1.Association of monocyte/high-density lipoprotein cholesterol ratio with periodontitis: a cross-sectional study based on the NHANES database

Objective:
To investigate the association between monocyte to high-density lipoprotein cholesterol ratio (MHR) and periodontitis and to provide new epidemiologic evidence on the factors affecting periodontitis.
Methods:
Data on MHR, periodontitis, and other covariates were selected from the NHANES(National Health and Nutrition Examination) database for 3 cycles of subjects in 2009-2010, 2011-2012, and 2013-2014, and a total of 8 456 study subjects were included. The study participants were grouped according to the prevalence of periodontitis (presence or absence), and three regression models (unadjusted covariates, partially adjusted covariates, and fully adjusted covariates) were constructed to analyze the relationship between MHR and periodontitis by using a weighted logistic regression method with stepwise adjustment for confounders. MHR was divided into four groups from Q1 to Q4 according to quartiles from small to large for weighted trend analysis, and the nonlinear relationship between MHR (continuous) and periodontitis was analyzed using a restricted cubic spline with subgroup analysis and sensitivity analysis.
Results:
All three logistic regression models showed a positive association between MHR and periodontitis (OR = 2.92, 95%CI: 2.14-3.99, P<0.001 (not adjusted); OR = 1.97, 95%CI: 1.39-2.78, P<0.001 (partially adjusted); OR = 1.62, 95%CI: 1.10-2.39, P = 0.017 (fully adjusted)). Trend analysis showed a significantly higher risk of developing periodontitis in the Q4 group compared with the Q1 group in both single (OR = 1.92, 95% CI: 1.58-2.33, P<0.001) and multifactorial analyses (OR = 1.30, 95% CI: 1.03-1.64, P = 0.029). Restricted cubic spline results did not support a nonlinear relationship between MHR and periodontitis (P for nonlinear>0.05), subgroup analysis showed no significant interaction between the covariates and MHR (P>0.05), and sensitivity analysis also showed a positive correlation between MHR and periodontitis (OR = 1.67, 95%CI: 1.31-2.14, P<0.001).
Conclusion
MHR is positively associated with the risk of developing periodontitis.
2.Houshihei San Repairs Skeletal Muscle Injury After Ischaemic Stroke by Regulating Ferroptosis Pathway

ObjectiveTo investigate the pharmacodynamic effects of Houshihei San (HSHS) recorded with the effects of treating wind and limb heaviness on muscle tissue injury after middle cerebral artery occlusion (MCAO) in rats through the ferroptosis pathway. MethodsThirty SD male rats were selected and randomly grouped as follows: sham, MCAO, deferoxamine mesylate, high-dose HSHS (HSHS-H, 0.54 g·kg-1), and low-dose HSHS (HSHS-L, 0.27 g·kg-1), with 6 rats in each group. A laser scattering system was used to evaluate the stability of the MCAO model, and rats were administrated with corresponding agents by gavage for 7 days. During the administration period, behavioral, imaging and other methods were used to systematically evaluate the skeletal muscle tissue injury after MCAO and the therapeutic effect in each administration group. Hematoxylin-eosin staining was employed to evaluate the cross-section of muscle cells. Subsequently, immunohistochemistry was used to detect tumor suppressor p53 and glutathione peroxidase 4 (GPX4) in the soleus tissue. Western blot was employed to determine the protein levels of p53, GPX4, myogenic differentiation 1 (MyoD1), nuclear factor E2-related factor 2 (Nrf2), Myostatin, solute carrier family 7 member 11 (SLC7A11), muscle ring-finger protein-1 (MuRF1), and muscle atrophy F-box protein (MAFbx) to verify the therapeutic effect in each group. ResultsCompared with the MCAO group, HSHS enhanced the locomotor ability and promoted muscle regeneration, which suggested that the pharmacological effects of HSHS were related to the inhibition of muscle tissue ferroptosis to reduce the expression of muscle atrophy factors. Behavioral and imaging results suggested that compared with the MCAO group, HSHS ameliorated neurological impairments in rats on day 7 (P<0.01), enhanced 5-min locomotor distance and postural control (P<0.01), strengthened grasping power and promoted muscle growth (P<0.01), stabilized skeletal muscle length and weight (P<0.01), and increased the cross-section of muscle cells (P<0.01). Compared with the MCAO group, HSHS promoted the increases in glutathione and superoxide dismutase content and inhibited the increase in malondialdehyde content (P<0.05,P<0.01). Ferroptosis pathway-related assays suggested that HSHS reduced the p53-positive cells and increased the GPX4-positive cells (P<0.01). HSHS ameliorated muscle function decline after stroke by promoting the expression of GPX4, Nrf2, SLC7A11, and MyoD1 and inhibiting the expression of p53, Myostatin, MurRF1, and MAFbx to reduce ferroptosis in the muscle (P<0.01). ConclusionHSHS, prepared with reference to the method in the Synopsis of Golden Chamber, can simultaneously reduce the myolysis and increase the protein synthesis in the skeletal muscle tissue after ischemic stroke by regulating the ferroptosis pathway.
3.Treatment of skeletal class Ⅱ high angle malocclusion patient by clear aligner therapy combined with orthognathic surgery:A case report and literature review

