Aim To predict the mechanism of action of ginsenoside Rg1(G-Rg1)paired with hirudin in the treatment of myocardial fibrosis in acute myocardial in-farction(AMI)based on the network pharmacology ap-proach,and to validate it by in vivo and in vitro experi-ments.Methods The corresponding targets of G-Rg1 and Hirudin were collected using SwissTargetPredic-tion,TargetNet,ETCM and ChEMBL databases, and the targets related to AMI and myocardial fibrosis were collected using GeneCards,OMIM and DisGeNET da-tabases.The drug-disease intersection targets were subjected to protein-protein interaction network(PPI)network analysis, gene ontology(GO)functional en-richment analysis and kyoto encyclopedia of genomes(KEGG)pathway enrichment analysis.Key targets and pathways were validated using an AMI mouse mod-el induced by ligation of the anterior descending branch of the left coronary artery in mice versus a hypoxia-in-duced injury model of human cardiac microvascular en-dothelial cells(HCMECs).Results G-Rg1 paired with hirudin had 229 drug targets,816 AMI and myo-cardial fibrosis disease targets, and 65 intersecting tar-gets.PPI analysis showed that tumor necrosis factor(TNF),interleukin-1[3(IL-1 β),transforming growth factor beta-1(TGF-β1),nuclear factor kappa-B(NF-κB),and interleukin-6(IL-6)might be the core tar-gets of G-Rg1 paired with Hirudin in the treatment of post-MI myocardial fibrosis;KEGG was enriched for a total of 141 pathways involving endocrine and metabo-lism,inflammation,and immunity, mainly TNF signa-ling pathway,PI3K/Akt signaling pathway and TGF-βsignaling pathway. In vivo experiments confirmed that G-Rg1 paired with hirudin attenuated myocardial fibro-sis after AMI in mice, and down-regulated the expres-sion of TNF-α,IL-1β,NF-κB,TGF-β1,and Smad2/3 proteins in myocardial tissues. In vitro experiments confirmed that G-Rg1 paired with Hirudin inhibited cellular NF-κB/TGF-β1 pathway, reduced hypoxia-in-duced cellular TNF-α and IL-1β expression, and su-perimposed NF-κB inhibitor significantly reduced IL-1 β expression and attenuated cellular inflammatory re-sponse.Conclusions G-Rg1 with Hirudin treats post-MI myocardial fibrosis by regulating TNF-α,IL-1 β and other targets and NF-KB/TGF-β1 pathway, reflecting its multi-pathway and multi-target action characteris-tics,and providing a pharmacological basis for the treatment of post-MI myocardial fibrosis with G-Rg1 with Hirudin.

National Journal of Andrology 2025;31(5):412-420
OBJECTIVE:
The aim of this study is to investigate the risk factors of prognosis in patients with early-onset prostate cancer treated without surgery. A nomogram will be constructed and validated to predict overall survival (OS) of patients with early-onset prostate cancer treated without surgery.
METHODS:
The clinical data was obtained from the National Cancer Institute's Surveillance, Epidemiology, and End Results (SEER) database on prostate cancer patients aged 18-55 years who were treated without surgery between 2010 and 2015. The clinical data set was divided into training set and validation set according to 7∶3 ratio, including age, race, marital status, Gleason score, prostate specific antigen (PSA) and other 8 factors. And significant variables were screened by univariate Cox regression analysis. Multivariate Cox regression analysis was used to identify the influence factors. Stepwise regression method was used to select the most influential factors on the total OS, and R software was used to build a nomogram model. The accuracy and prediction ability of the model were verified by drawing receiver operating characteristic (ROC) and Calibration Plot. The clinical benefit of the model was evaluated by decision curve analysis (DCA).
RESULTS:
A total of 8 212 patients who met the criteria were randomly assigned to the training set (n=5 752) or validation set (n=2 460), with no statistical difference between the two groups (all P>0.05). Six factors were identified through univariate and multivariate Cox regression analysis including marital status, N stage, M stage, radiotherapy, PSA and Gleason score, which were most closely associated with the OS of prostate cancer patients, and a column graph model was constructed based on these factors. The Consistency index (C-index) of the model in the training set and the verification set were 0.802 and 0.794, respectively. And the apparent diffusion coefficient (AUC) was 0.851, 0.855 and 0.855 for training sets 1, 3 and 5 years, and 0.694, 0.860 and 0.832 for verification sets 1, 3 and 5 years. The calibration chart showed a good agreement between the predicted and actual values of the model. In the analysis of decision curve, the model showed good clinical application value.
CONCLUSION
The prediction model based on marital status, radiotherapy, M stage, N stage, PSA and Gleason score for early-onset prostate cancer patients without surgical treatment has certain reference value which is expected to become an effective tool for clinicians to treat in future prospective studies on large and multi-center samples.

OBJECTIVE:
To precise classify sacral meningeal cysts, effective guide minimally invasive neurosurgery and postoperative personalized rehabilitation by multiple dimensions radiographic reconstruction MRI.
METHODS:
From March to December 2021, based on the original 3D-fast imaging employing steadystate acquisition (FIESTA) scanning sequence, 92 patients with sacral meningeal cysts were pre-operatively evaluated by multiple dimensional reconstruction MRI. The shape of nerve root and the leakage of cyst were reconstructed according to the direction of nerve root or leakage track showed on original MRI scans. Sacral canal cysts were accurately classified as including nerve root and without nerve root, so as to accurately design the incision of skin and formulate corresponding open range of the posterior wall of the sacral canal. Under the microscope intraoperation, the shape of the nerve roots inside cysts or leakage track of the cysts without nerve roots were verified and explored. After the reinforcement and shaping operation, several reexaminations of multiple dimensional reconstruction MRI were performed to understand the deformation of the nerve root and hydrops in the operation cavity, so as to formulate a persona-lized rehabilitation plan for the patients.
RESULTS:
Among the 92 patients with sacral mengingeal cyst, 58 (63.0%) cysts with nerve root cyst, 29 (31.5%) cysts without nerve root cyst, and 5 (5.4%) cysts with mixed sacral canal cyst. In 58 patients with nerve root cysts, the accuracy of preoperative clinical classification on MRI image reached 96.6% (56/58) through confirmation by operating microscope. Only 2 cases of large single cyst with nerve root on the head of cyst were mistaken for without nerve root type. In 29 patients with sacral cyst without nerve root, the accuracy of preoperative image reached 100% through confirmation by operating microscope. The accuracy of judging the internal nerve root and leakage of 12 cases with recurrent sacral cyst was also 100%. Two cases of delayed postoperative hydrops were found one month after operation. After rehabilitation treatment by moxibustion and bathing, the hydrops disappeared 4-6 months after operation.
CONCLUSION
Multiple dimensional reconstruction MRI can precisely make clinical classification of sacral meningeal cysts before operation, guide minimally invasive neurosurgery effectively, and improve the rehabilitation effect.
