The blood-brain barrier (BBB) is a physical and biochemical barrier that precisely regulates brain homeostasis and plays a central role in controlling the transport of endogenous and exogenous drugs and related metabolites across the blood-brain interface. These functions of the BBB are mediated by its major components, including brain microvascular endothelial cells (BMECs), tight junction protein complexes, and influx and efflux transporter proteins. One of the pathological features of ischemic stroke (IS) is BBB disruption, which plays an important role in the development of post-stroke brain injury and subsequent neurological dysfunction. Therefore, given the increasing incidence of IS, there is an urgent need to develop new therapeutic strategies to prevent BBB dysfunction and thereby protect injured brain tissue after IS. This study describes the pathological mechanisms by which BMEC injury after IS leads to BBB dysfunction and elucidates the association between BMECs and IS, including the regulation of apoptosis, autophagy, inflammatory responses, oxidative stress, neurotoxic effects, and cerebral edema. In addition, this article summarizes Chinese herbal medicines that may prevent and treat IS by targeting BMECs. These include monomeric compounds and single herbs such as flavonoids, glycosides, phenols, phthalides, terpenoids, and Styrax. Traditional Chinese medicine (TCM) compound formulas and preparations include oral formulations such as Buyang Huanwu decoction, Sailuotong, Naoxintong capsules, Dandeng Tongnao capsules, and Shexiang Tongxin dropping pills, as well as injectable preparations such as Tongluo Jiunao injection, Xingnaojing injection, Danshen polyphenolic acid for injection, Yiqi Fumai injection, and Shuxuetong injection. This study aims to explore the protective effects of TCM against IS through targeted regulation of BMEC function, providing new insights into the mechanisms of IS and endovascular therapeutic strategies.

Results:A total of 276 children were included in this study, with an age ranging from 6 to 154 months with a median of 34 months. The weight ranged from 6.2 to 63.2 kilograms, with a median of 13.5 kilograms. The operation time ranged from 23 to 90 minutes, with a median of 45 minutes. One hundred and thirty-five patients underwent day surgery and 141 patients underwent non-day surgery. The average age and average weight of the patients in the day surgery and non-day surgery groups was (43.7±30.8) months vs.(42.4±30.2) months and (15.0±6.4) kg vs.(14.8±6.9) kg, respectively. There was no statistically significant difference in age and weight between the two groups (all P>0.05).There was no difference in perioperative data and incidence of perioperative complications between the two groups (all P>0.05).However, the length of hospital stay of the day surgery group was significantly shorter than that of the non-day surgery group[(1.0±0) days vs.(2.5±0.9) days, t=-20.993, P<0.001], and the hospitalization cost of the day surgery group was also significantly lower than that of the non-day surgery group[(50 800±1 100) yuan vs.(53 500±3 900) yuan, t=-10.040, P<0.001].Both families of the two groups were satisfied with the treatment results, and there was no difference in satisfaction between the two groups ( χ2=0.501, P=0.479). Conclusions:The day surgery management model of Da Vinci robotic surgery for the treatment of pediatric patent ductus arteriosus is safe and feasible. To a certain extent, it can reduce the length of hospital stay, treatment costs, and treatment burden.

Mitochondrial transcription factor B1(TFB1M)is mainly involved in mitochondrial DNA transcription and related to the development of hepatocellular carcinoma and breast cancer. However, its role in colon cancer is unclear. In this study, the expression level of TFB1M in colon cancer and its prognosis were analyzed based on TCGA and other databases and IHC assays.Differentially expressed genes(DEGs)were screened and subjected to the analysis of functional enrichment, mutation analysis, immune cell infiltration, and drug sensitivity analysis.CCK-8 and flow cytometry were used to detect the effects of overexpression of TFB1M on the viability, apoptosis and cell cycle distribution of colon cancer cells. Our results showed that the expression level of TFB1M was significantly up-regulated in colon canc-er,and its expression level was correlated with the N stage and TNM stage(P<0.05).The prognosis of colorectal cancer patients in the high TFB1M expression group was worse.Functional enrichment results showed that TFB1M was related to leukocyte-mediated immunity, immunoglobulin production and other signaling pathways.Mutation results showed that high-frequency mutated genes, such as ZFHX4,RYR2,PIK3CA and FAT4,had significantly higher mutation frequencies in the TFB1M high-expression group(all P<0.05).In addition, the expression level of TFB1M was significantly higher in the PIK3CA and FAT4 mutation groups(all P<0.05).Immune infiltration results showed that the percentage of CD4+memory activated T cells was increased in the TFB1M high expression group, while the percentage of Treg cells was reduced. The drug sensitivity results showed that patients in the TFB1M high expression group might be more sensitive to Tozasertib, cytarabine, vincristine, etc.,while patients in the TFB1M low expression group might be more sensitive to Dasatinib,JQ1,ERK_2440,etc.The results of cellular experiments showed that over-expression of TFB1M enhanced viability, reduced apoptosis and increased the percentage of S-phase and G2/M-phase cells in colon cancer cells.Altogether, the results indicated that TFB1M might play a key role in the growth of colon cancer cells by regulating immune cell infiltration and function.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
