Temporomandibular joint osteoarthritis is a common chronic degenerative disease,in which joint pain is the most signif-icant symptom,but its pathogenesis is still unclear.Significant subchondral bone lesions can occur in the early stage of osteoarthritis progression,and more and more experimental evidence shows that subchondral bone lesions play an important role in the pain caused by osteoarthritis.Osteoclasts,osteocytes,osteoblasts,endothelial cells,new generating nerves and blood vessels in the sub-chondral bone matrix microenvironment interact with each other and participate in the process of osteoarthritis pain.Therefore,regu-lating the subchondral bone matrix microenvironment is expected to become a new strategy to control joint pain.
2.Development and validation of the rapid health aging assessment scale for the Chinese population

Objective:To develop a rapid assessment scale for healthy aging suitable for the Chinese population.Methods:Based on existing healthy aging assessment scales, national standards, and expert consensus, an initial Healthy Aging Rapid Assessment Scale was drafted through two rounds of expert consultation. A pre-survey was conducted with 3 220 subjects recruited from Guangzhou between July 2023 and July 2024. Items were screened through item analysis and exploratory factor analysis to form the final scale. Reliability and validity of the final scale were validated across five cities: Guangzhou, Dongguan, Shenzhen, Baoding, and Chuxiong.Results:The initial version comprised 36 items, while the finalized scale contained 18 items across three dimensions: metabolic health, mental health, and cognitive health. Test-retest reliability ranged from 0.71 to 0.81 across all study sites. The Spearman-Brown coefficient varied between 0.91-0.96, Cronbach′s α between 0.77-0.83, comparative fit index (CFI) between 0.90-0.98, goodness-of-fit index (GFI) between 0.90-0.99, and root-mean-square error of approximation (RMSEA) between 0.03-0.09. For the three dimensions, reliability and validity metrics demonstrated consistency: Spearman-Brown coefficients 0.87-0.99, Cronbach′s α 0.77-0.83, CFI 0.90-0.98, GFI 0.90-0.99, and RMSEA 0.03-0.09 across four regions.Conclusion:The developed Healthy Aging Rapid Assessment Scale for the Chinese population exhibits robust reliability and validity.

BACKGROUND:
While emotional distress, encompassing anxiety and depression, has been associated with negative clinical outcomes, its impact across various clinical departments and general hospitals has been less explored. Previous studies with limited sample sizes have examined the effectiveness of specific treatments (e.g., antidepressants) rather than a systemic management strategy for outcome improvement in non-psychiatric inpatients. To enhance the understanding of the importance of addressing mental health care needs among non-psychiatric patients in general hospitals, this study retrospectively investigated the impacts of emotional distress and the effects of early detection and management of depression and anxiety on hospital length of stay (LOS) and rate of long LOS (LLOS, i.e., LOS >30 days) in a large sample of non-psychiatric inpatients.
METHODS:
This retrospective cohort study included 487,871 inpatients from 20 non-psychiatric departments of a general hospital. They were divided, according to whether they underwent a novel strategy to manage emotional distress which deployed the Huaxi Emotional Distress Index (HEI) for brief screening with grading psychological services (BS-GPS), into BS-GPS ( n = 178,883) and non-BS-GPS ( n = 308,988) cohorts. The LOS and rate of LLOS between the BS-GPS and non-BS-GPS cohorts and between subcohorts with and without clinically significant anxiety and/or depression (CSAD, i.e., HEI score ≥11 on admission to the hospital) in the BS-GPS cohort were compared using univariable analyses, multilevel analyses, and/or propensity score-matched analyses, respectively.
RESULTS:
The detection rate of CSAD in the BS-GPS cohort varied from 2.64% (95% confidence interval [CI]: 2.49%-2.81%) to 20.50% (95% CI: 19.43%-21.62%) across the 20 departments, with a average rate of 5.36%. Significant differences were observed in both the LOS and LLOS rates between the subcohorts with CSAD (12.7 days, 535/9590) and without CSAD (9.5 days, 3800/169,293) and between the BS-GPS (9.6 days, 4335/178,883) and non-BS-GPS (10.8 days, 11,483/308,988) cohorts. These differences remained significant after controlling for confounders using propensity score-matched comparisons. A multilevel analysis indicated that BS-GPS was negatively associated with both LOS and LLOS after controlling for sociodemographics and the departments of patient discharge and remained negatively associated with LLOS after controlling additionally for the year of patient discharge.
CONCLUSION
Emotional distress significantly prolonged the LOS and increased the LLOS of non-psychiatric inpatients across most departments and general hospitals. These impacts were moderated by the implementation of BS-GPS. Thus, BS-GPS has the potential as an effective, resource-saving strategy for enhancing mental health care and optimizing medical resources in general hospitals.

