1.Mechanism of tight junctional function injury of testicular Sertoli cells induced by high-fat diet based on NF-κB/NLRP3 signaling pathway

Aim To investigate the effect of high-fat diet on the tight junction function injury of Sertoli cells through the NF-κB/NLRP3 signaling pathway in mice and to explore the underlying mechanism.Methods Male C57BL/6J mice were fed with high-fat or normal diet for five months.The body and gonadal organ weight of mice were measured,and their indices were calculated.The sperm concentration,the sperm viabili-ty,the testicular histomorphology and the expression levels of tight junction proteins ZO-1,Occludin and Claudin-11 were measured.TM4 cells were treated with palmitic acid(PA)for 24 h.Cell viability was detected by CCK-8 method.Then,TM4 cells were di-vided into different groups treated with PA(0,50,100,200 and 300 μmnol·L-1),and the expression lev-els of tight junction proteins ZO-1,Occludin and Clau-din-11 were detected by Western blot.The tight junc-tion permeability of TM4 cells were detected by transepithelial electrical resistance(TEER)and FITC-dextran.The expression levels of mRNA and proteins for the NF-κB/NLRP3 pathway-related factors were de-tected by RT-qPCR and Western blot.Results The results from animal experiments showed that high-fat diet increased body weight and seminal vesicle weight of mice,and decreased testicular index,epididymal in-dex,sperm concentration and sperm motility of mice.High-fat diet also caused testicular tissue structure damage and down-regulated the expression levels of tight junction proteins ZO-1 and Occludin,without af-fecting the expression of Claudin-11.In vitro,PA sig-nificantly down-regulated the expression levels of ZO-1,Occludin and Claudin-11 in TM4 cells,increased the cell permeability,as well as up-regulated the mRNA and protein expression levels of NLRP3/NF-κB signa-ling pathway-related factors in TM4 cells.Conclusions High-fat diet can impair the function of tight junction of testicualr Sertoli cells,and the machanism may be related to the activation of the NF-κB/NLRP3 signaling pathway,resulting in Sertoli cell inflammation in mice.

3.Twelve cases of occupational acute 2- (2-thiophenyl) scopolamine glycolate poisoning

2- (2-thiophenyl) scopolamine glycolate is produced through an ester exchange reaction between scopine and methyl 2, 2-dithienylglycolate, and is used to synthesize tiotropium bromide. In this paper, the treatment process of 12 patients with acute 2- (2-thiophenyl) scopolamine glycolate poisoning was reviewed and their clinical data were analyzed. After a large amount of 2- (2-thiophenyl) scopolamine glycolate was absorbed through the respiratory tract, the poisoning patients presented with central nervous system injury as the main clinical manifestations, accompanied by symptoms such as blurred vision and irritability. After fluid replacement, ovulation induction and symptomatic supportive treatment, all patients improved and were discharged. The patients were followed up. After 6 months, only 3 patients had residual headache and dizziness. It is indicated that early intervention for acute 2- (2-thiophenyl) scopolamine glycolate poisoning has a good prognosis. Meanwhile, occupational protection should be strengthened to prevent the occurrence of poisoning.
4.Expression of Type 2 Innate Lymphoid Cell and IL-9 and Other Related Cytokines in Chronic Lymphocytic Leukemia

