1.Correlation between serum RAR,FAM19A5 and LECT2 expression and glycolipid metabolism in patients with type 2 diabetes mellitus complicated with metabolic syndrome

Objective To investigate the relationship between the serum red blood cell distribution width(RDW)to albumin(ALB)ratio,the expression of family with sequence similarity 19 member A5(FAM19A5)and leukocyte cell-derived chemotaxin 2(LECT2)with the glucose and lipid metabolism indexes in type 2 diabetes mellitus(T2DM)patients with metabolic syndrome(MS).Methods A total of 119 patients with T2DM were selected as the study group,and patients were divided into the MS group(67 cases)and the non-MS group(52 cases)based on whether they were complicated with MS.Meanwhile,93 healthy individuals who underwent physical examinations in our hospital during the same period were selected as the control group.RDW was detected by an automatic blood cell analyzer,and ALB was detected by an automatic biochemical analyzer.The ratio of RDW to ALB(RAR)was calculated.The serum levels of FAM19A5 and LECT2 were detected by enzyme-linked immunosorbent assay(ELISA).The correlation between serum and glycolipid metabolism indicators was analyzed by Pearson's method.The diagnostic value of serum RAR,FAM19A5 and LECT2 for MS in T2DM patients was analyzed by receiver operating characteristic(ROC)curve.Results Compared with the control group,the study group exhibited significantly higher levels of total cholesterol(TC),triacylglycerol(TG),low-density lipoprotein cholesterol(LDL-C),fasting blood glucose(FBG),serum RAR,FAM19A5 and LECT2,while high-density lipoprotein cholesterol(HDL-C)was significantly lower(P<0.05).Compared to the non-MS group,body mass index(BMI),duration of T2DM,history of hypertension,blood pressure,waist circumference,TG,fasting insulin(FINS),FBG,glycated hemoglobin(HbA1c),homeostasis model assessment-insulin resistance(HOMA-IR),serum RAR,FAM19A5 and LECT2 were significantly increased,and HDL-C was decreased in the MS group(P<0.05).Serum RAR,FAM19A5 and LECT2 were positively correlated with TG,FINS,FBG,HbA1c and HOMA-IR,and which was negatively correlated with HDL-C(P<0.05).The area under the curve(AUC)for the diagnosis of T2DM patients with MS using RAR,FAM19A5 and LECT2 individually were 0.821,0.807 and 0.793,respectively,and the combined AUC was 0.952,indicating that the combined diagnostic efficacy of RAR,FAM19A5 and LECT2 was superior to their individual efficacies(P<0.05).Conclusion Serum levels of RAR,FAM19A5 and LECT2 are elevated in T2DM patients with MS.These biomarkers exhibit a strong association with indices of glucose and lipid metabolism.The combination of these three markers has significant clinical diagnostic value for T2DM patients with MS.
2.Application of smart wearable device in postoperative rehabilitation of patient with glioma

Objective:To investigate the application effect of smart wearable device in the postoperative rehabilitation of patients with glioma and its influence on postoperative recovery.Methods:Eighty patients with glioma who underwent surgical treatment at the First Affiliated Hospital of Harbin Medical University between January 2022 and September 2024 were selected,and they were randomly divided into an observation group and a control group using a random number table,with 40 cases in each group.Patients in the control group received routine follow-up and rehabilitation management after surgery.Patients in the observation group received smart wearable devices to monitor their postoperative recovery on the basis of the management for control group.The study evaluated the scores of the Cancer-Related Fatigue Scale(CFS),exercise amount(steps and activity time),sleep quality(total sleep time and sleep efficiency),and vital signs(heart rate and blood oxygen saturation)at the 1st week,1st month,and 3rd month after surgery for both groups.Results:At the 1st week after surgery,the differences of CFS scores,daily steps and activity time between two groups were not significant(P>0.05).At the 1st month and 3rd month after surgery,the CFS score of observation group was lower than that of control group,and the differences were significant(t=8.914,11.268,P<0.05).Additionally,the daily average steps of observation group were higher than that of control group,and the differences were significant(t=15.259,20.024,P<0.05).Furthermore,the activity time of observation group was significantly longer than that of control group,and the difference was significant(t=8.994,10.667,P<0.05).In the observation group,the heart rate and amplitude of blood oxygen fluctuation at the 1st month and 3rd month after surgery were less than them at the 1st week after surgery,and the recovery after surgery in patients was more stability,while the difference was not significant among them(P>0.05).Conclusion:Smart wearable device can effectively monitor the postoperative rehabilitation of patients with glioma,and help patients to improve fatigue,and increase physical activity,and enhance sleep quality,and stabilize vital signs by real-time feedback.

5.Remote monitoring of neonatal jaundice in newborns with ABO hemolytic disease.

OBJECTIVE:
To explore the feasibility of remote monitoring of neonatal jaundice in newborns with ABO hemolytic disease.
METHODS:
Forty six neonates of gestational age >35 weeks with ABO hemolytic disease admitted to Women's Hospital, Zhejiang University School of Medicine from January 20th, 2020 to February 29th, 2020 were enrolled in the study (study group). The newborns were followed up at home after discharge, the transcutaneous bilirubin (TCB) levels were measured by parents using the provided device and the results were sent to the doctor by smart phone using the installed APP. Fifty six newborns with ABO hemolytic disease admitted in 2018 who received conventional outpatient follow-up after discharge served as the control group. The demographic characteristics, total serum bilirubin (TSB) level during hospitalization, number of outpatient visit and rate of re-admission due to rebound hyperbilirubinemia were compared between the two groups.
RESULTS:
There were no significant differences between the two groups in gestational age, birth weight, delivery mode, gender, length of the first hospitalization, TSB level before phototherapy and before discharge, and the managements during the first hospitalization (all
CONCLUSIONS
The remote follow-up for neonatal jaundice at home can effectively reduce the number of outpatient visits without increasing the risk of readmission and severe neonatal hyperbilirubinemia for newborns with ABO hemolytic disease.

6.Endobronchial Ultrasound-guided Transbronchial Needle Aspiration in the Diagnosis of Intrathoracic Metastasis from Extrapulmonary Malignancy

Background and objective Endobronchial ultrasound guided transbronchial needle aspiration (EBUS-TBNA) has been widely applied in diagnosing mediastinal and hilar adenopathy. hTis study is further to evaluate value and safety of EBUS-TBNA in diagnosing intrathoracic metastasis from extrapulmonary malignancy. Methods Prospectively analysis of 41 patients suspected intrathoracic metastasis from previous diagnosed/concurrent extrapulmonary malignancies in Shanghai Chest Hospital, with radiologic ifndings showing mediastinal/hilar lymph node enlargement or intrapulmonary lesion requiring EBUS-TBNA examination for pathological diagnosis. Results 41 candidate patients enrolled, and 67 mediastinal/hilar lymph nodes and 5 intrapulmonary lesions were aspirated. 14 intrathoracic metastasis, 10 primary lung cancer, 9 reactive lymphadenitis, 4 sarcoid-like reactions, and 1 tuberculosis was diagnosed by EBUS-TBNA. Sensitivity and accuracy of EBUS-TBNA in diagnosing intra-thoracic metastasis was 87.50%and 95.12%, respectively. Immunohistochemistry (IHC) was performed in 18 malignant tumors to obtain deifnite type or origin, twelve intrathoracic metastasis and 6 primary lung cancer were further conifrmed. Conclusion EBUS-TBNA is a safe, effective method for the diagnosis of intrathoracic metastasis from extrapulmonary malignancy. IHC can provide additional evidence for distinguishing extrapulmonary malignancy from primary lung cancer.
