1.The Association of Iodixanol With Renal and Cardiovascular Safety in Patients With ST-Elevation Myocardial Infarction Undergoing Primary Percutaneous Coronary Intervention:A Prospective Cohort Study

Background:
This study was performed to characterize the incidence, costs, and risk factors associated with renal and cardiovascular adverse outcomes following primary percutaneous coronary intervention (pPCI) in patients with ST-elevation myocardial infarction (STEMI).
Methods:
Patients with STEMI who underwent pPCI using iso-osmolar contrast were enrolled at 39 centers. The incidence of acute kidney injury (AKI) and major adverse renal and cardiovascular events (MARCE) was analyzed, as well as inpatient costs. Logistic regression analysis was performed to identify risk factors.
Results:
Among 2,293 patients, the incidence of AKI and MARCE within 72 hours post-pPCI was 4.14% (n = 95) and 4.40% (n = 101), respectively. AKI and/or MARCE were associated with systolic blood pressure (AKI: odds ratio [OR], 1.009; 95% confidence interval [CI], 1.000–1.018), hypertension (AKI: OR, 1.815; 95% CI, 1.133–2.906; MARCE: OR, 1.760;95% CI, 1.118–2.769), anterior wall infarction (AKI: OR, 1.895; 95% CI, 1.196–3.004; MARCE:OR, 1.939; 95% CI, 1.240–3.032), Killip class (AKI: OR, 1.465; 95% CI, 1.117–1.922; MARCE:OR, 1.467; 95% CI, 1.131–1.903), and serum creatinine (SCr; MARCE: OR, 1.006; 95% CI, 1.000–1.012). Hospitalization costs for patients with STEMI who developed AKI or MARCE were significantly higher than for those without AKI (9,595 ± 5,795 vs. 8,279 ± 3,872 USD, P = 0.003) or without MARCE (9,890 ± 5,616 vs. 8,255 ± 3,859 USD, P < 0.001).
Conclusions
In patients with STEMI undergoing pPCI with iso-osmolar contrast, the incidence of AKI and MARCE was associated with higher hospitalization costs. Systolic blood pressure, hypertension, anterior wall infarction, Killip class, and SCr were identified as risk factors for these outcomes.
2.Efficacy and safety of a facilitated percutaneous coronary intervention with half-dose recombinant staphylokinase in ST-segment elevation myocardial infarction

Objective To investigate the clinical efficacy and safety of facilitated percutaneous coronary intervention(PCI)with half-dose recombinant staphylokinase(r-SAK)in patients with ST-segment elevation myocardial infarction(STEMI)who are expected to undergo PCI within 120 minutes.Methods From October 2021 to August 2022,a total of 200 STEMI patients in eight centers were included and randomly assigned in a 1﹕1 ratio to either r-SAK group or control group.Patients received loading doses of aspirin and ticagrelor and intravenous heparin and were randomized to receive an intravenous bolus of either 5 mg r-SAK or normal saline prior to PCI.The outcomes were set as ST-segment resolution(STR)at 60-90 minutes after PCI,the proportion and transition of pathological Q waves on the 5th day after PCI,and the proportion of high-sensitivity cardiac troponin T(hs-cTnT)peaking within 12 hours of onset.The safety outcome was major bleeding events defined as Bleeding Academic Research Consortium(BARC)≥type 3 bleeding during hospitalization.Results Compared with the control group,the r-SAK group had a higher proportion of STR≥70%within 60-90 minutes after PCI(58.3%vs.40.3%,P=0.009);a lower proportion of pathological Q waves(59.1%vs.74.1%,P=0.040);a lower rate of Q wave progression(14.8%vs.43.2%,P<0.001);a higher rate of Q wave disappearance(12.5%vs.3.7%,P=0.027);and a higher proportion of hs-cTnT peaking within 12 hours of symptom onset[31/40(77.5%)vs.17/33(51.5%),P=0.027].Regarding the safety outcome,no significant difference in BARC≥type 3 bleeding was found between the two groups during hospitalization(P>0.05).Conclusions For STEMI patients who were expected to undergo primary PCI within 120 minutes of symptom onset,the facilitated PCI with half-dose r-SAK significantly increased the proportion of STR≥70%at 60-90 minutes after PCI,reduced the formation of pathological Q waves,and shortened the time to peak hs-cTnT,without increasing the risk of bleeding,which should be an alternative reperfusion strategy worthy of further study.

