Objective To explore the risk factors for recurrent laryngeal nerve(RLN)invasion in patients with papillary thyroid carcinoma(PTC)based on ultrasound image characteristics, construct a regression equation, and explore its predictive value for providing a reference for the clinical prevention and treatment of RLN invasion in PTC patients. Methods A retrospective analysis was conducted on 1 195 patients who were pathologically diagnosed with PTC in the Affiliated TCM Hospital of Guangzhou Medical University and Nanfang Hospital,Southern Medical University from January 2020 to November 2024.These patients were divided into invasion group(n=28)and non-invasion group(n=1167)according to whether they had RLN invasion or not.The clinical data, pathological data and ultrasound image characteristics were compared between two groups. The risk factors for RLN invasion in PTC patients were identified using multifactorial Logistic regression to construct a regression equation which was then evaluated using the likelihood ratioχ2.Moreover,the Hosmer-Lemeshow test was utilized to assess the goodness of fit,and the receiver operating characteristic(ROC)curve was plotted to analyze the predictive value of the regression equation for RLN invasion in PTC patients, and the area under the curve(AUC)was calculated. Results The proportions of patients with tumor located in the posterior sagittal position, maximum tumor diameter≥1 cm,capsule contact and lymph node metastasis were higher in invasion group than in non-invasion group(P<0.05).Multifactorial Logistic regression analysis identified tumor located in the posterior sagittal position(OR=1.079,95%CI:1.031-1.129),maximum tumor diameter≥1 cm(OR=2.408,95%CI:1.256-4.617),capsule contact(OR=3.171,95%CI:1.724-5.833),and lymph node metastasis(OR=3.425,95%CI:1.884-6.226)as independent risk factors for RLN invasion in PTC patients(P<0.05).The regression equation was constructed as followed:logit(P)=-8.552+tumor located in the posterior sagittal position×0.076+maximum tumor diameter≥1 cm×0.879+capsule contact×1.154+lymph node metastasis×1.231.The likelihood ratioχ2 evaluation of the regression equation suggested that the regression equation was effectively constructed(P<0.001).Hosmer-Lemeshow test for the goodness of fit of the regression equation showed that the regression equation was well fitted(P>0.05).ROC curve analysis revealed that when logit(P)>0.875,the AUC value was 0.901(95%CI:0.882-0.917),with a sensitivity of 75.00%and a specificity of 88.95%.Conclusion The occurrence of RLN invasion in PTC patients is associated with tumor located in the posterior sagittal position, maximum tumor diameter≥1 cm,capsule contact, and lymph node metastasis, and the regression equations constructed based on these factors can well predict the occurrence of RLN invasion in PTC patients and is expected to provide an effective guidance for clinical practice.

Attention refers to the body's capacity to maintain alert awareness of internal and external stimuli.Attention deficit can impair an individual's learning capacity and working proficiency, resulting in a heavy burden to the family and society.Vagus nerve stimulation(VNS)is an emerging neuro regulatory technique that can achieve reciprocal interaction between the central and peripheral nervous systems, which has demonstrated great potential for the treatment of neurological and psychiatric diseases;however,it has not been used clinically to treat attention-deficient developmental disorders in children or neurodegenerative diseases in the aged population. We briefly review the anatomical and physiological bases of VNS,and its application status and potential use in treating attention deficit diseases.A potential mechanism by which VNS participates in attention-related nervous regulation is proposed,to lay a theoretical foundation for the application of VNS in the field of attention nervous regulation.

Objective To explore the correlation between fasting blood glucose(FBG)level and fractional flow reserve(FFR)in patients with borderline coronary artery disease, and to clarify its potential influence on FFR measurement. Methods From August 2020 to August 2023,the data of 135 patients with coronary atherosclerotic heart disease who received coronary angiography and FFR evaluation in the Fourth Affiliated Hospital of Harbin Medical University were retrospectively collected.According to the exclusion and inclusion criteria,85 cases of borderline diseased vessels of single coronary artery with stenosis degree of 50%-80%were screened out,and they were divided into FBG≥6.1 mmol/L group(47 cases)and FBG<6.1 mmol/L group(38 cases). The baseline data, angiographic and functional indexes of the two groups were compared, and the correlation between FBG and FFR was analyzed. Results Compared with the FBG<6.1 mmol/L group, the FBG≥6.1 mmol/L group had a higher proportion of FFR negative results(72.3%vs.23.7%,P<0.001),and the FFR measurement values were generally increased[0.84(0.80,0.90)vs.0.75(0.68,0.80),P<0.001],with statistically significant differences.Pearson correlation analysis was performed on all lesions, and FFR>0.80(negative result)was positively correlated with FBG≥6.1 mmol/L(r=0.484,P<0.001).Conclusions Among the patients with borderline coronary artery disease(50%-80%stenosis)included in this study,FBG≥6.1 mmol/L is significantly correlated with FFR>0.80.For patients with borderline coronary lesions with elevated FBG,the influence of blood glucose factors should be carefully considered in clinical interpretation of FFR results.

Objective To report the diagnosis and treatment of a case of acute pulmonary embolism caused by thrombus detachment within a popliteal venous aneurysm and conduct a literature review, aiming to enhance the understanding of the disease. Methods The case data of a patient with pulmonary embolism caused by thrombus detachment within a popliteal venous aneurysm who was admitted to the Second Hospital of Lanzhou University were retrospectively analyzed. The relevant literature published from July 2014 to July 2023 in the CNKI and PubMed databases was retrieved, and the diagnosis and treatment methods of the disease were summarized. Results The patient in this case was a 61-year-old male, who was admitted was admitted to the hospital due to"intermittent chest tightness and shortness of breath for 5 days".Color Doppler ultrasound examination and pulmonary artery CT angiography(CTA)suggested popliteal venous aneurysm combined with intramural thrombus and pulmonary embolism. Therefore, the pulmonary embolism and popliteal venous aneurysm were treated surgically in stages. The patient recovered well after the operation and there were no symptoms of chest tightness or shortness of breath after discharge.A total of 11 literatures were retrieved, involving 11 patients, all of whom underwent surgical treatment;Among them,4 patients were treated with inferior vena cava filter placement, and 2 patients underwent popliteal venous aneurysm surgery after filter placement.Through literature review,it can be known that the etiology of popliteal venous aneurysm remains unclear, and there is no unified standard for diagnosis and treatment.Up to now, surgery is still the preferred option for treatment. Conclusions Patients with popliteal venous aneurysms may be asymptomatic but can lead to deep vein thrombosis of the lower limbs and subsequent pulmonary embolism.Surgical intervention is an effective treatment option. For patients with combined pulmonary embolism, especially those with recurrent pulmonary embolism,it is recommended to perform surgical treatment following placement of an inferior vena cava filter.
