1.The relationship between coronary artery measurements combined with C-reactive protein, erythrocyte sedimentation rate and coronary artery lesionsin with incomplete Kawasaki disease in children

Objective:To explore the relationship between coronary artery measurements combined with C-reactive protein(CRP), erythrocyte sedimentation rate(ESR) and coronary artery lesionsin(CAL) with incomplete Kawasaki disease (IKD) in children.Methods:Sixty-eight cases of pediatric IKD admitted to Huaibei Maternal and Child Health Care Hospital from January 2020 to April 2024 were selected as the IKD group, and 34 healthy children undergoing physical examination during the same period were selected as the control group according to the principle of 2∶1 pairing. Echocardiographic Z values including left coronary artery (LCA)-Z, right coronary artery (RCA)-Z, left anterior descending branch (LAD)-Z, left circumflex branch (LCX)-Z and CRP, ESR were compared between the two groups. General information of children with or without CAL were compared. The multivariate Logistic regression was used to screen the risk factors of IKD combined with CAL, and receiver operating characteristic (ROC) curve was used to analyze the predictive value of each index to IKD combined with CAL.Results:The LCA-Z, RCA-Z, LAD-Z, LCX-Z and the levels of CRP, ESR in the IKD group were higher than those in the control group: 3.26 ± 0.97 vs. 1.35 ± 0.28, 3.46 ± 0.92 vs. 1.01 ± 0.29, 3.18 ± 0.69 vs. 1.18 ± 0.34, 2.97 ± 0.68 vs.1.27 ± 0.34, (39.97 ± 9.03) mg/L vs. (4.21 ± 1.05) mg/L, (59.67 ± 16.34) mm/1 h vs. (12.85 ± 2.43) mm/1 h, there were statistical differences ( P<0.05). The duration of fever, intravenous immunoglobulin (IVIG) treatment response, IVIG start time, creatine kinase (CK), creatine kinase-MB(CK-MB), and N-terminal pro-B-type natriuretic peptide (NT-proBNP) between the children with IKD and CAL, IKD without CAL: (10.02 ± 2.45) d vs. (7.68 ± 1.43) d, 65.22%(15/23) vs. 88.89%(40/45), 60.87%(14/23) vs. 86.67%(39/45), (236.78 ± 59.74) U/L vs. (192.67 ± 35.41) U/L, (45.19 ± 9.85) U/L vs. (33.18 ± 9.87) U/L, (1.78 ± 0.32) μg/L vs. (0.92 ± 0.20) μg/L, there were statistical differences ( P<0.05). Logistic regression analysis showed that the duration of fever, IVIG treatment response, IVIG initiation time, CK, CK-MB, NT-proBNP, echocardiographic Z value, CRP and ESR levels were independent risk factors for IKD combined with CAL ( P<0.05). ROC curve analysis results showed that the area under the curve of IKD combined with CAL predicted by LCA-Z, RCA-Z, LADE-Z, LCX-Z, CRP and ESR was the largest, which was 0.917 (95% CI 0.824 - 0.970), which was higher than that diagnosed by each index alone ( P<0.05). Conclusions:There is a certain relationship between the echocardiographic Z values, CRP and ESR in children with IKD and CAL, which can be used to assist in the evaluation of IKD complicated with CAL.

3.Comparative study of myocardial perfusion and prognosis in patients with acute myocardial infarction treated by ticagrelor or clopidogrel

Objective To compare the effects of ticagrelor and clopidogrel on platelet aggregation rate, myocardial perfusion and prognosis in patients with acute myocardial infarction. Methods One hundred and sixty-nine patients with acute ST segment elevation myocardial infarction (STEMI) and emergency percutaneous coronary intervention (PCI) were recruited and randomly divided into ticagrelor group (85 cases) and clopidogrel group (84 cases). The TIMI blood flow before and after PCI was recorded, and the ST segment fall rate of 1 h ECG after PCI was calculated. The platelet aggregation rate was measured. After 12 months' follow-up, the incidence of major adverse cardiac events (MACE) was recorded. The Logistic regression analysis was used to discover the factors of MACE. Results One hundred and sixty-nine patients with acute STEMI were recruited including 85 cases treated with ticagrelor and 84 cases in clopidogrel group. The ECG ST segment fall rate after PCI in ticagrelor group was significantly higher than that in clopidogrel group: (61.3 ± 30.7)% vs. (47.8 ± 26.6)%, P<0.05. The platelet aggregation rate 2 h, 24 h and 7 d after PCI in ticagrelor group was significantly lower than that in clopidogrel group (P<0.05). MACE occurred in 19 cases (22.4%) in ticagrelor group and in 21 cases (25.0%) in clopidogrel group, and there was no significant difference (P > 0.05). Multiple Logistic regression analysis revealed that age and ECG ST segment fall rate were independent risk factors for MACE (P<0.05). Conclusions Compared with clopidogrel, ticagrelor can effectively reduce platelet aggregation rate and accelerate ST segment fall in STEMI patients, but their long-term prognosis is similar.
4.Vitamin D3 regulates mononuclear phagocyte polarization induced by serum from patients with ankylosing spondylitis.

