Objective:To investigate the training needs of general practitioners in community health center in health care and management of older patients for developing a curriculum for the Extension for Community Health Outcomes (ECHO) project accordingly. Methods:This was a qualitative study. The semi-structured interviews were conducted with 20 community health center general practitioners (GPs) using focus groups between March 2024 and April 2024. The interview data were collated using Nvivo12 software and coded using thematic analysis. The focus of the interviews was on the challenges GPs face in health care of older people in the community and relevant training needs. Results:Three male and 17 female GPs took part in the interviews. The interviews showed that GPs in comminity health service centers faced multiple challenges in the helth management of older patients, including implementation of holistic treatment plans, time-consuming comprehensive patient assessment, lack of standardized medication selection, unclear referral processes and difficulties in establishing a trusting doctor-patient relationship. The study suggested that the design and development of training programs should encompass eight core components: differentiated end-of-life care for patients, nutritional advice, positive psychological intervention, software data-based health assessment, systematic geriatric medicine, updated clinical research findings, medication selection and treatment guidelines, and insulin resistance management. Conclusion:Through qualitative investigation, this study explored the challenges faced by general practitioners in providing health care for older patients and relevant training needs, which provids evidence for designing and developing corresponding curricula for ECHO projects.

Objective To investigate the causal relationship between frailty and epilepsy based on the traditional Chinese medicine(TCM)theory of"epilepsy caused by frailty"utilizing a two-sample Mendelian randomization approach,to enrich the modern connota-tion of the"epilepsy caused by frailty"theory and provide a theoretical basis for treating epilepsy from a frailty perspective in TCM.Methods Data on frailty and epilepsy were extracted from publicly available genome-wide association study summary datasets.Single nucleotide polymorphism(SNP)were selected as instrumental variable under the conditions of P<5 x 10-8,a linkage disequilibrium co-efficient of 0.001,and a linkage disequilibrium region width of 10000kb.Mendelian randomization analysis was performed using five methods:MR-Egger regression, weighted median, inverse variance weighting, simple mode and weighted mode, with inverse variance weighting as the primary analysis method. The causal relationship between frailty and epilepsy was interpreted using OR and 95%CI.Pleiotropy was assessed using the MR-Egger intercept and MR-PRESSO analysis methods, while heterogeneity was analyzed using MR-Egger Cochran's Q test, and sensitivity analysis was conducted using the leave-one-out method. Results The results of inverse variance weighting and weighted median analysis indicated that there was a positive causal relationship between frailty and epilepsy. This result passed pleiotropy tests, confirming its robustness. Conclusion Frailty is associated with an increased risk of epilepsy, which is consistent with the TCM understanding of"epilepsy caused by frailty".It is suggested that in clinical treatment, besides conventional ap-proaches such as resolving phlegm, eliminating blood stasis, calming wind, and opening orifices, there should be a greater emphasis on tonifying frailty and nurturing the body's vital energy.

Objective To explore the patterns of functional gait asymmetry during the support phase and the differentiation of bilateral foot functions,to gain insights into the human movement compensa-tion mechanisms and stability control characteristics,so as to provide theoretical references for the pre-vention and rehabilitation treatment of unilateral foot and ankle injuries. Methods Twelve young females with normal feet were recruited, and the time percentage of each phase of the support period, the change of longitudinal arch angle, the vertical ground reaction force and the center of plantar pressure trajectory of the dominant and non-dominant feet were collected by using a three-dimensional motion capture and a high-frequency plantar pressure testing system when the subjects walked at the optimum speed with the incremental loading of weights[0%body weight(BW),10%BW and 20%BW].The ef-fects of laterality(dominant foot,non-dominant foot)and load(0%BW,10%BW,20%BW)on arch changes and gait parameters, and the interactions between them, were analyzed using a two-way re-peated-measures ANOVA.Results The factors of laterality and load interacted significantly on arch changes, but not on gait parameters.(2)Compared with 0%BW,the time percentage of the buffering phase of the nondominant feet with the load of 20%BW increased(P<0.05),while that of the domi-nant ones decreased(P<0.05),with an increase in the buffering load of both feet with the load of 20%BW(P<0.05)and the buffering force excursion of both feet with the load of 10%BW(P<0.05).More-over,changes in longitudinal arch angle increased(P<0.05)with the load of 10%and 20%BW,the pedal extension load rate increased(P<0.05)with the above BW load in the non-dominant feet, and the horizontal slope of pedal extension decreased(P<0.05)with 20%BW load in the dominant feet.Compared with 10%BW,when the feed was loaded 20%BW,the time percentage in the buffering phase and the buffering force offset of the dominant feet decreased(P<0.05),while the buffering load of both feet increased(P<0.05).(3)Compared with the non-dominant feet, for the dominant feet, the buffering load rate at the load of 10%and 20%BW and changes in the longitudinal arch angle at the load of 20%BW decreased(P<0.05),while the horizontal slope of the pedal extension without load in-creased(P<0.05).Conclusion Walking with load induces function differentiation in bilateral feet. The dominant foot has more efficient horizontal extension without load and better buffering function in the vertical direction with loads, while the non-dominant feet perform a more adequate weight-bearing and extension function vertically with larger load.It is suggested that unilateral injury feet should not be given too much load, and it is inappropriate to select a single foot to represent and evaluate the func-tion and injury risk of the double feet in loaded walking.

Journal of Beijing University of Traditional Chinese Medicine 2024;47(7):934-938
In Treatise on Cold Pathogenic and Miscellaneous Diseases, there are five articles concerning"diet as usual".When many doctors annotate such articles, they mostly interpret"diet as usual"as normal diet or because of stomach qi not affected by disease, ignoring the true significance of"diet as usual"and its role in clinical differential diagnosis.Through sorting out and summarizing the relevant provisions of"diet as usual",combining with the comments of Shuowen Jiezi and various ancient and modern doctors on the relevant provisions of"diet as usual"to explore the meaning behind it,the author believes that"diet as usual"can only be understood as"diet as before".Because it exists in a variety of diseases,it cannot be blindly extended to"normal diet"."Diet as usual"has two functions in clinical differential diagnosis:on the one hand, the stomach is empty, and no solid no drink blocks the qi movement,or there is stagnant heat in the stomach and intestines, but has not yet formed dry feces;on the other hand, when the middle jiao becomes one of the pathogenic factors of the disease,"diet as usual"can exclude the influence of the middle jiao.
