OBJECTIVE:
To explore the effect of acupuncture therapy on dysphagia in patients with Parkinson's disease.
METHODS:
This randomized controlled study lasted 42 days and included 112 patients with Parkinson's disease and dysphagia. Participants were randomly assigned to the experimental and control groups (56 cases each group) using the completely randomized design, all under routine treatment. The experimental group was given acupuncture therapy. The primary outcome was Penetration-Aspiration Scale (PAS). The secondary outcomes were (1) Standardized Swallowing Assessment (SSA), and (2) nutritional status including body mass index (BMI), serum albumin, prealbumin, and hemoglobin. Adverse events were recorded as safety indicators.
RESULTS:
One participant quitted the study midway. There were no significant differences in baseline assessment (P>0.05). After treatment, both groups showed significant improvement in PAS, SSA and nutritional status except for BMI of the control group. There were significant differences between the two groups in the PAS for both paste and liquid, SSA (25.18±8.25 vs. 20.84±6.92), BMI (19.97±3.34 kg/m2vs. 21.26 ±2.38 kg/m2), serum albumin (35.16 ±5.29 g/L vs. 37.24 ±3.98 g/L), prealbumin (248.33 ±27.72 mg/L vs. 261.39 ±22.10 mg/L), hemoglobin (119.09±12.53 g/L vs. 126.67±13.97 g/L) (P<0.05). There were no severe adverse events during the study.
CONCLUSION:
The combination of routine treatment and acupuncture therapy can better improve dysphagia and nutritional status in patients with Parkinson's disease, than routine treatment solely. (registration No.

PURPOSE:
Lateral patellar compression syndrome (LPCS) is characterized by a persistent abnormally high stress exerted on the lateral articular surface of the patella due to lateral patellar tilt without dislocation and lateral retinaculum contracture, leading to anterior knee pain. The purpose of this study is to evaluate the efficacy and prognosis of lateral retinaculum release (LRR) combined with chondroplasty in the treatment of LPCS.
METHODS:
This retrospective study evaluated 40 patients who underwent LRR combined with chondroplasty for LPCS between 2020 and 2021. The assessment included improvement in postoperative tenderness and knee joint function. Patients were evaluated using the Lysholm, Tegner, and International Knee Documentation Committee 2000 scoring systems, as well as the visual analog scale, both preoperatively and postoperatively, with the paired comparisons analyzed using a t-test. Additionally, intraoperative observations were made regarding knee joint lesions, including cartilage damage and osteophyte formation, with analysis by the Chi-square test.
RESULTS:
The visual analog scale score for tenderness showed a significant decrease after surgery (p < 0.001). Evaluation of knee joint function also indicated significant improvements, as demonstrated by increased Lysholm, Tegner, and International Knee Documentation Committee 2000 scores postoperatively (p < 0.001, p = 0.011, p < 0.001, respectively). Furthermore, all LPCS patients included in the study presented with cartilage injuries and osteophyte formation. Significant differences were noted in the incidence of cartilage damage and osteophyte formation at different locations within the knee among patients with LPCS.
CONCLUSION
LRR combined with chondroplasty is an effective surgical approach for treating patients with LPCS, with satisfactory recovery observed at the 1-year follow-up. Additionally, the incidence of cartilage damage and osteophyte formation in LPCS patients varies significantly depending on the specific location within the knee joint.

The single photon counting imaging detector based on microchannel plate(MCP)has the characteristics of high sensitivity and low dark count rate, and has been applied to the optical remote sensing detection of weak ultraviolet spectral signals in space. In this work,by using planar array single photon counting imaging detector as the detector,flat-field concave grating as the splitter, and electrodeless discharge lamp(EDL)as the excitation light source,a dispersion detection system suitable for hydride generation-atomic fluorescence spectrometry(HG-AFS)was developed. The wavelength calibration of the system was carried out,and the negative high pressure and EDL stability time of the planar array single photon counting imaging detector were analyzed and optimized. The characteristic emission spectral lines of As and Bi elements excited in the wavelength range of 180-320 nm were analyzed, and the scattering interference in the wavelength range of 257.3-306.7 nm was discussed. The results showed that the AFS dispersion detection system based on the planar array single photon counting imaging detector could detect and analyze the HG-AFS fluorescence signal initially, and the influence of scattering interference on the detection results was effectively avoided. The system had the advantages including simple structure,no refrigeration and temperature control,no moving parts and simultaneous measurement of multi-band.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

