ObjectiveTo investigate the clinical characteristics, diagnostic challenges, and treatment strategies in pediatric patients with ureteropelvic junction obstruction (UPJO) complicated with ipsilateral ureterovesical junction obstruction (UVJO). MethodsA retrospective analysis was conducted on clinical data of pediatric patients admitted to our hospital between January 2014 and December 2024 with a confirmed diagnosis of UPJO complicated with UVJO. The collected clinical parameters included gender, age at presentation, initial diagnosis, lesion laterality, imaging findings, surgical procedures, and follow-up outcomes. ResultsThe study cohort comprised 14 pediatric patients (male=10 and female=4) with a median age of 3.1 months at presentation. The initial diagnoses were UPJO in 9 cases and UVJO in 5 cases. Preoperatively, only 6 cased showed ureteral dilation on MRU. The definitive diagnosis was confirmed by intraoperative retrograde pyelography in 5 cases, postoperative antegrade pyelography via a nephrostomy tube in 7 patients, and combined application of the two modalities in 2 patients. For primary surgical intervention, 9 patients underwent pyelo-plasty, 2 underwent ureteral reimplantation, 2 received percutaneous nephrostomy, and 1 underwent ureteral orifice dilation. Intraoperative nephrostomy tube placement was performed in 10 patients. With a median follow-up duration of 5.5 years, 4 patients required revision surgery. Of these, only 1 patient , who had an initial diagnosis of UPJO, required staged management for persistent obstruction, whereas UPJ obstruction resolved after ureteroneocystostomy in the remaining 3 patients with an initial diagnosis of UVJO. Among the 10 patients who did not undergo revision surgery, 8 showed delayed distal urinary drainage on APG, yet remained asymptomatic with continuous improvement of hydronephrosis after nephrostomy tube clamping. ConclusionsPreoperative diagnosis of concurrent UPJO and ipsilateral UVJO in pediatric patients is clinically challenging. Further intraoperative evaluation is mandatory when antegrade or retrograde ureteral stent placement fails during surgery. For patients with an initial diagnosis of UPJO, pyeloplasty is the first-line treatment. Routine retrograde pyelography is not recommended, whereas nephrostomy tube with subsequent postoperative APG evaluation can prevent unnecessary ureteroneocystostomy. For patients with an initial diagnosis of UVJO accompanied by severe ureteral dilation, primary ureteroneocystostomy is feasible initial management strategy.

The lung function of patients with occupational pneumoconiosis (hereinafter referred to as "pneumoconiosis") combined with chronic obstructive pulmonary disease (COPD) declines rapidly. Inhaled pharmacotherapy is crucial in the treatment of patients with pneumoconiosis combined with COPD. At present, pressurized metered-dose inhalers, dry powder inhalers, and soft mist inhalers are commonly used to deliver bronchodilators (including long-acting β2-adrenergic agonists, long-acting muscarinic antagonists, or their fixed-dose combinations) in inhaled pharmacotherapy. Peak inspiratory flow (PIF) is a commonly used indicator for evaluating the effectiveness of inhalation therapy. Patients with good coordination of hand and mouth, and a high PIF (≥30 L/min) can be treated with pressurized metered-dose inhalers or dry powder inhalers, whereas patients with poor coordination or insufficient PIF (<30 L/min) should be treated with pressurized metered-dose inhalers combined with a spacer or with nebulizers. In addition, the effectiveness of inhalation therapy is affected by individual factors, such as treatment adherence and pulmonary function, as well as drug-related factors, including the use of inhaled corticosteroids and the selection of bronchodilators. Inhaled pharmacotherapy appears to have substantial potential in the treatment of pneumoconiosis complicated with COPD. Future efforts should focus on the development of more effective inhaled drugs with fewer adverse effects and more user-friendly inhalation devices, as well as on optimizing inhalation treatment regimens to enable precision therapy and improve therapeutic efficacy.

China Occupational Medicine 2024;51(4):466-471
The appearance and degree of damage caused by metallic mercury injection at different sites were disparate, and different from common mercury poisoning. This study reviewed literature on poisoning caused by injection of metallic mercury in different body parts, summarized the health impairments and corresponding treatment principles. The causes of such accidents were mostly intentional injury or suicide, self-harm, with a minority due to feudal superstition, tattoos, and improper treatment. Subcutaneous or deep intramuscular injection of mercury primarily caused local inflammation reactions in the early stages. Intraocular injection might lead to inflammation, tissue necrosis, and blindness. Intravenous injection might lead to local or systemic acute reactions. The injection at local sites might cause harms to respiratory, nervous, urinary, and circulatory systems and reproductive health if individuals fail to boost mercury excretion promptly. For mercury treatment, the vacuum sealing drainage and extensive removal of deposits were preferred for mercury subcutaneous, muscular, or deep tissue injected individuals. For mercury intraocular injected individuals, the prompt surgical removal of mercury drops and, if necessary, enucleation of the eyeball were preferred. For mercury intravenous injected individuals, in addition to debridement surgery, treatment such as plasma exchange, hemoperfusion, hemodialysis, and bronchoalveolar lavage could be used. For mercury poisoning caused by injection in different body parts, mercury expulsion and symptomatic treatment are recommended, in addition to psychological therapy.

