1.Clinical Observation of Zhuanggu Kangsong Soft Extract Combined with Percutaneous Vertebroplasty in the Treatment of Osteoporotic Vertebral Compression Fractures

Objective To evaluate the clinical efficacy of Zhuanggu Kangsong Soft Extract combined with percutaneous vertebroplasty(PVP)in the treatment of osteoporotic vertebral compression fractures(OVCF).Methods A total of 90 eligible OVCF patients admitted to Guangzhou Integrated Chinese and Western Medicine Hospital between January 2023 and May 2024 were enrolled and randomly divided into an observation group(n=45)and a control group(n=45)using a random number table.Both groups underwent PVP.After treatment,the control group received conventional western medication orally(calcium carbonate D3 tablets+calcitriol soft capsules),while the observation group received additional Zhuanggu Kangsong Soft Extract treatment orally.Western medication was administered for 6 months,and herbal treatment lasted 3 months.Changes in Visual Analogue Scale(VAS)scores for pain,Oswestry Disability Index(ODI)scores for low back function,bone mineral density(BMD)T-values,and serum bone metabolism markers were observed before and after treatment.Results(1)At postoperative day 3,3 months,and 6 months,both groups showed significant improvements in VAS and ODI scores compared to those before treatment(P<0.05).The observation group demonstrated superior improvements in VAS and ODI scores at 3 and 6 months compared to the control group(P<0.05).(2)At 3,6,and 12 months postoperatively,BMD T-values were improved in both groups(P<0.05),with the observation group exhibiting greater improvements at 6 and 12 months(P<0.05).(3)Serum N-terminal osteocalcin and 25-hydroxyvitamin D3[25(OH)D3]levels increased in both groups at 1,3,and 6 months(P<0.05).The observation group showed more pronounced elevations in these markers at 3 and 6 months(P<0.05).Conclusion The combination of Zhuanggu Kangsong Soft Extract and PVP demonstrates significant therapeutic benefits for OVCF by effectively alleviating pain,enhancing BMD,and improving activities of daily living,which highlights its high clinical value.
2.Observation on the Clinical Efficacy of Haloperidol Combined with Auricular Point Pressing Therapy in Children with Tic Disorders

Objective To evaluate the clinical efficacy of haloperidol combined with auricular point pressing therapy in children with tic disorders.Methods A total of 120 pediatric patients diagnosed with tic disorders at the Department of Pediatric Rehabilitation,Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine,Hebei Province from March 2022 to March 2024 were enrolled.Participants were randomly divided into an observation group(n=60)and a control group(n=60)using a random number table.The control group received haloperidol alone,while the observation group received additional auricular point pressing therapy.Treatment duration was 12 weeks.After 3 months,clinical efficacy was assessed by comparing:Yale Global Tic Severity Scale(YGTSS)scores,serum 5-hydroxytryptamine(5-HT)and dopamine(DA)levels,immune function markers(CD3+,CD4+),and incidence of adverse reactions between groups.Results(1)The overall efficacy rate in the observation group was 91.67%(55/60),while that in the control group was 76.67%(46/60).The efficacy of the observation group was superior to that of the control group,with a statistically significant difference(P<0.05).(2)After treatment,the YGTSS scores of children in both groups was significantly improved(P<0.05),and the observation group showed a significantly greater improvement in YGTSS scores than the control group,with a statistically significant difference(P<0.05).(3)After treatment,the levels of 5-HT and DA in both groups of children were significantly improved(P<0.05),and the observation group showed a significantly greater improvement in 5-HT and DA levels than the control group,with a statistically significant difference(P<0.05).(4)After treatment,CD3+and CD4+levels were significantly improved in both groups(P<0.05),and the observation group showed a significantly greater improvement in CD3+and CD4+levels than the control group,with a statistically significant difference(P<0.05).(5)There was no statistically significant difference in the incidence of adverse reactions between the observation group and the control group,with a statistically significant difference(P>0.05).Conclusion The combination of haloperidol and auricular point pressing therapy significantly improves clinical symptoms and immune function in children with tic disorders,demonstrating both efficacy and safety.
3.Factors influencing good blood glucose control in elderly diabetic patients

