1.Effect of tele-supervised home-based pulmonary music therapy on chronic obstructive pulmonary disease

ObjectiveTo explore the effect of tele-supervised home-based pulmonary music therapy on respiratory function, exercise capacity and clinical symptoms in patients with chronic obstructive pulmonary disease (COPD). MethodsA total of 96 COPD patients admitted to the Second People's Hospital of Lianyungang from January, 2023 to June, 2024 were selected. They were divided into control group and observation group (48 cases each) using block randomization combined with a random number table. The control group received routine treatment and nursing intervention, while the observation group added tele-supervised home-based pulmonary music therapy, for six months. Evaluations were conducted before intervention and one, three and six months after intervention. ResultsThree cases in the control group and two in the observation group dropped out. Compared to the control group, the observation group was better in forced expiratory volume in one second percentage of predicted (FEV1%) level six months after intervention (χ2 = 10.481, P < 0.001), maximal inspiratory pressure three months (Z = -2.045, P = 0.044) and six months (Z = -3.643, P = 0.033) after intervention, maximal expiratory pressure three months (Z = 2.111, P = 0.036) and six months (Z = 4.025, P < 0.001) after intervention, exercise capacity one month (t = 2.266, P = 0.028), three months (t = 3.943, P = 0.028) and six months (t = 6.703, P < 0.001) after intervention, fatigue scores three months (Z = -4.352, P < 0.001) and six months (Z = -5.718, P < 0.001) after intervention, clinical symptoms three months (t = -2.698, P = 0.007) and six months (t = -2.898, P = 0.003) after intervention, and clinical control levels three months (t = -3.402, P < 0.001) and six months (t = -5.036, P < 0.001) after intervention. ConclusionTele-supervised home-based pulmonary music therapy is beneficial in improving respiratory function, exercise capacity, fatigue, clinical symptoms and clinical control levels in patients with COPD.

4.The decolorization efficiency and influencing factors of low-temperature plasma on crystal violet wastewater

Objective To investigate the decolorization performance and key influencing factors of a novel low temperature plasma(LTP)on crystal violet(CV)simulated wastewater.Methods In this experiment,CV was used as the target pollutant and treated with LTP generated by argon atmospheric pressure plasma jet;the absorbance(A590)and chemical oxygen demand(COD)of CV solution were systematically evaluated before and after LTP treatment by adjusting the treatment volume,initial pH value,NaCl concentration,addition of Fe 2+and other parameters.The dynamic changes of active particles in the liquid were monitored.Results Compared with those of the control group,the A590 and COD values of LTP treatment groups were significantly reduced(P<0.05).At the treatment time of 180 s,the decolorization rate and COD removal rate of CV solution reached 62.34%and 50.22%,respectively.Under the same treatment time(>90 s),the A590 values in the 0.1 mol/L NaCl group,pH=9 or 11 group,and Fe2+group were lower than those in LTP-only treatment group,but the A590 values in the n-butanol group were higher(P<0.05).The A590 values in 2 mL or 3 mL group were higher than those in 1 mL group(P<0.05).Compared with CV solution treated with LTP,the H2O2 content in the activated water was higher while the NO3content was significantly lower than those in CV solutions(P<0.05).Conclusion LTP has an efficient decolorization ability for CV solution.The decolorization of CV solution could be promoted by NaCl addition,Fe2+inclusion or high pH values,but was inhibited by the increase in treatment volume or the addition of n-butanol.The?OH and H2O2 generated by LTP played a key role in the decolorization process.
5.Treatment of Granulation Tissue Hyperplasia after Tracheostomy Based on the Kenang (窠囊) Theory

Granulation tissue hyperplasia after tracheotomy is a common clinical complication. Endoscopic treatment can temporarily relieve airway obstruction, however, it is associated with a high recurrence rate and poor long-term prognosis. Based on the traditional Chinese medicine (TCM) Kenang (窠囊) theory and combined with modern pathological mechanisms, this paper explores its correlation with the pathogenesis of post-tracheotomy granulation tissue hyperplasia. Drawing from clinical experience in applying the Kenang theory for treatment, this paper proposes that the fundamental pathogenesis of this condition lies in qi deficiency and organ dysfunction, while phlegm and blood stasis interlocking serve as the symptomatic manifestations. The treatment focuses on resolving phlegm and promoting blood circulation, dispersing nodules and eliminating stagnation, regulating qi flow, and reinforcing the body's vital energy while expelling pathogenic factors. This approach aims to dissolve phlegm and blood stasis, dissipate the Kenang, and ultimately prevent and treat granulation tissue hyperplasia.

8.Clinical characteristics and prognoses of 13 patients with Morvan syndrome with obvious electromyography characteristics

