1.Wide beam-harmonic motion imaging for locating focus of high intensity focused ultrasound:An in vitro study

Objective To observe the effectiveness of ultrasound wide beam(WB)-based harmonic motion imaging(HMI)(WB-HMI)for locating irradiation focus of high intensity focused ultrasound(HIFU)for in vitro tissue.Methods WB-HMI technology was developed with acoustic radiation force and ultrasound imaging as the key technology.For in vitro porcine tenderloin and bovine liver tissue,different amplitude modulation(AM)frequencies(25-100 Hz)and excitation acoustic power(0.7-28 W)were used to achieve WB-HMI localization of HIFU irradiation focus,and the differences of WB-HMI localization of HIFU irradiation focus under different parameter combinations were observed.Taken the actual focus position on ultrasonic image after HIFU as the standard and the focus positioning error<1 mm as the effective standard of WB-HMI locating irradiation focus,the locating success rate was calculated.Results The larger the acoustic power,the larger the displacement amplitude of irradiation focus by WB-HMI at the same AM frequency,while the smaller the AM frequency,the larger the displacement amplitude of irradiation focus located by WB-HMI under the same acoustic power.Under different AM frequencies,for in vitro porcine tenderloin,the success rate of WB-HMI for locating HIFU radiation focus was higher than 90.00%when acoustic power was 15 W or 22 W,whereas the success rate showed a decreasing trend when the acoustic power was 28 W.For in vitro bovine liver tissue,the success rate of WB-HMI localization was 100%when acoustic power was ≥7.0 W.Conclusion WB-HMI could be used to effectively locate HIFU irradiation focus for isolated tissue.
2.Research progress on the relationship between LINC01018-E2F1-CDK6 and malignant tumor

It is known that LINC01018 and CDK6 are associated with the occurrence, progression and recurrence of malignant tumor. Our preliminary data showed that the expression of LINC01018 was down-regulated and that of CDK6 was up-regulated, which were related with the malignancy grade. Bioinformatics suggested that E2F1 binding site exist in the CDK6 promoter region, and LINC01018 might interact with E2F1. Thus we speculate that the expression of LINC01018 in malignant tumor is decreased. The interaction between LINC01018 and E2F1 activates E2F1, promotes the transcription activation of CDK6, and affects tumor proliferation, invasion and metastasis. All these are expected to reveal the effect and mechanisms of LINC01018 in the development and regulation of malignant tumor, and provide new ideas and evidences for its treatment.
3.Establishment of LINC01018 overexpression colon cancer cell line and study on its biological characteristics

Objective:To establish a colon cancer cell line which overexpressing LINC01018 stably and study its biological characteristics.Methods:The expression of LINC01018 in HCoEpiC and HT-29 cells were detected by real time fluorescence quantitative polymerase chain reaction (qRT-PCR). HT-29 cells were infected with LINC01018 overexpression lentivirus to screen and establish HT-29 cell lines which overexpressing LINC01018 stably. The effect of LINC01018 on the proliferation, invasion and migration of HT-29 cells were detected by cell counting kit-8 (CCK-8) assay and Transwell assay separately. The expression of CDK6 and matrix metalloproteinase-2 (MMP-2) in HT-29 cells was detected by Western blot.Results:The expression of LINC01018 in HT-29 cells was significantly lower than that in the human colonic epithelial cells (HCoEpiC). HT-29-L18 cell lines which overexpressing LINC01018 stably was screened successfully. Overexpression of LINC01018 significantly inhibited the cell proliferation, invasion and migration, and reduced the protein expression of CDK6 and MMP-2 in HT-29 cells.Conclusions:The expression of LINC01018 was decreased abnormally in colon cancer cells. Up-regulation of LINC01018 expression can inhibit the proliferation, invasion and migration of colon cancer cells, which may be related to CDK6 and MMP-2.
4.Effect of motion and immobilization on shoulder function early after arthroscopic rotator cuff repair

