the moderately differentiated group showed low enhancement,while 50%lesions in the highly differentiated group still showed equal enhancement,and there were significant differences in the enhancement levels of HCC with different differentiation degrees(P<0.01).The highly differentiated group showed multiple CEUS patterns,the moderately differentiated group mainly showed"fast advance and fast retreat"and"fast advance and slow retreat"patterns,and 90.0%of the low differentiated group showed"fast advance and fast retreat"patterns.There were significant differences in CEUS patterns between HCC with different degrees of differentiation(P<0.01).Conclusion Sonazoid-CEUS has certain value in evaluating the differentiation degree of HCC.
