The Expert Consensus on Clinical Application of Qinbaohong Zhike Oral Liquid in Treatment of Acute Bronchitis and Acute Attack of Chronic Bronchitis (GS/CACM 337-2023) was released by the China Association of Chinese Medicine on December 13th, 2023. This expert consensus was developed by experts in methodology, pharmacy, and Chinese medicine in strict accordance with the development requirements of the China Association of Chinese Medicine (CACM) and based on the latest medical evidence and the clinical medication experience of well-known experts in the fields of respiratory medicine (pulmonary diseases) and pediatrics. This expert consensus defines the application of Qinbaohong Zhike oral liquid in the treatment of cough and excessive sputum caused by phlegm-heat obstructing lung, acute bronchitis, and acute attack of chronic bronchitis from the aspects of applicable populations, efficacy evaluation, usage, dosage, drug combination, and safety. It is expected to guide the rational drug use in medical and health institutions, give full play to the unique value of Qinbaohong Zhike oral liquid, and vigorously promote the inheritance and innovation of Chinese patent medicines.
2.Cholesterol paradox in the community-living old adults: is higher better?

OBJECTIVE:
To evaluate the associations of lipid indicators and mortality in Beijing Elderly Comprehensive Health Cohort Study.
METHODS:
A prospective cohort was conducted based on Beijing Elderly Comprehensive Health Cohort Study with 4499 community older adults. After the baseline survey, the last follow-up was March 31, 2021 with an average 8.13 years of follow-up. Cox proportional hazard model was used to estimate the hazard ratios (HR) with 95% CI for cardiovascular disease (CVD) death and all-cause death in associations with baseline lipid indicators.
RESULTS:
A total of 4499 participants were recruited, and the mean levels of uric acid, body mass index, systolic blood pressure, diastolic blood pressure, fasting plasma glucose, total cholesterol (TC), triglyceride, and low-density lipoprotein cholesterol (LDL-C) showed an upward trend with the increasing remnant cholesterol (RC) quarters (Ptrend < 0.05), while the downward trend was found in high-density lipoprotein cholesterol (HDL-C). During the total 36,596 person-years follow-up, the CVD mortality and all-cause mortality during an average 8.13 years of follow-up was 3.87% (95% CI: 3.30%-4.43%) and 14.83% (95% CI: 13.79%-15.86%) with 174 CVD death participants and 667 all-cause death participants. After adjusting for confounders, the higher level of TC (HR = 0.854, 95% CI: 0.730-0.997), LDL-C (HR = 0.817, 95% CI: 0.680-0.982) and HDL-C (HR = 0.443, 95% CI: 0.271-0.724) were associated with lower risk of CVD death, and the higher level of HDL-C (HR = 0.637, 95% CI: 0.501-0.810) were associated with lower risk of all-cause death. The higher level of RC (HR = 1.276, 95% CI: 1.010-1.613) increase the risk of CVD death. Compared with the normal lipid group, TC ≥ 6.20 mmol/L group and LDL-C ≥ 4.10 mmol/L group were no longer associated with lower risk of CVD death, while RC ≥ 0.80 mmol/L group was still associated with higher risk of CVD death. In normal lipid group, the higher levels of TC, LDL-C and HDL-C were related with lower CVD death.
CONCLUSIONS
In community older adults, higher levels of TC and HDL-C were associated with lower CVD mortality in normal lipid reference range. Higher RC was associated with higher CVD mortality, which may be a better lipid indicator for estimating the CVD death risk in older adults.

Genomic studies of cancer cell alterations,such as mutations,copy number variations(CNVs),and translocations,greatly promote our understanding of the genesis and development of cancers.However,the 3D genome architecture of cancers remains less studied due to the complexity of cancer genomes and technical difficulties.To explore the 3D genome structure in clin-ical lung cancer,we performed Hi-C experiments using paired normal and tumor cells harvested from patients with lung cancer,combining with RNA sequenceing analysis.We demonstrated the feasibility of studying 3D genome of clinical lung cancer samples with a small number of cells(1×104),compared the genome architecture between clinical samples and cell lines of lung cancer,and identified conserved and changed spatial chromatin structures between normal and cancer sam-ples.We also showed that Hi-C data can be used to infer CNVs and point mutations in cancer.By integrating those different types of cancer alterations,we showed significant associations between CNVs,3D genome,and gene expression.We propose that 3D genome mediates the effects of cancer genomic alterations on gene expression through altering regulatory chromatin structures.Our study highlights the importance of analyzing 3D genomes of clinical cancer samples in addition to cancer cell lines and provides an integrative genomic analysis pipeline for future larger-scale studies in lung cancer and other cancers.

