This article introduces the clinical experience of Professor WANG Jihong in treatment of post-stroke hypermyotonia with Lai's tongyuan therapy (therapy for removing obstruction and consolidating vital essence). Regarding the pathogenesis of this disease, Professor WANG Jihong emphasizes a "trinity" approach, "the mind disturbed, qi and blood unbalanced, and yuan (vital essence) not consolidated". In treatment, "the unblocking, conducting and nourishing are taken as the fundamental" to "prevent from over-dredging, balancing yin and yang, and avoiding over-replenishing". In clinical practice, the combined measure is adopted with acupuncture, tuina and herbal medicine. In acupuncture, the acupoints are selected according to the front-back combination of the points of the conception vessel and the governor vessel (basic prescription: Yintang [GV24+], Baihui [GV20] and Niesanzhen; prescription for promoting the governor vessel: Dazhui (GV14), Jinsuo [GV8], Xinshu [BL15], Pishu [BL20] and etc.; and the prescription for consolidating the vital essence: Danzhong (CV17), Zhongwan [CV12], Tianshu [ST25], Qihai [CV6]) and etc., as well as those on the four limbs for "opening, closing and pivoting of yin and yang" (on the upper limbs of the affected side: Jiquan [HT1], Jianyu [LI15], Quchi [LI11], Quze [PC3] and etc.; on the lower limbs of the affected side: Huantiao [GB30], Liangqiu [ST34], Xuehai [SP10], Yanglingquan [GB34]) and etc. The above points co-act on regulating the conception vessel and the governor vessel, and balancing yin and yang. Tuina is delivered on the governor vessel, the bladder meridian of foot-taiyang and wind points on the area between the head and nape, so as to promote the governor vessel, benefit the orifices, strengthen the spine and relax tendons. Herbal medicine works on nourishing the liver to soften tendons, nourishing blood to remove wind, regulating qi to replenish the spleen, consolidating the root to cultivate the vital essence, activating blood circulation to unblock meridians, and removing stasis to resolve phlegm. Tongyuan therapy provides a certain instruction in treatment of post-stroke hypermyotonia.

Bisphenol A(BPA)is an environmental endocrine disruptor commonly found in industri-al products such as plastics, resin coatings, and paper coatings, and it poses potential reproductive hazards.Despite extensive research, studies examining its effects on the F1 generation of rabbits are limited. This study established a BPA exposure model in pregnant female rabbits to investigate its impact on reproductive hormones, apoptosis, oxidative stress, inflammatory responses, and tissue integrity in weaned female offspring. The results indicate that BPA exposure induces oxidative stress and inflammation in F1 rabbits, disrupts hormonal balance, and affects antioxidant enzymes and inflammatory mediators through the modulation of the Nrf2 and NF-κB signaling pathways, ultimately leading to apoptosis and tissue damage.

Objective To investigate the relationship between enlarged perivascular spaces in the hippocampus(H-EPVS)and cognitive function in patients with type 2 diabetes mellitus(T2DM),and to explore whether the H-EPVS can be used as an imaging marker of cognitive impairment in T2DM.Methods A total of 93 T2DM patients were selected(including 56 patients with cognitive impair-ment and 37 patients with normal cognition). All patients received head MRI scanning and neuropsychological tests. The differences of the H-EPVS counts and various parameters between the two groups were compared.Multiple linear regression was used to evalu-ate the association between H-EPVS and cognitive function. Results Compared with the T2DM patients with normal cognition, the cognitive impairment had significantly higher total H-EPVS counts(P=0.015),and worse performance of global cognitive function, processing speed, immediate memory and delayed recall(P<0.05).After adjusting for confounding factors by multiple linear regres-sion,total H-EPVS were associated with global cognitive function and immediate memory. Conclusion The H-EPVS are correlated with global cognitive function and memory function in patients with T2DM.The H-EPVS may be used as an imaging marker of cog-nitive impairment in T2DM patients.

Objective To investigate the relationship between the ABCB1 rs1045642 polymorphism and clopidogrel resistance (CR) among Chinese Han patients with ischemic stroke. Methods In total,110 patients with acute ischemic stroke were included in the study.Venous blood samples (2 mL) were collected after oral administration of 75 mg/d clopidogrel;the platelet aggregation rate was measured before clopidogrel administration and after 7 days of clopidogrel administration.CR is defined as no more than 10% change in the platelet aggregation rate over two times.Based on the results, patients were divided into the CR group and non-CR (NCR) group.Genomic DNA was extracted, the target gene was amplified by PCR,and the SNP was determined by RFLP.Results The TT genotype frequency of ABCB1 rs1045642 was significantly higher in the CR group than in the NCR group (P =0.003).T allele carriers in CR group show much more (57.1% vs 34%,P =0.001) compared with those in the NCR group. Conclusion The ABCB1 rs1045642 polymorphism may be an independent risk factor and the T allele may be a gene indicating risk for CR among Chinese Han patients with ischemic stroke.

Objective To retrospectively evaluate the clinical effects of end-to-end suture in repairing acute closed Achilles tendon ruptures. Methods Twenty-eight consecutive patients(20 males and 8 females, aged from 19 to 48 years with a mean of 36.5) with acute closed Achilles tendon rupture admitted from Nov.2000 to Jun.2006 were involved in present study. All patients were produced to have complete Achilles tendon rupture by MRI,and they received surgical procedure within 4 days after injury. An incision of 1cm long was made medial to the tendon, sparing the lesser saphenous vein and sural nerve. The tendon stumps were minimally debrided and end-to-end sutured using the Kessler or Bunnell techniques. The patients were followed-up for 12 to 36 months(mean 20 months),the therapeutic effects were evaluated by Arner-Lindholm scoring system, and postoperative complications were documented during the follow-up period. Results Of the 28 patients,19 got excellent therapeutic effect and 9 got good as evaluated by Arner-Lindholm scoring system, the satisfactory rate was 100%.All the patients regained normal gait, and rehabilitative training was initiated 3 months after operation. The postoperative complications included superficial wound infection in one case and skin edge necrosis in one case, resulting in a 7.1% incidence of complication. No peripheral nerve injury or Achilles tendon re-rupture occurred during the follow-up period. Conclusions An excellent therapeutic effect and minimal postoperative complication may be expected with end-to-end suture for repairing acute closed Achilles tendon rupture.
