1.Literature analysis of tislelizumab-induced Stevens-Johnson syndrome and toxic epidermal necrolysis

OBJECTIVE To analyze the clinical characteristics of Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) induced by tislelizumab, providing evidence for clinical medication safety. METHODS Case reports of tislelizumab-related SJS/TEN were retrieved from CNKI, VIP, Wanfang Data, PubMed, ScienceDirect, and Embase. Descriptive analysis was performed. RESULTS Seventeen cases from 17 publications were included (SJS 4 cases, TEN 13 cases). Among them, there were 10 males and 7 females. Twelve patients were aged between 70 and 79 years. The predominant tumor type was lung cancer (10 cases). Thirteen patients received combination therapy with chemotherapeutic drugs. The median onset time of SJS/ TEN was 26 (4, 104) days. Nine patients developed SJS/TEN after the first administration of the drug. Sixteen patients exhibited prodromal rash symptoms, primarily characterized by severe skin damage such as skin detachment, accompanied by mucosal injury. Sixteen patients improved after symptomatic treatment, while one patient died. CONCLUSIONS Tislelizumab-associated SJS/TEN risk is higher in elderly patients, males, those with lung cancer and those receiving combination chemotherapy. Mucosal lesions and atypical rashes may indicate the early onset of SJS/TEN. During clinical use, pharmaceutical care can be carried out through measures such as identifying high-risk populations, closely monitoring skin symptoms from the first administration to the fifth treatment cycle, and enhancing patient education. When relevant symptoms occur, the medication should be promptly discontinued and symptomatic treatment should be administered to ensure the patient’s medication safety.
2.Design and writing practice of standardized training cases for professional master students based on organ systems

With reference to the teaching reform of standardized training cases based on organ systems, a teaching case writing team has been established, integrating the teaching experts in basic medicine, clinical medicine, humanities medicine, and other fields. Through collective lesson preparation and discussion, non-fictional comprehensive scenario writing techniques have been used based on organ systems and oriented by competency development, and the teaching cases for postgraduate students were written on the basis of real cases, which lays a solid foundation for the hierarchical and progressive cultivation and improvement of the quality of competency cultivation for postgraduate students through case teaching based on organ systems.
3.Application of hierarchical and progressive teaching in standardized residency training: difficulties and countermeasures

By identifying and analyzing the problems and difficulties in the implementation process of student stratification, goal stratification, practice stratification, and assessment stratification in standardized residency training, this article proposes to fully leverage the main role of professional bases in hierarchical and progressive teaching for standardized residency training and strengthen the specific responsibilities of professional base teaching groups in discussing hierarchical and progressive teaching plans, evaluation standards, and assessments. It is also necessary to scientifically implement student stratification, achieve consistent stratification goals, strengthen the training of hierarchical and progressive teaching concepts and teaching abilities for teachers, enhance the ability for homogeneous and hierarchical evaluation, and develop countermeasures that combine stage training goal process evaluation with dynamic personalized teaching by mentors, so as to provide a useful reference for further promoting the reform of standardized residency training using a hierarchical and progressive teaching model.
4.A case report of cerebral dural arteriovenous fistula with psychiatric and behavioral abnormalities and progressively worsening bilateral thalamic edema

The clinical and imaging manifestations of the dural arteriovenous fistula(DAVF)located in the great cerebral vein area with progressive bilateral thalamic edema lack specificity and are easily confused with a variety of diseases.The authors reported a case presented with psychiatric and behavioral abnormalities,followed by bilateral thalamic lesions.Initially,the diagnosis was considered as a glioma of the thalamus,encephalitis and straight sinus thrombosis,but the clinical and imaging findings progressively worsened.Through multi-modal imaging examinations,a DAVF in the great cerebral vein territory was finally diagnosed.By reporting the case and reviewing the literature,this paper aimed to explore the clinical manifestations and imaging features of the disease to reduce the incidence of misdiagnosis.
5.The application of fuorescein sodium for the microscopic resection of medulloblastoma

