1.Impact of hypothyroidism on renal function and its mechanism revealed by 99Tc m-DTPA renal dynamic imaging

Objective:To quantitatively evaluate the impact of hypothyroidism on renal function and address its underlying mechanisms using 99Tc m-diethylene triamine pentaacetic acid (DTPA) renal dynamic imaging. Methods:A total of 35 patients with differentiated thyroid cancer (12 males, 23 females, age (44.8±10.7) years), who were referred for radioiodine remnant ablation, were consecutively enrolled prospectively from October 2022 to October 2023 in Shanghai Sixth People′s Hospital Affiliated to Shanghai Jiao Tong University School of Medicine. Serum examinations of thyroid- and renal function-relevant parameters and 99Tc m-DTPA renal dynamic imaging were performed before and 4 weeks after stopping taking levothyroxine ( L-T 4). Paired t test, Wilcoxon signed rank test and Wilcoxon rank sum test were used to analyze data. Results:After L-T 4 withdrawal, glomerular filtration rate (GFR) derived from renal dynamic imaging, estimated GFR (eGFR), and cystatin C level significantly decreased ((95.80±18.09) vs (89.86±15.58) ml/min; t=4.83, P<0.001; (108.27±18.39) vs (87.97±16.24) ml·min -1·1.73 m -2;t=9.37, P<0.001; (0.72±0.12) vs (0.64±0.15) mg/L; t=3.73, P=0.001). GFR decreased more robustly in male patients than in females (6.16(3.62, 14.89) vs 3.83(0.57, 8.25) ml/min; Z=-1.98, P=0.048). Additionally, 99Tc m-DTPA renal dynamic imaging revealed that the peak times of both left and right kidneys were significantly prolonged (3.03(2.53, 3.78) vs 3.04(2.53, 4.13) min; Z=-3.85, P<0.001; 3.14(2.46, 4.20) vs 3.50(2.74, 4.50) min; Z=-3.44, P=0.001), but the left and right renal half-excretion times remained stable ( Z values: -0.88, -0.48, P values: 0.381, 0.634). Conclusion:Based on the 99Tc m-DTPA renal dynamic imaging results, it can be demonstrated that hypothyroidism can lead to a significant decrease in renal function, and the possible reason might be the reduction in the blood perfusion of kidneys.
2.Melatonin alleviated acute myocardial infarction by inhibiting ferroptosis

AIM:To investigate whether melatonin can ameliorate acute myocardial infarction(AMI)by in-hibiting ferroptosis.METHODS:H9C2 cells were cultured in AnaeroPack system with low sugar and serum-free medium for 10 h to construct a cell model of AMI.Then cells were treated with melatonin and ferroptosis inducer erastin.The cell activity,reactive oxygen species(ROS),lipid peroxidation,mitochondrial membrane potential(MMP),and ferroptosis related protein expression were detected.A rat model of AMI induced by isoprenaline(ISO)injection was established to evaluate the effects of melatonin,in which the myocardial infarction size,cardiac injury,pathological changes,oxidative stress,iron ion and ferroptosis related protein expression were examined.RESULTS:Melatonin decreased the oxidative stress,lipid peroxidation and expression of ferroptosis protein in cardiomyocytes induced by hypoxia,but these effects could be impeded by the ferroptosis inducer erastin.Furthermore,in vivo experiments,we also found that melatonin im-proved the myocardial infarction size,cardiac injury,pathological changes,oxidative stress,and alleviated iron ion accu-mulation and ferroptosis.CONCLUSION:The cardioprotective effects of melatonin in AMI are associated with the inhi-bition of ferroptosis.

5.Study on the mutation characteristics of streptomycin genes of Multidrug-Resistant Mycobacterium Tuber-culosis in Jiangxi Province