ObjectiveTo investigate the expression of glial cell line-derived neurotrophic factor (GDNF) and androgen receptor (AR) in testicular peritubular cells (TPCs) of cryptorchidism mouse models and explore the theoretical significance of cryptorchidism-induced spermatogenesis dysfunction. MethodsA total of 30 five-week-old male ICR rats were divided randomly by using random number table method into 6 groups. Cryptorchidism was surgically induced in 3 randomly selected groups and the other 3 groups underwent sham surgery as the control groups. On days 4, 7 and 14 after surgery, we harvested the mice testes of the 3 groups and their corresponding control groups, then measured the testicular volumes, analyzed the testicular histopathology and detected the mRNA and protein expression levels of AR and GDNF in TPCs by immunofluorescence, real-time PCR and Western blot. ResultsIn normal control groups, on days 4, 7 and 14 after surgery, the testicular volumes were (125.58±19.22) mm3,(123.45±20.12) mm3, (140.09±13.62) mm3 , respectively. Clear layers of spermatogenic cells were well arranged and abundant sperm cells were found. Peritubular cells were morphologically homogeneous, with slim-spindle appearance and normal cell thickness. The mRNA expression levels of AR were 1.00±0.05, 1.06±0.07 and 1.19±0.13; GDNF mRNA 1.00±0.04, 1.09±0.05, and 1.10±0.07. The protein expression levels of AR were 1.01±0.01, 0.79±0.02 and 1.01±0.04; GDNF protein (18.68±0.43) pg/mL, (14.39±0.36) pg/mL and (16.88±0.37) pg/mL. In cryptorchidism groups, on days 4, 7 and 14 after surgery, the testicular volumes were (115.64±3.91) mm3, (69.51±14.97) mm3 and (44.86±5.56) mm3, respectively. Spermatogenic cells were disorganized, seminiferous tubules were disrupted, peritubular cells shrank, bent and fractured. The mRNA expression levels of AR were 0.76±0.06, 0.53±0.04, and 0.29±0.02; GDNF mRNA 0.72±0.05, 0.42±0.02 and 0.30±0.03. The protein expression levels of AR were 0.54±0.02, 0.98±0.04 and 0.31±0.01; GDNF protein (8.50±0.34) pg/mL, (17.44±0.32) pg/mL and (6.83±0.34) pg/mL. Statistically significant differences (P < 0.05) were found in 7-day and 14-day testicular volumes between control and cryptorchidism groups but not in the 4-day testicular volume (P > 0.05). Testicular volumes, AR and GDNF mRNA and protein expression in control groups had no statistically significant difference (P > 0.05), while those in cryptorchidism groups showed a trend of gradual decline in the amount and the differences between groups were statistically significant (P < 0.05). ConclusionsIn surgery-induced cryptorchidism mice, after the induction, the expression of AR and GDNF in TPCs showed a gradual decrease over time. AR and GDNF play a major role in mediating the TPCs damage in cryptorchidism. This study provides a theoretical basis for mechanism researches of cryptorchidism-induced spermatogenesis dysfunction.