Objective:To explore the difference of type 2 innate lymphoid cell(ILC2),IL-9 and related cytokines between chronic lymphocytic leukemia(CLL)patients and normal individuals,as well as the correlation between ILC2 and IL-9 and other cytokines in CLL patients.Methods:Flow cytometry was used to detect the expression of ILC2 and regulatory T cells(Tregs)in peripheral blood of 26 CLL patients at initial diagnosis and 10 healthy controls.RT-qPCR was used to detected IFN-γ,TGF-β,IL-9 and IL-10 mRNA in peripheral blood mononuclear cell(PBMC).ELISA was used to detect serum IFN-γ,TNF-α,TGF-β,IL-9,IL-10 and IL-21.ILC2 and IL-9 were observed in the cervical lymph node of 12 CLL patients at initial diagnosis and 12 patients with reactive lymphoid hyperplasias by multiplex indirect immunofluorescence staining.Spearman test was used to analyze the correlation between peripheral blood ILC2 and IL-9,IL-9 and IL-21,IFN-γ mRNA and IL-10 mRNA in CLL patients.Pearson test was used to analyze the correlation between TNF-α and TGF-β in CLL patients.Results:Compared with control group,the proportions of ILC2 and Tregs were significantly increased in CLL group(both P<0.05).The mRNA expressions of IFN-γ,IL-9,IL-10 and TGF-β in PBMCs of CLL patients were significantly increased(all P<0.05).In CLL patients,the expressions of IFN-γ,TNF-α,TGF-β,IL-9 and IL-10 in serum were significantly increased(all P<0.01),while IL-21 slightly increased without statistical difference(P>0.05).In CLL patients,the peripheral blood ILC2 was positively related to IL-9(r=0.56),IL-9 was positively related to IL-21(r=0.397),IFN-γ mRNA was positively related to IL-10 mRNA(r=0.623),and TNF-α was positively related to TGF-β(r=0.577).Compared with reactive lymphoid hyperplasias patients,the mean fluorescence intensities of GAT A3 and CRTH2 representing ILC2 and IL-9 in cervical lymph nodes were significantly increased in the CLL group(all P<0.001),and showed colocalization.Conclusion:In CLL patients,the proportions of ILC2 and IL-9 in peripheral blood and lymph nodes increase,and ILC2 and IL-9 show colocalization in lymph nodes.There is a positive correlation between ILC2 and IL-9 in the peripheral blood of CLL patients,the ability of ILC2 to secrete IL-9 is increased,and ILC2 may affect the occurrence and development of CLL through IL-9.
5.Research Progress on the Correlation between Pathogenesis of Idiopathic Pulmonary Fibrosis Combined with Lung Cancer

Idiopathic pulmonary fibrosis(IPF)and lung cancer(LC)pose a serious threat to human health,the prognosis of IPF is poor,the 5-year survival rate is low,and the incidence rate and mortality of LC are high.At present,IPF combined with LC(IPF-LC)has attracted much attention,but the pathogenesis is unclear.Epidemiology shows that,the risk of LC in IPF patients is 7 times than that of the general population,the incidence rate varies in different regions,which is related to smoking rate and diagnostic criteria.In terms of pathogenesis,chronic inflammation,angiogenesis,signaling pathways,aging,gene mutations,epithelial mesenchymal transition(EMT),and immune dysfunction are common pathogenic factors of both,and pathological changes such as smoking and pulmonary fibrosis increase the risk of LC.Thoroughly studying these mechanisms is of great significance for the early diagnosis,treatment strategy formulation,and improvement of patient prognosis of IPF-LC.
6.Effectiveness of home management based on ecological systems theory in patients undergoing endoscopic colon polypectomy

Objective:To explore the effectiveness of home management based on ecological systems theory (EST) in the nursing of patients undergoing endoscopic colon polypectomy.Methods:Convenience sampling was used to select 562 patients undergoing endoscopic colon polypectomy admitted to the China-Japan Union Hospital of Jilin University from July 2023 to January 2024 as study subjects. The patients were divided into a control group ( n=280) and an intervention group ( n=282) . Control group was given routine postoperative nursing and intervention group was treated with EST-based home management on the basis of control group. The incidence of postoperative complications, colonoscopy review rate, and scores on the Self-Management Ability Scale (SMAS) , the Connor-Davidson Resilience Scale (CD-RISC) , and the Short Form-36 Health Survey (SF-36) before and after the intervention were compared between the two groups. Results:After intervention, the overall incidence of postoperative complications in the two groups was not statistically significant (χ 2=0.124, P>0.05) . The colonoscopy review rate of patients in intervention group was higher than that of control group, and the difference was statistically significant (χ 2=12.204, P<0.01) . After intervention, the scores of the dimensions in SMAS and the total scores of both groups were higher than those before intervention, and the scores of the dimensions in SMAS and the total scores of the patients in intervention group were higher than those in control group, and the differences were statistically significant (all P<0.05) . After intervention, the scores of the dimensions in CD-RISC and the total scores of the patients in both groups were higher than those before intervention, and the scores of the dimensions in CD-RISC and the total scores of the patients in intervention group were higher than those in control group, and the differences were statistically significant (all P<0.05) . After intervention, the scores of all dimensions in SF-36 of both groups were higher than those before intervention, and the scores of all dimensions in SF-36 in intervention group were higher than those in control group, and the differences were statistically significant (all P<0.05) . Conclusions:EST-based home management is effective in patients undergoing endoscopic colon polypectomy, and can enhance postoperative patients' compliance with colonoscopy review, increase their self-management ability, and improve their psychological condition and quality of life.
7.Comparison of Three Drowning-related Plankton Testing Methods in Drowning Diagnosis