Objective:To investigate the effect of passive motion and immobilization on shoulder function early after arthroscopic repair of rotator cuff tears.Methods:A retrospective case-control study was conducted to analyze the clinical data of 78 patients with rotator cuff tear admitted to Xinhua Hospital, Shanghai Jiaotong University School of Medicine from January 2016 to December 2017. There were 36 males and 42 females, aged 35-78 years [(62.7±3.2)years]. There were 36 patients with medium-sized tears (1-3 cm), 31 with small tears (<1 cm), and 11 with partial articular supraspinatus tendon avulsion (PASTA). All patients underwent arthroscopic rotator cuff repair. Forty-three patients started rehabilitation exercise immediately after operation (motion group). Thirty-five patients were immobilized with shoulder abduction brace for 6 weeks, and started rehabilitation exercise at week 7 (immobilization group). The range of motion, visual analogue pain score (VAS), simplified shoulder joint function test (SST) and Constant shoulder joint score were compared between the two groups before surgery, 6 weeks, 3 months and 12 months after surgery. The healing results were assessed by ultrasound 12 months after surgery. Complications were observed.Results:All the patients were followed up for 12-16 months [(13.7±1.3)months]. There were 7 patients with shoulder joint stiffness in each group (motion group: 16%, immobilization group: 20%) ( P<0.05). There were no significant differences between the two groups in VAS, SST or Constant score at postoperative 6 weeks, 3 months and 12 months ( P>0.05). The forward flexion and external rotation with the arm at the side in immobilization group was (124.9±12.9)° and 25(20, 30)° at postoperative 6 weeks, significantly improved in motion group [(136.6±16.7)°, 30(25, 40)°] ( P<0.05). There were no significant differences between the two groups in forward flexion and external rotation with the arm at the side at postoperative 3 and 12 months ( P>0.05). There were no significant differences between the two groups in internal rotation at postoperative 6 weeks, 3 months, and 12 months ( P>0.05). All rotator cuffs were healed verified by ultrasound at postoperative 12 months. No infection or implant displacement occurred after operation. Conclusions:For arthroscopic repair of medium-sized tears, small tears and PASTA, early postoperative rehabilitation exercises have advantage in improving range of motion only at early stage when compared to immobilization, which disappears with time. Moreover, the two methods have no significant differences in improving postoperative pain and shoulder function.
5.Risk factors for varus collapse after locking plate fixation of unstable proximal humerus fracture

Objective:To identify potential risk factors of varus collapse after unstable proximal humerus fracture treated with locking plates.Methods:A retrospective case series study was conducted on data of 146 patients with unstable proximal humerus fracture stabilized by locking plates at Xinhua Hospital, Shanghai Jiaotong University School of Medicine from January 2008 through December 2014. These patients were classified into varus collapse group ( n=39) and non-varus collapse group ( n=107) according to the occurrence of varus collapse. The gender, age, bone mineral density, cause of injury, fracture Neer classification, fracture type (varus or valgus), surgical timing, surgical techniques (medial support, cancellous bone graft, suture augmentation), number of humeral head screws and reduction quality were recorded. Potential risk factors were evaluated using univariate analysis and multivariate Logistic regression. The subjective reliability analysis was performed for Neer classification and medial fracture assessments. Results:Varus collapse group had higher ratio of osteoporosis, varus fracture, lack of medial column support, absence of suture augmentation and varus malreductin compared to non-varus collapse group ( P<0.05). While the two groups had no significant differences in gender, age, fracture classification, allogeneic cancellous bone transplantation and number of humeral head screws ( P>0.05). Moreover, the Logistic regression analysis indicated that osteoporosis, varus fracture, lack of medial column support, absence of suture augmentation and varus malreduction were major independent risk factors for varus collapse in proximal humerus fractures ( P<0.05). Among these risk variables, the lack of medial column support showed the strongest correlation of varus collapse after proximal humerus fractures treated with locking plates ( OR=9.62), and varus malreduction was another remarkable risk factor ( OR=8.39). The reliability of Neer classification and medial fracture assessments between interobservers and intraobservers was good. Conclusion:The risk factors for varus collapse after unstable proximal humerus fracture treated with locking plate are osteoporosis, varus fracture, lack of medial column support, absence of suture augmentation and varus malreduction.
6.A clinical and natural history research on mitochondrial encephalomyopathy, lactic acidosis and stroke-like episodes

Objective To summarize the clinical features,natural history and causes of death of mitochondrial encephalomyopathy,lactic acidosis and stroke-like episodes (MELAS).Methods We retrospectively evaluated the clinical findings of 64 patients diagnosed as MELAS more than 3 years (death cases excluded) in Huashan Hospital from January 2005 to March 2017 and analyzed the natural course and the causes of death of the disease.Results Among 64 patients,the male-to-female ratio was 1.3 ∶ 1.Median onset age was 20.5 (16.8) years.The peak of incidence age was from 14 to 22 years.The most common features of MELAS in acute phase were seizures (48/64,75.0%),headache (41/64,64.1%),blurred vision (37/64,57.8%),nausea and vomiting (27/64,42.1%),fever (25/64,39.1%),mental and behavioral disorder (24/64,37.5%).Lactate dehydrogenase (31/60,51.6%),resting blood lactic acid (43/58,74.1%) and cerebral spinal fluid lactic acid (9/9) were elevated.Abnormal findings in electroencephalogram (36/40,90.0%),electrocardiogram (37/47,78.7%),electromyography (25/41,61.0%) were detected.In this cohort,20 patients (20/64,31.3%) with MELAS were dead.A Kaplan-Meier survival curve showed the estimated overall median survival time was 12 years.The median survival time of the group onset before sex maturity (≤ 14 years) was 8 years and that in the group onset after sex maturity (> 14 years) was 21 years.The causes of death were cardiogenic incidence (4/20,20.0%),pulmonary infection (4/20,20.0%),lactic acidosis (2/20,10.0%) and status epilepticus (2/20,10.0%).Conclusions MELAS is usually presented in young people associated with high mortality rate.The leading causes of death are cardiogenic,pulmonary infection and lactic acidosis.
7.Estimation of spectral oscillatory power of resting-state EEG in subjective tinnitus

