1.Effect of Huangqin Qingre Chubi Capsules-containing Serum on CircRNA_0001543/NF-κB Expression in Co-cultured PBMCs and Human FLSs from Patients with Ankylosing Spondylitis

ObjectiveThis study aims to explore the effects of Huangqin Qingre Chubi capsules-containing serum on the expression of CircRNA_0001543/nuclear factor-kappa B (NF-κB) in co-cultured peripheral blood mononuclear cells (PBMCs) and human fibroblast-like synoviocytes (FLSs) from patients with ankylosing spondylitis (AS). MethodsVenous blood was collected from patients with AS to isolate PBMCs. FLSs were co-cultured with AS patients' PBMCs, and FLSs were harvested after co-culture for subsequent experiments. The normal control group consisted of normal FLSs, while the model group comprised co-cultured AS PBMCs and FLSs to simulate AS pathology. The Huangqin Qingre Chubi capsules group involved adding Huangqin Qingre Chubi capsules-containing serum to the co-cultured cells(6.48 g·kg-1). To investigate the effect of HQC-containing serum on the viability of co-cultured cells, and the experiment was divided into the following groups based on the dilution concentration: blank group, 10% HQC group, 20% HQC group, and 30% HQC group.To study the influence of the optimal concentration of HQC-containing serum on cytokine and pathway indicators in each group, the experiment was divided into three groups: normal group, model group, and optimal concentration HQC-containing serum group.For the validation of the transfection efficiency of the CircRNA_0001543 interference plasmid, the experiment was divided into the following groups: blank group, si-NC group (with transfection reagent), si-circ_0001543-1 group (with transfection reagent and interference plasmid No. 1 targeting circ_0001543), si-circ_0001543-2 group (with transfection reagent and interference plasmid No. 2 targeting circ_0001543), and si-circ_0001543-3 group (with transfection reagent and interference plasmid No. 3 targeting circ_0001543).For the validation of the transfection efficiency of the CircRNA_0001543 overexpression plasmid, the experiment was divided into the following groups: blank group, OE-NC group (with transfection reagent), and OE-circ_0001543 group (with transfection reagent and overexpression plasmid targeting circ_0001543).To study the effects of CircRNA_0001543 interference/overexpression on cytokine and pathway indicators in each group, the experiment was divided into the following groups: si-NC group, si-CircRNA_0001543 group, OE-NC group, and OE-CircRNA_0001543 group. Enzyme-linked immunosorbent assay (ELISA) was used to detect levels of interleukin-1β (IL-1β), IL-10, IL-37, and tumor necrosis factor-α (TNF-α). Real-time quantitative polymerase chain reaction (Real-time PCR) was utilized to measure the expression of CircRNA_0001543, IκBα, and NF-κB p65. ResultsAfter 48 hours, 30% Huangqin Qingre Chubi Capsules-containing serum significantly inhibited the proliferation of co-cultured PBMCs and FLSs, which was determined to be the optimal experimental drug-containing serum concentration. Compared with those in the normal group, the expressions of NF-κB p65 mRNA, IκBα mRNA, IL-1β, and TNF-α in the model group were significantly increased (P<0.01), while the expressions of CircRNA_0001543 mRNA, IL-10, and IL-37 were significantly decreased (P<0.01). Compared with those in the model group, the expressions of NF-κB p65 mRNA, IκBα mRNA, IL-1β, and TNF-α in the Huangqin Qingre Chubi Capsules-containing serum group were significantly decreased (P<0.05), and the expressions of CircRNA_0001543 mRNA, IL-10, and IL-37 were significantly increased (P<0.05), with the most prominent changes in the 30% drug-containing serum group (P<0.01). Compared with that in the si-NC group, the expression of CircRNA_0001543 was significantly reduced in the si-CircRNA_0001543 group (P<0.01). Compared with that in the OE-NC group, the expression of CircRNA_0001543 was significantly increased in the OE-CircRNA_0001543 group (P<0.01), indicating that the si-CircRNA_0001543 and OE-CircRNA_0001543 plasmids were successfully transfected. Based on the optimal drug-containing serum of Huangqin Qingre Chubi Capsules, si-CircRNA_0001543 transfection led to significantly increased expressions of NF-κB p65 mRNA, IκBα mRNA, IL-1β, and TNF-α and decreased the expressions of IL-10 and IL-37 (P<0.01). In contrast, OE-CircRNA_0001543 transfection significantly decreased the expressions of NF-κB p65 mRNA, IκBα mRNA, IL-1β, and TNF-α (P<0.01) and increased the expressions of IL-10 and IL-37 (P<0.01). ConclusionHuangqin Qingre Chubi capsules-containing serum can improve immune inflammation in AS by increasing the expression of CircRNA_0001543, regulating the NF-κB pathway, suppressing pro-inflammatory cytokines, and enhancing anti-inflammatory cytokine expression.
2.Synaptic Vesicle Glycoprotein 2A Slows down Amyloidogenic Processing of Amyloid Precursor Protein via Regulating Its Intracellular Trafficking.