The development of Traditional Chinese Medicine(TCM)medical alliances plays a pivotal role in enhancing grassroots TCM service capabilities and meeting public demand for TCM healthcare.However,challenges persist in establishing these alliances,including insufficient Party leadership at primary TCM institutions and deficiencies in clinical services,talent de-velopment,and emergency care capacity.This study examines innovative Party building approaches in public hospitals within the new era context,analyzing practical cases of alliance development.Our findings demonstrate that integrating Party building into the governance structure of medical alliances not only strengthens Party leadership at primary TCM institutions but also significant-ly promotes TCM service development.Systematic analysis of case hospital practices reveals several key insights.Firstly,strengthening top-level design through Party committee leadership is crucial.Secondly,addressing the most pressing public healthcare concerns with genuine commitment forms the foundation.Thirdly,deep integration of Party building with core medical services represents the essential approach.Lastly,policy-responsive innovation based on consolidated achievements serves as the key driver.

6.Impact of changes in tear cytokines on the ocular surface of patients with advanced refractory glaucoma after surgery

Objective To investigate the postoperative changes of tear cytokines in patients with advanced refractory glaucoma and their impact on ocular surface-related indicators.Methods In total,120 patients(240 eyes)with advanced refractory glaucoma admitted from January 2023 to June 2024 were selected as the observation group,all of whom underwent glaucoma drainage device(GDD)implantation.Another 85 healthy individuals(170 eyes)who underwent ophthalmic examinations during the same period were selected as the control group.The changes in tear cytokine levels[interleukin-1α(IL-1α),interleukin-2(IL-2),tumor necrosis factor-α(TNF-α),transforming growth factor-β(TGF-β),interleukin-10(IL-10),vascular endothelial growth factor(VEGF)]and ocular surface-related indicators[tear film break-up time(BUT),Schirmer I test(SIt)values,and corneal fluorescein staining(CFS)scores]were observed and recorded at 1 d before surgery and at 1,7,30,and 60 d after surgery in the observation group,and the levels of the above indicators were compared with those in the control group.Pearson correlation analysis was used to investigate the correlation between changes in IL-1α,IL-2,TNF-α,TGF-β,IL-10,VEGF levels and SIt values,CFS scores,and BUT.Results There were significant differences in the levels of IL-1α,IL-2,TNF-α,TGF-β,IL-10,and VEGF in the observation group when compared at 1 d before surgery and at 1,7,and 30 d after surgery(P<0.05).There was no significant difference in the levels of IL-1α,IL-2,TNF-α,TGF-β,IL-10,and VEGF in the observation group when compared at 1 d before surgery and at 60 d after surgery(P>0.05),nor significant difference compared with the control group(P>0.05).The differences in BUT,SIt values,and CFS scores in the observation group at 1 d before surgery and at 1,30,and 60 d after surgery were statistically significant(P<0.05),and the levels of the above indicators in the observation group were comparable to those in the control group at 60 d after surgery(P>0.05).Pearson correlation analysis showed that IL-1α,IL-2,TNF-α,TGF-β,IL-10,and VEGF in the observation group were positively correlated with SIt value and CFS score,and negatively correlated with BUT(P<0.05).Conclusion In the early postoperative period(1-30 d),the levels of IL-1α,IL-2,TNF-α,TGF-β,IL-10,and VEGF in the tears of patients with advanced refractory glaucoma are significantly increased,and returned to preoperative levels at 60 d after surgery.The above cytokines are positively correlated with SIt values and CFS scores,and negatively correlated with BUT.

8.The feasibility of bone mineral density screening using a proximal femur radiomics model derived from abdomen-pelvic CT scans

Objective To develop an automated bone mineral density(BMD)assessment model based on proximal femur images from abdomen-pelvic CT scans and to analyze its application value in opportunistic osteoporosis(OP)screening.Methods A retrospective selection was conducted on 351 patients who underwent abdomen-pelvic plain CT examination.The patients were randomly divided into training set(n=245)and test set(n=106)in a ratio of 7∶3.All images were transferred to a quantitative computed tomography(QCT)post-processing workstation to measure the BMD of the left proximal femur.According to the QCT BMD T-score,the patients were divided into osteoporosis(T-score-2.5),osteopenia(-2.5<T-score<-1)and normal bone density(T-score≥-1).The left proximal femur was dissected using an automatic segmentation model,and two three-class BMD assessment radiomics models were constructed using random forest(RF)and logistic regression(LR)classifiers,respectively.The receiver operating characteristic(ROC)curves were generated,and the area under the curve(AUC),sensitivity,specificity and other metrics were calculated to evaluate the diagnostic performance of the two models.The DeLong test was used to compare differences between the models.Results In the test set,the AUC of the RF and LR models for identifying osteoporosis were 0.953 and 0.954,respectively.The AUC for identifying osteopenia were 0.894 and 0.870,and the AUC for identifying normal bone density were 0.975 and 0.982,respectively.The comparison of model performance showed no statistically significant differences between the RF and LR models in identifying the three bone states in both the training and test sets(P>0.05).Conclusion Both the RF and LR radiomics models,constructed based on abdomen-pelvic plain CT scans,can be used for opportunistic BMD screening with high diagnostic efficiency.
9.Efficacy of remimazolam-based anesthesia in daytime laparoscopic cholecystectomy: a multicenter, non-inferiority, randomized controlled trial

