AIM To identify the in vivo metabolites of Cynanchum auriculatum Royle ex Wight extract in functional dyspepsia rats by UHPLC Q-Exactive Plus Orbitrap HRMS.METHODS The rat models for functional dyspepsia were established.The analysis was performed on a 40℃ thermostatic Hypersil GOLD C18 column(2.1 mm×100 mm,1.9 μm),with the mobile phase comprising of water(containing 0.1％formic acid)-acetonitrile(containing 0.1％formic acid)flowing at 0.3 mL/min in a gradient elution manner,and electrospray ionization source was adopted in positive and negative ion scanning.RESULTS Total 4 prototypes(baishouwubenzophenone,deacylmetaplexigenin,qingyangshengenin,syringic)and 110 metabolites were identified,12 of which were common metabolites in feces and urine,56 of which were unique metabolites in urine,42 of which were unique metabolites in feces.The metabolic pathway of prototypes contained phase Ⅰ metabolism(reduction,oxidization,etc.),phase Ⅱ metabolism(sulfonation,glucuronidation,etc.)and phase Ⅰ,Ⅱ composite reactions.CONCLUSION This effective and comprehensive method can lay the theoretical foundation for further discovery of potential active metabolites in C.auriculatum.

Objective:To explore the potential synergistic protective mechanism of Glycyrrhizae Radix et Rhizoma-Granati Pericarpium formula compound by using the methods and tools of network pharmacology，and provide a basis for the modernization of traditional Chinese medicine（TCM） compounds and the discovery of new drugs. Method:Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform （TCMSP） was used to obtain the active components of Glycyrrhizae Radix et Rhizoma-Granati Pericarpium formula and their corresponding targets. The obtained targets were input to the UniProt database to inquire the gene names corresponding to the targets. By searching the CTD database，Genecards database and OMIM database of disease-related websites，the anti-sunburn targets were obtained. The interaction of the active targets was analyzed with online STRING database to screen the predicted core targets. The gene ontolog（GO） gene function enrichment analysis and Kyoto encyclopedia of genes and genomes（KEGG） pathway enrichment analysis of the predictive targets were performed by using DAVID database. Cytoscape 3.6.1 software was used to make "drug-component-target" network diagram，"protein-protein interaction" network diagram and "component-target-pathway" network diagram. Online website Draw Venn Diagram was used to show the relationship between disease targets and drug predicted targets. R Studio software was used to draw the functional enrichment analysis diagram of GO gene and KEGG pathway. Molecular docking between the active ingredients and the core targets was performed using GOLD software. Result:The 16 active compounds were collected，such as liquiritin，glycyrrhizin，kaempferol and quercetin. The active components mainly acted on 5 core targets：protein kinase B1（AKT1），interleukin（IL）-6，vascular endothelial growth factor（VEGFA），tumor necrosis factor（TNF） and tumor suppressor gene （TP53） and played a role in anti-sunburn effect primarily through these pathways such as hepatitis B，pathways in cancer，toxoplasmosis，chagas disease（American trypanosomiasis），and TNF signaling pathway. Conclusion:Based on the method of network pharmacology，the present study has preliminarily explored the anti-sunburn targets and pathways of Glycyrrhizae Radix et Rhizoma-Granati Pericarpium formula，and further verified the characteristics of multi-component and multi-target treatment of diseases in TCM，so as to provide certain scientific ideas for the modernization research of Chinese herbal compound prescriptions.

AIM: To investigate the effect of panretinal photocoagulation(PRP)combined with intravitreal injection of conbercept as an adjuvant to neovascular glaucoma secondary to central retinal vein occlusion(CRVO).

METHODS: This was a prospective case-control study. Patients diagnosed with neovascular glaucoma(stage Ⅰor Ⅱ)secondary to CRVO in our hospital from January 2014 to March 2019 were enrolled and randomly divided into two groups. Patients in combination group were treated with both PRP and intravitreal conbercept injection. In the other group(PRP only group)patients were treated with PRP only. Changes of best corrected visual acuity(BCVA), intraocular pressure(IOP)and iris neovascularization(NVI)were observed before treatment, 1wk, 1mo, 3mo, 6mo and 9mo after treatment.

