1.Correlation of metabolic comorbidities and insulin resistance with CKD in an elderly population taking physical exam

Objective To explore the relationship of metabolic comorbidities and insulin resistance(IR)with chronic kidney disease(CKD)among elderly individuals undergoing health exam.Methods A cross-sectional observational study was conducted on 8358 older adults who took health exam in Chinese PLA General Hospital between December 2009 and May 2021.According to the guidelines for CKD diagnostic criteria,they were divided into CKD group(983 cases)and non-CKD(7375 cases).Clinical data was collected,and the eGDR was calculated.Quasi-Bayesian method was used for causal mediation analysis.Results The prevalence of metabolic comorbidi-ties including hypertension,CHD,DM,hyperlipidemia,and hyperuricemia was significantly higher in the CKD group than the non-CKD group(P<0.01).The eGDR was obviously lower in the CKD group than the non-CKD group[6.88±2.09 mg/(kg·min)vs 8.41±2.12 mg/(kg·min),P<0.01].Logistic regression analysis revealed that,without adjusting covariates,each 1-unit increase in eGDR was associated with a 29%reduction in the risk of developing CKD(OR=0.714,95%CI:0.691-0.738,P<0.01),and after adjusting covariates,eGDR remained signifi-cantly negatively association with the risk of CKD(P<0.01).Mediation analysis indicated that DM and brachial-ankle pulse wave velocity accounted for the highest proportions of the mediating effect in the relationship between eGDR and CKD(14.2%and 12.5%,respectively).Conclusion In the elderly population undergoing health exam,reduced insulin sensitivity is significantly asso-ciated with the development of CKD.Diabetes and arteriosclerosis exert mediating effect in this association.
2.Correlation between atherogenic index of plasma and prognosis of patients with acute myocardial infarction

Objective To investigate the effect of baseline atherogenic index of plasma(AIP)on the long-term prognosis of patients with acute myocardial infarction(AMI).Methods A total of 712 AMI patients admitted to the hospital from January 2018 to December 2019 were continuously included as subjects and divided into a low-value group(AIP<0.280,n=237),a median-value group(AIP 0.280~0.852,n=238)and a high-value group(AIP>0.852,n=237)according to the baseline AIP tertiles.The primary endpoint was defined as the occurrence of major cardiovascular adverse events(MACEs).Multivariate Cox regression was used to analyze the independent influencing factors of MACEs.The nonlinear relationship between AIP and the risk of MACEs was analyzed with restricted cubic spline plots.Kaplan-Meier curve was used to analyze survival differences between groups.Subgroup analysis assesses the consistency of AIP's predictive value to MACEs.Results With the increase of AIP tertile groups,the proportion of dyslipidemia and MACEs increased,white blood cell count,fasting blood glucose,triglyceride,total cholesterol,low density lipoprotein and AIP increased,and high-density lipoprotein decreased,with statistical significance(P<0.05).Multivariate Cox regression analysis showed that AIP was an independent risk factor for MACEs(HR=2.024,95%CI:1.211~3.381,P=0.007).The results of restricted cubic spline analysis show that there is an L-shaped nonlinear effect relationship between AIP and the risk of MACEs(P-nonlinear=0.008).When AIP>0.613,the risk of MACEs in AMI patients increases with the increase of AIP.Kaplan-Meier survival curve analysis results show:With the increase of AIP,the cumulative incidence of MACEs in AMI patients increased significantly(Log-rank test,P=0.032).Compared with the low-value group,the risk of MACEs in the high-value group increased by 131%(HR=2.311,95%CI:1.261~4.234,P=0.007).The results of subgroup analysis showed that the P value of interaction within each subgroup was not significant,and the ability of AIP to predict MACEs was applicable to all subgroups.Conclusion Increased AIP at baseline is an independent predictor of poor long-term prognosis in patients with AMI.
3.Clinical study on osteogenic effect of sticky bone and autologous iliac cancellous bone graft in repairing unilateral alveolar cleft

