Objective Under the background of DIP payment reform,the data quality of medical record home pages has become a core factor affecting the efficiency of hospital medical insurance settlement and economic benefits.Methods This study aimed to address the issues in the completion of medical record home pages by residents in tertiary hospitals and constructed an improvement system based on the Six Sigma method and RBRVS.Results In 2023,the defect rate of medical record home pages was 5.97％,with major defects concentrated in the names of surgical procedures(19.9％)and the filling of primary diag-noses(18.15％).By establishing a closed-loop management scheme under the"structure-process-outcome"theoretical frame-work-including standardized entry processes,a"three-medical linkage"supervision mechanism,and an RBRVS-based perform-ance appraisal system-the error rate of medical record home pages dropped to 2.04％in 2024 after applying Six Sigma improve-ments.Conclusion The RBRVS performance indicators,through differentiated weight design,can motivate physicians'initia-tive and improve the data quality of medical record home pages,thereby achieving accurate medical insurance settlement.It is recommended to explore intelligent verification paths combined with AI technology in the future to achieve synergistic optimization of medical record quality and hospital operations.

[Objective]To observe the clinical effect of"lung-intestinal treatment"method for adjuvant treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)mechanical ventilation patients with syndrome of lung heat and Fu-viscera excess.[Methods]Sixty-two patients with AECOPD in Pulmonary and Critical Care Medicine(PCCM)and Intensive Care Unit(ICU)of the Third Affiliated Hospital of Zhejiang Chinese Medical University were selected and divided into conventional treatment group and treatment group by random number table,31 cases in each group.The conventional treatment group was treated with western medicine,and the treatment group was added with lung-intestinal treatment on the basis of the conventional treatment group.The traditional Chinese medicine(TCM)syndrome points,blood gas index,ventilator parameters,mechanical ventilation time,complications and clinical effects were observed.[Results]Compared with before treatment,the conventional treatment group had improvement in the blood gas index,ventilator parameters,TCM syndrome(cough and asthma,sputum,abdominal distension and constipation,fever,chest tightness)and total points after treatment(P＜0.05);after treatment,the blood gas index,ventilator parameters,TCM syndrome(cough and asthma,sputum,abdominal distension and constipation,fever,chest tightness,loss of appetite)were relieved and total scores were all improved in the treatment group(P＜0.05).Compared with the conventional treatment group,the treatment group had significantly improved blood gas index,ventilator parameters and clinical effects,the TCM symptoms(chest tightness,cough and asthma,abdominal distension and constipation,loss of appetite)were relieved and total scores were improved(P＜0.05),and the time of mechanical ventilation and complications were reduced compared with the conventional treatment group(P＜0.05).[Conclusion]The treatment of AECOPD mechanical ventilation patients with"lung-intestinal treatment"method has clear cu rative effect,which can relieve clinical symptoms,shorten the time of mechanical ventilation,and the efficacy is better than that of western medicine alone.

Objective:To explore the classification performance of combined model constructed from CT signs combined with radiomics for discriminating COVID-19 pneumonia and other viral pneumonia.Methods:The clinical and CT imaging data of 181 patients with viral pneumonia confirmed by reverse transcription-polymerase chain reaction in 15 hospitals of Yunnan Province from March 2015 to March 2020 were analyzed retrospectively. The 181 patients were divided into COVID-19 group (89 cases) and non-COVID-19 group (92 cases), which were further divided into training cohort (126 cases) and test cohort (55 cases) at a ratio of 7∶3 using random stratified sampling. The CT signs of pneumonia were determined and the radiomics features were extracted from the initial unenhanced chest CT images to build independent and combined models for predicting COVID-19 pneumonia. The diagnostic performance of the models were evaluated using receiver operating characteristic (ROC) analysis, continuous net reclassification index (NRI) calibration curve and decision curve analysis.Results:The combined models consisted of 3 significant CT signs and 14 selected radiomics features. For the radiomics model alone, the area under the ROC curve (AUC) were 0.904 (sensitivity was 85.5%, specificity was 84.4%, accuracy was 84.9%) in the training cohort and 0.866 (sensitivity was 77.8%, specificity was 78.6%, accuracy 78.2%) in the test cohort. After combining CT signs and radiomics features, AUC of the combined model for the training cohort was 0.956 (sensitivity was 91.9%, specificity was 85.9%, accuracy was 88.9%), while that for the test cohort was 0.943 (sensitivity was 88.9%, specificity was 85.7%, accuracy was 87.3%). The AUC values of the combined model and the radiomics model in the differentiation of COVID-19 group and the non-COVID-19 group were significantly different in the training cohort ( Z=-2.43, P=0.015), but difference had no statistical significance in the test cohort ( Z=-1.73, P=0.083), and further analysis using the NRI showed that the combined model in both the training cohort and the test cohort had a positive improvement ability compared with radiomics model alone (training cohort: continuous NRI 1.077, 95 %CI 0.783-1.370; test cohort: continuous NRI 1.421, 95 %CI 1.051-1.790). The calibration curve showed that the prediction probability of COVID-19 predicted by the combined model was in good agreement with the observed value in the training and test cohorts; the decision curve showed that a net benefit greater than 0.6 could be obtained when the threshold probability of the combined model was 0-0.75. Conclusion:The combination of CT signs and radiomics might be a potential method for distinguishing COVID-19 and other viral pneumonia with good performance.