Objective:To summarize the clinical manifestations, neuroelectrophysiological characteristics and prognoses of Movan syndrome (MoS), and provide references for early diagnoses and prognoses.Methods:A retrospective analysis was performed. The clinical data, such as clinical symptoms, treatments and prognoses, laboratory test results and electrophysiological test results, of 13 patients with confirmed MoS in Department of Neurology, He'nan Provincial People's Hospital from January 2018 to October 2023 were collected.Results:Ten male MoS patients and 3 female ones were included. Main clinical manifestations of 13 patients with MoS included myokymia, pain, numbness of limbs, itching all over the body, hyperhidrosis, urinary and defecation disorder, tachycardia, insomnia, anxiety and depression. Ten patients completed the autoimmune encephalitis antibody detection: 3 only had positive anti-contactin-associated protein-like 2 (CASPR2) antibody, 2 only had positive anti-leucine-rich glioma-inactivated protein1 (LGI1) antibody, and 2 had both positive anti-CASPR2 antibody and anti-LGI1 antibody. Eleven patients completed tumor screening and 4 tumors (thymoma [ n=2], lung squamous cell carcinoma [ n=1] and adrenal non-Hodgkin's lymphoma [ n=1]) were noted. Ten patients completed electrocardiogram, including 3 patients with resting tachycardia and 2 patients with ST segment elevation. All patients completed the electromyographic examination; 12 patients showed abnormal motor unit potential, including myokymia potential, fasciculation potential and neuromyotonic potential; F-wave and/or M-wave post-discharge potentials were found in all patients. Follow up was performed for 1-12 months; in 9 non-tumor patients, 5 were improved in 6 patients accepted immunotherapy and one was improved in 3 patients received symptomatic treatment; in 4 tumor patients, only one was improved in 3 received immunotherapy. Conclusion:Myokymia, pain, urinary and defecation disorder, and severe insomnia are typical symptoms for MoS patients; serum anti-CASPR2/LGI1 antibody and electromyography results provide evidences for MoS diagnosis; early immunotherapy can improve the MoS prognosis, and MoS patients combined with tumors have poor clinical prognosis.
9.Impact of simulator-MDT-PBL integrated teaching on the quality of clinical internship teaching in gynecology department

Objective:To investigate the impact of simulator-multidisciplinary treatment (MDT)-problem-based learning (PBL) integrated teaching on the quality of clinical internship teaching in the Gynecology Department.Methods:A retrospective analysis was conducted on the data of 86 clinical interns in the Gynecology Department who completed one month of internship at Xijing Hospital, Air Force Medical University from May 2021 to May 2022. The interns who received routine teaching from May to October 2021 were enrolled into control group ( n=43), and those who received simulator-MDT-PBL integrated teaching from November 2021 to May 2022 were enrolled into observation group ( n=43). The two groups were compared for differences in professional abilities (measured by Readiness for Interprofessional Learning Scale [RIPLS]), humanistic care ability (measured by humanistic care ability scale), critical thinking ability (measured by critical thinking disposition inventory-Chinese version [CTDI-CV]), and autonomous learning ability (measured by college student autonomous learning scale) after one month of teaching. The chi-square test and t-test were conducted using SPSS 19.0. Results:The two groups showed significantly increases in RIPLS scores [team collaboration: (38.45±6.72) vs. (36.39±5.65); professional identity: (28.11±5.85) vs. (26.43±5.33); role responsibilities: (10.83±1.24) vs. (9.52±1.46)], humanistic care ability scale scores [understanding: (68.73±8.15) vs. (61.55±8.93); patience: (53.45±6.03) vs. (49.57±6.74); encouragement: (56.83±7.24) vs. (52.75±7.46)], and all dimensions of CTDI-CV scale and college student autonomous learning scale, with all scores higher in the observation group than in the control group (all P<0.05). Conclusions:The simulator-MDT-PBL integrated teaching can help improve the professional ability, humanistic care ability, critical thinking ability, and autonomous learning ability of clinical interns in the Gynecology Department, thus improving the quality of clinical internship teaching in the Gynecology Department.
10.Effects of Ppp1r17 down-regulation on drinking-related behaviors and AKT/GSK-3β/CREB signaling pathway activation in mice

AIM:To investigate the impact of hippocampal protein phosphatase 1 regulator subunit 17(Ppp1r17)down-regulation on drinking-related behavior in mice,and to explore the protein kinase B(PKB/AKT)/glyco-gen synthase kinase-3β(GSK-3β)/cAMP response element binding protein(CREB)signaling pathway of Ppp1r17 in the development of alcohol dependence.METHODS:Forty male C57BL/6J mice were randomly divided into four groups(n=10):control group,shPpp1r17① group,shPpp1r17② group and shPpp1r17③ group.The mRNA and protein expression levels in the hippocampal tissues were evaluated after 3 weeks of AAV-shPpp1r17 injection.Twenty male C57BL/6J mice were randomly divided into a control group and a shPpp1r17 group(n=10).An open-field test,conditioned place prefer-ence(CPP)test and righting reflex test were performed.Furthermore,the localization and expression of AAV-shPpp1r17 were detected after 3 weeks of AAV-shPpp1r17 injection.Twenty male C57BL/6J mice were randomly divided into four groups(n=5):the empty virus+water group,the empty virus+EtOH group,the shPpp1r17+water group and the shPpp1r17+EtOH group.The EtOH group mice drank 9%alcohol continuously for 30 d.The protein expression levels of Ppp1r17,p-AKT,AKT,p-GSK-3β,GSK-3β,p-CREB and CREB were detected by Western blot.RESULTS:(1)The results of qPCR showed that Ppp1r17 was successfully suppressed by shPpp1r17.(2)The behavioral results showed that the shPpp1r17 group mice exhibited enhanced exercise ability and reduced anxiety-like emotions,and the downregulation of Ppp1r17 increased CPP scores and reduced the sensitivity of the mice to alcohol.(3)Immunofluorescence showed that AAV-shPpp1r17 was specifically expressed in the hippocampus.(4)Western blot analysis revealed that Ppp1r17 and the p-AKT/AKT,p-GSK-3β/GSK-3β and p-CREB/CREB ratios were significantly increased after treatment of EtOH.Howev-er,the protein expression of Ppp1r17 was significantly reduced,and the AKT/GSK-3β/CREB pathway was activated after knockdown of Ppp1r17 in the hippocampus.CONCLUSION:Ppp1r17 may suppress CREB phosphorylation through the AKT/GSK-3β/CREB pathway,thereby influencing alcohol drinking preference and locomotor behavior in mice.
