1.Independent and Interactive Effects of Air Pollutants, Meteorological Factors, and Green Space on Tuberculosis Incidence in Shanghai.

OBJECTIVE:
To assess the independent and combined effects of air pollutants, meteorological factors, and greenspace exposure on new tuberculosis (TB) cases.
METHODS:
TB case data from Shanghai (2013-2018) were obtained from the Shanghai Center for Disease Control and Prevention. Environmental data on air pollutants, meteorological variables, and greenspace exposure were obtained from the National Tibetan Plateau Data Center. We employed a distributed-lag nonlinear model to assess the effects of these environmental factors on TB cases.
RESULTS:
Increased TB risk was linked to PM 2.5, PM 10, and rainfall, whereas NO 2, SO 2, and air pressure were associated with a reduced risk. Specifically, the strongest cumulative effects occurred at various lags: PM 2.5 ( RR = 1.166, 95% CI: 1.026-1.325) at 0-19 weeks; PM 10 ( RR = 1.167, 95% CI: 1.028-1.324) at 0-18 weeks; NO 2 ( RR = 0.968, 95% CI: 0.938-0.999) at 0-1 weeks; SO 2 ( RR = 0.945, 95% CI: 0.894-0.999) at 0-2 weeks; air pressure ( RR = 0.604, 95% CI: 0.447-0.816) at 0-8 weeks; and rainfall ( RR = 1.404, 95% CI: 1.076-1.833) at 0-22 weeks. Green space exposure did not significantly impact TB cases. Additionally, low temperatures amplified the effect of PM 2.5 on TB.
CONCLUSION
Exposure to PM 2.5, PM 10, and rainfall increased the risk of TB, highlighting the need to address air pollutants for the prevention of TB in Shanghai.

2.Effectiveness and Safety of Baidu Jieduan Granules for COVID-19: A Retrospective Observational Multicenter Study.

OBJECTIVE:
To evaluate the effectiveness and safety of Baidu Jieduan Granules (BDJDG) to treat common type coronavirus disease 2019 (COVID-19).
METHODS:
This multicenter, retrospective, and observational clinical trial included 230 common COVID-19 patients in Leishenshan, Huangshi, and Laohekou Hospitals in Wuhan from January 21 to March 26, 2020. The included patients were further divided into two subgroups according to the use of supplemental oxygen, mild and moderate groups. During the first 14 d of hospitalization, all patients were administered BDJDG combined with conventional Western medicine, and observed for continuous 28 d. Primary outcomes were disease progression rate and discharge rate. Secondary outcomes included negative conversion time of nucleic acid, hospitalization duration, clinical symptom subsidence time, and symptom regression rate.
RESULTS:
A total of 230 common COVID-19 patients were analyzed (138 in moderate group and 92 in mild group). By day 28, the disease progression rate was 4.3% and the discharge rate was 95.7%. All mild cases recovered and were discharged from hospital. The median negative conversion time of nucleic acid of all 230 COVID-19 patients was 12 d [inter-quartile range (IQR) 3.5-17], the median hospitalization duration was 15 d (IQR 12-20). The median time to fever, cough, and fatigue recovery was 4 d (IQR 2-6), 8 d (IQR 5-12), and 8 d (IQR 5-11). The recovery rate of fever, cough, and fatigue was 94.6%, 90.5%, and 93.5%. The median time to clinical improvement was 12 d (IQR 10-17). Compared with the baseline, total leukocyte counts, neutrophil counts, lymphocyte counts, and platelet counts were increased significantly on days 7 and 14 (P<0.01). C-reactive protein markedly increased on day 3 and significantly decreased on days 7 and 14 (P<0.01). No serious adverse events occurred during treatment.
CONCLUSION
BDJDG may be effective and safe for treatment of common type COVID-19. (Registration No. ChiCTR2000030836).

Diabetes mellitus complicated with cerebral infarction is the commonest and most serious vascular complication of diabetes mellitus. With a high disability and mortality rate, it seriously threatens human health. Because the pathogenesis is still unclear, more and more scholars have focused on the research of diabetic cerebral infarction at home and abroad. Traditional Chinese medicine(TCM) compounds have a remarkable curative effect in the treatment of diabetic cerebral infarction. Its mechanisms of action mainly include anti-hypertension, reduction of blood sugar and lipid, promotion of vascular regeneration and vascular endothelial function, anticoagulation, anti-thrombosis, improvement of nerve function defect, reduction of infarct volume, improvement of hemorheological, inhibition of inflammation and platelet aggregation, and promotion of collateral circulation. Through literature search, this paper summarizes the research progress of the mechanisms of TCM compounds in treating diabetic cerebral infarction in recent five years at home and abroad, in order to provide reference for clinical treatment.
4.Toxicity of Buxue Shengru Granules on perinatal rats

AIM To investigate the toxicity of Buxue Shengru Granules (BSG,Astragali Radix,Angelicae sinensis Radix,Paeoniae Radix alba,etc.) to perinatal SD rats and their offspring to determine its safety.METHODS The respective BSG administration of 0,1,3,10 g/kg to the four groups rats,each with twenty-four female rats,started from the day 15 of gestation (GD15) through weaning when they were procured for the identification of the general condition,body weight and food ingestion.The offspring had their postnatal appearance observed the physiological development,reflexes development,behavior and fertility detected as well.RESULTS BSG gave neither significant influence on the rats' general condition,time of birth,body weight,and histological changes of organs during perinatal period;nor remarkable impact on the offspring's growth,development,nerves,endocrine,and reproductive system.CONCLUSION Data on BSG to rats maternal weight gain and food intake and to their offspring developmental landmarks,sexual maturation,or reflexes suggest that BSG gives no perinatal toxicity.
5.Effects of di-butyl phthalate on the reproductive system of adolescent male rats.

