Diabetic nephropathy（DN） is a common and severe microvascular complication of diabetes. In recent years， the classical herbal formula Shenqi dihuang decoction has demonstrated unique advantages in the clinical treatment of DN. This article conducts a systematic review of the mechanisms of action and clinical applications of Shenqi dihuang decoction in the treatment of DN. It reveals that the mechanism by which this formula improves DN involves multi-target synergistic regulation. For instance, Shenqi dihuang decoction exerts multiple pharmacological effects by regulating signaling pathways including phosphatidy linostiol 3-kinase/protein kinase B， AMP-activated protein kinase/silent information regulator 1/forkhead box O1， and nuclear factor erythroid 2-related factor 2/heme oxygenase-1 pathways.These effects include regulating glucose and lipid metabolism， inhibiting oxidative stress， reducing inflammation， improving insulin resistance， modulating cell death （apoptosis/autophagy/ferroptosis/pyroptosis）， and preventing renal fibrosis. Existing clinical studies indicate that Shenqi dihuang decoction and its modified formulas， alone or in combination with other therapeutic methods， can significantly improve glucose and lipid metabolism， reduce proteinuria， and delay renal function decline in patients with DN. These effects are superior to those of Western medicines such as irbesartan， valsartan， and empagliflozin， and the treatment demonstrates good safety. Future research should leverage systems biology and artificial intelligence technologies to further elucidate the integrated mechanisms in the treatment of DN by Shenqi dihuang decoction， thereby advancing the precision and standardization of its clinical application.

Human lactate dehydrogenase A (LDHA) is an NADH-dependent oxidoreductase that catalyzes the conversion of pyruvate to lactate, playing a crucial role in the aerobic glycolysis of tumor cells. Additionally, LDHA is responsible for catalyzing the conversion of glyoxylate to oxalate in the liver, excessive accumulation of which leads to hyperoxaluria. Representative LDHA inhibitors reported to date include GNE-140, NCATS-SM1441, and CHK-336. In this study, NCATS-SM1441 was used as the lead compound. Based on the analysis of its cocrystal binding mode with LDH, the key carboxythiazole and aromatic pharmacophores were retained, while the original substituted pyrazole linker was replaced with urea linker moieties to simplify the structure. As a result, nine urea derivatives were designed and synthesized. Subsequently, on the basis of structure-activity relationship and molecular docking studies, nine target triazole compounds were further designed. Ultimately, triazole compound 25 was found to exhibit the most potent LDHA inhibitory activity (IC50 = 1.59 μmol/L) in vitro. Molecular dynamics simulations were employed to analyze the interactions between compound 25 and key amino acid residues. Finally, the prediction of physicochemical properties and the evaluation of cell viability were performed for compound 25. The study provides an experimental basis for future development of novel LDHA inhibitors with improved drugability.

Objective To explore the analytical methods combining multicolor flow cytometry with bioinformatics techniques for analyzing myeloid cells in the gastrocnemius of mice after muscle injury, and provide an experimental foundation for the study of muscle regeneration mechanisms. Methods Immune cells were collected from single-cell suspensions of mouse gastrocnemius using multicolor flow cytometry, and data were analyzed by the t-distributed stochastic neighbor embedding（t-SNE）algorithm supplemented by manual gating techniques. The tSNE algorithm was used in multicolor flow cytometry to guide the setting of manual gates, optimize the identification and classification of cell populations. Results Compared to the sham surgery group, the proportions of dendritic cells, granulocytes, macrophages, and monocytes at the site of muscle injury in the model group significantly increased, with the increasing in monocytes being particularly notable. Conclusion The application of the tSNE algorithm combined with manual gating techniques in multicolor flow cytometry demonstrated that this method could effectively guide the setting of manual gates and enhance the efficiency of distinguishing immune cell types. Through this combined technology, the function and subtypes of myeloid cells in the mouse gastrocnemius could be analyzed more accurately.