4.Current status and influencing factors of insufficient hyperopia reserve in preschool children

AIM: To analyze the current status and influencing factors for insufficient hyperopia reserve in preschool children from Changzhi City, Shanxi Province, and to provide reference and basis for myopia prevention and control in this district.METHODS: A stratified cluster random sampling strategy was used to select 2 854 preschool children(5 708 eyes)from 29 child-care centers in Changzhi City between January and May 2024. Hyperopia reserve was assessed through measurements and questionnaire surveys. Totally 2 820 cases(5 640 eyes)were finally included, with 34 cases excluded(32 cases of uncooperativeness and 2 cases of distractibility). The univariate analysis and multivariate Logistic regression were performed to analyze the associated influencing factors of insufficient hyperopia reserve.RESULTS: A total of 580 preschool children with insufficient hyperopia reserve were detected, with an incidence of 20.57%. Logistic regression analysis revealed that male(OR=1.723, 95% CI: 1.419-2.093), maternal myopia(OR=2.210, 95% CI: 1.681-2.906), paternal myopia(OR=1.426, 95% CI: 1.059-1.921), myopia in both parents(OR=2.761, 95% CI: 2.110-3.612), preterm infants(OR=1.740, 95% CI: 1.294-2.342), the mean daily sleep duration <10 h(OR=1.272, 95% CI: 1.024-1.579), and the mean daily outdoor activity time <2 h(OR=1.222, 95% CI: 1.005-1.485)were risk factors for insufficient hyperopia reserve(all P<0.05). Conversely, using blackout curtains during the day and turning off lights at night(OR=0.598, 95% CI: 0.405-0.883)were identified to be protective factors(P<0.05).CONCLUSION: Sex, genetics, gestational age, sleep duration and environmental conditions, and outdoor activity time are potentially associated with insufficient hyperopia reserve in preschool children. Caregivers should prioritize the management of these risk factors to prevent the occurrence of myopia.
5.Construction of an ultrasound dynamic image segmentation model for thyroid nodules

Objective:To construct a thyroid nodule segmentation model using ultrasound dynamic images and explore its potential for assisting in the screening of thyroid nodules.Methods:A total of 126 patients with thyroid nodules(comprising 150 nodules)who were diagnosed and treated at Xuzhou Cancer Hospital from April 2024 to December 2024 were prospectively enrolled. Two-dimensional ultrasound was performed to capture short-axis and long-axis video images of thyroid nodules,forming a dynamic ultrasound image dataset. The dataset was divided into training,validation,and test sets in a ratio of 6∶1∶3. After the training loss curve converged,the model that performed well on the validation set was selected for testing. Three-fold cross-validation was employed for training and testing. All 300 ultrasound videos were divided into three subsets. In each experiment,two subsets were used as the training set,and one subset was used as the test set to evaluate the model's generalization ability. A collaborative spatiotemporal diffusion model was established based on the dynamic trends and tissue texture details of thyroid nodules. Six widely used segmentation metrics were employed to evaluate the model's application capabilities.Results:The study included 126 patients with 150 thyroid nodules,300 dynamic ultrasound images,and video lengths of 3-4 seconds per nodule,resulting in 12 312 segmented images. The size of the thyroid nodules was(10.7 ± 10.6)mm(transverse diameter)×(8.4 ± 6.3)mm(anteroposterior diameter). Among the nodules,62(41.3%)had clear boundaries,while 88(58.7%)had indistinct boundaries;61(40.7%)exhibited regular shapes,while 89(59.3%)were irregular;66(44.0%)had a taller-than-wide aspect ratio;and 70(46.7%)showed microcalcifications. The collaborative diffusion model based on dynamic ultrasound image segmentation achieved the following scores:a Jaccard score of(69.22 ± 0.03)%,a Dice score of(79.16 ± 0.18)%,a Precision score of(86.70 ± 0.17)%,a Recall score of(77.82 ± 0.04)%,an Sα score of(85.26 ± 0.01)%,and an Eθmn score of(90.58 ± 0.17)%. Compared to other models,this model demonstrated significant improvements across all evaluation metrics,achieving the highest values in each metric with increments of over 8% and 1%,respectively. Conclusions:The collaborative diffusion model with a dynamic controller,constructed based on dynamic ultrasound images of thyroid nodules,demonstrates excellent performance in ultrasound image segmentation. It improves the accuracy of thyroid nodule screening,thereby providing a valuable auxiliary diagnostic tool for clinical practice.