Objective:To investigate blood glucose management in elderly diabetic patients and the factors that affected achieving high rates of achieving it.Methods:The quality of blood glucose control by elderly diabetic patients who visited five community health service centers in Beijing in June 2022 was surveyed retrospectively using a questionnaire.Participants were divided into the high-quality group and not high-quality groups by the criteria of glycosylated hemoglobin(HbA 1C)<7% without hypoglycemia and weight gain.Single factor analysis and multiple-stepwise Logistic regression analysis were used to identify the relative risk of factors affecting the achievement of good glucose management. Results:A total of 287 valid questionnaires were collected, including 80 cases(27.9%)in the high-quality group and 207 cases(72.1%)in the not high-quality group.There were significant differences in sex, course of disease, occurrence of hypoglycemia, number of chronic complications, medication, alcohol consumption, dietary changes, missed medication doses, prolonged outpatient visit intervals, and depressive mood between the two groups( P<0.05).Multivariate stepwise logistic regression analysis showed that insulin use, missed medication ≥ 1 time/week, and prolonged outpatient visit intervals were independent risk factors for good control( P<0.05). Conclusions:The percentage of elderly diabetes patients with good blood glucose control was low.Factors affecting the rate included insulin use, medication compliance, and prolonged outpatient visit intervals.
4.Patterns of Immunocyte Infiltration in Ovarian Cancer and Their Clinical Significance

Objective To explore the patterns of the immunocyte infiltration in ovarian cancer,and their influence on clinical out-come and chemotherapy response.Methods The clinical information and gene expression profiles of 2206 ovarian cancer patients were downloaded from TCGA and GEO publicly databases.The CIBERSORT was used to evaluate the distribution of 22 immune cells and their effects on survival,and chemotherapy response.Unsupervised clustering was used to classify cases and analyze differences in clinical out-comes between subgroups.Immunofluorescence staining was performed to illustrate the macrophages infiltration in ovarian cancer samples collected from the First Affiliated Hospital,Sun Yat-sen University.Results Naive B cells,M2 macrophages,and resting memory CD4+T cells were significantly associated with poor prognosis,while follicular helper T cells were significantly associated with good prog-nosis.Plasma cells,M1 macrophages,and activated memory CD4+T cells were associated with chemotherapy sensitivity,and M2 macro-phages were associated with chemotherapy resistant.Furthermore,three subgroups were identified by unsupervised clustering which showed different distribution ratios of macrophages and T cells,and significant differences in overall survival among subgroups.The infil-tration of macrophages in metastasis tissue of ovarian cancer was more than that in primary tumor.Conclusion The immunocyte infiltra-tion in ovarian cancer is associated with clinical prognosis and chemotherapy resistance.Immune cell targeting therapy or combination chemotherapy may be the potential treatment choices for ovarian cancer.
5.Study on changes in management capabilities of chronic disorders in elderly diabetic patients during the COVID-19 pandemic

Objective:To investigate changes in the self-management ability of chronic diseases in elderly diabetic patients during the novel coronavirus pneumonia pandemic.Methods:A multi-stage cluster sampling method was used to retrospectively collect diabetes management information from elderly diabetic patients treated in five community health service centers in Beijing in June 2022, including 295 elderly diabetic patients, and changes in management of their self-care practices such as diet, exercise, medical treatment and medication, blood glucose monitoring, complication management, and psychological states before and after the outbreak(2019-2022)were analyzed.Results:In 295 elderly diabetic patients, increased glycated hemoglobin( P<0.05)and body mass index(BMI)( P<0.05)were observed during the pandemic.Among 115 patients with underlying diabetic complications, 75.7%(87)of patients had worsening symptoms of chronic complications of diabetes and a shortened weekly exercise time[124(60, 300)min vs.140(60, 300)min, Z=6.00, P<0.05].The number of weekly blood glucose tests was reduced(5.6±4.2 vs.5.8±4.3, t=3.17, P<0.05).The weekly time spent interacting with family members increased( P<0.05).There were no significant differences in body weight, sleep time, and number of times of forgetting to take medication per week before and after the outbreak( P>0.05).21.0% of patients had a decrease in the number of hospital visits, and 72.2% of patients experienced low mood and increased adverse psychological emotions during the pandemic. Conclusions:The pandemic has had a negative impact on the management of chronic diseases in elderly diabetic patients, mainly reflected in the reduction of exercise time and blood glucose monitoring.It also affected regular hospital visits of patients, resulting in negative psychological emotions, poor blood glucose control, increased BMI, and aggravated symptoms of chronic complications, compared with their pre-pandemic conditions.
6.Effect of COVID-19 epidemic on management of diabetic patients