RESULTS: Sixty patients 60 eyes were enrolled, with the mean age of 54.45±6.08 years. 56.67% were males. There were no significant difference between two groups in age and gender. After treatment, all the indexes of the two groups improved obviously with significant different(F=154.992, 92.519, 30.696, 82.374, all P<0.001), visual acuity improved, IOP decreased, and NVI decreased at different time points. The difference was statistically significant compared with the control group one week after treatment(F=50.870, 24.265, 13.125, 11.829, all P<0.001), and it was maintained continuously.

CONCLUSION: For eyes with CRVO secondary with neovascular glaucoma, intravitreal injection of conbercept combined with panretinal photocoagulation PRP is better than panretinal photocoagulation PRP only.

OBJECTIVEThrough reviewing the relevant literature from the past decades, to summarize the assessment and management of fractures of the clavicle, and provide an overview of the clinical results of a range of treatment options.

DATA SOURCESThe data analyzed in this review are mainly from articles included in PubMed and EMBASE, published from 1960 to 2015.

STUDY SELECTIONStudies involving assessment of fractures of the clavicle were reviewed. Further literatures were gathered regarding the conservative and surgical treatment of these fractures, including the methods of fixation and the surgical approaches used. Both conservative and surgical treatments were then compared and contrasted.

RESULTSThrough retrieving and reading the abstract, a total of 42 representative articles were selected, which covered all aspects of the conservative treatment and surgical treatment, and compared the advantages and disadvantages of different treatment options.

CONCLUSIONSAlthough the majority of recent data suggest that surgery may be more appropriate as it improves functional outcome and reduces the risk of complications, we recommend that the treatment should be individually assessed.

OBJECTIVETo accurately calculate the protein requirements in Chinese young adults using the indicator amino acid oxidation technique.

METHODSNine women and ten men received a restricted daily level of protein intake (0.75, 0.82, 0.89, 0.97, and 1.05 g/kg), along with L-[1-13C]-leucine. Subjects' protein requirement was determined by a biphasic linear regression crossover analysis of F13CO2 data. In doing so, a breakpoint at the minimal rate of appearance of 13CO2 expiration specific to each level of dietary protein was identified. This trial was registered with the Chinese clinical trial registry as ChiCTR-ONC-11001407.

RESULTSThe Estimated Average Requirement (EAR) and the Recommended Nutrient Intake (RNI) of protein for healthy Chinese young adults were determined to be 0.87 and 0.98 g/(kg•d), respectively, based on the indicator amino acid oxidation technique.

CONCLUSIONThe EAR and RNI of mixed protein are 5% and 16% that are lower than the current proposed EAR and RNI (0.92 and 1.16 g/(kg•d), respectively), as determined by the nitrogen balance method. The respective EAR and RNI recommendations of 0.87 and 0.98 g/(kg•d) of mixed protein are estimated to be reasonable and suitable for Chinese young adults.

OBJECTIVEDoubly labeled water (DLW) method is the gold standard for measuring total energy expenditure (TEE). We used this method to measure TEE in Chinese young men.

METHODSSixteen healthy young men age 23±1 years with body mass index 22.0±1.4 kg/m2 were recruited. TEE was measured by the DLW method, and basal energy expenditure (BEE) was determined by indirect calorimetry. We also conducted 24-h activity, energy balance and factorial approach to estimate energy requirements of the subjects.

RESULTSTEE of subjects by DLW method was 9.45±0.57 MJ/day (2258±180 kcal/day). The 24-h activity was 10.80±0.33 MJ/day (2582±136 kcal/day). The energy requirement, derived from energy balance observations, was 9.93±1.32 MJ/day (2373±315 kcal/day). The BEE of 6.65±0.28 MJ/day (1589±67 kcal/day), calculated by the adjusted Schofield equation, was significantly higher (P<0.001) than that measured by indirect calorimetry, 5.99±0.66 MJ/day (1433±158 kcal/day). The TEE derived from the factorial approach was 10.31±0.43 MJ/day (2463±104 kcal/day).

CONCLUSIONThe TEE of Chinese young men measured by the DLW method was about 10% lower than the current recommended nutrient intake (RNI), suggesting that the RNI for Chinese men maybe overestimated. Further studies are warranted to determine the value of the estimated energy requirement.