Skeletal class Ⅱ malocclusion is characterized by maxillary protrusion,mandibular retrognathia,or a combination of both,and often accompanied by vertical dimensional discrepancies;treatment is complex,and combined orthodontic-orthognathic surgery is needed for the adult patients.Clear aligner therapy has gradually been applied in complex orthodontic cases.However,limited cases have been reported domestically and internationally regarding the application of clear aligner therapy combined with orthognathic surgery.This article presented a case of a patient with skeletal class Ⅱ high-angle malocclusion treated with the combined therapy and analyzed the clinical efficacy of the treatment appraoch to provide reference for the clinical practice.Extraction of impacted wisdom teeth 18,28,38,and 48,as well as orthodontic teeth 15,25,34,and 44,was performed in stages before orthodontic treatment.Clear aligner therapy was used for preoperative orthodontics.In sagittal plane,a super-complete class Ⅱ canine and molar relationship and a 13-14 mm overjet of the anterior teeth were established.The maxillary and mandibular arch morphology was matched horizontally.The orthognathic surgery included maxillary LeFort Ⅰ osteotomy,bilateral sagittal split ramus osteotomy(BSSRO)and chinplasty.Fine occlusal adjustment was conducted after operation.After treatment,the skeletal relationship between upper and lower jaw was corrected to normal;subspinale-nasion-supramental angle(ANB)was improved from 12.3° to 4.7°;the patient established the class Ⅰ canine and molar relationship,with normal overjet and overbite;root parallelism was good and there was no obvious root resorption;the facial soft tissue profile was significantly improved,and nasion-subnasale-pogonion angle(N-Sn-Pg)was improved from 143.9° to 162.8°.The curative effect was stable 1 year after operation.Clear aligner therapy can efficiently complete combined orthodontic and orthodontic surgery in the complex cases.Compared with the fixed appliance,it is more beneficial to the patients'need for beauty and the maintenance of periodontal health.
4.Orthodontic combined with orthognathic treatment of a Class Ⅱ malocclusion patient with idiopathic condylar resorption:A case report and literature review

