Rectal cancer, which accounts for approximately 40% of colorectal cancers, remains a major clinical concern. Recent advances in diagnostic imaging, surgical techniques, radiotherapy, and systemic treatment have steadily improved rectal cancer outcomes. Considering this, the Korean Rectal Cancer Multidisciplinary (KRCM) Committee has aimed to provide clinicians and policymakers with up-to-date, evidence-based clinical practice guidelines to support optimal decision-making, reflecting current evidence, the Korean healthcare context, and patient values and preferences. The Clinical Practice Guidelines for Rectal Cancer version 2.0 were developed through multidisciplinary collaboration with related academic societies, building upon and updating the KRCM Clinical Practice Guidelines version 1.0 (titled “Multidisciplinary guidelines for the management of rectal cancer”). These consensus guidelines of the KRCM were established based on a comprehensive literature review, evidence synthesis, with recommendation development guided by the GRADE (Grading of Recommendations Assessment, Development and Evaluation) methodology, and consideration of applicability in real-world clinical practice under the national health insurance system. Each recommendation has been presented with its strength and level of evidence.

Background: Poor outcomes after acute ischemic stroke (AIS) revascularization are anticipated in patients with dementia due to advanced age, comorbidities, and reduced disease awareness. This study investigated clinical characteristics and short-term outcomes of AIS revascularization in patients undergoing dementia treatment. Methods: We retrospectively reviewed patients who underwent AIS revascularization therapy at eight centers of the Catholic University of Korea. Patients receiving anti-dementia medication for cognitive impairment before stroke onset were analyzed. Clinical characteristics and outcomes, including the modified Rankin Scale (mRS) score, hemorrhagic transformation, and mortality, were collected. Results: Among 1,379 patients who received AIS revascularization over 5 years, 13 (0.94%; mean age 83 years; 8 females) had a preexisting diagnosis of dementia (Alzheimer’s disease in 11, mild cognitive impairment in 2) and were on anti-dementia medication. Two patients underwent intravenous thrombolysis combined with endovascular therapy, whereas 11 underwent endovascular therapy alone. Most patients (11/13, 84.6%) presented with an unclear onset time. Stroke lesions were located in the right middle cerebral artery (MCA) in seven cases, left MCA in three, bilateral MCAs in one, and posterior circulation in two. Hemorrhagic transformation occurred in six patients (46%), of whom three (23%) were symptomatic. The mRS scores at discharge were 6 in two patients, 5 in one patient, 4 in nine patients, and 3 in one patient. Four patients died within 3 months of stroke onset. Conclusion The proportion of patients with cognitive impairment receiving anti-dementia medication among those undergoing stroke revascularization therapy was very small. The rates of hemorrhagic transformation and mortality were high. None of the patients achieved favorable outcomes at discharge.

Background: Adolescent unintended pregnancies can have detrimental effects on maternal and fetal health, impacting the social lives of young parents. Globally, there is an emphasis on preventing unintended pregnancies in adolescents through comprehensive contraceptive education. This study examined contraception practices and determinants among sexually active Korean adolescents. Methods: This study analyzed raw data from the 2018 and 2021 Korea Youth Risk Behavior Survey conducted by the Korea Disease Control and Prevention Agency. The cohort consisted of 6,042 adolescents who reported sexual experience, categorized into those always practicing contraception and those not. We scrutinized factors related to sexual behavior, such as contraception and sex education, as well as other social and behavioral factors. Results: Among sexually active adolescents, 47.9% of males and 48.1% of females consistently used contraception. The most prevalent contraceptive method was condoms (male 58.1%; female 59.6%). However, a significant portion of adolescents were not using contraception (male 29.4%; female 21.7%). Females who did not consistently use contraception exhibited higher pregnancy rates (2.4% vs. 11.3%). Consistent contraceptive users tended to live with their families and reported less sexual experience after drinking alcohol. Despite a decline in sexual education in schools from 2018 to 2021, the proportion of adolescents consistently using contraception increased. Conclusion To prevent unintended pregnancies among sexually active adolescents, consistent use of appropriate contraceptive methods is crucial.Comprehensive education on contraception should be extended beyond schools to include homes and primary healthcare settings. Policymakers and healthcare providers should consider these consequences when devising strategies to reduce unintended pregnancies among adolescents.