OBJECTIVE To explor e the characteristics of spectral oscillatory power of resting-state electroencephalography(EEG) in subjective tinnitus patients and to lay the foundation for study of central mechanism of tinnitus.METHODS 31 subjects(15 subjects with subjective tinnitus and 16 age matching healthy subjects) underwent a 128-channel resting-state EEG analyses. After a series of preprocessing, data were segmented into 8 frequency bands, including δ(0.5~3.5 Hz), θ(4~7.5 Hz), α1(8~10 Hz), α2(10~12 Hz), β1(13~18 Hz), β2(18.5~21 Hz), β3(21.5~30 Hz) and γ(30.5~44 Hz). The group differences of spectral power were analyzed by independent t test. Correlation between spectral power of each frequency band and tinnitus subjective symptoms were also analyzed. RESULTS Significant higher spectral power of the α1, β and γ bandwere found in the left and right temporal areas of tinnitus group compared with that of normal group.Tinnitus subjects also had higher spectral powerof the δ and θ band in temporo-parietal areas than that of the normal group. There was no significant difference of spectral power in other frequency bands. Moreover, signif icant positive cor relation were found between tinnitus loudness and spectralpower of right anterior lateral(R=0.66, P =0.007) and right anterior medial(R=0.58, P =0.031) areas. CONCLUSION Tinnitus subjects have higher spectral oscillatory power on right and left temporal lobe and temporo-parietal area. A positive correlation exsit between tinnitus loudness andspectral power of right anterior lateral and medial areas in tinnitus subjects, whichindicate that central reorganizationexsit in tinnitus reorganization andγ band maybe considered as a possible biomarkerforthe tinnitus subjective symptom.
8.Effects of different types of triglycerides on hepatic and renal function and lipometabolism of patients with acute necrotizing pancreatitis: a prospective study

Objective To investigate the effects of structured triglyceride (STG) and physical mixed medium chain/long chain triglycerides (MCT/LCT) on hepatic and renal function and lipometabolism of patients with acute necrotizing pancreatitis (ANP).Methods The clinical data of 30 patients with ANP who were admitted to the PLA General Hospital between January 2012 and June 2014 were prospectively analyzed.A double-blind,randomized,controlled study was performed in 30 patients who were allocated into the experimental group (15 patients received STG) and the control group (15 patients received physical mixed MCT/LCT).All the patients received isometrical nitrogen and isocaloric parenteral nutrition more than 5 days.The levels of alanine transaminase (ALT),aspartate transaminase (AST),glutamyl-transpeptidase (GGT),alkaline phosphatase (ALP),creatinine (Cr),blood urea nitrogen (BUN),triglyceride (TG) and total cholesterol (TC) were assayed before nutritional support treatment and at day 1,3 and 5 after nutritional support therapy.The measurement data with normal distribution was presented as (x) ± s.The skew distribution data were described as M (range).The comparison between groups were evaluated with an independent sample t test or one-way ANOVA.The count data were analyzed using the chi-square test.Results A total of 30 patients were screened for eligibility.The levels of ALT,AST,GGT,ALP,Cr,BUN,TG and TC were changed within a certain range at day 1,3 and 5 after nutritional support treatment.The levels of ALT,AST,GGT,ALP,Cr,BUN and TC before treatment and at day 5after treatment were changed from 29.0 U/L,25.4 U/L,83.2 U/L,(193 ± 115) U/L,(124 ± 97) μmol/L,(8±6)mmol/L and (2.4±1.1)mmol/L to 29.4 U/L,33.0 U/L,77.7 U/L,(172±74)U/L,(117 ±103)μmol/L,(8 ± 5) mmol/L and (2.3 ± 1.0) mmol/L in the experimental group,and from 23.8 U/L,22.9 U/L,96.2 U/L,(148 ± 108) U/L,(82 ± 57) μmol/L,(9 ± 7) mmol/L and (2.5 ± 0.7) mmol/L to 21.3 U/L,24.5 U/L,127.4 U/L,(179 ± 126) U/L,(80 ± 54) μmol/L,(10 ± 6) mmol/L and (2.4 ±0.8) mmol/L in the control group,respectively.There were no significant differences in the changing trends of the levels of ALT,AST,GGT,ALP,Cr,BUN and TC between the 2 groups (F =0.647,1.186,0.282,0.553,0.862,0.182,0.369,P>0.05).The level of TG in the experimental group from pre-treatment to day 5 after treatment was changed from (1.5 ± 0.6) mmol/L to (1.5 ± 0.7) mmol/L,with increasing trend from pre-treatment to day 1 after treatment and reaching the normal level at day 3 and 5 after treatment.The level of TG in the control group from pre-treatment to day 5 after treatment was changed from (1.5 ± 0.6) mmol/L to (2.4 ± 0.6) mmol/L,with increasing trend from pre-treatment to day 1,3 and 5 after treatments.There were significant differences in the changing trends of TG before and after nutritional support therapy between the 2 groups (F =7.940,P < 0.05).Conclusion STG and physical mixed MCT/LCT don't influence the hepatic and renal function of patients with ANP undergoing parenteral nutritional support therapy,while STG has a better effect of lipometabolism compared with physical mixed MCT/LCT.Registry This study was registered with the UMIN Clinical Trial Registry with the registry number of UMIN000016958
9.Percutaneous nephroscopic necrosectomy for post-operatively resident infection of severe acute pancreatitis.