Objective To investigate the safety and efficacy of sodium fluorescein-guided microsurgery in children with medulloblastoma,and to analyze the surgical efficacy and prognosis.Among them,12 cases underwent unilateral telovelar approach,2 cases underwent bilateral telovelar approach,and 6 cases underwent telovelar approach combined with transvermian approach.The intraoperative dose of sodium fluorescein was 2 mg/kg.Methods The clinical data of 20 patients with medulloblastoma treated with fluorescein sodium assisted microsurgery from January 2018 to August 2023 in Xinjiang Autonomous Region People's Hospital were retrospectively analyzed.Results Of the 20 patients,12 were male and 8 were female.The mean age of onset was(7.9±3.7)years.In all cases,there was clear tumor fluorescence,none of the cases had adverse reaction associated with the use of sodium fluorescein.There were 16 cases of gross-total resection,3 cases of near-total resection,1 case of partial resection,1 case of intracranial infection,2 case of subcutaneous effusion,2 cases of cerebellar mutism.The follow-up time was from 3 to 72 months.5 cases did not receive sufficient radiotherapy and chemotherapy,and 9 cases died due to tumor progression or recurrence.In all cases,the longest overall survival was 72 months,the mean survival time was 39.2 months,and the median survival time was 41.2 months.Conclusion Fluorescein sodium assisted microsurgery is safe and effective in pediatric medulloblastoma surgery.
6.Relationship between social exclusion and relapse tendency in female drug rehabilitation patients

Objective:To explore the influence of social exclusion on the relapse tendency of female drug re-hab patients,and the mediating role of self-esteem and the moderating role of positive psychological capital.Method:A total of 288 female drug rehabilitation patients were selected,and the Social Exclusion Question-naire(SEQ)was used to assess the degree of social exclusion experienced by the patients.The Relapse Tendency Questionnaire(RPQ)was used to evaluate the current tendency of drug relapse.The Rosenberg Self-Esteem Scale(RSES)was used to evaluate the self-esteem level of the patients,and the Positive Psychological Capital Question-naire(PPCQ)was used to evaluate their level of positive psychological capital.Results:The SEQ scores of female drug rehab patients were positively correlated with the RPQ scores(β=0.41,P<0.001),The RSES scores had a mediating effect between the scores of SEQ and RPQ,with a mediation effect size of 0.09(95%CI:0.04-0.15,accounting for 20.6%of the total effect).The PPCQ scores had a moderating effect between the scores of RSES and RPQ(β=-0.08,P<0.05)and between the scores of SEQ and RPQ(β=0.10,P<0.05).Conclusion:So-cial exclusion could not only directly affect but also indirectly affect the relapse tendency through self-esteem of fe-male drug rehab patients.Positive psychological capital could moderate the relationship between social exclusion,self-esteem,and relapse tendency.
7.Analysis of factors related to false positives of treadmill exercise test in patients with hypertensive response to exercise

Objectives To explore factors associated with false positive treadmill exercise testing(TET)in patients with hypertensive responses to exercise.Methods Between November 2019 and October 2022,154 patients with positive TET and hypertensive response to exercise were selected from the de-partment of cardiac rehabilitation of Jiangsu Province Official Hospital,and divided into a false posi-tive group of 72 and a true positive group of 82.Then,demographic and TET-related parameters were compared between the two groups.Results There was a significant difference between the two groups in gender(P<0.05).Compared with the male,the female were more likely to get false-positive results.Moreover,the systolic blood pressure(SBP)at the time of ST segment depression in the false positive group was 203.87±8.70 mmHg,significantly different from the true positive group of 187.25±16.66 mmHg(P<0.05).According to logistic regression analysis,only the SBP at the time of ST seg-ment depression and gender were significantly correlated with TET false positives(P<0.05).The cut-off value of SBP at the time of ST segment depression was 188 mmHg,with a sensitivity of 1,a specific-ity of 0.625,and an area under the curve of 0.813.Conclusions In patients with hypertensive re-sponse to exercise,SBP at the time of ST segment depression can be considered as a predictor of false positive TET.When its value is no less than 188 mmHg,TET-positive conclusions should be drawn with caution,especially for female patients.
8.Quality standard for Zhujieshen Formula Granules based on standard decoction

