Objective: To identify biomolecular markers closely related to the occurrence of kidney renal clear cell carcinoma (KIRC) and verify their expression levels in clinical samples. Methods: Stage Ⅰ KIRC mRNA sequencing data were obtained from The Cancer Genome Atlas (TCGA).Principal component analysis (PCA) was used for dimensionality reduction to screen differentially expressed genes (DEGs),which then underwent GO and KEGG analyses.Weighted gene co-expression network analysis (WGCNA) was used to screen genes significantly related to KIRC,and a protein-protein interaction (PPI) network was constructed to screen hub genes. The diagnostic value of hub genes was evaluated with receiver operating characteristic (ROC) curve, and their prognostic value was analyzed using survival curve plots. The correlation between the mRNA expressions of hub genes and the pathological stages of KIRC was analyzed.Clinical samples of 20 patients with stage Ⅰ KIRC treated in our hospital were included, and the expressions of the hub genes in cancerous and adjacent tissues were detected with reverse transcription real-time quantitative polymerase chain reaction (RT-qPCR),Western blotting, and enzyme-linked immunosorbent assay (ELISA). Results: A total of 8223 DEGs were screened out, including 4092 up-regulated ones and 4131 down-regulated ones.GO analysis showed that DEGs were related to bioadhesion, plasma membrane composition, and transporter activity.KEGG analysis showed that DEGs were related to pathways such as cell adhesion molecules,cytokine-cytokine receptor interactions, and interactions between viral proteins and cytokines and cytokine receptors.WGCNA analysis obtained 171 genes that were significantly related to stage Ⅰ KIRC.The hub gene, lymphocyte cytosolic protein 2 (LCP2),screened out by the PPI network, was significantly related to stage Ⅰ KIRC.The area under the ROC curve was 0.96.The expression level was negatively correlated with the overall survival rate of patients. The expression of LCP2 was related to the stage and lymph node metastasis.Clinical verification showed that the mRNA and protein relative expressions of LCP2 in KIRC tissues were significantly higher than those in adjacent tissues (P<0.000 1). Conclusion: LCP2 is significantly up-regulated in stage Ⅰ KIRC tissues and can be used as a potential biomarker for the early diagnosis and treatment of KIRC.

Objective To analyze the PLCβ4 gene mRNA expression and its clinical significance in hepatocellular carcinoma (HCC) based on TCGA database. Methods Based on the data on 424 clinical samples (including 374 cases of HCC tissues and 50 cases of nontumor liver tissues) in the TCGA database, Kaplan–Meier method, Cox regression analysis, and immune infiltration analysis were performed to evaluate the relationship between PLCβ4 gene and the clinical characteristics and survival prognosis of HCC patients. Correlation analysis between PLCβ4 gene and 24 types of immune cells was applied to investigate the relationship between PLCβ4 gene and immune cell infiltration and mRNA expression level of TP53 gene, a high-frequency mutation gene in HCC. In addition, paraffin sections of highly, moderately, and poorly differentiated tumor tissues and normal liver tissues from HCC patients were collected. The histopathological observation was carried out via HE staining method, and the expression levels of PLCβ4 and Ki-67 proteins in each clinical sample were verified through the immunohistochemical method. Results The expression level of PLCβ4 gene in HCC was significantly higher than that in normal tissues (P<0.01), and all patients in the PLCβ4 high-expression group had a significantly longer overall survival than those in the low-expression group (P<0.05), which suggested that PLCβ4 substantially affected the prognosis of HCC patients. Correlation analysis showed that the expression level of PLCβ4 gene was highly correlated with immune cell infiltration and the expression level of TP53 gene. As verified by clinical sample experiments, HE staining experiments and immunohistochemical results revealed that PLCβ4 gene expression in HCC tissue samples was significantly higher than that in normal tissues (P<0.001), and it was negatively correlated with the degree of differentiation. Conclusion PLCβ4 may serve as an independent prognostic factor in HCC and is expected to be a novel molecular target for HCC treatment.

Objective:To investigate the status quo of sexual function and marital adjustment of postpartum women and their spouses, so as to analyze the subject-object effects of sexual function and marital adjustment. Methods:This was a cross-sectional study. A total of 240 postpartum women and their spouses who gave birth in PLA Rocket Force Characteristic Medical Center from October 2022 to February 2023 were selected as the research objects by the convenient sampling method. The general data questionnaire, Female Sexual Function Index (FSFI), Male Sexual Function Index (MSFI) and Locke-Wallace Marital Adjustment Test were used to investigate them. Pearson correlation analysis and actor-partner interdependence model (APIM) were used to explore the relationship between sexual function and marital adjustment of postpartum women and their spouses. Results:In this study, 480 questionnaires were distributed to 240 couples, and 450 valid questionnaires were collected, with an effective recovery rate of 93.75% (450/480). The sexual function index score of 225 postpartum women was (24.89±4.97), and that of their spouses was (49.64±7.99). The marital adjustment scores of postpartum women was (96.42±22.64) and that of their spouses was (103.38±23.93). The results of APIM showed that the sexual function of postpartum women and their spouses could predict their own marital adjustment (β-values of 0.765 and 0.176, P<0.01). In terms of object effect, the sexual function of postpartum women could predict the marital adjustment of their spouses (β=0.632, P<0.01), but the sexual function of their spouses could not predict the marital adjustment of postpartum women ( P>0.05) . Conclusions:Postpartum women and their spouses have sexual dysfunction and marital disorder, their own sexual function is closely related to marital adjustment, and women's sexual function can also positively affect their spouses' marital adjustment. Medical staff should take postpartum women and their spouses as a whole to formulate intervention measures to promote the recovery of postpartum women's sexual function, balance family relations, enhance the satisfaction of sexual life between husband and wife and improve the level of marital adjustment.

