ObjectiveThis paper aims to conduct a secondary analysis of a randomized controlled trial on the treatment of myelodysplastic syndrome (MDS) with deficiency of both the spleen and kidney and blockage of toxin and blood stasis with an arsenic-containing traditional Chinese medicine compound, by applying the mixed model for repeated measure (MMRM) and the method of stratified composite outcome with win ratio. The analysis includes the assessment of hematological efficacy and the composite outcome evaluation of adverse reactions, so as to more comprehensively assess the therapy of this regimen. MethodsThe MMRM and win ratio methods were used to evaluate the efficacy of a prospective,multi-center,double-blind,randomized controlled study. The blood routine (hemoglobin concentration, neutrophil count, and platelet count) and biochemical indexes (aspartate aminotransferase, alanine aminotransferase, serum creatinine, and serum ferritin) of the patients were detected at the time of enrollment and at the end of each course of treatment in the laboratory department of Xiyuan Hospital. The patients' syndromes at the time of enrollment and after treatment were recorded and scored according to the therapy standard of traditional Chinese medicine for diseases and syndromes. MMRM was used to analyze the blood routine indexes of the experimental group and the control group. This method has the advantages of high data reliability and dynamic efficacy under intervention and time. The win ratio method was used to evaluate the composite outcome of traditional Chinese medicine syndrome scores and biochemical indexes according to the priority and to verify the clinical safety of arsenic-containing traditional Chinese medicine compound. ResultsThe results of MMRM analysis showed that the hemoglobin concentration of patients in the group with arsenic-containing traditional Chinese medicine compound increased significantly compared with that before treatment in the group, while that in the placebo group decreased significantly (P<0.01). When compared with that after treatment in the placebo group, the hemoglobin concentration of patients in the group with arsenic-containing traditional Chinese medicine compound increased significantly, and the mean difference of least squares (LS) was statistically significant (P<0.01). When compared with those before treatment in the group, there were no statistically significant differences in the neutrophil count and platelet count in both groups. After treatment, there were no statistically significant differences in the neutrophil count, platelet count, and the mean difference of LS between the two groups. The analysis results of win ratio showed that the group with arsenic-containing traditional Chinese medicine compound had a significant advantage in the comparison of composite outcomes, with a win ratio (95% CI) of 2.01 (1.24-3.27) (P<0.01),and that the possibility of "winning" in terms of safety was 2.01 times that of the placebo group. The safety advantage of the group with arsenic-containing traditional Chinese medicine compound mainly came from the traditional Chinese medicine syndrome scores, renal function indexes, and iron reserve capacity indexes, and the number of winning times was less than that of losing times in the comparison of liver function outcomes. ConclusionThe MMRM analysis proves that the arsenic-containing traditional Chinese medicine compound can significantly improve the hemoglobin concentration of patients with myelodysplastic syndrome with refractory cytopenia and multilineage dysplasia (MDS-RCMD) of the type of deficiency of both the spleen and kidney and blockage of toxin and blood stasis. This conclusion is not interfered with by time trends and individual relationships and methodologically improves the credibility of the therapy of the arsenic-containing traditional Chinese medicine compound in treating MDS. Four outcomes are evaluated by the win ratio method, namely traditional Chinese medicine syndromes, liver function, renal function, and iron reserve capacity, proving that the arsenic-containing traditional Chinese medicine compound has the comprehensive advantages of improving the survival quality of the patients and reducing adverse reactions. The win ratio outcome provides clear comparative indexes for the evaluation of adverse reactions, making it easier for regulatory authorities, medical staff, and patients to understand the safety of the arsenic-containing traditional Chinese medicine compound in clinical application.

Given the lack of annotations for key lung organs and tissues in existing public datasets, this study collected 863 cases of chest CT scan images and constructed the first comprehensive dataset containing annotations of pulmonary vessels, airways, and nodules using a semi-automated method that combines computer vision algorithms with manual corrections by radiologists. On this basis,a lung nodule segmentation model based on multi-task learning is proposed.By incorporating annotations of pulmonary vessels(pulmonary arteries and veins)and the trachea to enhance model's ability to learn lung features, the proposed model reduces the false discovery rate in lung nodule detection, and improves generalization ability.Additionally, the use of larger image patches further optimizes model performance. The trained VAAN_128 model achieves the best performance, with a Dice coefficient of 0.694 and a false discovery rate of 0.210 for lung nodule segmentation. Moreover,it simultaneously provides accurate segmentation results of pulmonary vessels and the trachea, assisting in the formulation of more precise diagnosis and treatment plans.Based on the VAAN_128 model,a software system for navigation and localization in biopsy procedures is developed. In clinical practice, this system can assist physicians in accurately locating lung nodules, distinguishing critical tissues, and improving preoperative planning efficiency. This provides precise and efficient technical support for early diagnosis and disease monitoring of lung diseases, and is of great significance for path planning in clinical navigation system and future lung imaging research.