Objective·To explore the clinical efficacy and safety of sticky bone[deproteinized porcine bone mineral granules combined with advanced platelet-rich fibrin(A-PRF)and liquid platelet-rich fibrin(L-PRF)]in the repair of unilateral alveolar cleft.Methods·Patients diagnosed with unilateral alveolar cleft who met the inclusion and exclusion criteria were recruited from December 1,2023 to August 31,2024.The patients were randomly divided into 2 groups.The experimental group received sticky bone grafts,and the control group received autologous iliac cancellous bone grafts for alveolar cleft repair.The primary efficacy index was the bone resorption rate at 6 months post-surgery,calculated by measuring the grafted bone volume immediately after surgery and at 6 months using Simplant Pro 17.01 software based on patients'computed tomography data collected before surgery,immediately after surgery,and 6 months post-surgery.Secondary efficacy indices included bone density at 6 months after surgery,the occurrence of postoperative complications in patients,and the scores on the oral health-related quality of life scale.Prism 10 software was used for data analysis,and t-test and Pearson correlation analysis methods were adopted.Results·Seventeen patients with unilateral alveolar cleft were included in the experimental group,and 15 in the control group.The remaining bone volume in the experimental group at 6 months after surgery was more than that in the control group.The bone resorption rate in the experimental group was 33.24%±17.16%,significantly lower than 66.31%±17.98%in the control group(P<0.001).One patient in the experimental group had vestibular mucosal dehiscence accompanied by bone powder discharge 2 weeks after surgery;3 patients in the control group had vestibular mucosal dehiscence accompanied by cancellous bone discharge 1 month after surgery,with one case of grafted bone necrosis 4 months after surgery.The bone density in the experimental group at 6 months after surgery was significantly higher than that in the control group(P<0.001),and the scores on the oral health-related quality of life scale were lower.Conclusion·Compared with iliac cancellous bone,when sticky bone is used in the treatment of patients with unilateral alveolar cleft,it shows a lower bone resorption rate and higher bone density,indicating better osteogenic ability.Fewer number of complications and lower scores on the oral health-related quality of life scale suggest greater safety,thereby supporting its strong potential for clinical promotion and application.

5.Natural products targeting NLRP3 inflammasome for metabolic dysfunction-associated fatty liver disease: the known unknowns.

Metabolic dysfunction-associated fatty liver disease (MAFLD), characterized by fatty acid overload, secondary chronic inflammation, and fibrosis, has become the most prevalent chronic liver disease globally. While no effective pharmacotherapy exists for MAFLD, mitigating inflammatory responses represents a promising approach to preventing the progression from steatosis to severe steatohepatitis. The NOD-like receptor family pyrin domain containing 3 (NLRP3) inflammasome, which detects endogenous danger and stress signals, has emerged as a significant target for inflammatory disease treatment, as transcriptional inactivation of its components demonstrates the therapeutic potential for MAFLD. Natural products targeting NLRP3 inflammasome activation have shown promising efficacy in MAFLD therapy. This review synthesizes the current understanding of NLRP3 inflammasome activation and therapeutic targets for NLRP3 homeostasis. Additionally, natural products reported to inhibit NLRP3 inflammasome for MAFLD improvement are categorized according to their mechanisms of action. The review also addresses limitations and future directions regarding natural products targeting NLRP3 inflammasome in MAFLD treatment. Enhanced understanding of NLRP3 inflammasome activation mechanisms in MAFLD and the identification of novel natural products supported by mechanistic research will significantly advance MAFLD treatment.

6.Treatment of large bone defects in load-bearing bone: traditional and novel bone grafts.