OBJECTIVE:
To reveal the effects and potential mechanisms by which synaptic vesicle glycoprotein 2A (SV2A) influences the distribution of amyloid precursor protein (APP) in the trans-Golgi network (TGN), endolysosomal system, and cell membranes and to reveal the effects of SV2A on APP amyloid degradation.
METHODS:
Colocalization analysis of APP with specific tagged proteins in the TGN, ensolysosomal system, and cell membrane was performed to explore the effects of SV2A on the intracellular transport of APP. APP, β-site amyloid precursor protein cleaving enzyme 1 (BACE1) expressions, and APP cleavage products levels were investigated to observe the effects of SV2A on APP amyloidogenic processing.
RESULTS:
APP localization was reduced in the TGN, early endosomes, late endosomes, and lysosomes, whereas it was increased in the recycling endosomes and cell membrane of SV2A-overexpressed neurons. Moreover, Arl5b (ADP-ribosylation factor 5b), a protein responsible for transporting APP from the TGN to early endosomes, was upregulated by SV2A. SV2A overexpression also decreased APP transport from the cell membrane to early endosomes by downregulating APP endocytosis. In addition, products of APP amyloid degradation, including sAPPβ, Aβ 1-42, and Aβ 1-40, were decreased in SV2A-overexpressed cells.
CONCLUSION
These results demonstrated that SV2A promotes APP transport from the TGN to early endosomes by upregulating Arl5b and promoting APP transport from early endosomes to recycling endosomes-cell membrane pathway, which slows APP amyloid degradation.

Standardised emergency management protocols for hand injury in microsurgery is critical, as it directly determines ultimate clinical outcomes. This consensus consolidates expert insights regarding diagnostic and treatment procedure for hand injury in microsurgery, emergency support protocols and key points of emergency workflow optimisation. It summarises the opinions of experts and puts forward standardised recommendations to guide clinical practice in microsurgical treatment process, so as to further improve the quality of treatment for hand injury in microsurgery and maximise the protection of limb function and quality of life of patients.
4.Analysis of the Symptom Improvement Effect of Self-made Eczema and Dermatitis Conditioning Formula Combined with Qingpeng Ointment on Damp Heat Intrinsic Eczema

Objective:To explore the symptom improvement effect of a self-made eczema and dermatitis conditioning formula combined with Qingpeng ointment on damp heat type eczema.Methods:A prospective study was conducted on 70 patients with damp heat type eczema ffrom June 2023 to August 2024.Random number table method was used to divide them into two groups.Among them,35 cases were in the single group and treated with Qingpeng plaster alone,while the remaining 35 cases were in the combination group and treated with a self-made eczema and dermatitis conditioning formula orally combined with Qingpeng plaster.After one month of treatment,the clinical efficacy,TCM syndrome scores before and Post-treatment,disappearance time of main symptoms,and the long-term recurrence rate was also compared.Results:The total effective rate of the combined group was 91.43%,which was higher than that of the single group(71.43%,P<0.05);Before treatment,there was no significant difference in the scores of TCM syndromes between the two groups(P>0.05).Post-treatment,the scores of TCM syndromes related to the main symptoms(skin erythema,papules,blisters,obvious exudation,severe itching,and burning sensation in the skin lesions)and secondary symptoms(dry mouth,bitter mouth,yellow and red urine,sticky stool or constipation)in both groups decreased,and the combined group was lower than the single group(P<0.05);The disappearance time of skin burning sensation,exudation,erythema,itching,and skin lesion related symptoms in the combined group was shorter than that in the single group(P<0.05);Two groups of patients were followed up for six months Post-treatment,and there were no lost cases during the follow-up period.There was no difference between the combined group and the single group(x2=3.214,P=0.073).Conclusion:The combination of self-made eczema and dermatitis conditioning formula and Qingpeng ointment can improve the clinical efficacy of damp heat type eczema,reduce its TCM syndrome score,shorten the duration of main symptoms,and lower the long-term recurrence rate of patients.The mechanism may be related to the regulation of T lymphocyte subsets in eczema patients by the self-made eczema and dermatitis conditioning formula,and the reduction of the body's inflammatory response.
5.Fucoidan Provokes Ferroptosis via Inhibition of the PI3K/Akt Signaling Pathway in Human Osteosarcoma 143B Cells

