1.Comparison of 127° small and 135° large stem angle prostheses in total hip arthroplasty.

OBJECTIVE:
To investigate the effect of total hip arthroplasty(THA) with the prosthesis of 127° small neck stem angle and 135° large neck stem angle.
METHODS:
From January 2014 to June 2016, 84 patients with THA were selected, including 44 males and 40 females, aged 45 to 72(53.4±8.1) years old, 68 patients with necrosis of the femoral head(32 on the left and 36 on the right), 16 patients with serious osteoarthritis of the hip caused by other reasons, and the course of disease was 9 to 36 (24.0±5.5) months. Forty-two patients in each group were evaluated by Harris score, visual analog score(VAS), length measurement of lower limbs, biomechanical evaluation of different angles of the neck stem. The complications and quality of life 24 months after operation were compared.
RESULTS:
Two patients in each group were lost, the rest were followed up for 30 to 36 (33.0±1.6)months. The Harris score and the length of both lower limbs were measured before and 1, 6, 12, 24 months after operation. The difference of Harris score and the length of both lower limbs in the two groups was significantly improved compared with that before operation(
CONCLUSION
THA with large and small neck stem angle prosthesis can better recover the function of hip joint, but large neck stem angle can reduce the degree of postoperative pain and improve the quality of life of patients.

2.Aldosterone induces inflammatory cytokines in penile corpus cavernosum by activating the NF-κB pathway.

Emerging evidence indicates that aldosterone and mineralocorticoid receptors (MRs) are associated with the pathogenesis of erectile dysfunction. However, the molecular mechanisms remain largely unknown. In this study, freshly isolated penile corpus cavernosum tissue from rats was treated with aldosterone, with or without MRs inhibitors. Nuclear factor (NF)-kappa B (NF-κB) activity was evaluated by real-time quantitative PCR, luciferase assay, and immunoblot. The results demonstrated that mRNA levels of the NF-κB target genes, including inhibitor of NF-κB alpha (IκB-α), NF-κB1, tumor necrosis factor-alpha (TNF-α), and interleukin 6 (IL-6), were higher after aldosterone treatment. Accordingly, phosphorylation of p65/RelA, IκB-α, and inhibitor of NF-κB kinase-β was markedly increased by aldosterone. Furthermore, knockdown of MRs prevented activation of the NF-κB canonical pathway by aldosterone. Consistent with this finding, ectopic overexpression of MRs enhanced the transcriptional activation of NF-κB by aldosterone. More importantly, the MRs antagonist, spironolactone blocked aldosterone-mediated activation of the canonical NF-κB pathway. In conclusion, aldosterone has an inflammatory effect in the corpus cavernosum penis, inducing NF-κB activation via an MRs-dependent pathway, which may be prevented by selective MRs antagonists. These data reveal the possible role of aldosterone in erectile dysfunction as well as its potential as a novel pharmacologic target for treatment.

3.IMPEDIMETRIC IMMUNOSENSOR BASED ON NANOBODY AND GOLD NANOPARTICLES MODIFIED GRAPHENE PAPER FOR DETECTION OF SCHISTOSOMA JAPONICUM CIRCULATING ANTIGEN

In this study,a new type of impedimetric immunosensor was constructed based on graphene paper for detection of Schistosoma japonicum circulating antigen.The prepared graphene paper was modified with nanobody and gold nanoparticls and the surface morphology and crystal structure of the graphene paper was characterized by scanning electron microscope,atomic force microscope,raman spectroscopy.At the same time,specificity and sensibility of this immuosensor were analyzed.It is demonstrated that the developed impedimetric immunosensor possesses excellent specificity and sensibility,the detection level of S.japonicum soluble egg antigen(SEA) reaches 0.01 ng/mL;The detection rate of suspected schistosomiasis patients was 66.7%,which was better than that (56.7%) of ELISA,and the coincidence rate with ELISA was 90%.The immunosensor has high sensitivity and specificity for detecting S.japonicum circulating antigen.The method is simple,fast and has good potential for application.
4.PRELIMINARY STUDY ON THE COLLOIDAL GOLD IMMUNOCHROMATOGRAPHIC ASSAY BASED ON NANOBODY OF SOLUBLE EGG ANTIGENS FROM SCHISTOSOMA JAPONICUM