AIM To establish the quality standard of the zhujieshen Formula Granules based on standard decoction.METHODS The contents and transfer rates of ginsenoside Ro and chikusetsu saponin Ⅳa in standard decoction and formula granules were determined by HPLC,after which the transfer rates were calculated.The HPLC characteristic chromatograms of standard decoctions were established,after which cluster analysis and principal component analysis were adopted.Then the HPLC characteristic chromatograms of formula granules were established.RESULTS Nine common peaks were found in the HPLC characteristic chromatograms of seventeen batches of standard decoctions with the similarities of more than 0.9(except for S6,S12),which were clustered into two categories,and the accumulative variance contribution rate of three principal components reached 86.7%.The contents of ginsenoside Ro in three batches of formula granules were 83.1-88.6 mg/g,and the transfer rates were 53.1%-55.5%.The contents of chikusetsusaponin Ⅳa were 14.8-15.0 mg/g,and the transfer rates were 47.4%-48.1%.Nine common peaks were found in the HPLC characteristic chromatograms of three batches of formula granules with the similarities of 0.998,0.998 and 0.999,respectively.CONCLUSION This reasonable and reliable method can comprehensively evaluate the quality of Zhujieshen Formula Granules,and provide a reference for the quality control.
9.A case report of severe acute kidney injury caused by near drowning and literature review

This article reports a case of acute kidney injury (AKI) following near drowning with reduced urine output as the main manifestation. The patient was a 37-year-old middle-aged man. His renal function deteriorated sharply after accidentally falling into the water, and renal pathology showed acute tubular injury. After hemodialysis treatment, urine output increased significantly, and renal function and proteinuria improved significantly. AKI following near drowning lacks typical clinical manifestations and is prone to delayed diagnosis and treatment. The patients with a history of near drowning should be followed up to determine whether they are complicated by AKI.
10.Study on medication law of traditional Chinese medicine compound for prevention and treatment of viral pneumonia based on national patents

Objective To analyze the medication rules of compound patents for the prevention and treatment of viral pneumonia through data mining technology,and to provide new ideas for clinical diagnosis and treatment and new drug research and development.Methods Summarizing the patents of traditional Chinese medicine compounds for the prevention and treatment of viral pneumonia in Patent Announcement module of China National Intellectual Property Administration website,search time is from database establishment to December 31,2023,and the statistics on the frequency of use,medicinal properties,efficacy classification,association analysis,and cluster analysis were carried out with the help of data mining software such as Excel 2019,IBM SPSS Statistics 26.0,and IBM SPSS Modeler 18.0.Results Through screening,a total of 218 compound patents met the inclusion criteria,involving 424 kinds of Chinese materia medica,and the top 5 herbs with the highest frequency of use were Glycyrrhizae Radix et Rhizoma,Lonicerae Japonicae Flos,Forsythiae Fructus,Scutellariae Radix,and Platycodon Radix.The medicinal properties are bitter and cold,and mostly belong to the lung meridian.There were 4 medicinal groups of pair medicine,13 medicinal groups of triple medicine,and 1 medicinal group with four related medicine exhibiting strong correlation.A total of 6 high-frequency combinations of traditional Chinese medicine were obtained by cluster analysis.Conclusion The basic pathogenesis of viral pneumonia is that internal deficiency and evil enter,heat toxin is trapped in the ling,the phlegm and blood stasis is occurred inside,and affecting the lung's function of purging and eliminating.In clinical practice,the deficiency tonic drugs Astragali radix and Glycyrrhizae Radix et Rhizoma,are used to strengthen resistance,the heat-clearing drugs Lonicerae Japonicae Flos,Forsythiae Fructus,the phlegm antitussive and antiasthmatic drugs,Platycodon Radix,Armeniacae Semen Amarum,etc,are used to treat the symptoms,simultaneous treatment of the symptoms and root cause,prevention and treatment are organically combined.