Objective:To analyze the perioperative risks and prognosis in patients with differentiated thyroid cancer (DTC) complicated by Graves' disease (GD) .Methods:Clinical data of 880 DTC patients who underwent radical thyroidectomy at the Department of General Surgery, the First Medical Center of Chinese PLA General Hospital between Jan. 2016 and Dec. 2022 was retrospectively collected. The cohort included 119 DTC patients with GD (hyperthyroid group) and 761 DTC patients with normal thyroid function without GD (normal group) . Use propensity score matching (PSM) was used to obtain cohorts with balanced baseline characteristics between the hyperthyroid and normal groups. Perioperative risks and prognos were compared between the two groups. Results:For perioperative risk comparison, after 1∶2 matching, 116 patients were included in the hyperthyroid group and 232 patients in the normal group. The operation time was longer in the hyperthyroid group compared to normal group ([107.26±30.27]min vs. [96.45±32.51]min, P=0.003) . There were no statistical differences between the two groups in intraoperative blood loss, postoperative drainage volume or postoperative hospital stay ( P>0.05) . There were no statistical difference in the incidence of complications such as temporary or permanent hypoparathyroidism, temporary or permanent recurrent laryngeal nerve injury, and lymphatic leakage between the two groups. For prognostic comparison, after 1∶2 matching, 117 cases were included in the hyperthyroid group and 234 cases in the normal group. During follow-up period, 16 patients experienced DTC recurrence, including 5 cases in the hyperthyroid group and 11 cases in the normal group. There was no statistical difference in reccurence-free survival rates between the two groups (5-year RFS: 95.7% vs. 95.3%, P=0.907) . Multivariate Cox regression analysis revealed GD was not the risk factor for DTC recurrence. Conclusions:With standardized and adequate preoperative preparation, the perioperative risks of DTC patients complicated by GD were not increased compared to patients without GD. Additionally, GD does not increase the risk of postoperative tumor recurrence in DTC patients.

Gefitinib,a first-generation epidermal growth factor receptor(EGFR)tyrosine kinase inhibi-tor(TKI),exerts significant therapeutic efficacy in the treatment of non-small cell lung cancer(NSCLC)by selectively targeting mutant forms of EGFR.However, the development of acquired resistance signifi-cantly limits its long-term clinical benefits.Cell division cycle 20(CDC20),a key regulator of cell cycle progression, has been implicated in the tumorigenesis and progression of various malignancies.Neverthe-less,its role and underlying regulatory mechanisms in the acquisition of drug resistance in NSCLC remain largely unexplored. This study aimed to elucidate the molecular mechanisms by which CDC20 contributes to gefitinib resistance in NSCLC.Gefitinib-resistant cell lines,HCC827/GR(ICs0 0.05±0.01 μmnol/L vs 36.24±6.21 μmol/L)and PC9/GR(IC50 0.02±0.01 μmol/L vs 25.36±5.57 μmol/L),were established through stepwise drug induction, exhibiting markedly increased IC50 values compared to their parental counterparts.Bioinformatics analysis revealed that the transcriptional level of CDC20 is signifi-cantly upregulated in lung cancer tissues and is associated with poor patient prognosis.Western blotting analysis confirmed elevated CDC20 protein levels in the resistant HCC827/GR and PC9/GR cells relative to the parental HCC827 and PC9 cells. To further investigate the functional role of CDC20 in NSCLC ge-fitinib resistance,CDC20 was knocked out using CRISPR/Cas9 technology. This genetic intervention sig-nificantly restored gefitinib sensitivity in resistant cells(IC 50 37.08±6.15 μmol/L vs 10.49±1.83μmol/L,7.23±1.55 μamol/L),while concurrently promoting apoptosis and inducing G2/M phase cell cycle arrest.Conversely,CDC20 overexpression decreased drug sensitivity in parental cells and notably attenuated gefitinib-induced apoptosis and cell cycle arrest.Mechanistically,CDC20 depletion was found to inhibit activation of the PI3K/Akt/mTOR signaling pathway, upregulate pro-apoptotic proteins such as cleaved-Caspase 3 and Bax,and downregulate the anti-apoptotic protein Bcl-2.Collectively,these find-ings demonstrate that CDC20 mediates gefitinib resistance in NSCLC through modulation of the PI3K/Akt/mTOR signaling pathway, thereby identifying CDC20 as a potential therapeutic target for overcoming resistance to EGFR-targeted therapies.