OBJECTIVETo investigate the method and effect of percutaneous nephroscopic necrosectomy (PNN) for post-operatively resident infection of severe acute pancreatitis (SAP).
METHODSData of the 15 SAP patients with post-operatively resident infection treated by PNN from June 2008 to December 2014 in Chinese People's Liberation Army General Hospital were reviewed. Twelve of the patients underwent the laparotomy within 1 week, 1 in 3(rd) week, 1 in 4(th) week and the other one on the 127(th) day. All of the referrals firstly received (multi-)percutaneous catheter drainage (PCD), and then PNN operation according to the disease, followed by continuous irrigation-drainage.
RESULTSEleven patients were healed after received only one PNN operation, 2 patients for twice, 1 for three times and 1 for four times. The average post-operative time of hospital stay was 66.2 days (10-223 days). The complications after operation contained colon fistula (n = 1), abdominal hemorrhage (n = 1), pancreatic pseudocyst (n = 1), severe pulmonary infection (n = 1). Three patients eventually died.
CONCLUSIONSPercutaneous nephroscopic necrosectomy is a minimally invasive approach which could prevent the complicated re-laparotomy operation, result in less complication. It is an ideal method for treating SAP patients with post-operatively resident infection.

10.Analysis of distribution and drug resistance of 1 705 strains of common pathogenic bacteria in primary hospital

Objective To understand the regional distribution and drug resistance of clinical isolates of bacteria flora ,in order to provide the basis for the diagnosis and treatment of bacterial infection .Methods According to the national clinical test proce‐dures operation separation strains ,HX‐21 bacteria identification/susceptibility analyzer bacteria identification and drug sensitive test .Results Among 1 705 strains of isolated bacteria ,Gram‐positive cocci accounted for 39 .8% ,Gram‐negative bacteria accounted for 60 .2% ;separation rate from high to low were:benzene azole resistance westwood 13 .3% coagulase negative staphylococcus , pseudomonas aeruginosa ,12 .0% benzene azole resistance westwood staphylococcus aureus 11 .3% ,did not produce the ultra broad spectrum beta lactamase 10 .7% ,e .coli to produce ultra broad spectrum beta‐lactamase e .coli 9 .9% ,acinetobacter was 7 .1% ,pro‐ducing ultra broad spectrum beta‐lactamase pneumonia klebsiella bacteria 5 .9% ,did not produce the ultra broad spectrum beta‐lac‐tamase pneumonia klebsiella bacteria 5 .7% ,benzene azole westwood sensitive coagulase negative staphylococcus 4 .8% ,enterococ‐cus was 4 .8% ,benzene azole westwood sensitive staphylococcus aureus 4 .5% ,etc .Conclusion The common pathogenic bacteria is gram‐negative bacilli in the common pathogenic bacteria .In negative bacilli infection ,acinetobacter ,pseudomonas aeruginosa ,e .coli , klebsiella pneumoniae ,etc were more common .In staphylococcus aureus strains ,benzene azole westwood drug‐resistant strain ratio is higher than benzene azole westwood sensitive strain rate for 3 times .In addition to the vancomycin and teicoplanin sensitive ,other commonly used antibiotics shows different degrees of resistance .