A colloidal gold immunochromatographic assay for detection of Schistosoma japonicum based on nanobody was established. Two nanobodies, VHH-48 and VHH-52 against soluble egg antigens (SEA) of Schistosoma japonicum have been expressed in Bacillus subtilis and purified by Ni-NTA affinity chromatography. In order to confirm the optimal labeling pH and optimum concentration of nanobody proteins,the colloidal gold solution was prepared by trisodium citrate reduction, VHH-48 and VHH-52 labelled with colloidal gold were used as probes,and the SEA and goat anti-mouse IgG were coated onto the nitrocellulose membrane to form the test line and control line respectively. The colloidal gold immunochromatographic assay with double nanobody sandwich was established. Test strips were assembled with the optimized double-sandwich nanobody combination,sensitivity was evaluated with a serial dilution of SEA antigen and serum from patients of schistosomiasis positive. The specificity of the test strips was determined with sera from other parasites such as Paragonimus. Results showed every 1 mL colloidal gold labeled with VHH-48 and VHH-52 mixed with 16 and 12 μL 0.2 mol/L K2CO3respectively to reach the optimum pH. The colloidal gold solution was adjusted to the optimum pH to determine the optimal protein concentrantion. The optimal protein concentration of VHH48 and VHH-52 were 14 and 10 μg/mL. The best reaction model was using the VHH-52 to label with colloidal gold and coat onto the nitrocellulose membrane to form the test line. The detective sensitivity of SEA was 3.4 ng/mL and the serum positive rate of Schistosomiasis patients was 2.27%. Detection of other parasite-infected sera showed good specificity. The colloidal gold dipstick based on nanobody against SEA laid a certain foundation for the application of nanobody in the rapid diagnosis of Schistosomiasis.
5.Identification of the active material of anti-hepatic fibrosis from Amydae Carapax.

OBJECTIVETo identify the active material of anti-hepatic fibrosis from Amydae Carapax.
METHODSMembrane separation technology was adopted to screen active fraction in Amydae Carapax, and the active components were isolated from the active fraction using gel chromatography and high performance liquid chromatography. The purified active components in Amydae Carapax were further analyzed using 4700 series time-of-flight mass spectrometer.
RESULTSProteins and peptides of Amydae Carapax with molecular weight less than 6000 were proved to have biological activity. 8 components (Bj1-Bj8) were isolated from the active fraction. Bj4, Bj6 and Bj7 were screened as active components. Bj7 was further purified, resulting in 7 components (Bj701-Bj707). Bj704 and Bj707 showed significant biological activity. Mass spectrometry showed three molecular ion peaks with highest abundance, i.e. m/e 526, 542 and 572, i.e. m/e 526, 542 and 572, in Bj707 -A The amino acid sequences of above three peptide compounds were NDDY (Asn-Asp-Asp-Tyr), NPNPT (Asn-Pro-Asn-Pro-Thr), and HGRFG (His-Gly-Arg-Phe-Gly), respectively. And M572 was the most abandunt components.
CONCLUSIONThree active peptide compounds of anti-hepatic fibrosis of Amydae Carapax were identified.

6.Urodynamic studies on benign prostatic hyperplasia combined with diabetes mellitus.

OBJECTIVE:
To evaluate urinary dynamics in the diagnosis, differential diagnosis, disease grades, complications, prognosis, and curative effect on benign prostatic hyperplasia (BPH) patients accompanied with diabetic mellitus (DM), and to investigate the clinical significance of urodynamic studies.
METHODS:
A total of 106 BPH patients with lower urinary tract symptoms were studied, aged 53-92(63.21+/-7.18) years. The mean duration of voiding symptoms was 3-12 years. These patients were divided into 2 groups: Group A consisted of 59 BPH patients while Group B consisted of 47 BPH patients combined with DM. Both groups were evaluated the international prostate symptom score(IPSS), urine flow rate, and residual urine volume. All patients underwent urodynamic examination. Correlation analysis was conducted to determine the correlation among the patients. The urodynamic results in different groups were compared.
RESULTS:
The IPSS in Group A was higher than Group B, while the Qmax was lower than Group B. The unsatisfactory results of Group A was higher than Group B. There was significant difference between the 2 groups (P<0.05). There was no significant difference in residual urine volume between the 2 groups (P>0. 05). There was significant difference between the 2 groups in detrusor instability of the bladder, low compliance bladder, and damaged detrusor function (P<0.05).
CONCLUSION
The influence of DM on the function of the bladder in BPH patients is significant. Urodynamics can provide objective basis for the selection of therapeutic methods, and forecast the postoperative effect. It has an important reference value in BPH patients accompanied with DM.