The outbreak of COVID-19 as an acute communicable disease has also changed the epidemiological status of diabetes mellitus and other noncommunicable chronic diseases. During the COVID-19 epidemic period, it was observed that there were increased morbidity of diabetes, difficulties in blood sugar control and increased acute complications for diabetic patients. This may be attributed to lifestyle changes during the epidemics, such as the reduced exercise time and increased sedentary time, more snacks and sugary food intake, as well as anxiety and depression. However, it is not known the long-term impact of COVID-19 epidemic on the management of diabetic patients, so it is necessary to closely monitor the exposed diabetic patients in the future.
7.Impact of achievement goals on the academic achievement of medical students

Objective:To investigate the impact of achievement goals on the academic achievement of medical students.Methods:In May to June of 2019, an online questionnaire survey was performed for data collection, and the stratified sampling method was used for sampling. Demographic factors, Achievement Goal Scale, and Academic Achievement Scale were used in this study, and descriptive statistics, the Pearson correlation analysis, and the multivariate linear hierarchical regression analysis were performed to analyze the impact of achievement goals on the academic achievement of medical students.Results:For these medical students, the mean value of achievement-near to the goal orientation was (3.45±0.77), the mean value of achievement-dodge the goal orientation was (3.43±0.80), and the mean value of mastery goal orientation was (3.85±0.66). Achievement-near to the goal orientation had a significant positive impact on academic achievement ( β=0.336, P<0.01), and mastery goal orientation also had a significant positive impact on academic achievement ( β=0.540, P<0.01). Conclusion:The achievement goals dominated by mastery goal orientation is the most beneficial dimension for the development of medical students.
8.Clinical characteristics of paroxysmal nocturnal hemoglobinuria (PNH) complicated with ischemic bowel disease

Paroxysmal nocturnal hemoglobinuria (PNH) is a rare acquired hematopoietic stem cell disease. Gastrointestinal involvement is rarely seen in PNH. This study aims to analyze the clinical features in PNH patients complicated with ischemic bowel disease. Clinical date of 6 patients were collected at Peking Union Medical College Hospital from January 2010 to December 2020. The clinical manifestations, laboratory tests,imaging, endoscopic,and histopathological features and treatment were analyzed.Five in 6 patients were men, with a median age of 31 years old at onset. Most of disease course were recurrent episodes of chronic disease, with abdominal pain (5/6) and gastrointestinal bleeding (5/6). Laboratory examinations showed pancytopenia, reticulocytosis, elevated serum lactate dehydrogenase, high D-dimer and C-reactive protein levels in all patients. Multiple segments of small intestine were the most commonly involved and colon was also affected. Abdominal CT scan showed thickening and roughness or exudation of the intestinal wall (6/6), increased mesenteric density or “comb sign”(4/6), and cholestasis or gallbladder stones (5/6). Endoscopic manifestations included irregular shallow ulcers in the annular cavity (5/6), swelling mucosa with well-defined margins (6/6). Pathological biopsy revealed chronic inflammation of mucosa. The efficacy of steroids combined with anticoagulant therapy was better than that of steroids alone. Ischemic bowel disease in PNH patients is different from typical ischemic enteritis. Young patients, involvement of intestine with multiple segments are common characteristics. The anticoagulant is an essential agent for these patients.
9.Research progress on the antihypertensive effect of sodium-glucose synergistic transporter 2 inhibitors

Diabetes mellitus is a chronic metabolic disease, in which the abnormality of glucose and lipid metabolism may cause multisystem damage. Sodium-glucose synergistic transporter 2 (SGLT2) inhibitors are a novel type of hypoglycemic drug that can lower blood sugar level by inhibiting the absorption of glucose through renal tubules. Studies have shown that SGLT2 inhibitors also have a lowering effect on blood pressure, but the mechanism is not fully elucidated. In this article the hypotensive effects of SGLT2 inhibitors and possible mechanisms are reviewed.
10.Physical fitness assessment and management of patients with diabetes mellitus

Physical fitness is the basic ability necessary to meet daily living activities,including walking, running, jumping, throwing, climbing, and so on. Adequate physical fitness is essential for the prevention and management of metabolic diseases. Impaired physical fitness is common in patients with diabetes mellitus and its chronic complications. This article reviews the impact of diabetes and its complications on physical fitness, current methods for assessment and management to provide a reference for comprehensive prevention and treatment of diabetic patients.