Artificial intelligence （AI） technologies， especially machine learning algorithms， have achieved remarkable advances in medical image analysis in recent years. As a noninvasive， real-time， and cost-effective imaging modality， vascular ultrasound is one of the most active areas for the application of AI technologies in medicine. This article reviews the latest research advances in the application of AI in vascular ultrasound image recognition， automatic segmentation， diagnostic assistance， and risk prediction in 2021—2025， covering the aspects of transcranial Doppler， carotid artery， abdominal aortic aneurysms， aortic dissection， peripheral artery disease， arteriovenous fistula， venous thrombosis， and the inferior vena cava. This article highlights the key technological innovations， clinical value， and research challenges of AI within these areas.

Objective:To establish the characteristic spectrum of Liushenqu standard decoction using ultra-high performance liquid chromatography (UPLC); To determine the contents of related index components; To evaluate the quality of Liushenqu standard decoction.Methods:UPLC method was used to establish characteristic spectrum of Liushenqu standard decoction. Chromatographic Fingerprint Similarity Evaluation System (2012 edition) was used for similarity analysis, the characteristic peak was assigned, and the content of its index components was determined.Results:The characteristic peaks of Liushenqu standard decoction were calibrated and 8 components were identified, namely uridine, adenosine, guanosine, 5-hydroxymethylfurfural, tryptophan, vanillic acid, ferulic acid and shaftaside. The contents of uridine, adenosine, tryptophan ferulic acid and shaftaside in 10 batches of Liushenqu standard decoction were simultaneously determined, and ranged from 0.036 1～0.383 9 mg/g, 0.030 7～0.170 2 mg/g, 0.007 0～0.060 2 mg/g, 0.001 0～0.005 0 mg/g, 0.000 8～0.013 8 mg/g, respectively. The transfer rates ranged from 44.2% to 50.8%, 60.1% to 67.7%, 60.4% to 76.4%, 62.7% to 77.4%, 50.7% to 61.4%, respectively.Conclusion:The established UPLC characteristic spectrum and content determination method are accurate and repeatable, which can provide references for quality control of Liushenqu standard granules.

Objective:To establish UPLC characteristic spectrum of Hujiao standard decoction and the determination method for the content of piperine.Methods:15 batches of freeze-dried powder of Hujiao standard decoction were prepared according to the traditional decocting method. The range of paste yield was determined, and the UPLC characteristic spectrum of the standard decoction was established. High-resolution mass spectrometry and control products were used to identify common peaks. Based on the common peak area, the weights of each peak were compared using entropy weight method, and correlation analysis and similarity evaluation were conducted using clustering analysis and grey correlation method; a method for determining the content of piperine in Hujiao decoction pieces and freeze-dried powder of standard decoction was simultaneously established, and the transfer rate was calculated.Results:The extraction rate of 15 batches of freeze-dried powder of Hujiao standard decoction ranged from 10.4% to 16.8%, with an average of 14.0%. The content of piperine ranged from 12.2 to 30.0 mg/g, with an average of 18.5 mg/g, and the transfer rate ranged from 4.0% to 7.8%, with an average of 5.8%. Six common peaks were identified in the established characteristic spectrum and identified by high-resolution mass spectrometry and control products respectively. Peak 1 was N-trans-feruloyltyramine, peak 3 was piperine and the similarity was 0.959-1.000. Clustering analysis and grey correlation analysis showed that there was little difference between quality of Piperis Fructus and origins.Conclusion:In this study, the characteristic spectrum and content determination method of freeze-dried powder of Hujiao standard decoction are established, which can provide references for quality detection and control of Hujiao standard decoction or its derivative products.

Objective:To establish the quality standard of the standard decoction of wine-processed Coptidis Rhizoma by studying the extraction rate, fingerprint and component quantitative analysis.Methods:ccording to the Technical Requirements for Quality Control and Standard Formulation of Chinese Medicine Formula Granules, 15 batches of the standard decoction of wine-processed Coptidis Rhizoma were prepared, and the paste rate was determined; HPLC fingerprints of 15 batches of standard decoction of wine-processed Coptidis Rhizoma were established, and evaluated by combining similarity evaluation, clustering analysis, principal component analysis and orthogonal partial least squares discriminant analysis; the contents of berberine, epiberberine, pamadine, and safranine in the samples of the 15 batches were determined and analyzed their transfer rates.Results:A total of 15 batches of standard decoction samples were calibrated with 11 common peaks, referring to the recognition of 8 components. The similarity between the samples and the control product was greater than 0.900; the clustering analysis could cluster the 15 batches of samples into 2 classes; the results of the principal component analysis showed that the cumulative variance contribution rate of the 3 principal component factors was 89.388%; the OPLS-DA screened out the 3 components of the quality difference; the 15 batches of samples out of the paste rate was 15.7% -20.8%, and the mass fractions of berberine, epiberberine, safranine, and palmatine were 18.47%-24.38%, 2.82%-3.49%, 5.08%-6.69%, and 4.84%-6.68%, respectively, with transfer rates of 41.7%-61.7%, 46.9%-68.7%, 39.8%-61.5%, and 43.8%-65.2%.Conclusion:The fingerprint and content determination method established in this study is accurate, stable, simple, and can be used for the quality control and evaluation of the standard decoction of wine-processed Coptidis Rhizoma.