Large bone defects in load-bearing bone can result from tumor resection, osteomyelitis, trauma, and other factors. Although bone has the intrinsic potential to self-repair and regenerate, the repair of large bone defects which exceed a certain critical size remains a substantial clinical challenge. Traditionally, repair methods involve using autologous or allogeneic bone tissue to replace the lost bone tissue at defect sites, and autogenous bone grafting remains the "gold standard" treatment. However, the application of traditional bone grafts is limited by drawbacks such as the quantity of extractable bone, donor-site morbidities, and the risk of rejection. In recent years, the clinical demand for alternatives to traditional bone grafts has promoted the development of novel bone-grafting substitutes. In addition to osteoconductivity and osteoinductivity, optimal mechanical properties have recently been the focus of efforts to improve the treatment success of novel bone-grafting alternatives in load-bearing bone defects, but most biomaterial synthetic scaffolds cannot provide sufficient mechanical strength. A fundamental challenge is to find an appropriate balance between mechanical and tissue-regeneration requirements. In this review, the use of traditional bone grafts in load-bearing bone defects, as well as their advantages and disadvantages, is summarized and reviewed. Furthermore, we highlight recent development strategies for novel bone grafts appropriate for load-bearing bone defects based on substance, structural, and functional bionics to provide ideas and directions for future research.

7.Relationship between nonalcoholic fatty liver disease and long-term prognosis in patients with acute myocardial infarction

Objective:To investigate the relationship between nonalcoholic fatty liver disease (NAFLD) and the long-term prognosis in patients with acute myocardial infarction (AMI).Methods:It was a retrospective cohort study. A total of 712 patients diagnosed with AMI who were admitted to the People′s Hospital of Xinjiang Uygur Autonomous Region from January 2018 to December 2019 were continuously included as subjects. The fatty liver index (FLI) was used to evaluate the degree of hepatic steatosis. Subjects were divided into No NAFLD group (FLI?30), grade 1 NAFLD group (30≤FLI?60), and grade 2 NAFLD group (FLI≥60). The endpoint event was defined as the occurrence of major cardiovascular adverse events (MACEs). The Cox proportional risk model was used to assess the risk of MACEs. The Kaplan-Meier curve was used to analyze the survival differences between the groups, and subgroup analysis was performed for age, gender, and complicity with hypertension, diabetes, dyslipidemia, and abdominal obesity.Results:During the follow-up period, the incidence of MACEs was 10.92% (26/238) in the No NAFLD group, it was 18.01% (47/261) in the grade 1 NAFLD group, and 24.41% (52/213) in the grade 2 NAFLD group, there was significant difference in the incidence of adverse events among the groups ( χ2=9.136, P?0.05). After adjusting for confounding factors, compared with the No NAFLD group, the risk of MACEs in grade 1 NAFLD group was increased by 69.0%( HR=1.690, 95% CI: 1.026-2.783, P=0.039) and 131.2%( HR=2.312, 95% CI: 1.415-3.773, P=0.001), respectively. Kaplan-Meier curve analysis indicated that the cumulative incidence of MACEs was significantly increased and the prognosis was worse in grade 2 NAFLD group (Log-rank test χ2=13.500, P=0.001). The results of subgroup analysis suggested that grade 2 NAFLD could significantly increase the risk of MACEs in all age groups, women, dyslipidemia, normal body type, and people with or without hypertension or diabetes. In particular, it had higher predictive value in women and normal-size people (interaction P?0.05). Conclusion:The degree of NAFLD is closely related to the long-term prognosis of AMI patients, the more severe the NAFLD, the higher the risk of adverse events in AMI patients.
8.Correlation between arterial stiffness and incident chronic kidney disease among old adults taking health check-up