OBJECTIVE:
To investigate the effect of vitamin D3 on polarization of monocyte macrophages induced by serum from patients with ankylosing spondylitis (AS).
METHODS:
Twenty AS naïve patients and 20 healthy controls from Wenzhou People's Hospital during January 2016 and December 2017 were enrolled. The macrophages were differentiated from THP1 cells induced by phorbol 12-myristate 13-acetate (PMA), and then co-cultured with the serum from healthy subjects (control group) or AS patients. Vitamin D3 was added in the medium mixed with serum from AS patients. Flow cytometry was used to analyze the ratio of CD68 and CD206 positive cells, and RT-PCR was performed to detect the mRNA expression of inducible nitric oxide synthase(iNOS) and arginase-1(Arg-1).
RESULTS:
THP1 cells could be polarized into mononuclear-macrophages with the induction of PMA. The proportion of CD206 positive cells in AS-serum group was lower than that in the control group (=9.434, <0.05), while the proportion of CD68 positive cells was higher than that in the control group (=43.920, <0.05). The proportion of CD206 positive cells in vitamin D3 group was higher than that in AS-serum group (=8.895, <0.05), while the proportion of CD68 positive cells was lower than that in AS-serum group (=9.089, <0.05). mRNA expression of Arg-1 in AS-serum group was lower than that in the control group (=8.899, <0.05), while mRNA expression of iNOS was higher than that in the control group (=3.656, <0.05). mRNA expression of Arg-1 in vitamin D3 group was higher than that in AS-serum group (=6.219, <0.05), while mRNA expression of iNOS was lower than that in AS-serum group (=5.876, <0.05).
CONCLUSIONS
Vitamin D3 can regulate the polarization of mononuclear macrophages for immunoregulation in patients with AS.

5.Effect of febuxostat on epithelial-to-mesenchymal transition of kidney tubules, serum interleukin-6 and transforming growth factor β1 in hyperuricemic rats

Objective To observe the effect of febuxostat on epithelial-to-mesenchymal transition (EMT) of kidney tubules and the levels of serum IL-6 nad transforming growth factor (TGF) β1 in hyperuricemic rats.Methods Forty male SD rats were divided into 4 groups:normal control group (NC group),oteracil potassium group (OP group),oteracil potassium with febuxostat group (OF group) and oteracil potassium with benzbromarone group (OB group).Each group had 10 rats and balanced in body weights.To induce hyperuricemia,rats were given oteracil potassium by gastric garage once a day for eight weeks.Rats in OF group and OB group were given either febuxostat or benbromarone starting with oteracil potassium,and rats in NC group was given saline only.Blood samples were taken before,and at the end of 4 and 8 weeks of the treatments and serum uric acid,creatinine,blood usea nitrogen (BUN),IL-6 and TGFβ1 contents were measured at each time point.Renal pathological changes were observed via HE and Masson staining,and the expression of α-SMA and E-cadherin were detected by immunohistochemistry.Results Compared with those in NC group,the levels of serum uric acid,creatinine,BUN,IL-6 and TGFβ1 in the another three groups were increased significantly (all P < 0.01).However,the IL-6 and TGFβ1 contents in OF group were much lower than those in OP group (P <0.01).HE and Masson staining showed that OF group had less damage and tubulointerstitial fibrosis than OP group and OB group (P <0.01).Moreover,the expression of α-SMA was significantly down-regulated (P < 0.01) and that of E-cadherin was significantly up-regulated in OF group compared with those in OP group.Conclusion Febuxostat treatment significantly inhibited EMT and reduced the levels of IL-6 and TGFβ1 in hyperuricemia rats.
6.Effect of Tripterygium wilfordii combined with methotrexate on short-term effect of rheumatoid arthritis and the levels of IL-6, IL-10 and TNF- in peripheral blood

Objective To investigate the effects of tripterygium glycosides combined with methotrexate in treatment of rheumatoid arthritis (RA) curative effect and peripheral blood of patients with IL-6, effects of IL-10 and TNF- alpha. Methods 80 patients with RA from February 2014 to August 2016 in our hospital were retrospectively analyzed. According to the different treatment methods by for the observation group, the control group (40 cases). To compare and analyze the curative effect before and after short-term treatment and treatment of patients in the 2 groups were the main clinical symptoms and signs of change and ESR, RF, CRP, IL-6, TNF- alpha, IL-10 index. Results The total effective rate of observation group (92.50%) was significantly higher than the control group (77.50%), the difference was statistically significant (P<0.05), 2 groups of patients before treatment of main clinical symptoms and signs, there was no significant difference, after treatment, clinical symptoms and signs of 2 groups of patients were compared with those before treatment was statistically significant to alleviate the differences (P<0.05), and the observation group of main clinical symptoms and signs were lower than the control group and the remission (down), the difference was statistically significant (P<0.05).Before ESR, the treatment of 2 patients with RF, CRP, TNF- IL-6, alpha, IL-10 index level, no statistical significance, the 2 groups after treatment in patients with ESR, RF, CRP, IL-6, TNF- alpha, IL-10 index level than before treatment with significant difference (P<0.05), and the observation group ESR, RF, CRP, TNF- alpha, IL-6 average index of water compared with the control group decreased significantly, IL-10 index increased significantly than the control group. The difference was statistically significant (P<0.05). Conclusion Tripterygium glycosides combined with methotrexate in treatment of rheumatoid arthritis And it can effectively inhibit the expression of peripheral serum L-6 and TNF- alpha, promote the expression of IL-10 and alleviate the inflammatory reaction
7.A comparative study of nephroptosis in the second and final stage of medical selection of flying cadets