Objective To compare the application effects of plankton multiplex polymerase chain reac-tion-capillary electrophoresis(PCR-CE),SYBR Green Ⅰ real-time quantitative PCR(qPCR)and microwave digestion-vacuum filtration-automated scanning electron microscopy(MD-VF-Auto SEM)in the diagnosis of drowning.Methods Lung,liver and kidney tissues from 212 drowned corpses and 30 non-drowned corpses were examined respectively by the three drowning-related plankton testing methods,and the detection rates of plankton in each tissue by three methods were compared.Results In drowned corpses,the total detection rates of PCR-CE,qPCR,and MD-VF-Auto SEM were 93.9%,96.2%,and 95.3%,respectively,with no statistically significant difference(P>0.05).The detection rate of lung tissue by MD-VF-Auto SEM(100%)was higher than those of PCR-CE and qPCR(P<0.05),and there was no significant difference in the detection rates of the three methods in liver or kidney tissues(P>0.05).In non-drowning corpses,a small number of diatoms(less than 10 cells/10 g)were detected by MD-VF-Auto SEM method,only in liver and kidney tissues,while the other two methods yielded negative results for all tissues.Conclusion All three methods have good efficacy in the examination of drowned corpses.The MD-VF-Auto SEM method directly observes diatom morpho-logical characteristics through scanning electron microscopy,and the qualitative and quantitative analy-ses are intuitive and accurate.It has great advantages in the examination of difficult degradation samples.The PCR-CE method and qPCR method have a low sample demand(0.5 g),are easy to operate and have short detection time(4-7 h).They are easy to be applied in the grassroots depart-ments and are suitable for the rapid determination of drowned corpses in routin cases.The combina-tion of the two DNA methods with the MD-VF-Auto SEM method can increase the detection rate of plankton,ensuring the reliability of examination results.This combined use is of significant importance in the application of drowning diagnosis.
8.Feasibility study of using dual-energy CT virtual non-contrast images to replace true non-contrast images in photon and proton radiotherapy dose calculations