Objective To investigate the correlation between arterial stiffness(AS)and incident chronic kidney disease(CKD)among the elderly individuals taking health checkup.Methods A retrospective study was conducted on 857 elderly individuals without CKD at baseline who taking physical exams in our medical center from December 2009 to May 2021.Their clinical and labora-tory data were collected.Brachial-ankle pulse wave velocity(baPWV)was used to assess the se-verity of AS,and then the subjects were divided into normal elasticity group(201 cases),and moderate(490 cases)and severe AS group(166 cases).Kaplan-Meier curves were plotted to dis-play cumulative incidence rates of incident CKD across different AS groups.Restricted cubic splines(RCS)and Cox regression models were applied to analyze the correlation of baPWV and incident CKD risk.Results The severe AS group had significantly advanced age,greater ratio of hypertension,larger waist circumference,higher HR,SBP and DBP,increased urinary albumin/creatinine ratio(UACR),elevated levels of TG and fasting blood glucose,and baPWV than the normal elasticity group(P<0.05).During the follow-up period,37 participants developed CKD.The incidence of CKD was obviously higher in the severe AS group than the normal arterial elas-ticity group(9.04%vs 3.48%).RCS analysis revealed a U-shaped relationship between baPWV and incident CKD risk.When baPWV ≥1 400 cm/s,each standard deviation increase in baPWV indicates the risk of incident CKD increasing by 71%(HR=1.71,95%CI:1.30-2.25,P<0.01).Regardless of adjustment for covariates or not,baPWV remained positive correlation with inci-dent CKD risk(P<0.05).Conclusion Among the elderly individuals undergoing health check-up,increased AS severity is significantly associated with higher risk of incident CKD when baP-WV ≥1400 cm/s.
9.Clinical characteristics of adult patients with eosinophilic esophagitis in China

Objective:Eosinophilic esophagitis (EoE), an allergen/immune-mediated chronic disorder of the esophagus, is witnessing an incremental rise in its global incidence. However, data in China are very limited. This study aimed to clarify the characteristics of EoE in Chinese adults at our medical center.Methods:We collected information, including clinical characteristics and endoscopic findings, relative to all cases of adult patients who met the pathological diagnostic criteria for EoE at our institution from January 2019 to May 2024. Patients diagnosed with reflux esophagitis (RE) were matched in a 1∶1 ratio based on age and sex, and comparisons were made between the two groups.Results:The overall detection rate of EoE in our hospital was approximately 0.05%, with a trend increasing yearly. In total, 61 patients met the diagnostic criteria for EoE, of whom 42 (68.9%) were male. The median age was 53 years. The clinical manifestations of EoE patients were diverse, with common symptoms including dysphagia, acid reflux, and upper abdominal pain. Serological examination revealed that 23.1% (9/39) of patients had increased absolute eosinophil counts. Allergic diseases were observed in 73.8% (45/61) patients. Most patients exhibited typical mucosal changes in the esophagus during endoscopy, the median EREFS score was 3 points. The most commonly administered treatment included proton pump inhibitors, and 13 patients received steroid treatment. During a follow-up period, 42 patients (68.9%) exhibited clinical symptom improvement or pathological remission. In comparison to Western countries, patients with EoE in China typically presented with milder disease severity and a more favorable prognosis. No significant differences were observed between younger and older adult groups regarding clinical symptoms, the proportion of comorbid allergic diseases, medications, or the rate of improvement ( P for all>0.05). Additionally, patients with EoE more commonly presented with dysphagia (29.5% vs. 1.6%, χ2=15.96, P<0.001), comorbid allergic diseases (73.8% vs. 13.1%, χ2=45.67, P<0.001), increased eosinophil counts (23.1% vs. 3.3%, χ2=7.61, P<0.01), and a positive response to corticosteroids when compared to patients with RE. Conclusions:This study systematically determined the clinical characteristics of adult EoE patients in China. Compared to Western countries, patients from China typically presented with milder disease severity and a more favorable prognosis. No significant clinical manifestations or prognosis differences were observed between younger and older patients. EoE can be differentiated from RE based on symptoms, the presence of comorbid allergic diseases, and the degree of serum eosinophil infiltration. For patients suspected of EoE, endoscopic esophageal biopsy will facilitate early diagnosis.