With continuous discovery of tumor immune targets and continuous changes in antibody research and development technology, antibody drugs are becoming more and more widely used in clinical practice. However, some targets are not only expressed on tumor cells, but also on red blood cells. Therefore, the clinical application of antibodies against the corresponding targets may interfere with the detection of blood transfusion compatibility, resulting in difficulty in blood matching or delay of blood transfusion. This consensus summarizes the current solutions for the interference of CD38 monoclonal antibody (CD38 mAb) in transfusion compatibility testing. After analyzing the advantages and disadvantages of different methods, polybrene and sulfhydryl reducing agents [dithiothreitol (DTT) or 2-mercaptoethanol (2-Me)], as a solution for CD38 mAb interference in blood compatibility testing, are recommended for Chinese patients, so as to eliminate blood transfusion interference produce by CD38 mAb and further provide a pre-transfusion workflow for clinicians and technicians in Department of Blood Transfusion.

Exposure to free silica induces silicosis and myofibroblasts are regarded as primary effector cells. Fibrocytes can differentiate into myofibroblast. Therefore, the present study was designed to investigate whether fibrocytes participate in silicosis. The rat model of silicosis was established. Hematoxylin-eosin stainings and Masson stainings were used to evaluate the histopathology and collagen deposition. Flow cytometry and immunofluorescence were performed to detect the number of fibrocytes and their contribution to myofibroblasts. Results showed that fibrocytes participate in silicosis. Trend analysis of different sources of myofibroblasts during silicosis indicated that fibrocytes and lung type II epithelial cell-derived myofibroblasts play an important role in the early stage of silicosis, while resident lung fibroblast-derived myofibroblasts play a predominant role during the fibrosis formative period.

7.Crystalline Silica Promotes Rat Fibrocyte Differentiation in Vitro, and Fibrocytes Participate in Silicosis in Vivo.

OBJECTIVEThe aim of this study was to investigate the effects of SiO2 on fibrocytes and whether fibrocytes participate in silicosis in vivo.
METHODSA macrophagocyte (AM)/fibrocyte coculture system was established, and AMs were treated with 100 μg/mL SiO2. Flow cytometry was used to detect the number of fibrocytes. Real-time PCR was performed to measure the expression of collagen I, collagen III, and α-SMA mRNA. The levels of collagen I, collagen III, and TGF-β1 protein were determined by ELISA. Immunohistochemical staining was performed to measure α-SMA protein expression. A rat silicosis model was induced by intratracheal instillation of SiO2. Lung histopathological evaluation was conducted using HE and Masson's trichrome staining after 1 and 9 weeks. The number of fibrocytes in peripheral blood or lung tissue of rat was detected by flow cytometry. Double-color immunofluorescence was applied to identify fibrocytes in the lung tissue.
RESULTSPeripheral blood monocytes were found to differentiate into fibrocytes in vitro in a time-dependent manner, and exposure to crystalline silica might potentiate fibrocyte differentiation. In addition, fibrocytes were able to migrate from peripheral blood to the lung tissue, and the number of fibrocytes was increased after SiO2 exposure.
CONCLUSIONSilica exposure potentiates fibrocyte differentiation, and fibrocytes may participate in silicosis in vivo.

