1.Mechanistic study of Tripterygium wilfordii multiglucoside in improving nephrotic syndrome via regulating the HIF-1α/miR-155-5p/Nrf2 pathway

OBJECTIVE To study the improvement effect and mechanism of Tripterygium wilfordii multiglucoside (TWM) on nephrotic syndrome in rats. METHODS The nephrotic syndrome model was established by intravenous injection of adriamycin via the tail vein. The modeling rats were randomly divided into the model group (distilled water), prednisone group (10 mg/kg), and TWM high- and low-dose groups (10 and 5 mg/kg, respectively). Additionally, blank group (distilled water) without model induction was established. Each group consisted of 9 rats. Rats in each group were administered the corresponding drugs or distilled water by gavage, once a day, for 6 consecutive weeks. The histopathological morphology of kidney tissues in rats was observed; the levels of 24-hour urinary protein (24 h-UTP) and serum biochemical indicators [albumin (ALB), blood urea nitrogen (BUN), serum creatinine (SCr), cholesterol (CHOL), and triglyceride (TG)] in rats were determined; the levels of oxidative stress indicators [superoxide dismutase (SOD), malondialdehyde (MDA)] in kidney tissue of rats were determined; expressions of hypoxia-inducible factor-1α (HIF-1α)/microRNA-155-5p (miR-155-5p)/nuclear factor erythriod 2- related factor 2 (Nrf2) signaling pathway-related mRNA and protein in the renal tissues of rats were detected. RESULTS Compared with the blank group, the rats in the model group exhibited disordered renal tissue structure, with a small amount of glomerular necrosis and edema of the renal tubular epithelial cells. 24 h-UTP, serum levels of SCr, BUN, CHOL and TG, MDA content, mRNA and protein expressions of HIF-1α and Keap1 as well as the expression of miR-155-5p in renal tissues were increased significantly ( P <0.05). Serum level of ALB, SOD level in renal tissue as well as mRNA and protein expressions of Nrf2 were decreased significantly ( P <0.05). Compared with the model group, TWM high-dose and low-dose groups exhibited significant improvements in renal injury, with notable reversals in the levels of the above quantitative indicators ( P <0.05). CONCLUSIONS TWM can alleviate oxidative stress-induced damage and thereby improve nephrotic syndrome in rats by regulating the HIF-1α/miR-155-5p/Nrf2 signaling pathway.
2.Effectiveness of a newly developed YouTube diabetes education: A multicentre randomized controlled trial

Introduction:
Adequate knowledge of diabetes is essential for effective self-management and glycemic control. Emerging trends in
digital platforms, video-based education offer a scalable and accessible approach to patient learning. However, evidence
on the effectiveness of validated YouTube-based diabetes education in the Malaysian population remains limited. We
aimed to evaluate the impact of a newly developed, tri-language YouTube diabetes education programme on clinical and
knowledge outcomes.
Methodology:
In this multicentre study, patients with diabetes mellitus were randomly assigned to intervention (YouTube-based education)
and control groups (standard care). The intervention consisted of newly developed and validated short educational videos
on YouTube regarding diabetes, complications and lifestyle changes, delivered weekly via WhatsApp over 9 weeks. Videos
were available in English, Malay and Chinese. The control group received standard care and counseling. Demographic
data, hemoglobin A1c (HbA1c), and knowledge scores were collected at baseline and 3 months post-intervention.
Results:
A total of 109 patients (62.4% female; mean age 53.2 years; mean diabetes duration 16.3 years) were enrolled in the
study, with similar baseline characteristics between groups. The intervention group demonstrated significantly greater
improvement in glycemic control compared to the control group (HbA1c: −1.22 ± 1.36% vs. −0.24 ± 1.05%; p <0.001). While
knowledge scores improved in both groups, the increase was significantly greater in the intervention group (2.66 vs. 0.80;
p = 0.001). Notably, improvement in knowledge score predicted HbA1c reduction (b = 0.091; 95% CI 0.01, 0.172; p = 0.028).
Conclusion
The tri-language YouTube-based diabetes education programme significantly improved both glycemic control and patients’
knowledge. This accessible, scalable video-based education represents an effective strategy to enhance diabetes selfmanagement, offering flexible, on-demand learning for diverse populations.

3.Association between A Body Shape Index (ABSI) and sarcopenia among older adults in China: evidence from the CHARLS

Objective:To explore the association between A Body Shape Index (ABSI) and sarcopenia in Chinese older adults.Methods:Totally 1 728 participants in waves 1 and 3 of the China Health and Retirement Longitudinal Survey (CHARLS) were enrolled in this cohort-based cross-sectional analysis. ABSI was calculated by using anthropometric measurements from CHARLS. According to the Asian Working Group for Sarcopenia (AWGS) 2019 consensus update on sarcopenia, low muscle strength and low muscle mass were used as the diagnostic criteria for sarcopenia. Logistic regression and restricted cubic spline (RCS) were used to assess the relationship between ABSI and sarcopenia, and subgroup and interaction analyses were conducted to evaluate the influence of other factors on this association.Results:High ABSI showed a significant association with the risk of sarcopenia among older adults in China, and RCS analysis demonstrated a positive linear correlation between ABSI and sarcopenia. Additionally, the relationship of ABSI with sarcopenia was significantly modified by educational level and depressive status.Conclusion:ABSI, a novel anthropometric measure, serves as an effective threshold for assessing sarcopenia risk in the Chinese older population, providing a basis for developing clinical interventions and public health strategies for the prevention and early identification of sarcopenia.