Adult medical education has its own particularity.Its quality is affected by students' work units,families,schools,their own and other factors.Changing concepts,optimizing management,promoting personal quality education,setting up early warning mechanism,and doing full co-ordination are effective ways to improve the quality of adult medical education.

Objective To analyze the common outer membrane proteins (OMP)from Porphyromonas gingivalis (Pg)by surface enhanced laser desorption/ionization time-of-flight mass spectrometry (SELDI-TOF-MS) and to provide antigens for the subsequent experiments in screening vaccine for periodontitis therapy. Methods Four strains of Pg were cultured under anaerobic conditions. The common OMP was extracted through ultracentrifugation and SELDI-TOF-MS was employed to detect the expressions of proteomes by chip H50. The data was analyzed by Biomarker Wizard. ResultsFour kinds of strains of OMP fingerprint spectrum were obtained. Seventy-one proteins of PgATCC33277, 74 proteins of PgW83, 76 proteins of PgW301 and 72 proteins of Pg381 were captured by chip H50. Thirteen common proteins were identified according to fingerprint spectrum. There was only 1 of the 13 common proteins identified in NCBI protein bank. ConclusionsSELDI-TOF-MS has good reproducibility and high sensibility and can be used to identify the common OMP of Pg. The 13 proteins have a potential value in the screening vaccine candidate antigen sites for periodontitis.

OBJECTIVETo measure the energy expenditure of reading and watching TV for young women.

METHODSAll 30 girls in the college were selected according to questionnaire, medical examination, blood test, liver function and thyroid gland level. Energy expenditure of reading and watching TV was measured respectively by the Cosmid K4b2 portable gas analysis system from Italy. Basic metabolic rate (BMR) and resting metabolic rate (RMR) were measured as to analyzing results conveniently.

RESULTSThe energy expenditure in reading was (226.35 +/- 56.07) kJ/h and the heart rate was (69 +/- 5) bmp. For watching TV it was (220.79 +/- 65.69) kJ/h and the heart rate was (68 +/- 5) bmp. BMR was (178.53 +/- 53.51) kJ/h and the heart rate was (62 +/- 6) bmp. RMR was (214.76 +/- 44.56) kJ/h and the heart rate was (65 +/- 6) bpm.

CONCLUSIONSThe energy expenditure in reading should be higher than that of watching TV, however, the difference was not obvious. The expenditure rates might get closer to those of RMR. It might be regarded as an expenditure of very light physical active in Chinese.

Objective To explore the diagnosis of small anterior communicating artery aneurysms and discuss the microsurgical techniques and the timing of surgical intervention.Methods Thirty-two cascs of small anterior communicating artery aneurysms were reviewed for the methods for lesion detection,imaging features of the lesions,microsurgical procedures and the patients'clinical outcome. Results CT displayed subarachnoid hemorrhage(SAH)in all the 32 cases.Magnetic resonance angiography(MRA) was performed in 13 cases and computed tomographic angiography (CTA)in 8 cases. All the 32 patients underwem a total of 35 examinations With digital subtract angiography(DSA).The time between operation and aneurysm rupture was less than 3 daysin 6 cases,4 to 14 days in 10 cases.10 to 14 days in 13 cases and over 30 days in 3 cases.Neck clipping of the aneurysm through the pterional approach was performed in all thc cases with tracheal intubation and general anesthesia,and hemorrhage due to ancurysm ruptures occurredin 5 cases during the operation.Death occurred in 1 case after the operation,and 5 patients developed hydrocephalus and received subsequent ventriculoperitoneal shunting.After the operation,22 patients showed good recovery,6 were capable of independent living, 2 needed assistance for walking and in daily activities, and 1 patient remained bed-ridden and needed nursing care.Follow-up of 27 patients for 3 to 12 months found no occurrence of hemorrhage or death.Conclusion DSA Can be the primary choice for diagnosis of small anterior communicating artery aneurysms.Earlyinterventions should be administered for grade I or Ⅱpatients,but for the other patients,the surgeries can be performed 2 or 3 weeks after the hemorrhage.Proficient microsurgical skills can be crucial for successful clipping of the aneurysms and for prevention and effective management of hemorrhage due to aneurysm rupture.