In recent years,a growing body of research has demonstrated that acupuncture can be used to effectively treat a diverse range of diseases,including functional gastrointestinal disorders,cardiovascular diseases,as well as anxiety and depression,through the modulation of the paraventricular hypothalamic nucleus(PVN).Acupuncture may exert its therapeutic effect either by modulating specific neurons within the PVN,such as corticotropin releasing hormone(CRH)neurons,or by regulating the release of hormones,such as oxytocin(OXT)and vasopressin(VP),and the activity of neural circuits associated with the PVN.This review summarizes the mechanisms by which PVN is involved in acupuncture treatment,including its regulatory mechanisms in gastrointestinal diseases,cardiovascular diseases,and negative emotions and pain.Future research should be conducted to further explore the precise mechanisms by which acupuncture regulates PVN to treat diseases,focusing on clarifying the specific processes of signaling pathway transduction,and exploring the specific effects of acupunture of different acupoint combinations and stimulation frequencies and intensity on PVN.

Objective To investigate the clinical diagnostic value of lung ultrasound(LUS)com-bined with serum interleukin(IL)-1β and IL-12p70 for pediatric community-acquired pneumonia(CAP).Methods A total of 136 pediatric patients with suspected CAP were enrolled and divided in-to CAP group(95 patients)and non-CAP group(41 patients)based on chest computed tomography(CT)results as the gold standard.The LUS characteristics,LUS scores,serum levels of IL-1 β and IL-12p70 were compared between the two groups at admission.Receiver operating characteristic(ROC)curves were plotted to analyze the diagnostic efficacy of LUS combined with serum IL-1 β and IL-12p70 for CAP.Results The incidence rates of interstitial disease,pleural line abnormalities,pleural effusion,lung consolidation,and bronchial signs were higher in the CAP group than those in the non-CAP group(P＜0.05).The serum levels of IL-1 β,IL-12p70 and LUS scores were also high-er in the CAP group than in the non-CAP group(P＜0.05).ROC curve analysis showed that the area under the curve for the diagnosis of CAP using LUS scores combined with serum IL-1 β and IL-12p70 at admission was 0.981,with a sensitivity of 93.68％,a specificity of 95.12％,and a diagnostic effica-cy was significantly higher than that of each index alone(P＜0.05).Conclusion LUS combined with serum IL-1 β and IL-12p70 has high diagnostic value for pediatric CAP.

Metabolic dysfunction-associated steatotic liver disease(MASLD)is the most prevalent chronic liver disease globally,encompassing the entire spectrum of fatty liver pathogenesis.It progresses from simple steatosis to metabolic-associated steatohepatitis(MASH),involving injury and inflammation,with or without fibrosis,ultimately leading to cirrhosis and hepatocellular carcinoma,which affects approximately a quarter of the world's population.Liver biopsy remains the gold standard for differentiating MASH from steatosis and assessing advanced fibrosis.However,its limitations,including costliness,invasiveness,and sampling bias,have spurred the development of noninvasive diagnostic techniques.In addition,there are no FDA-approved drugs for the treatment of MASLD.Enumerating noninvasive diagnostic markers that have the potential to replace liver biopsy were summarized,and the current treatment options for MASLD were discussed,with clinical trials designed to evaluate the efficacy and safety of single agents or combination therapies to halt or reverse disease progression,which could provide new insights for the clinical diagnosis and treatment of MASLD.