Objective To understand the molecular characteristics of Streptomycin(SM)resistance in multidrug-resistant tuberculosis(MDR-TB)in Jiangxi Province,and to explore the relationship between SM resistant genes(rpsL,rrs and gidB)mutations and SM resistant phenotypes in Beijing genotype TB.Methods 106 non-replicated MDR-TB isolates were collected from Gaoxin Branch of The First Affiliated Hospital of Nanchang University and Jiangxi Provincial Chest Hospital from January to December 2021,and tested for drug-resistance phenotypes,whether they were Beijing genotype or not and the characteristics of rpsL,rrs and gidB gene mutations.Chi-square test was performed to determine whether rpsL,rrs and gidB mutations were related to genotypes and drug-resistance phenotypes.Results Among 106 cases of MDR-TB,76 cases were resistant to SM.A total of 58 cases had rpsL 43A>G mutation,8 cases had 88A>G mutation,5 cases had rrs mutation,and 3 cases had gidB mutation.Statistical analysis showed that the coincidence rate of gene mutation and phenotypic drug-resistance detection was 89.6%,and the specificity and sensitivity were 86.7%and 90.8%,respectively.The isolated rate of Beijing genotype TB was 88.7%,and the drug-resistant gene mutations were mainly concentrated in rpsL and rrs,while the drug-resistant mutations of non-Beijing genotype were mainly concentrated in gidB;in addition,Beijing genotype bacteria were more prone to gene mutations(P = 0.013),but there was no difference in phenotypic drug-resistance.Conclusions Mutations in rpsL,rrs,and gidB genes have a good coincidence rate with phenotypic drug-resistance,and molecular biology can be used to detect directly drug-resistance genes to predict bacterial resistance;TB genotypes are strongly associated with streptomycin resistance characteristics.
6.Exploration and Prospect of Quantitative Evaluation of Integrity Risk Prevention and Control in Public Hospitals

Objective To explore the quantitative evaluation of integrity risk prevention and control in public hospitals,provide reference for improving the quality and efficiency of integrity risk prevention and control.Methods Self-designed"Inspection Standards for Integrity Risk Prevention and Control of Power Matters in Public Hospitals"was used to score and rate the power matters provided by each functional department/clinical department of West China Hospital of Sichuan University from three aspects:the clarity of power operation process,the accuracy of finding integrity risk points,the effectiveness of prevention and control measures.Results A total of 236 power matters of the hospital were inspected for integrity risk prevention and control,and according to the inspection criteria,57 items were rated as first grade,103 items were rated as second grade,and 76 items were rated as third grade,accounting for 24.15%,43.64%and 32.20%,respectively.The score for the special work of integrity risk prevention and control was 5.82±1.92 points,of which the process dimension score was 2.11±0.75 points,the risk points dimension score was 1.89±0.92 points,the prevention and control dimension score is 1.89± 0.79 points,which reflects the problems of unclear workflow,inaccurate finding of individual risk points,and unspecified prevention and control measures in some units.Conclusion Hospitals should focus on the concreteness,accuracy,salience and quantification in the long-term construction of integrity risk prevention and control from the aspects of thought,behavior,effectiveness and evaluation.
7.Influence of spleen-qi deficiency syndrome in type 2 diabetes mellitus on Ankle-Brachial Index

Objective:To investigate the influence of T2DM Spleen-qi Deficiency syndrome on Ankle-Brachial Index (ABI).Methods:The clinical data of 298 patients with T2DM who were hospitalized in Dongzhimen Hospital, Beijing University of Chinese Medicine from January 2019 to December 2021 were retrospectively analyzed. According to the diagnostic criteria of spleen-qi deficiency syndrome, the patients were divided into two groups: spleen-qi deficiency syndrome group and non-spleen-qi deficiency syndrome group. There were 142 patients in the spleen-qi deficiency syndrome group and 156 patients in the non-spleen-qi deficiency syndrome group. The differences of ABI between the two groups were compared, and the correlation between spleen-qi deficiency syndrome and clinical indicators (gender, age, body mass index, course of diabetes, history of hypertension, smoking history, fasting glucose, total cholesterol, triglyceride, platelet, hemoglobin, serum albumin, blood urea nitrogen, serum creatinine, estimated glomerular filtration rate) and ABI in T2DM patients was analyzed.Results:The left ABI [1.09 (1.00, 1.19) vs. 1.13 (1.03, 1.22)] and the right ABI [1.09 (0.96, 1.17) vs. 1.12 (1.02, 1.20)] in T2DM spleen-qi deficiency syndrome group were significantly lower than those in non-spleen-qi deficiency group ( P<0.05).The left ABI was significantly correlated with spleen-qi deficiency syndrome ( r=0.122, P=0.035) and estimated glomerular filtration rate ( r=0.137, P=0.018), and the right ABI was significantly correlated with spleen-qi deficiency syndrome ( r=0.123, P=0.034) and PLT ( r=-0.115, P=0.047). After correcting for other confounding factors by multiple linear regression analysis, there was significantly correlation between spleen-qi deficiency syndrome and ABI. Conclusion:Compared with the non-spleen-qi deficiency syndrome group, T2DM patients in the spleen-qi deficiency group had a lower ankle-brachial index and were more likely to develop peripheral arterial disease.
8.Rhodojaponin VI indirectly targets Cav2.2 channels via N-ethylmaleimide-sensitive fusion protein to alleviate neuropathic pain.