6.Research and Application of Traditional Chinese Medicine Knowledge Organization System

The knowledge organization work in the field of Traditional Chinese Medicine(TCM)aims to establish a systematic TCM knowledge system,contributing to the discovery,sharing,and application of knowledge resources.From the perspective of knowledge organization,this paper classifies the TCM knowledge system based on technological development generations and degrees of structurali-zation.Seven representative and distinctive knowledge organization systems in the TCM field were selected,including encyclopedias,dictionaries,classification schemes,subject headings,ontologies,semantic networks,and knowledge graphs.The characteristics of each knowledge system are identified and future research directions are analyzed.The progress of knowledge organization application re-search relies on the support of knowledge organization systems.This paper elaborates on the application scope of TCM knowledge organ-ization systems from three aspects:literature indexing and retrieval,clinical decision support,and intelligent knowledge service con-struction.Finally,it emphasizes that knowledge organization models will develop toward achieving multimodal,fine-grained knowledge organization and advanced reasoning applications,continuously improving the TCM knowledge organization system and fully exploiting its application potential.
7.Characteristics of Tumor-Associated Macrophages in Different Regions of Breast Cancer and Their Correlation with Clinicopathology via Contrast-Enhanced Ultrasound

Purpose To investigate the distribution characteristics of tumor-associated macrophages(TAM)in different regions of breast cancer with enhanced contrast-enhanced ultrasound(CEUS),and to further explore the relationship between TAM and CEUS indicators and clinicopathology in different regions of breast cancer.Materials and Methods A total of 119 patients with suspected breast cancer admitted to the Cancer Hospital Affiliated to Xinjiang Medical University from March 2021 to March 2023 were prospectively included.CEUS was applied to the tumor,and ultrasound-guided puncture biopsy was also taken.The lesions diagnosed as breast cancer by pathology was outlined the central area,marginal area and normal area next to the cancer,and was obtained the time intensity curves of different areas.The tissues were taken for immunohistochemistry and flow cytometry,and the TAM cells were stained and distinguished.The characteristics of TAM in different regions of breast cancer and its correlation with clinical pathology were analyzed,respectively.Results By immunohistochemistry and flow cytometry,there were significant differences in the number of TAM infiltration in the border area,central area and adjacent area of breast cancer(immunohistochemistry:F=382.326,P<0.05;flow cytometry:F=24.955,P<0.05).The characteristics of CEUS in three different regions showed that the TAM in the central region of breast cancer increased when filling defect appeared(t=2.631,P<0.05),but the TAM in the peripheral region was more(t=2.999,P<0.05).After angiography,lesions showed high perfusion,and there was significantly more TAM in the edge and central area of the lesion than that in normal area next to the cancer(t=5.529,P<0.05;t=2.584,P<0.05).Clinical stage was related to the TAM in three regions.When the clinical stage was high,there were more TAM in all three regions(t=6.658,2.367,2.400,all P<0.05).Histological grading was high,and TAM in all three areas was high(F=101.151,16.922,26.822,all P<0.05).Conclusion There was a decreasing trend of TAM in the marginal area,central area and adjacent tissues of breast cancer during CEUS.The edge region has more malignant CEUS characteristics than that in the central region and the normal region adjacent to cancer;and the number of TAM is more in breast cancer with late clinical grading,poor tissue differentiation and obvious contrast-enhanced ultrasound malignant characteristics.The distribution characteristics of TAM represent the malignant degree and metastatic probability of breast cancer to a certain extent,and TAM is a factor related to the invasion of breast cancer.

OBJECTIVE:
To assess the independent and combined effects of air pollutants, meteorological factors, and greenspace exposure on new tuberculosis (TB) cases.
METHODS:
TB case data from Shanghai (2013-2018) were obtained from the Shanghai Center for Disease Control and Prevention. Environmental data on air pollutants, meteorological variables, and greenspace exposure were obtained from the National Tibetan Plateau Data Center. We employed a distributed-lag nonlinear model to assess the effects of these environmental factors on TB cases.
RESULTS:
Increased TB risk was linked to PM 2.5, PM 10, and rainfall, whereas NO 2, SO 2, and air pressure were associated with a reduced risk. Specifically, the strongest cumulative effects occurred at various lags: PM 2.5 ( RR = 1.166, 95% CI: 1.026-1.325) at 0-19 weeks; PM 10 ( RR = 1.167, 95% CI: 1.028-1.324) at 0-18 weeks; NO 2 ( RR = 0.968, 95% CI: 0.938-0.999) at 0-1 weeks; SO 2 ( RR = 0.945, 95% CI: 0.894-0.999) at 0-2 weeks; air pressure ( RR = 0.604, 95% CI: 0.447-0.816) at 0-8 weeks; and rainfall ( RR = 1.404, 95% CI: 1.076-1.833) at 0-22 weeks. Green space exposure did not significantly impact TB cases. Additionally, low temperatures amplified the effect of PM 2.5 on TB.
CONCLUSION
Exposure to PM 2.5, PM 10, and rainfall increased the risk of TB, highlighting the need to address air pollutants for the prevention of TB in Shanghai.