OBJECTIVETo explore the effects of di-butyl phthalate (DBP) on the reproductive system of adolescent male rats.
METHODSSprague-Dawley (SD) rats aged 5 weeks were assigned to receive corn oil (vehicle control) or DBP orally at 10, 100 and 500 mg/(kg x d) for 30 days. After the exposure, the testis, epididymis, liver and pituitary of the rats were weighted and their ratios to the body weight obtained. Histopathological changes of the testis and epididymis were examined by Hematoxylin-eosin staining, the levels of testosterone (T), luteinizing hormone (LH) and follicle stimulating hormone (FSH) in the serum were measured by radioimmunoassay, and the relative mRNA expressions of the steroidogenesis acute regulatory protein (StAR), proliferating cell nuclear antigen (PCNA), cytochrome P450 cholesterol side chain cleavage enzyme (P450scc) and scavenger receptor (SR) were detected by real-time quantitative RT-PCR.
RESULTSDBP induced significant histopathological changes in the testicular tissue at 100 and 500 mg/(kg x d), and decreased the testicular and epididymal weights, inhibited the mRNA expressions of StAR and PCNA, reduced the levels of T and LH, and elevated the level of FSH at 500 mg/(kg x d). At the dose of 10 mg/(kg x d), DBP increased serum LH and FSH and the mRNA expression of P450scc. While the SR mRNA expression showed no significant changes in all the groups.
CONCLUSIONHigh level of DBP has apparent toxic effect on reproductive system of male rats. Low - dose DBP can increase the level of serum gonadotropin LH and affect the mRNA expression of P450scc in the testis.

6.P27Kip1 expression and its prognostic implication in breast carcinoma: a meta-analysis.

To evaluate the relationship between p27Kip1 low expression in breast cancer and its prognostic implication in breast carcinoma patients. Methods All data that were associated with the study of the relationship between p27Kip1 and the prognosis for breast cancer was pooled from Cochrane library, PubMed, Embase and Medlinebase. The outcome was measured using the risk ratio (RR). Data pooling was performed by RevMan 4. 2. Results 6457 patients from 20 studies were included in this meta-analysis. RR estimate of overall survival (OS) for patients with low level p27Kip1 was 2.07 [1.66,2.60] (P<0.01). For disease free survival (DFS), the pooled RR was 1.27 [1.10,1.47] (P<0.05). The combined RR estimate of relapse free survival (RFS) for patients with low level of p27Kip1 was 1.49 [0.92, 2.42] (P >0.05). In patients with lymph node negative breast carcinoma, the combined RR for OS and RFS were 1.98 [1.34,2.91] (P <0.01) and 1.28 [0.45,3.65] (P > 0.05), respectively. Among the patients with lymph node positive breast carcinoma, the combined RR for OS and RFS was 1.92 [1.31, 2.82] (P=0.0009) and 1.35 [0.96,1.89] (P>0.05) respectively. Conclusions Low level of p27Kip1 appears to be an independent prognostic factor to OS and DFS of breast cancer patients but not to RFS. Additional studies with large patient number and widely accepted practical methods are required to derive the precise prognostic significance of p27Kip1 expression in breast cancer patients.

7.Effects of ischemia postconditioning on isehemia-reperfusion injury and reperfusion injury salvage kinase signal transduction pathways in isolated mouse hearts

Objeetive To explore the effects of ischemia postconditioning(IPC)on ischemia-reperfusion(I/R)injury and associated reperfusion injury salvage kinase(RISK)signal transduction palthways changes in isolated mouse hearts.Methods Langendofff perfused C57/BL mouse hearts were divided to 6 groups:(1)control:30 min global ischemia and 2 h reperfusion(I/R);(2)IPC with 3 episodes,IPC with 3 episodes of 10 s of ischemia and 10 s reperfusion after 30 rain ischemia and before 2 h reperfusion:(3)IPC with 6 episodes,IPC with six episodes of 10 s of ischemia and 10 s reperfusion after 30 min ischemia and before 2 h reperfusion:(4)delayed IPC,IPC with 3 episodes of 10 s of ischemia and 10 s repeflusion after 30 min ischemia and at one minute after reperfusion;(5)IPC+ERK1/2 inhibitor PD98059(10-5mol/L for 15 min);(6)L/R+ERK1/2 inhibitor PD98059(10-5mol/L for 15 min).The effects of IPC on hemodynamics,coronary artery flow,creatine kinase(CK)and lactate dehydrogenase (LDH)release,myocardial SOD,MDA,phospho-protein kinase(P-ERK1/2)and phospho-protein kinase B(P-Akt)as well as myocardial infarction size were measured.Results IPC with 3 episodes and IPC with 6 episodes significantly and equally improved myocardial function,increased myocardial SOD,reduced CK and LDH release and myocardial infarction size compared with IR group(all P<0.01)while these parameters were similar between I/R hearts and delayed IPC hearts.IPC significantly increased myocardial ERK1/2 phosphorylation.PD98059 inhibited the phosphorylation of ERK1/2 and abolished the cardioDrotective effects induced by IPC.Conclusions IPC obviously attenuated I/R injury in isolated mouse hearts,the cardioprotection of IPC was not enhanced because of increasing of IPC episodes and disappeared in delayed IPC.The cardioprotective effects of IPC were mediated through ERK1/2-MAPK signal transduction pathway.