Neuropathic pain is a chronic disease that severely afflicts the life and emotional status of patients, but currently available treatments are often ineffective. Novel therapeutic targets for the alleviation of neuropathic pain are urgently needed. Rhodojaponin VI, a grayanotoxin from Rhododendron molle, showed remarkable antinociceptive efficacy in models of neuropathic pain, but its biotargets and mechanisms are unknown. Given the reversible action of rhodojaponin VI and the narrow range over which its structure can be modified, we perforwmed thermal proteome profiling of the rat dorsal root ganglion to determine the protein target of rhodojaponin VI. N-Ethylmaleimide-sensitive fusion (NSF) was confirmed as the key target of rhodojaponin VI through biological and biophysical experiments. Functional validation showed for the first time that NSF facilitated trafficking of the Cav2.2 channel to induce an increase in Ca2+ current intensity, whereas rhodojaponin VI reversed the effects of NSF. In conclusion, rhodojaponin VI represents a unique class of analgesic natural products targeting Cav2.2 channels via NSF.
9.Macrophage migration inhibitory factor protects bone marrow mesenchymal stem cells from hypoxia/ischemia-induced apoptosis by regulating lncRNA MEG3.

OBJECTIVES:
This research was performed to explore the effect of macrophage migration inhibitory factor (MIF) on the apoptosis of bone marrow mesenchymal stem cells (BMSCs) in ischemia and hypoxia environments.
METHODS:
The cell viability of BMSCs incubated under hypoxia/ischemia (H/I) conditions with or without pretreatment with MIF or triglycidyl isocyanurate (TGIC) was detected using cell counting kit-8 (CCK-8) analysis. Plasmids containing long noncoding RNA (lncRNA) maternally expressed gene 3 (MEG3) or β-catenin small interfering RNA (siRNA) were used to overexpress or downregulate the corresponding gene, and the p53 signaling pathway was activated by pretreatment with TGIC. The influences of MIF, overexpression of lncRNA MEG3, activation of the p53 signaling pathway, and silencing of β-catenin on H/I-induced apoptosis of BMSCs were revealed by western blotting, flow cytometry, and terminal deoxynucleotidyl transferase (TdT)-mediated dUTP nick-end labeling (TUNEL) staining.
RESULTS:
From the results of CCK-8 assay, western blotting, and flow cytometry, pretreatment with MIF significantly decreased the H/I-induced apoptosis of BMSCs. This effect was inhibited when lncRNA MEG3 was overexpressed by plasmids containing MEG3. The p53 signaling pathway was activated by TGIC, and β-catenin was silenced by siRNA. From western blot results, the expression levels of β-catenin in the nucleus and phosphorylated p53 (p-p53) were downregulated and upregulated, respectively, when the lncRNA MEG3 was overexpressed. Through flow cytometry, MIF was also shown to significantly alleviate the increased reactive oxygen species (ROS) level of BMSCs caused by H/I.
CONCLUSIONS
In summary, we conclude that MIF protected BMSCs from H/I-induced apoptosis by downregulating the lncRNA MEG3/p53 signaling pathway, activating the Wnt/β-catenin signaling pathway, and decreasing ROS levels.

10.The impact of mindfulness on nurses' professional quality of life

Objective To provide a scientific basis for nursing managers to promote the professional quality of life of nurses.Methods Totally 373 nurses from three general hospitals in Jinan by convenience sampling technique were recruited and assessed with the Professional Quality of Life Scale Version 5 (ProQOL-V) and the Five Facet Mindfulness Questionnaire.The relationship between professional quality of life and mindfulness was analyzed.Results The average score of compassion satisfaction was (29.71±6.61) points,the average score of compassion fatigue was (52.82±8.04) points,which were in the middle level of them.Compassion satisfaction was correlated with mindfulness,observation,description,non-reactiveness and non-judgment (P < 0.05).Compassion fatigue was correlated with mindfulness,observation,awareness of the action,non-reactiveness and non-judgment (P < 0.05).Multiple regression analysis showed that the three dimensions (observation,non-reactiveness and non-judgment) of mindfulness could predict 15% variance of compassion satisfaction and the three dimensions (observation,awareness of the action and non-judgment) of mindfulness could predict 34.1% variance of compassion fatigue after controlling demographic variables.Conclusions Mindfulness can effectively predict nurses' professional quality of life.Nursing managers should take some mindfulness training in order to improve their professional quality of life.