8.Effects of miR-7-5p on the proliferation,apoptosis,and immune escape of esophageal squamous cell carcinoma KYSE-150 cells by regulating FOXM1

Objective:To investigate the effects of miR-7-5p on the proliferation,apoptosis,and immune escape of esophageal squamous cell carcinoma(ESCC)KYSE-150 cells by regulating forkhead box M1(FOXM1).Methods:The targeted binding sites of miR-7-5p and FOXM1 were verified by the dual-luciferase reporter gene assay.The cancer tissues and adjacent paracancerous tissues as well as the basic clinical data of 56 ESCC patients hospitalized at Changzhou Geriatric Disease Hospital affiliated to Soochow University between January 2022 and October 2024 were collected.qRT-PCR was used to detect the expression levels of miR-7-5p and FOXM1 in ESCC tissues,and analyze the relationship between their expressions and clinicopathological characteristics.Normal cultured KYSE-150 cells were selected as the Control group Plasmids were transfected into KYSE-150 cells using Lipofectamine 3000 transfection reagent,and the cells were assigned into the Mimic-NC group,the miR-7-5p mimic group,the miR-7-5p mimic+OE-NC group,and the miR-7-5p mimic+OE-FOXM1 group.EdU staining and CCK-8 assay were used to detect the proliferation ability of KYSE-150 cells.Flow cytometry was performed to detect the apoptosis of KYSE-150 cells and CD8+T cells.Western blot assay was performed to detect the expression levels of PD-L1,FOXM1,BAX,and PCNA proteins in KYSE-150 cells.A nude mouse transplanted tumor model of KYSE-150 cells was established to observe the effects of miR-7-5p overexpression on the growth of the transplanted tumors and the expressions of Ki-67 and FOXM1 in the tissues.Results:miR-7-5p could target and negatively regulate FOXM1(P<0.05).miR-7-5p expression was low and FOXM1 expression was high in ESCC tissues(both P<0.05).The expressions of miR-7-5p and FOXM1 were significantly correlated with TNM stage and differentiation degree,respectively(all P<0.05).The rates of EdU-positive cells,cell proliferation ability,CD8+T cell apoptosis rate,as well as PD-L1,PCNA,and FOXM1 mRNA and protein in the miR-7-5p overexpression group were significantly reduced(all P<0.05),while the apoptosis rate,miR-7-5p,and BAX increased significantly(all P<0.05).Meanwhile,overexpression of FOXM1 could reverse the above effects(all P<0.05).Overexpression of miR-7-5p decreased the mass and volume of transplanted tumors,and the expressions of Ki-67 and FOXM1 proteins in transplanted tumor tissues(all P<0.05).Conclusion:Overexpression of miR-7-5p can significantly inhibit the proliferation and immune escape of KYSE-150 cells and promote cell apoptosis,which may be achieved by targeted negative regulation of FOXM1.

10.Chinese surgical robot-assisted surgery for parotid tumor:a case report

Robotic surgery is known as the"third technological revolution"in the field of surgery,and is an important milestone in the development of modern surgery.However,our country's innovative surgical robot industry is still in its early stages,and it is only being utilized in certain surgical fields.To explore the effectiveness of the application of do-mestic surgical robot in oral and maxillofacial surgery,the author successfully completed a case of benign parotid tumor resection with the assistance of a domestic autonomous robot.The operation was successful,facial nerve function was preserved,and postoperative wound healing was good.