Objective:To systematically evaluate the differences in CT values between virtual non-contrast (VNC) images and true non-contrast (TNC) images generated from dual-energy CT (DECT), and to validate the feasibility of VNC images replacing TNC images in dose calculations for photon and proton radiotherapy plans.Methods:A retrospective analysis was conducted on the imaging data of 40 patients with solid tumors (20 cranial, 10 thoracic and 10 abdominal cases) who underwent DECT scans at Cancer Hospital of Shandong First Medical University from February 2022 to May 2023. VNC and TNC images were registered slice-by-slice. The differences in CT values of anatomical structures were compared, and Pearson correlation analysis was used to evaluate the correlation of CT values of different anatomical structures in VNC and TNC images. For structures with significant differences, linear regression models (TNC=β×VNC+α) were established using the least squares method. In the Varian Eclipse 15.5 treatment planning system, photon and proton radiotherapy plans based on TNC images and VNC images, as well as the proton radiotherapy plan based on the VNC images corrected by the regression models, were respectively designed. Dose differences of radiotherapy plans designed based on the two images were evaluated. To evaluate dose variations in regions adjacent to the clinical target volume (CTV), two 2-mm-thick annular reference structures were generated on the axial slice containing the largest cross-section of the CTV, extending cranially and caudally from the CTV. These structures were designated as Ring_p and Ring_d, respectively.Results:The differences in CT values between VNC and TNC images were mainly concentrated in the bony structure. The CT values difference between TNC and VNC images was (409.07±53.38) HU for the skull in 20 cranial tumor patients ( t=13.88, P<0.001), and (118.66±20.90) HU for the vertebral bone in 10 thoracic and 10 abdominal tumor patients ( t=10.43, P<0.001). The CT values of the skull and spine showed high correlation between TNC and VNC images ( r=0.98, P<0.001; r=0.99, P<0.001). The regression models established respectively were: TNC=1.859×VNC+33.896 (skull), and TNC=1.827×VNC+5.491 (spine). For photon radiotherapy plans based on TNC and VNC images, the D mean of the CTV were (60.00±0.00) and (60.00±0.00) Gy respectively, with D mean of Ring_p were (61.17±1.69) and (61.01±1.67) Gy, and Ring_d were (55.26±2.06) and (55.20±1.94) Gy, respectively. The relative dose differences in D mean between the two image types were 0 ( t<0.01, P>0.999), 0.33% ( t=0.30, P=0.766), and 0.19% ( t=0.07, P=0.947), all with no statistically significant differences. For proton radiotherapy plans based on TNC and VNC images, the D mean of the CTV were (61.73±0.32) and (61.67±0.26) Gy (RBE), respectively, with D mean of Ring_p were (61.19±0.44) and (60.53±1.22) Gy (RBE), and Ring_d were (60.97±0.67) and (59.80±4.26) Gy (RBE), respectively. The relative dose differences in D mean between the two image types were 0.24% ( t=0.63, P=0.530), 1.80% ( t=1.45, P=0.156), and 3.56% ( t=2.26, P=0.030), with a statistically significant difference in the Ring_d region. In the proton radiotherapy plan designed based on the corrected VNC images, the D mean of the CTV was (61.75±0.32) Gy (RBE), Ring_p was (61.43±0.71) Gy (RBE), and Ring_d was (59.96±2.80) Gy (RBE). The relative dose differences in D mean between TNC images and corrected VNC images were 0.16% ( t=0.19, P=0.850), 0.76% ( t=1.32, P=0.196), and 2.22% ( t=1.93, P=0.061), respectively, with no statistically significant differences. Conclusions:The differences in CT values between VNC and TNC images in DECT mainly exist in bony structures, particularly in the skull and vertebrae. For patients with cranial tumors, VNC images can be directly used in photon radiotherapy planning. In contrast, for proton therapy, after being corrected by the regression model, VNC images can effectively replace TNC images for the dose calculations of radiotherapy plan.
9.Construction and investigation of work capability evaluation indicator system for different positions of offshore workers

Objective To investigate the work capability need of offshore workers,and then to construct the evaluation indicator system of work capability for different positions of offshore workers.Methods Literature search,cluster analysis,key events,weight calculation and expert assessment were applied in this study.Results After the indicator screening,weight calculation and expert verification,the work capability evaluation indicator system was established,which contained 5 dimensions and 24 items.The indicators were sequenced according to the weight calculation,and the coefficient of variation of each indicator was less than 20%.Conclusion The construction of the work capability evaluation indicator system for different positions of offshore workers not only improves the content of the offshore job performance evaluation system,but also meets the demand for enhancing offshore operation support.It has greatly practical significance and promotion value.
10.Establishment and exploration of a quality monitoring indicator system for Investigator-Initiated ophthalmology clinical research in China

Objective:To develop an evaluation index system for the quality management process of investigator-initiated trials(IITs) in ophthalmology, providing a scientific tool and reference for improving clinical research quality and standardized management.Methods:Key quality management elements of ophthalmology IITs were identified through a literature review and experts interviews. A three-round Delphi consultation was conducted with experts from 16 provinces and municipalities in China who specialize in ophthalmic clinical research management. The importance of each index was scored, and weighting calculations were performed. Based on ranking and feasibility, a validated quality index system for ophthalmology IITs was established.Results:A three-tier index system was developed, comprising three first-level indicators (structure, process, and outcome), 15 secondary indicators, and 44 tertiary indicators. The expert authority coefficient was 0.87, and the response rates for the three Delphi rounds were 100%, 92.58%, and 100%, respectively. Among the first-level indicators, the highest weight was assigned to ″Structure″ (0.443 2). The top three secondary indicators were ″Data analysis report″ (0.098 3), ″Study protocol execution and data archiving″ (0.096 0), and ″Research team″ (0.094 1). At the tertiary level, the highest-ranked indicators were ″Data cleaning, verification, and database locking″ (0.049 2), ″Archiving and preservation of research data″ (0.048 8), and ″Execution of statistical analysis as planned″ (0.048 4).Conclusions:The ophthalmology IIT process quality evaluation index system developed in this study is scientifically grounded and will provides a valuable reference for standardizing quality management and further empirical research in ophthalmology IITs.