Fucoidan(FUC)is a natural seaweed-derived drug.Previously,our experiments have shown that FUC can significantly inhibit the cell viability of human osteosarcoma 143B cells and induce cell death,but the mechanism remains unclear.Ferroptosis,a novel form of cell death,has emerged as an important target for tumor therapy.This study aims to investigate whether FUC induces ferroptosis of 143B cells and elucidate its underlying molecular mechanisms.CCK-8 and LDH assays result showed that FUC(10,100,400 μg/mL)significantly reduced cell viability of 143B cells and induced cell death.Calce-in-AM staining,FeRhoNox-1 staining,and C11 BODIPY 581/591 staining indicated that FUC obviously increased the levels of labile iron pool(LIP),Fe2+,and lipid reactive oxygen species(Lip ROS)in 143B cells.Chemical colorimetric analysis revealed that FUC markedly decreased intracellular Glutathi-one(GSH)contents.Real-time quantitative PCR showed that FUC dramatically reduced the mRNA lev-els of ferroptosis-related factors solute carrier family 7 member 11(SLC7A11)and glutathione peroxidase 4(GPX4),while increasing the mRNA levels of prostaglandin endoperoxide synthase 2(PTGS2)and acyl-CoA synthetase long-chain family member 4(ACSL4).Western blotting analysis demonstrated that FUC significantly reduced the protein levels of SLC7A11 and GPX4,and the ratios of p-PI3K/PI3K,p-AktSer473/Akt,and p-AktThr308/Akt,but increased the protein level of ACSL4.Immunofluorescence staining showed that FUC obviously inhibited the nuclear translocation of p-AktSer473.The ferroptosis in-hibitor ferrostatin-1(Fer-1)and iron chelator deferoxamine(DFO)remarkably suppressed cell death in-duced by FUC in 143B cells.Additionally,the PI3K/Akt pathway activator 740Y-P significantly inhibi-ted FUC-induced iron overload and lipid peroxidation in 143B cells,and restored the protein levels of SLC7A11 and GPX4.In conclusion,FUC can induce ferroptosis of 143B cells by inhibiting the PI3K/Akt signaling pathway,which may be a potential target for the prevention and treatment of osteosarcoma.
6.Application of the new "D 3-teaching" mode in enhancing ophthalmic surgery skills

Objective:To explore and evaluate the effectiveness of the new "D 3-teaching" mode in surgical training for ophthalmology standardized residency training. Methods:A total of 40 residents at the Eye Center of The Second Affiliated Hospital of Zhejiang University School of Medicine who participated in the standardized residency training program in Zhejiang Province from September 2020 to June 2024 were selected as the "D 3-teaching" group. This group received an innovative surgical training mode integrating digital resources, three-dimensional teaching, and dry lab simulation. Moreover, another 42 residents who had completed the standardized training program in the 2018 and 2019 were selected as the traditional teaching group. This group received traditional teaching methods involving theoretical lectures, text- and image-based teaching, and video demonstrations. The effectiveness of both teaching modes was assessed by comparing the pass rates of examinations, ICO-OSCAR skill mastery rates, learning periods, and satisfaction survey. Results:After training, there was no significant difference in the pass rates between the two groups. However, the skill mastery rate in the "D 3-teaching" group was significantly higher than that of the traditional teaching group (28/40, 70.00% vs. 15/42, 35.71%, χ2=9.66, P=0.002). Additionally, the learning period in the "D 3-teaching" group was significantly shorter than that of the traditional teaching group [(13.14±5.91) months vs. (21.33±7.73) months, t=3.88, P<0.001]. Furthermore, the scores for satisfaction with all teaching parameters in the "D 3-teaching" group were significantly higher than those in the traditional teaching group. Conclusions:The "D 3-teaching" mode is an innovative surgical teaching method in ophthalmology standardized residency training that significantly improves trainees' skill acquisition, enhances training outcomes, shortens the learning period, and has received high recognition from trainees. This mode is worthy of wider application and promotion.