Objective:To evaluate the efficacy of remimazolam-based anesthesia in daytime laparoscopic cholecystectomy.Methods:In this multicenter, non-inferiority, randomized controlled trial, 300 American Society of Anesthesiologists Physical Status classification Ⅰ or Ⅱ patients of either sex, aged 18-60 yr, with body mass index of 18-28 kg/m 2, who underwent daytime laparoscopic cholecystectomy under general anesthesia with tracheal intubation at Sir Run Run Shaw Hospital Affiliated to Zhejiang University School of Medicine, the Fourth Affiliated Hospital of Zhejiang University School of Medicine, Jinhua Hospital Affiliated to Zhejiang University School of Medicine, Hangzhou First People′s Hospital Affiliated to Westlake University School of Medicine, Ningbo No. 2 Hospital, and the Second Affiliated Hospital of Nanchang University from August 2021 to August 2023, were selected and divided into 2 groups ( n=150 each) using a random number table method: remimazolam group (R group) and propofol group (P group). Anesthesia was induced as follows: Sufentanil was intravenously injected at a rate of 0.5 μg/kg, remimazolam was intravenously injected at a rate of 0.3 mg/kg in group R, propofol was intravenously injected at a rate of 2.0-2.5 mg/kg in group P, and cisatracurium besilate was intravenously injected at a rate of 0.2 mg/kg after loss of consciousness in two groups. The patients were mechanically ventilated after tracheal intubation. Anesthesia was maintained as follows: Remimazolam was intravenously injected at a rate of 0.5-1.0 mg·kg -1·h -1 in group R, propofol was intravenously injected at a rate of 4-10 mg·kg -1·h -1 in group P, and remifentanil was intravenously infused at a rate of 0.25-2.00 μg·kg -1·min -1, maintaining intraoperative bispectral index value of 40-60. The success rate of sedation was recorded, and non-inferiority tests were conducted. The time to loss of consciousness, emergence time, extubation time, recovery time of orientation, time of stay in post-anesthesia care unit and occurrence of delayed emergence were recorded. Liver function and renal function were measured before operation and within 24 h after operation. The occurrence of abnormal alanine transaminase, abnormal aspartate transaminase, abnormal creatinine and abnormal urea was recorded. The occurrence of adverse reactions during and after operation was recorded. Results:The success rates of sedation were 98.6% and 99.3% in group R and group P, respectively, there was no statistically significant difference in the success rate of sedation between the two groups ( P>0.05), and the difference in the success rates of sedation between the two groups was -0.007 (95% confidence interval-0.0301-0.0161), which met the pre-set non-inferiority criteria(95% confidence interval >-0.055). Compared with group P, the time to loss of consciousness and recovery time of orientation were significantly prolonged, and the incidence of delayed emergence was increased ( P<0.05), and no statistically significant changes were found in the emergence time, extubation time, time of stay in post-anesthesia care unit and severity of postoperative nausea and vomiting in group R ( P>0.05). There was no statistically significant difference in the abnormal rates of alanine transaminase, aspartate transaminase, creatinine and urea before and after operation between the two groups ( P>0.05). Conclusions:The efficacy of remimazolam-based anesthesia in daytime laparoscopic cholecystectomy is not inferior to that of propofol-based anesthesia.

With the growing emphasis on maternal and child oral health, the significance of managing oral health across preconception, pregnancy, and infancy stages has become increasingly apparent. Oral health challenges extend beyond affecting maternal well-being, exerting profound influences on fetal and neonatal oral development as well as immune system maturation. This expert consensus paper, developed using a modified Delphi method, reviews current research and provides recommendations on maternal and child oral health management. It underscores the critical role of comprehensive oral assessments prior to conception, diligent oral health management throughout pregnancy, and meticulous oral hygiene practices during infancy. Effective strategies should be seamlessly integrated across the life course, encompassing preconception oral assessments, systematic dental care during pregnancy, and routine infant oral hygiene. Collaborative efforts among pediatric dentists, maternal and child health workers, and obstetricians are crucial to improving outcomes and fostering clinical research, contributing to evidence-based health management strategies.