Purpose: The purpose of this study was to identify the effects of nursing work environment and social support on the reality shock of new graduate nurses in university hospitals. Methods: The subjects were 153 new graduate nurses with less than 12 months of clinical experience at two university hospitals in the G province, and data were collected from September 15 to October 7, 2020. Data were analyzed using the SPSS/WIN 26.0 program for frequency, average, t-test, ANOVA, Pearson correlation coefficient, multiple regression. Results: Factors influencing reality shock were nursing work environment (β=-.39, p<.001) and social support (β=-.25, p=.002), and gender (female) (β=.20, p=.001), and the explanatory power was 44.8% (F=9.99, p=.002). Conclusion Our study shows that nursing work environment and social support play an important role in the reality shock of new graduate nurses. It is important to strengthen nursing work environment and social support to alleviate the reality shock of new graduate nurses. The findings of this study may serve as a basis for developing strategies for mitigating the reality shock of new graduate nurses.

BACKGROUND/OBJECTIVES: Recently, herbal medicines have gained attention for the treatment of benign prostatic hyperplasia (BPH), a common disease in elderly men. In this study, we aimed to determine the effect of ethanol extract of Asparagi radix (EAR), which is traditionally used to treat various diseases, on BPH development using a testosteroneinduced BPH model.MATERIALS/METHODS: Testosterone propionate (TP)-treated Sprague–Dawley rats were used to establish a BPH model in vivo. EAR was orally administered along with TP, and finasteride was used as a positive control. All rats were sacrificed at the end of the experiment, and pathological changes in the prostate tissue and levels of key biomarkers associated with BPH pathogenesis were assessed. RESULTS: Oral administration of EAR significantly inhibited TP-induced BPH by reducing the prostate weight, lumen size, and epithelial thickness in a concentration-dependent manner. EAR also significantly abrogated the expression of 5α-reductase type 2 (SRD5A2), proliferating cell nuclear antigen, and prostate-specific antigen (PSA) induced by TP.Additionally, serum levels of testosterone, dihydrotestosterone, and PSA were elevated in the TP-induced group but decreased in the EAR-treated group. EAR also decreased the expression levels of the androgen receptor (AR) and its coactivators in TP-induced BPH model rats. CONCLUSION Our findings revealed that EAR protected against BPH by inhibiting 5α-reductase activity and AR signaling pathway, suggesting its potential for BPH treatment.

Hepatic dysregulation of lipid metabolism exacerbates inflammation and enhances the progression of metabolic dysfunction-associated steatotic liver disease (MASLD). STAT3 has been linked to lipid metabolism and inflammation. Jolkinolide B (JB), derived from Euphorbia fischeriana, is known for its pharmacological anti-inflammatory and anti-tumor properties. Therefore, this study investigated whether JB affects MASLD prevention by regulating STAT3 signaling. JB attenuated steatosis and inflammatory responses in palmitic acid (PA)-treated hepatocytes. Additionally, JB treatment reduced the mRNA expression of de-novo lipogenic genes, such as acetyl-CoA carboxylase and stearoyl-CoA desaturase 1. Interestingly, JB-mediated reduction in inflammation and lipogenesis was dependent on STAT3 signaling. JB consistently modulated mitochondrial dysfunction and the mRNA expression of inflammatory cytokines by inhibiting PA-induced JAK/STAT3 activation. This study suggests that JB is a potential therapeutic agent to prevent major stages of MASLD through inhibition of JAK/STAT3 signaling in hepatocytes.