Objective:To preliminarily explore the application of directional electrodes with perceivable subthalamic nucleus-deep brain stimulation (STN-DBS) for Parkinson′s disease (PD).Methods:A retrospective analysis was conducted on 56 patients with primary PD who underwent STN-DBS treatment across multiple neurosurgical centers, including the Department of Neurosurgery of the First Medical Center of the Chinese People′s Liberation Army General Hospital, the Department of Neurosurgery of Hua′an Brain Hospital Affiliated to Anhui Medical University, and the Department of Neurosurgery of Hefei Second People′s Hospital, from January to December 2024. The cohort included 26 patients in the directional+perception group and 30 in the conventional group. The directional+perception group had activation contacts selected based on electrode branch contact locations and local field potential data recorded by the perceptible deep brain stimulation (DBS) system. The conventional group used contact testing to determine therapeutic contacts. Unified Parkinson′s Disease Rating Scale-Ⅲ (UPDRS-Ⅲ) assessments were performed in the medication-off state under continuous STN-DBS therapy at postoperative activation, 1, 3, and 6 months, comparing postoperative data with preoperative baseline. Initial programming outcomes were also compared between groups. Results:By combining directional electrodes with sensing capabilities, therapeutic contacts can be selected more quickly and effectively. The directional+ perception group showed significantly shorter initial programming time compared to the conventional group [(30.1±4.7) min vs (65.0±6.8) min, respectively], with a statistically significant difference ( t=-22.159, P<0.001). Compared to preoperative baseline, UPDRS-Ⅲ scores improved markedly at postoperative activation and at 1, 3, and 6 months, with improvements of 59.8%(20.6±5.2 vs 51.2±8.7), 62.1%(19.4±6.2 vs 51.2±8.7), 55.5%(22.8±7.2 vs 51.2±8.7), and 61.7%(19.6±13.9 vs 51.2±8.7), respectively. The scores of tremor showed the greatest improvement of 72.2% [2.5(0, 4.3) vs 9.0(0, 13.0)], 61.1% [3.5(0, 5.0) vs 9.0(0, 13.0)], 72.2% [2.5(0, 5.0) vs 9.0(0, 13.0)], 63.3% [0(0, 3.3) vs 9.0(0, 13.0)], respectively, followed by rigidity. Axial symptoms, postural stability, and gait improved moderately, while speech showed no significant change. Conclusions:In the treatment of PD, the combined use of directional electrodes and a perceivable DBS system allows precise selection of therapeutic contacts. This approach not only safely and effectively improves patients′ motor symptoms but also significantly reduces the time required for initial programming compared to conventional DBS systems, demonstrating clear clinical advantages.

Objective To examine the changing prevalence and antimicrobial resistance profiles of Burkholderia cepacia in 52 hospitals across China from 2015 to 2021.Methods A total of 9 261 strains of B.cepacia were collected from 52 hospitals between January 1,2015 and December 31,2021.Antimicrobial susceptibility of the strains was tested using Kirby-Bauer method or automated antimicrobial susceptibility testing systems according to a unified protocol. The results were interpreted according to the breakpoints released in the Clinical & Laboratory Standards Institute(CLSI)guidelines(2023 edition). Results A total of 9 261 strains of B.cepacia were isolated from all age groups, especially elderly patients. The proportion was 11.1%(1 032 strains)in children, significantly lower than the proportion in adults.About half(46.5%,4 310/9 261)of the strains were isolated from patients at least 60 years old and 42.3%(3 919/9 261)of the strains were isolated from young adults. Most isolates(71.1%)were isolated from sputum and respiratory secretions, followed by urine(10.7%)and blood samples(8.1%).B.cepacia isolates were highly susceptible to the five antimicrobial agents recommended in the CLSI M100 document(33rd edition,2023).B.cepacia isolates showed relatively higher resistance rates to meropenem and levofloxacin. However, the resistance rates to ceftazidime,trimethoprim-sulfamethoxazole,and minocycline remained below 8.1%.The percentage of B.cepacia strains resistant to levofloxacin was the highest compared to other antibiotics in any of the three age groups(from 12.4%in the patients<18 years old to 20.6%in the patients aged 60 years or older). Conclusions B.cepacia is one of the clinically important non-fermenting gram-negative bacteria.Accurate and timely reporting of antimicrobial susceptibility test results and ongoing antimicrobial resistance surveillance are helpful for rational prescription of antimicrobial agents and proper prevention and control of nosocomial infections.