The patients with skeletal Class Ⅱ high-angle malocclusion are frequently complicated by idiopathic condylar resorption(ICR),which may lead to temporomandibular joint(TMJ)dysfunction and dentofacial deformities.This article reports the diagnosis and treatment process of a 24-year-old female patient with skeletal Class Ⅱ high-angle malocclusion accompanied by ICR.The patient's chief complaints were anterior open bite and TMJ pain,and was diagnosed with ICR through clinical examination and imaging.After stabilizing condylar resorption with occlusal splint therapy,combined orthodontic-orthognathic treatment was performed.The 42-month follow-up revealed:well-aligned dentition with complete closure of diastemas,significant improvement of protrusive facial profile(ANB angle reduced by 4.2°),complete resolution of TMJ pain and clicking,and establishment of stable Class Ⅰ occlusion.Three-dimensional CT demonstrated satisfactory condylar bone remodeling and normalized joint space.Through multidisciplinary treatment,both occlusal function and facial aesthetics were significantly improved.This case demonstrates that orthodontic-orthognathic treatment should be performed after condylar stabilization in ICR patients,and occlusal splint therapy serves as an effective preoperative intervention.
5.Correlation between noninvasive hemodynamic parameters and major adverse cardiovascular events in patients with acute anterior wall myocardial infarction after percutaneous coronary intervention

Objective To discuss the correlation between noninvasive hemodynamic parameters and major adverse cardiovascular events(MACE)in patients with acute anterior wall myocardial infarction after receiving percutaneous coronary intervention(PCI).Methods A total of 132 patients with acute anterior wall myocardial infarction,who received PCI at the Affiliated People's Hospital of Shandong First Medical University of China between October 2021 and February 2024,were collected.At 24 h and 7 d after surgery,the hemodynamic parameters,including mean arterial pressure(MAP),cardiac index(CI),cardiac output(CO),stroke volume(SV),peripheral vascular resistance index(SVRI),were recorded.Spearman correlation analysis was used to analyze the relationship between Killip grade of cardiac function and hemodynamic parameters.According to the presence or absence of MACE within 6 months after PCI,the patients were divided into MACE group and non-MACE group.The predictive value of hemodynamic parameters for MACE was analyzed by receiver operating characteristic(ROC)curves.Results The postoperative 7-day levels of CO,CI and SV were higher than their postoperative one-day levels,while the postoperative 7-day level of SVRI was lower than its postoperative one-day level(P<0.05).Of the 132 patients,Killip classification of grade Ⅰ was seen in 39,grade Ⅱ in 62,grade Ⅲin 23 and grade Ⅳ in 8.The postoperative 7-day levels of CO,CI and SV in the patients with Killip gradeⅢ-Ⅳ were lower than those in the patients with Killip grade Ⅰ-Ⅱ,while the level of SVRI in the patients with Killip grade Ⅲ-Ⅳ was higher than that in the patients with Killip grade Ⅰ-Ⅱ(P<0.05).The results of Spearman correlation analysis showed that Killip grade of cardiac function was negatively correlated with the postoperative 7-day levels of CO,CI and SV,while positively correlated with the postoperative 7-day level of SVRI after PCI(r=-0.518,r=-0.480,r=-0.416 and r=0.493 respectively,all P<0.05).Six months after PCI,34 patients developed MACE.The levels of CO,CI and SV in MACE group were lower than those in the non-MACE group,while the level of SVRI in MACE group was higher than that in the non-MACE group(P<0.05).The area under the ROC curve(AUC)of MAP,CO,CI,SV,SVRI and combination of the five indicators for predicting MACE was 0.620,0.687,0.676,0.649,0.710 and 0.860 respectively,and the AUC value of the combination of the five indicators was the greatest one.Conclusion In patients with acute anterior wall myocardial infarction after receiving PCI,the changes in the levels of MAP,CO,CI,SV and SVRI can reflect cardiac function level to a certain extent and can predict the occurrence of MACE events in the short term.
6.Analysis of the current situation of defect of vertebral column among primary, middle, and high school students in Minhang District, Shanghai