Objective:To preliminarily disclose the biological properties of recombinant human tau441 (P301S) protein, such as aggregation, antigenicity and immunogenicity.Methods:The recombinant plasmid tau441 (P301S) was expressed by prokaryotic expression system and purified by nickel column affinity chromatography. The protein concentrations were determined via BCA kit. The purity of protein was determined by SDS-PAGE gel coomassie brilliant blue staining. Western blot (WB) and negative staining transmission electron microscopy (TEM) were used to identify the recombinant proteins. The antigenicity was detected through indirect enzyme linked immunosorbent assay (ELISA), and the immunogenicity was detected by specific antibody titers of mouse immune serum.Results:The purity of recombinant human tau441 (P301S) was 70%. WB showed specific bands at relative molecular mass (Mr.×10 3) 64 and higher relative molecular mass. Negative staining TEM showed that tau441 (P301S) was aggregated, and the area was significantly larger than tau wild-type control protein (t=6.439, P=0.003). After 9 days of incubation at 4 ℃, tau441 (P301S) formed obvious fibrotic structure. Indirect ELISA result showed that tau441 (P301S) could be recognized by anti-tau monoclonal antibody HT7 (1∶80 000). The specific antibody titer of the immunized serum was 1∶128 000 and WB showed that the immunized serum recognized the brain lysate extract of Alzheimer’s disease (AD) transgenic mice. Conclusions:The recombinant human tau441 (P301S) protein had the characteristics of enhanced aggregation in vitro, but its antigenicity and immunogenicity were not changed.

Objective To explore the genetic mutation spectrum of patients with hypertrophic cardiomyopathy (HCM) and analysis the correlation of genotype phenotype.Methods Collect peripheral venous blood of the 51 cases unrelated HCM patients(35 male and 16 female) in the Beijing Anzhen Hospital of Capital Medical University from 2013 to 2016.Sequence whole exons of human and analysis seven major mutations of HCM including:MYBPC3、MYH7 、TNNT2、TNNI3 、MYL2 、TPM1 and ACTC1.Then compare the results with clinical characteristics.Results 24 patients(47.1％) had 22 kinds of pathogenicity or possibly pathogenicity mutations.The 90.9％ (20/22) of mutations only occurred one time,except MYH7 gene's 663 amino acid and the TNND gene's 157 amino acid which had twice.The mutations of MYBPC3,MYH7,TNNT2,TNNI3,MYL2,TPM1 and ACTC1 accounted for 45.8％ (11/24),20.8％ (5/24),12.5％ (3/24),8.3％ (2/24),8.3％ (2/24),4.2％ (1/24),and 0 respectively.No amphimutation had been found that causes illness or possibly.Through the comparison of clinical features between Genotype positive(24 cases) and negative(27 cases) patients:the incidence of syncope(19.6％ vs.7.8％,P ＜ 0.05),the largest left ventricular wall thickness[(22.8 ± 2.6) mm vs.(20.0 ± 3.4) mm,P ＜ 0.05],family history of HCM(20.8％ vs.0,P ＜0.05),percentage of apical hypertrophy(25.5％ vs.11.8％,P ＜ 0.05);The ratio of left ventricular outflow tract obstruction in MYH7 group was higher than MYBPC3 group (80.0％ vs.18.2％,P ＜ 0.05).Conclusion MYBPC3 is the most common mutation gene in HCM patients.Phenotype is more severe in geuotype positive patients than in genotype negative patients.Relationship between specific gene mutations and clinical phenotype requires further study.

