OBJECTIVE:
To assess the safety and topical efficacy of prim-O-glucosylcimifugin (POG) and investigate the molecular mechanisms of its therapeutic effects in atopic dermatitis (AD).
METHODS:
The effects of POG on human keratinocyte cell viability and its anti-inflammatory properties were evaluated using cell counting kit-8 assay and reverse transcription-quantitative polymerase chain reaction (RT-qPCR). Subsequently, the impact of POG on the differentiation of cluster of differentiation (CD) 4+ T cell subsets, including T-helper type (Th) 1, Th2, Th17, and regulatory T (Treg), was examined through in vitro experiments. Network pharmacology analysis was used to elucidate POG's therapeutic mechanisms. Furthermore, the therapeutic potential of topically applied POG was further evaluated in a calcipotriol-induced mouse model of AD. The protein and transcript levels of inflammatory markers, including cytokines, lymphocyte-specific protein tyrosine kinase (Lck) mRNA, and LCK phosphorylation (p-LCK), were quantified using immunohistochemistry, RT-qPCR, and Western blot analysis.
RESULTS:
POG was able to suppress cell proliferation and downregulate the transcription of interleukin 4 (Il4) and Il13 mRNA. In vitro experiments indicated that POG significantly inhibited the differentiation of Th2 cells, whereas it exerted negligible influence on the differentiation of Th1, Th17 and Treg cells. Network pharmacology identified LCK as a key therapeutic target of POG. Moreover, the topical application of POG effectively alleviated skin lesions in the calcipotriol-induced AD mouse models without causing pathological changes in the liver, kidney or spleen tissues. POG significantly reduced the levels of Il4, Il5, Il13, and thymic stromal lymphopoietin (Tslp) mRNA in the AD mice. Concurrently, POG enhanced the expression of p-LCK protein and Lck mRNA.
CONCLUSION
Our research revealed that POG inhibits Th2 cell differentiation by promoting p-LCK protein expression and hence effectively alleviates AD-related skin inflammation. Accurate timing of myelination is crucial for the proper functioning of the central nervous system. Here, we identified a de novo heterozygous mutation in TMEM63A (c.1894G>A; p. Ala632Thr) in a 7-year-old boy exhibiting hypomyelination. A Ca2+ influx assay suggested that this is a loss-of-function mutation. To explore how TMEM63A deficiency causes hypomyelination, we generated Tmem63a knockout mice. Genetic deletion of TMEM63A resulted in hypomyelination at postnatal day 14 (P14) arising from impaired differentiation of oligodendrocyte precursor cells (OPCs). Notably, the myelin dysplasia was transient, returning to normal levels by P28. Primary cultures of Tmem63a-/- OPCs presented delayed differentiation. Lentivirus-based expression of TMEM63A but not TMEM63A_A632T rescued the differentiation of Tmem63a-/- OPCs in vitro and myelination in Tmem63a-/- mice. These data thus support the conclusion that the mutation in TMEM63A is the pathogenesis of the hypomyelination in the patient. Our study further demonstrated that TMEM63A-mediated Ca2+ influx plays critical roles in the early development of myelin and oligodendrocyte differentiation.

Objective To investigate the risk factors for pneumonia complicating infectious mononucleosis(IM)in adults and construct a nomogram prediction model. Methods A retrospective analysis was conducted on 198 IM patients admitted to the Second Affiliated Hospital of Air Force Medical University from January 2015 to December 2021.Patients were divided into pneumonia group(n=52)and non-pneumonia group(n=146)based on whether pulmonary infection occurred during hospitalization. The baseline data(age, gender, place of onset, etc.),clinical manifestations(maximum body temperature, lymph node enlargement, splenomegaly, etc.),and inflammatory indicators[white blood cell count(WBC),C-reactive protein(CRP),etc.]were compared between the two groups.Kaplan-Meier curves were plotted to analyze the key indicators affecting the hospital stay of IM patients.Multivariate logistic regression was used to analyze the independent risk factors for pneumonia complicating IM in adults and construct a nomogram prediction model based on the identified risk factors. The predictive efficacy of the model was evaluated using the receiver operating characteristic(ROC)curve and the consistency of the model was assessed using the calibration curve. The fit of the model was evaluated using the Hosmer-Lemeshow test.Additionally, the sensitivity, specificity, and accuracy of the model were assessed using confusion matrix. Results Compared with non-pneumonia group, the pneumonia group had a significantly higher proportion of patients from rural areas, with body mass index(BMI)≥24 kg/m2,smoking history, hepatomegaly, fever duration of≥7 d,as well as increased total hospitalization costs and average daily hospitalization costs, and prolonged hospital stay(P<0.05).The proportion of patients with a history of antibiotic use was lower in the pneumonia group(P<0.05).Kaplan-Meier survival analysis showed that patients from rural areas, with BMI≥24 kg/m2,smoking history,no prophylactic use of antibiotics, fever duration≥7 d,and hepatomegaly had significantly prolonged hospital stays(P<0.05).Multivariate logistic regression analysis revealed that living in a rural area(OR=4.089,P<0.05),hepatomegaly(OR=4.082,P<0.05),and elevated WBC(OR=1.205,P<0.05)were independent risk factors for pneumonia complicating IM in adults, while the prophylactic use of antibiotics(OR=0.142,P<0.05)was an independent protective factor. The area under the ROC curve of the constructed nomogram prediction model was 0.827(95%CI 0.762-0.892),and the slope of the calibration curve was close to 1,and the Hosmer-Lemeshow test showed χ2=5.299,P=0.725,indicating good consistency and fit of the prediction model. The results of the confusion matrix assessment showed that the sensitivity of the model was 0.669(0.624-0.773),the specificity was 0.827(0.724-0.930),and the accuracy was 0.732(0.665-0.793).Conclusion The nomogram prediction model based on place of onset, hepatomegaly, the prophylactic use of antibiotics and WBC has excellent fit and discrimination, providing an effective quantitative tool for prognosis assessment of IM.

The emergence of artificial intelligence(AI)has had a significant impact on medical research and practice, both in terms of the number of studies and research paradigms, and has become an important tool for the development of pharmacoepidemiology. However, traditional AI has faced many challenges, while facilitating pharmacoepidemiology research, such as complex data processing, difficulty in identifying drug exposures and potential outcomes, and time-consuming and laborious study design and implementation. The rapid development of generative AI,represented by large language models(LLMs),has demonstrated a unique potential to enhance research efficiency, shift research paradigms, and facilitate knowledge discovery.LLMs are equipped with natural language understanding and generation capabilities.Through deep mining of multi-dimensional data resources,LLMs can quickly and accurately extract, analyze, summarize, and present the required information, which can not only help drug discovery, drug repurposing, pharmacovigilance and other pharmacoepidemiological tasks, but also provide powerful support for the whole process of research protocol design, data analysis, result interpretation and paper publication.Driven by LLMs,pharmacoepidemiology research is gradually moving into a new stage based on big data and automated analysis.Of course,LLMs also have problems of data bias,"illusion"of results, and ethical and legal regulation.By strengthening interdisciplinary cooperation, establishing a standardized evaluation system, improving ethical and regulatory guidance, enhancing data quality, strengthening practitioner training and capacity building, and promoting human-machine collaborative research modes,it is expected that the potential of LLMs in pharmacoepidemiology will be fully released, and it will provide a more scientific, rapid, and efficient technological support for drug regulation and public health decision-making.