ObjectiveTo investigate the general situation of defect of vertebral column among primary and middle school students in Minhang District of Shanghai and analyze the related factors, to provide a scientific basis for prevention and treatment. MethodsFrom September to October 2022, a total of 5 715 students were selected from two primary schools, two middle schools, and two high schools in Minhang District for physical examinations and screening for defect of vertebral column. ResultsTotally 219 students had defect of vertebral column, accounting for 3.83% of the sampled population. Anteroposterior spinal abnormalities were found in 4 individuals, accounting for 0.07%, and 218 students had scoliosis, accounting for 3.81%. The detection rate of defect of vertebral column was higher in girls (6.27%) than that in boys (1.51%), and higher in high school students (10.74%) than in primary school students (1.31%) and middle school students (10.97%). Students who are mildly underweight (5.95%) and who are moderately to severely underweight (7.46%) had a higher detection rate of defect of vertebral column than those with normal weight (4.54%), overweight (2.83%), and obesity (1.60%). The detection rate among students with poor vision (4.32%) was significantly higher than those with normal vision (2.24%), with all differences statistically significant (all P<0.05). ConclusionThe positive rate of defect of vertebral column in primary and middle school students in Minhang District, Shanghai is nearly 4%, with most cases being scoliosis. Factors such as being female, increasing age, being underweight, and poor vision are associated with a higher probability of detecting defect of vertebral column.
7.The relationship between students’ visual acuity and the visual environment of primary and secondary school classrooms in Minhang District, Shanghai

ObjectiveTo understand the visual environment sanitation in primary and secondary school classrooms in Minhang District, Shanghai, and to investigate the factors affecting the decline in students’ visual acuity. MethodsIn 2020, among all the primary and secondary schools in Minhang District, according to different types of classroom structures, levels, orientations, unilateral lighting, and bilateral lighting, a total of 20 231 students from 674 classrooms in primary and secondary schools were selected through stratified cluster sampling. Each monitored classroom was regarded as a research unit, and the naked eye vision of students was tested using a standard logarithmic vision chart during the 2020 and 2021 academic years. The change in average visual acuity between the two measurements was taken as the dependent variable, and the visual environment indices were considered as independent variables. The relationship between classroom visual environment indices and the decline in students’ naked eye vision was analyzed. ResultsThe qualification rates for per capita classroom area, window to floor area ratio, average illuminance of the blackboard, uniformity of blackboard illuminance, average illuminance of the desks, uniformity of desk illuminance, lighting coefficient of classroom, blackboard reflection ratio, back wall reflection ratio, and distance between lamps and desks were 79.53%, 88.58%, 46.74%, 70.33%,64.69%, 80.86%, 71.81%, 20.30%, 1.63%, and 97.53%, respectively. The average naked eye vision of primary and secondary school students in the 2020 academic year was 4.75±0.37 (right eye) and 4.76±0.37 (left eye), while in the 2021 academic year, it was 4.70±0.39 (right eye) and 4.71±0.38 (left eye). There was a significant decrease in the naked eye vision of primary and secondary school students in the 2021 academic year compared to that of 2020 (P<0.05). Univariate linear regression analysis showed a significant correlation between grade, average illuminance of the blackboard, average illuminance of the desks, uniformity of desk illuminance, lighting coefficient, and distance between lamps and desks, and the decline of students’ naked eye vision within one academic year (P<0.05). After adjusting for grade, multiple linear regression analysis revealed a significant correlation between the average illuminance of the blackboard, uniformity of desk illuminance, lighting coefficient, and distance between lamps and desks, and the decline of students’ naked eye vision within one academic year (P<0.05). Higher average illuminance of the blackboard, uniformity of desk illuminance, lighting coefficient, and distance between lamps and desks were protective factors for naked eye vision. ConclusionThe visual environment of primary and secondary school classrooms in Minhang District is a matter of concern. Average illuminance of the blackboard, uniformity of desk illuminance, lighting coefficient, and distance between lamps and desks are related to the degree of visual impairment in students over a year.
8.Disease burden of acute viral hepatitis in Guangdong Province, 1990-2019