Objective To compare the physical examination results of nephroptosis in the second stage and the final stage, to find the underlying reasons, and to explore better ways to detect nephroptosis.Methods A total of 1334 subjects participated in both the second stage and final stage of PLA air force medical selection of flying cadets, and physical examination results of both stages were compared.Results All the subjects recruited denied a history of abdominal pain or recurrent urinary tract infection, and urine tests were normal.The positive rate of nephroptosis in the second stage was 10.2%, compared to 5.5% in the final stage.The difference of physical examination results in the two stages was significant, and so was that of disqualification rates.Conclusion Results of physical examination of nephroptosis in the second stage are different from those in the final stage.It′s necessary to find better ways of detection of nephroptosis.

Objective To investigate the distribution and changing resistance proifle ofSalmonella isolates in hospitals across China during the period from January 2005 to December 2014.Methods Seventeen general hospitals and two children’s hospitals were involved in this program. Antimicrobial susceptibility testing was carried out by means of a unified protocol using Kirby-Bauer method or MIC determination. The results were analyzed according to CLSI 2014 breakpoints.Results The proportion ofSalmonella isolates increased with time from 0.2% in 2005 to 0.7% in 2014. A total of 3 478Salmonella strains were collected from 19 hospitals. The proportion ofSalmonella typhimurium andSalmonella enteritidis was 27.4% and 24.4%, respectively. During the 10-year period, theSalmonella strains showed highest resistance rate to ampicillin (33.3%-64.8%), but low resistance to cefoperazone-sulbactam (0-5.3%) and ciprofloxacin (2.4%-14.3%).S. typhimurium showed higher resistance rate thanS. typhi,S. paratyphi andS. enteritidis. About 76.8% and 50.5% ofS. typhimurium were resistant to ampicillin and trimethoprim-sulfamethoxazole. The average prevalence of multi-drug resistantSalmonellawas 3.9% in the ten-year period, the highest (7.5%) was in 2005, the lowest (1.5%) in 2013.Conclusions During the period from 2004 to 2015, majority of theSalmonella isolates in hospitals across China wasS. typhimurium andS. enteritidis. Ampicillin and trimethoprim-sulfamethoxazole are no longer appropriate for empirical treatment ofS. typhimurium infection due to high resistance rate.Salmonella isolates are relatively more susceptible to third-generation cephalosporins and quinolones. Ongoing monitoring is necessary to identify multi-drug resistant strains ofSalmonella.
9.Clinical Effect of Holmium Laser Enucleation of the Prostate in the Treatment of 40 Patients with Benign Prostatic Hyperplasia

Objective:To evaluate the safety and clinical effect of holmium laser enucleation of the prostate (HoLEP) for pati-ents with benign prostatic hyperplasia (BPH) .Methods:A total of 40 patients with BPH from Jan 2015 to Jul 2015 were underwent HoLEP.Pre-and postoperative complication were recorded and the differences of the clinical parameters were also compared, including peak urinary flow rate (Qmax), residual urine volume (PVR), International Prostate Symptom Score (IPSS), quality-of-life (QOL) .Results:A total of 34 patients were included after operation and 6 patients lost to follow-up. Qmax, PVR, IPSS, QOL after three months postoperatively were improved significantly compared with that of preoperative (P<0.01) .Complications included intraoperative bladder injury (1 case), postoperative stress incontinence (2 cases), urethral stricture (1 case) .No patients experienced the sign of TUR syndrome and blood transfusion for severe bleeding.Conclusion:HoLEP has the advantage of good therapeutic effects,wide scope of application,better safety performance,smaller risk and it’s worth popularizing in primary hospitals.
10.An empirical study on medical selection of flying cadets with arrhythmia between PLAAF and USAF

Objective To determine whether the candidates who are disqualified because of arrhythmia during medical selection of flying cadets in China are qualified or not according to United States Air Force ( USAF ) Medical Standards Directory( MSD) , in order to raise suggestions on revising PLAAF medical selection of flying cadets.Methods ECG was performed on all candidates who participated in the final stage of medical selection of flying cadets, and they were considered qualified or not according to USAF MSD.Results Arrhythmia accounted for the largest proportion of cardiovascular diseases( CVDs) in PLAAF medical selection of flying cadets,the constituent ratios of CVDs from 2012 to 2015 were significant different (P<0.01).There were differences between the two countries and the outcome of 81.48%of the candidates disqualified for premature systole in PLAAF was unknown according to USAF MSD.Conclusion There are some differences in medical selection standards of flying cadets between PLAAF and USAF, and we could revise PLAAF standards with reference to USAF standards.