Objective To explore the value of T2-mapping imaging in assessment of lumbar dorsal extensors group (multifidus,longissimus and iliocostalis) function before and after exercises in healthy volunteers.Methods Totally 50 healthy young volunteers underwent T2-mapping imaging before and after exercises.T2 value and cross-section area (CSA) of multifidus,longissimus and iliocostalis were measured at the level of L3 and 14 superior border.Differences of CSA and T2 value were compared between left and right side muscles before and after exercises.The correlation between CSA difference and T2 difference before and after exercises was analyzed.Results At the level of L3 and L4 superior border,CSA and T2 value of bilateral multifidus,longissimus and iliocostalis increased after exercises (all P＜0.05).At the level of L3 superior border,CSA after exercises and CSA difference of left multifidus were higher than those of right side (all P＜0.05).Before exercises,there were statistical differences of T2 values on left and right side of longissimus and iliocostalis at the level of L3 superior border and the longissimus at the level of L4 superior border (all P＜0.05).Positive correlations were found between CSA difference and T2 difference on bilateral iliocostalis at the level of L3 and L4 superior border,as well as on bilateral multifidus at the level of L4 superior border (all P ＜0.05).Conclusion T2-mapping imaging can be used to assess the activity state of lumbar dorsal extensors group before and after exercises.

Objective To establish new classification criteria for early rheumatoid arthritis (E-RA) based on large samples of early inflammatory arthritis patients and to evaluate the value of this criteria in China.Methods Patients who had arthritic complaints with disease duration less than one year were enrolled.They were divided into RA group and non-RA group according to the clinical diagnosis by experienced rheumatologists.The clinical and laboratory parameters were analyzed and those with high sensitivity or specificity were selected as the new classification criteria.Statistical analysis was carried out by using t test,x2 test and Logistic regression.Results ① A total of 803 patients with early inflammatory arthritis were included in this study.Five hundreds and fourteen patients were diagnosed as early RA and 251 were diagnosed as other rheumatic diseases,and the diagnosis of 38 patients remained unestablished by the end of follow-up.② New E-RA classification criteria were established based on the parameters with high sensitivity and/or specificity.The sensitivity of the new E-RA criteria was 84.4％,which was higher than 1987 ACR criteria (58.0％),while the corresponding specificities were similar,which were 87.4％ and 93.6％ respectively.③ Compared with the complex scoring system of 2010 ACR/EULAR criteria,the E-RA criteria was more simple and practical.The diagnostic sensitivity and specificity of E-RA criteria were higher than those of 2010 ACR/EULAR criteria reported in the literatures.④ New classification criteria based on scoring system using Logistic regression analysis was established.The sensitivity of this criteria was 86.4％,which was higher than 1987 ACR criteria (58.0％).Conclusion The diagnostic value of the E-RA criteria developed in this study for early RA is better than 1987 ACR criteria,and is more simple than 2010 ACR/EULAR criteria.It may be used as a new classification criteria for early RA diagnosis.

In order to study the effects of ethyl pyruvate on cardiomyocyte apoptosis following ischemia/reperfusion (I/R) in vitro and the expression of Bcl-2 and Bax proteins, isolated rat hearts were perfused in a Langendorff model. Twenty-four rats were randomly divided into 3 groups (n=8 in each group): control group was perfused for 120 min. In the I/R group, after 30 min stabilization the injury was induced by 30 min global ischemia followed by 60 min reperfusion. Ethyl pyruvate (EP) group was set up with the same protocol as I/R group except that it was supplied with 2 mmol/L EP 15 min before ischemia and throughout reperfusion. Myocardial malonaldehyde (MDA) content was measured. Myocardial apoptotic index (AI) was tested by terminal deoxynucleotidyl transferase mediated dUTP nick end labeling (TUNEL) method. The expression of anti-apoptotic protein Bcl-2 and pro-apoptotic protein Bax in cardiac myocytes was detected by immunohistochemistry. As compared with control group, the content of MDA, myocardial AI and the expression of Bcl-2, Bax proteins were increased significantly in I/R group, but the content of MDA, myocardial AI and the expression of Bax protein were decreased obviously and the expression of Bcl-2 protein was up-regulated in EP group (P<0.05). These results demonstrate that EP could inhibit apoptosis of cardiac myocytes possibly via alleviating oxidative stress, up-regulating Bcl-2 and down-regulating Bax proteins.