Objective:To examine the burden and trends of acute viral hepatitis in Guangdong Province from 1990 to 2019, and provide reference evidences for hepatitis prevention and control in the province.Methods:Data on acute viral hepatitis (hepatitis A, B, C, and E) in Guangdong from 1990 to 2019 were extracted from the Global Burden of Disease Study 2019 database. The incidence, prevalence, mortality, and disability-adjusted life years (DALY) data were analyzed by age and gender, and the estimated annual percentage change (EAPC) was calculated to describe the changing trends in disease burden.Results:From 1999 to 2019, the standardized incidence, prevalence, mortality, and DALY of acute viral hepatitis in Guangdong were higher than the national averages. In 2019, 51.43% (2 245 087/4 365 221) of acute viral hepatitis cases in Guangdong Province were mainly attributed to hepatitis B, and 77.18% (106/138) of deaths were due to acute hepatitis B. In different age groups, except for acute hepatitis B, which was more common in adults, the incidence rates of other types of viral hepatitis such as hepatitis A, B, and E showed an overall decreasing trend with age. The mortality rates of different types of acute viral hepatitis, except for the <5 age group, increased with age. The overall incidence and mortality rates of acute viral hepatitis were higher in men than in women.Conclusions:The overall burden of acute viral hepatitis in Guangdong declined in 2019, but remained higher than the national level. Further efforts are needed to strengthen hepatitis prevention and screening in different population in Guangdong Province, especially in children and the elderly.

10.Clinicopathological features of Sjogren′s syndrome complicated with liver injury

Objective:To study the clinicopathological features of Sjogren′s syndrome (SS) with liver injury and to improve the understanding of this disease.Methods:Forty-nine patients with SS complicated with liver injury were collected from Beijing Ditan Hospital, Capital Medical University from October 2008 to January 2022. All patients underwent ultrasound-guided liver biopsy, and all specimens were stained with HE. The histopathologic characteristics were observed and the pathologic indexes were graded. Immunohistochemical stains for CK7, CK19, CD38, MUM1 and CD10 were performed by EnVision method; and special histochemical stains for reticulin, Masson′s trichrome, Rhodanine, Prussian blue, periodic acid Schiff (PAS) and D-PAS stains were conducted .Results:The age of patients ranged from 31 to 66 years, including 3 males and 46 females. SS combined with drug-induced liver injury was the most common (22 cases, 44.9%), followed by autoimmune liver disease (13 cases, 26.5%, including primary biliary cholangitis in eight cases, autoimmune hepatitis in 3 cases, and PBC-AIH overlap syndrome in 2 cases), non-alcoholic fatty liver disease (NAFLD, 9 cases, 18.4%) and other lesions (5 cases, 10.2%; including 3 cases of nonspecific liver inflammation, 1 case of liver amyloidosis, and 1 case of porto-sinusoidal vascular disease). Among them, 28 cases (57.1%) were associated with obvious interlobular bile duct injury, mainly in SS combined with PBC group and drug-induced liver injury group. Twenty-three cases (46.9%) were associated with hepatocyte steatosis of varying degrees. In SS with autoimmune liver disease group, ISHAK score, degree of fibrosis bile duct injury, bile duct remodeling, lymphocyte infiltration of portal area, and plasma cell infiltration, MUM1 and CD38 expression; serum ALP and GGT, IgM; elevated globulin; positive AMA, proportion of AMA-M2 positive and IgM positive were all significantly higher than those in other groups(all P<0.05). Serum ALT, direct bilirubin and SSA positive ratio in SS combined with drug liver group were significantly higher than those in other groups(all P<0.05). The serum total cholesterol level in SS combined with PBC group ( P=0.006) and NALFD group ( P=0.011) were significantly higher than those in other groups ( P<0.05). Conclusions:The pathologic manifestations of SS patients with liver injury are varied. The inflammatory lesions of SS patients with autoimmune liver disease are the most serious, and the inflammatory lesions of SS patients with non-alcoholic fatty liver disease and non-specific inflammation are mild. Comprehensive analysis of liver histopathologic changes and laboratory findings is helpful for the diagnosis of SS complicated with different types of liver injury.