This study was purposed to investigate the practicality of matrix-assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF MS) for detection of single nucleotide polymorphisms (SNP) on jak2 gene in multiple myeloma (MM) cells. The DNA fragment containing 2 SNPs of jak2 (C428T and C643T) was amplified using PCR and was purified. The purified product was used as the template for primer extension (PEX). The small products of allele-specific reaction were purified, the SNPs on jak2 gene of 5 patients with MM and 5 healthy persons were detected by MALDI-TOF MS. The results showed that the distribution of genotype C428T and C643T was not different between MM patients and healthy persons, both of which are homozygous T/T. In conclusion, the method based on MALDI-TOF MS and PEX technique for detecting SNP in jak2 gene is rapid, accurate and reliable method, and can be used in clinical practice.

8.Analysis of the pelvic stability after type I resection of iliac tumor.

OBJECTIVETo analyze the pelvic stability after type I resection of iliac tumor.
METHODSSix adult cadaveric specimens were tested. The iliac subtotal resection models were established according to Ennecking's type I resection. Markers were affixed to the key region of the pelves. Axial loading from the proximal lumbar was applied by MTS load cell in the gradient of 0-500 N in the double feet standing state. Images in front view were obtained using CCD camera. Based on Image J software, displacements of the first sacral vertebrae (S1) of the resected pelves and the intact pelves were calculated using digital marker tracing method with center-of-mass algorithm.
RESULTSSerious instabilities were found in the resected pelves. S1 rotational movements around the normal side femoral head of the resected pelvis were found. The average vertical displacement of S1 of the resected pelvis was (7 +/- 3) mm under vertical load of 500 newtons, which were 8.3 times compared to the intact pelvis. The average angle of S1 rotation around the normal side femoral head of the resected pelvis was (4.0 +/- 1.8) degrees, which were 12.5 times compared to the intact pelvis.
CONCLUSIONSBiomechanical model of type I resection of iliac tumor are established. Essential pelvic reconstruction must be introduced because of the serious instability of the bone defection after tumor resection.

9.Phenylhexyl isothiocyanate reducing U266 cell line methylation level of p16 gene.

This study was purposed to investigate whether phenylhexyl isothiocyanate (PHI) can reduce p16 gene methylation level or not. The myeloma U226 cell line was cultured with PHI of 0, 5, 10 micromol/L for 72 hours, then DNA was extracted. Hydrosulfite was used to treat the genome DNA of healthy adult, PCR amplification was carried out by using this DNA as template. The gene chip detecting methylation changes of 3 CpG in promoter region of p16 gene was constructed by designing a pair of probes which contain one methylated and one unmethylated probes. This pair of probes was used to detect 3 consecutive sites of CpG island in p16 gene. The standard curve was constructed by using gene chip after the methylated and unmethylated DNA were mixed at different ratio. Then treated samples of U266 cells were dotted on gene chip, obtained results were compared with standard curve to get the quantitative results. The results indicated that the probes on chip had excellent reproducible ability and precision, the methylation level of p16 gene in U266 cells treated with 0, 5 and 10 micromol/L of PHI was determined as 78.2%, 61.7% and 54.8%, respectively. It is concluded that the PHI can reduce the methylation level of p16 gene in U266 cells.

10.Myocardial Toll like receptor 4 expression in a rat model of myocardial ischemia reperfusion injury

Objective To explore the role of TLR4 in myocardial ischemia reperfusion injury(MI/RI) by observing the dynamic TLR4 expression changes at mRNA and protein levels early after myocardial ischemia reperfusion.Methods Male SD rats were randomly divided into Sham and IR group and the rats were killed according to different reperfusion time(0,0.5,1,2,4 and 8 hours).Myocardial changes under light microscope and transmission electronic microscope were observed.TLR4 expressions at protein and mRNA levels were detected by immunohistochemistry and realtime RT-PCR respectively.Myocardial TNF-α was determined by ELISA.Results (1)Myocardial injury was observed in IR but not in Sham group and histopathological and uhrastructual changes in IR group remained unchanged up to 8 hours after reperfusion.(2)Positive TLR4 protein staining was visualized in both Sham and IR groups and significantly increased and peaked at 1 hour of reperfusion in IR group.(3)Compared to Sham group,TLR4 mRNA level was upregulated in myocardium in IR group and peaked at 1 hour of reperfusion.(4)Concentration of TNF-α in IR group was significantly higher than that of Sham group at corresponding time points(all P<0.05),and myocardial TLR4 mRNA level correlated positively with myocardial TNF-α (r=0.728,P<0.01).Conclusion Expression of TLR4 in myocardium during early after myocardial ischemia repeffusion was upregulated and actived TLR4 might play an important role in MI/RI through promoting myocardial TNF-α excretion.