To understand the molecular characteristics of the full-length genome of dengue serotype 2 virus (DENV-2) strains isolated in southwestern border region of Yunnan province,China,in 2013 and 2015 and their epidemiological feature.Dengue virus was cultured in C6/36 cell line.Viral RNA was extracted from isolates,then the full-length genome was amplified by RT-PCR.The homology and phylogenetic analyses were made on the nucleotide sequences by bioinformatics softwares including ClastalX1.83 and MEGA6 etc.Ten strains of DENV-2 were harvested from dengue fever cases,of these,6 strains from Ruili city of Dehong prefecture and 4 from Jinghong city of Xishuangbanna prefecture,Yunnan province.Phylogenetic tree and homology analysis based on structural and non-structural protein genes showed that 5 strains of the experimental virus belonged to Asian I genotype (AG-I) and the other 5 strains belonged to Cosmopolitan genotype (CG).Among them,the AG-I strains were isolated from Ruili in 2013 and 2015,and CG strains were isolated from Jinghong in 2015 as well as they have a closer genetic relationship with the strains isolated from Southeast Asia.Compared with prototype strain (New Guinea-C) of DENV-2,E and NS5 nucleotide homology of the Yunnan strains were 93.38%~93.77% and 92.80%~94.74% respectively.Yunnan strains compared with US_Hawaii strain,there were 44 and 150 different sites in amino acid of structural and non-structural proteins respectively.Only 29 and 86 different sites in amino acid of structural and non-structural proteins were found when compared to New Guinea-C strain,respectively.DENV-2 AG-I and CG transmitted in southwestern border region of Yunnan and AG-I was prevalent in Ruili in 2013 and 2015,and CG was prevalent in Jinghong in 2015.They have genetic diversity but multiple transmission sources were from neighboring countries of Southeastern Asia.It is obvious different in amino acid sites of structural and non-structural proteins between Yunnan strains and New Guinea-C strain.Their main amino acid sites for antigenicity,pathogenic and virulence of both AG-I and CG genotypes were shown without significant difference.
9.Effect on early tear film function of different adjustable suture in glaucoma trabeculectomy

Abstract?AlM:To compare the effects of different adjustable suture in glaucoma trabeculectomy on early tear film function.?METHODS:Sixty-eight cases of primary glaucoma ( 76 eyes) during January 2012 to June 2014 in our hospital were selected and divided into exposure conjunctival suture group ( 34 cases, 36 eyes ) and embedded conjunctival suture group ( 34 cases, 40 eyes ) according to treatment. Adjustable suture exposed conjunctival suture and embedding conjunctival suture was given to two groups, respectively. lntraocular pressure ( lOP ) before and after treatment 7, 14, 30d were observed and Schirmer test, tear break-up time, No Hikaru sensitivity and the occurrence of adverse reactions after treatment 1, 30d were recorded.?RESULTS: After the treatment, the mean lOP of two groups were decreased significantly ( P < 0. 05 ). The average lOP after treatment of 1d in the two groups were not statistically different (P>0. 05), after treatment 7, 14, 30d embedded conjunctival suture group was significantly higher than that of exposure conjunctival suture group ( P<0. 05). After 1d of treatment, Schirmer test, tear break-up time, No Hikaru sensitivity of two groups compared no significant difference (P<0. 05). After treatment 7, 14, 30d embedded conjunctival suture group Schirmer test, tear break- up time, was significantly superior to expose conjunctiva suture group (P<0. 05). The 30d package after treatment of conjunctival suture group buried adverse reaction rate was significantly lower than that of exposed conjunctival suture group (P<0. 05).? CONCLUSlON: Trabeculectomy operation with adjustable thread embedding conjunctival suture has few effects on the tear film function in patients with early postoperative complications, lower, and operation effect is better than that of exposed conjunctival suture.

Objective To investigate the effects of underwater navigation on the stress reaction and immune function in di-vers.Methods Brain load and stress reaction of 5 male divers working with different speeds, at different depths and different times both before and after underwater navigation were evaluated by using National Aeronautics and Space Administration-Task Load Index Scale.Changes in the concentrations of salivary cortisol and secretory immunoglobulin A ( sIgA) both before and after underwater train-ing were detected by radio-immunoassay.Results The subjective stress load experienced by the 5 divers reached peak value, when they came to the depth of 20 meters.The concentration of salivary cortisol during the first underwater training and at the depth of 20 m was increased significantly (P<0.05), however, no significant changes in the concentration of sIgA could be noted between different groups(P>0.05).Conclusion Stress reaction experienced by divers during underwater navigation increased with the increase in depth, but repeated training was beneficial to the recovery of stress reaction.