Classical swine fever(CSF),caused by classical swine fever virus(CSFV),is a severe in-fectious disease characterized by high fever and extensive bleeding, which is listed as a mandatory report disease by WOAH.As a single-stranded RNA envelope virus,CSFV has evolved a strategy for attachment and entry to the cell in the process of adapting to external environmental pressure.H owever, the underlying molecular mechanism remains largely unknown.Studies have shown that CSFV is mainly internalized through clathrin-mediated cross-membrane internalization and exists in the form of endosomes in the cytoplasm.Under certain conditions, the endosomal membrane fu-sion releases the genome for viral proliferation. At the same time,CSFV also depends on the inter-action with host proteins to inhibit the synthesis and secretion of host interferon, regulate host cell apoptosis, autophagy, pyroptosis and inflammatory response, and other life activities to evade the host's natural immunity, thus promoting the further replication of the virus in the host. However, the underlying specific mechanism needs further studying. Here,we summarize the molecular mechanism of CSFV internalization across cell membranes and the challenges of CSF prevention and control, with a view to providing theoretical assistance for CSF purification.

Objective:To evaluate the safety and necessity of vasopressor infusion through midline catheter. Methods:A convenient sampling method was used for a controlled study. A total of 88 adult patients who used vasopressors admitted to respiratory intensive care unit (RICU) of Fenyang Hospital in Shanxi Province from June 2022 to June 2023 were enrolled as the research subjects. A total of 44 patients who were infused with vasopressors through peripherally inserted central venous catheter (PICC) from June to December 2022 were enrolled as the PICC group, and 44 patients who were infused with vasopressors through midline catheter from January to June 2023 were enrolled as the midline catheter group. Both groups of patients used the modified Sedinger technique under the guidance of B-ultrasound for puncture and catheter placement. The middle 1/3 site between the cubital fossa and the axilla was selected. The catheters were 5 Fr double lumen. After catheter placement, the patients were followed until catheter removal, death, or 30 days (whichever came first). Based on the Infusion therapy standards of practice revised by American Infusion Nurses Society (INS), and combined with the results of previous preliminary tests, the safety evaluation was conducted on incomplete catheter obstruction, catheter-related bloodstream infection (CRBSI), phlebitis, thrombus within the catheter during extubation, redness of the puncture site (but no infection), and exudation of the puncture site in the two groups of patients. Results:There were no statistical differences in gender, age, catheter indwelling time, and primary disease between the two groups, indicating that the baseline data of the two groups were balanced and comparable. No CRBSI or phlebitis occurred in both groups during the observation period after catheterization. One patient in both groups had exudation at the puncture site [both were 2.27% (1/44)]. Compared with the PICC group, the incidence of incomplete catheter obstruction, thrombus within the catheter during extubation, redness of the puncture site (but no infection) in the midline catheter group were lowered [incomplete catheter obstruction: 4.55% (2/44) vs. 6.82% (3/44), thrombus within the catheter during extubation: 0% (0/44) vs. 2.27% (1/44), redness of the puncture site (but no infection): 0% (0/44) vs. 4.55% (2/44)], the overall incidence was significantly decreased [6.82% (3/44) vs. 15.91% (7/44), P < 0.01]. Conclusion:Administering vasopressor through a midline catheter can reduce the incidence of catheter-related complications, decrease the rate of central venous catheterization, and reduce the financial burden on patients.

Objective:To explore the clinical application effects of early pulmonary rehabilitation training based on bundle airway management in patients with tracheostomy and non-mechanical ventilation. Methods:Used convenience sampling, adult patients who underwent tracheostomy and non-mechanical ventilation admitted to Fenyang Hospital of Shanxi Province from January 2022 to May 2023 were enrolled. Patients were divided into control group and observation group based on their admission dates. The control group implemented bundle airway management based on the Airway care for patients with tracheostomy and non- mechanical ventilation. The observation group received additional pulmonary rehabilitation training implemented by a specialized respiratory rehabilitation team. The primary outcomes included hospital stay duration, improvement rate of muscle strength, diaphragmatic mobility, and successful extubation rate. Secondary outcomes included the incidence of pulmonary infection, improvement rate of self-care ability, and the duration of tracheal tube placement in extubated patients. Results:A total of 41 patients were included in the control group and 39 patients in the observation group. There were no significantly differences in gender, age, first diagnosis of disease, and activities of daily living (ADL) score at admission between the two groups, indicating balanced baseline data and comparability. Compared with the control group, the observation group had a significantly shorter hospital stay (days: 38.33±17.58 vs. 51.34±27.06), a higher improvement rate of muscle strength (76.92% vs. 39.02%), greater diaphragmatic mobility (cm: 1.86±0.64 vs. 1.44±0.57), and a higher success rate of tracheostomy tube extubation (38.46% vs. 17.07%), the duration of tracheal tube placement in successfully extubated patients was significantly reduced (days: 33.26±17.87 vs. 50.60±21.98), with all differences being statistically significant (all P < 0.05). Although the improvement rate of ADL score (56.41% vs. 36.59%) and the incidence of pulmonary infection (5.13% vs. 7.32%) in the observation group were better than those in the control group, but there were no differences (both P > 0.05). Conclusion:The addition of pulmonary rehabilitation training in bundle airway management can enhance exercise tolerance, improve respiratory function, promote sputum clearance, increase successful extubation rates, shorten hospital stays, and facilitate early recovery in patients with tracheostomy and non-mechanical ventilation.