Objectives:To construct a nomogram prediction model using common preoperative indicators for early weight loss (EWL) 1 year after laparoscopic sleeve gastrectomy (LSG).Methods:Relevant data of obese patients who had undergone LSG from January 2015 to May 2022 in Fujian Medical University Union Hospital and Quanzhou First Hospital Affiliated Fujian Medical University were analyzed. Patients with a history of major abdominal surgery, severe gastroesophageal reflux disease, pregnancy within 1 year after surgery, or who were lost to follow-up were excluded, resulting in a total of 200 patients in the study (190 from Fujian Medical University Union Hospital and 10 from Quanzhou First Hospital Affiliated Fujian Medical University). The participants were 51 men and 149 women of a mean age 29.9±8.2 years and a body mass index (BMI) 38.7±6.5 kg/m 2. All patients in this group underwent standardized LSG procedure. Achieving ideal weight (BMI≤25 kg/m 2) 1 year after LSG was defined as goal of EWL. Logistic regression analyses were performed to identify factors that independently influenced EWL. These factors were incorporated into the nomogram model. Receiver operating characteristic (ROC) curves (the larger the area under the curve [AUC], the better the predictive ability and accuracy of the model), likelihood ratio test (higher likelihood ratio indicates greater model homogeneity), decision curve analysis (higher net benefit indicates a better model), Akaike information criterion (AIC; smaller AIC indicates a better model), and Bayesian information criterion (BIC; smaller BIC indicates a better model) were used to validate the predictive ability of the column line diagram model. Results:In this study of 200 obese patients who underwent LSG surgery, 136 achieved EWL goal, whereas the remaining 64 did not. The rate of EWL goal achievement of the entire group was 68.0%. Compared with patients who did not achieve EWL goal, those who did had lower BMI, alanine transaminase, aspartate transaminase, triglycerides, and higher cholesterol. Additionally, the proportion of female was higher and the proportions of patients with fatty liver and hypertension lower in those who achieved EWL goal (all P<0.05). Univariate and multivariate logistic regression analysis revealed that preoperative BMI (OR=0.852, 95%CI: 0.796-0.912, P<0.001), alanine transaminase (OR=0.992, 95%CI: 0.985-0.999, P=0.024), presence of fatty liver (OR=0.185, 95%CI: 0.038-0.887, P=0.035) and hypertension (OR=0.374, 95%CI: 0.144-0.969, P=0.043) were independently associated with failure to achieve EWL goal. Cholesterol (OR=1.428, 95%CI: 1.052-1.939, P=0.022) was independently associated with achieving EWL goal. We used the above variables to establish an EWL nomogram model. ROC analysis, the likelihood ratio test, decision curve analysis, and AIC all revealed that the predictive value of the model was better than that of BMI alone (nomogram model vs. BMI: area under the curve 0.840 vs. 0.798, P=0.047; likelihood ratio: 58.785 vs. 36.565, AIC: 193.066 vs. 207.063, BIC: 212.856 vs. 213.660). Conclusion:Our predictive model is more accurate in predicting EWL after LSG compared with using BMI.

Objective:To investigate the therapeutic value of interventional embolization on feeding artery in intracranial hypervascular tumors.Methods:Forty-five patients with intracranial hypervascular tumors, admitted to and accepted interventional embolization of the feeding artery before craniotomy in Department of Neurosurgery, Huashan Hospital Affiliated to Fudan University from March 2019 to August 2022, were chosen; a retrospective analysis was performed on the clinical data of these patients. The imaging characteristics, pathological types, preoperative embolization indications, embolization effects and embolism-related complications were summarized to evaluate the safety and effecacy of preoperative interventional embolization. Results:Among the 45 patients, 21 patients had hemangioblastomas, 15 had meningiomas, 5 had hemangiopericytomas, and 4 had glomus jugular tumors. The technical success rate of interventional embolization was 97.8% (44/45); in this frustrated case, the middle meningeal artery was too circuitous for microcatheter to pass. Among the successful ones, 41 patients used liquid embolism agent onyx and 3 patients applied liquid embolism agent NBCA. Seven, 26 and the rest 11 patients achieved complete embolization, sub-total embolization and partial embolization, respectively. Four patients had embolism-related complications, including 2 with rupture of middle meningeal arteries, 1 with Marathon catheter failed to be pulled out, and 1 with functional glomus jugular tumor having pheochromocytoma crisis; these 4 patients were treated timely without serious complications. Conclusion:For intracranial hypervascular tumors, preoperative interventional embolization is safe and effective; it is necessary to master embolization indications and select appropriate embolization methods and materials.

COVID-19 is caused by a novel coronavirus-severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2), which has being spreading around the world, posing a serious threat to human health and lives. Neutralizing antibodies and small molecule inhibitors for virus replication cycle are the main antiviral treatment for novel coronavirus recommended in China. To further promote the rational use of antiviral therapy in clinical practice, the National Center for Infectious Diseases (Beijing Ditan Hospital Capital Medical University and the First Affiliated Hospital, Zhejiang University School of Medicine) invited experts in fields of infectious diseases, respiratory and intensive care to develop an Expert Consensus on Antiviral Therapy of COVID-19 based on the Diagnosis and Treatment Guideline for COVID-19 ( trial version 10) and experiences in the diagnosis and treatment of COVID-19 in China. The consensus is concise, practical and highly operable, hopefully it would improve the understanding of antiviral therapy for clinicians and provide suggestions for standardized medication in treatment of COVID-19.
