1.Cluster analysis of laryngomalacia in infants: insights into prognostic factors from a 10-year cohort

Purpose:
Laryngomalacia exhibits diverse morphological patterns, severities, and comorbidities. Defining clinical phenotypes could improve management and prognosis. This study aimed to identify and characterize phenotypes using cluster analysis and to evaluate prognostic factors.
Methods:
We retrospectively reviewed records of 195 children diagnosed with laryngomalacia between 2014 and 2023 using flexible laryngoscopy or bronchoscopy. Demographics, endoscopic findings, comorbidities, and outcomes up to 1 year of age were collected. Hierarchical cluster analysis was conducted using 10 clinical variables.
Results:
Four phenotypes emerged: cluster 1 (n= 75, 38.5%), Groningen Laryngomalacia Classification System (GLCS) type 1 dominant-mild; cluster 2 (n= 35, 17.9%), GLCS type 2 dominant-mild; cluster 3 (n= 40, 20.5%), severe with multiple comorbidities; and cluster 4 (n = 45, 23.1%), GLCS combined-type moderate. Distinct clinical courses were observed. Cluster 3 showed the highest rates of surgical intervention (32.5%, P < 0.001), pediatric intensive care unit admission (17.5%, P = 0.016), and Emergency Department (ED) visits (60.0%, P= 0.013) for respiratory problems during the first year. When stratified by comorbidities, children with multiple comorbidities, particularly those with major feeding problems had a higher risk of hospitalization (adjusted odds ratio [aOR], 2.65;95% confidence interval [CI], 1.11–6.33) and ED visits (aOR, 3.17; 95% CI, 1.39–7.23), even after adjusting for sex and severity.
Conclusion
Four clinically meaningful phenotypes of laryngomalacia were identified from the cluster analysis based on morphology, comorbidities, and disease severity. Children with multiple comorbidities accompanied by feeding problems had the greatest risk of hospitalization and ED visits for respiratory problems within the first year, even after adjusting for the severity of laryngomalacia.
2.Effects of Loneliness and Subjective Well-Being on Depression in Female Adolescents: A Longitudinal Moderated Mediation Model of the Parental Relationship

Objective:
Adolescent depressive symptoms are increasingly recognized as a global concern for youth mental health. Female adolescents consistently report higher levels of depressive symptoms than their male counterparts, yet the psychological mechanisms underlying depressive symptoms remain insufficiently understood. Our longitudinal study examined the relationship between loneliness and depressive symptoms in female adolescents and investigated the mediating effect of subjective well-being (SWB) and the moderating influence of the parental relationship.
Methods:
Data were drawn from the Korean Study of Adolescent Health, a longitudinal study on adolescent mental health. The sample included 104 female high school students (mean age=15.33 years) who completed self-report surveys at three time points between April 2022 and July 2023. Measures assessed loneliness, SWB, depressive symptoms, and relationships with mothers and fathers. Mediation and moderated mediation analyses were conducted using SPSS PROCESS (Models 4 and 14), controlling for age, socioeconomic status, and baseline depressive symptoms.
Results:
Mediation analyses indicated that SWB fully mediated the longitudinal association between loneliness and depressive symptoms. Loneliness predicted lower SWB, which subsequently predicted more depressive symptoms. Moderated mediation analysis revealed that maternal, but not paternal, relationship quality significantly moderated the SWB-depressive symptoms link. The indirect effect was significant only among adolescents reporting stronger maternal relationships.
Conclusion
These findings underscore the importance of SWB in the link between loneliness and depressive symptoms among female adolescents and highlight the protective role of a positive maternal relationship. The results have implications for targeted prevention and intervention strategies to enhance emotional well-being in female adolescents.
3.The effect of an internet of things-based mobile health management application for older adults depending on user engagement in South Korea: a secondary analysis of a quasi-experimental study

This study aimed to evaluate the effect of the TouchCare system, a digital health management system utilizing the internet of things (IoT), based on the usage levels of older adults. Methods: This is a secondary analysis of data from a quasi-experimental study examining the effects of an IoT-based digital healthcare system. Participants were equipped with the TouchCare application, a touch-tag, and context-aware artificial intelligence. Data on cognitive function, frailty, depressive symptoms, nutritional status, and fall efficacy were collected at baseline and after six months of using the system. The participants were divided into a high-engagement group (n = 22) and a low-engagement group (n = 24) based on how many days they used the application during the study. We used descriptive statistics, the paired t-test, the independent-samples t-test, and two-way mixed analysis of variance. Results: In total, 46 participants completed the evaluations (mean age, 76.6 years). Two-way mixed analysis of variance revealed no significant group-by-time interaction for cognitive function (p = .184), frailty (p = .338), depressive symptoms (p = .543), and nutritional status (p = .589). There was no significant difference in fall efficacy between the two groups (p = .091). The high-engagement group exhibited significant improvements in visuospatial and executive functions on the Montreal Cognitive Assessment (p = .029). Conclusion: The IoT-based mobile health management application demonstrated benefits in improving cognitive health among older adults. The findings suggest that active engagement with healthcare technology can positively affect health in this population, emphasizing the need for continuous support from nurses as health providers.
4.A review of the relationship between patient safety culture and safety activities: A systematic review focusing on the Korean version of the Hospital Survey on Patient Safety Culture 1.0

Purpose:
Forming a patient safety culture is the first step in addressing patient safety issues. This study therefore examines the patient safety culture among healthcare professionals in South Korea. It also identifies the factors influencing patient safety culture and related activities.
Methods:
Among the survey studies conducted in South Korea, we reviewed those that measured patient safety culture using the Korean version of the Hospital Survey on Patient Safety Culture 1.0. We identified the level of patient safety culture, the relationship between patient safety culture and patient safety activities, and the factors affecting patient safety culture.
Results:
A total of 640 studies were identified through searches of seven domestic and international databases, with 58 included in the analysis. Studies targeting nurses accounted for 96.6%. The general characteristics of patient safety culture included length of service, age, working hours, department of work, hospital size, conflict occurrence, unreported incidents, and subjective safety management evaluations. A statistically significant correlation between patient safety culture and patient safety activities was confirmed in 84.5% of studies. Patient safety culture was the most frequently reported factor influencing patient safety activities.
Conclusion
Strategies to enhance patient safety culture can promote patient safety activities in South Korean healthcare institutions. Considering organizational and organizational members characteristics when devising these strategies can enhance their effectiveness.

10.Community Pharmacists’ Awareness and Perceptions on the Role of Pharmacists in the Treatment and Management of Rare Disease Patients in Korea

Background:
Despite the increasing prevalence of rare diseases and growing medical expenses, community pharmacists’ role in rare disease management remains understudied in South Korea, even as the government emphasizes the importance of specialized healthcare professionals in rare disease management.
Methods:
An online survey was conducted among 75 community pharmacists in South Korea from May to June 2022. The survey assessed pharmacists’ awareness of rare diseases, current practices related to rare disease management, and perceived roles of community pharmacies in rare disease care. Data were analyzed using descriptive statistics.
Results:
While most pharmacists (98.67%) were familiar with the term “rare disease”, only 33.33% accu-rately understood its legal definition. One-third (33.33%) of respondents had experience with rare disease patients, with an aver-age of 7 patients per pharmacy. The main services provided were medication counseling (44.00%) and health supplement advice(28.00%). Pharmacists identified high accessibility (61.33%) as their primary advantage but cited lack of specialized knowledge(48.00%) and limited patient contact (56.00%) as major challenges. The majority (74.67%) expressed interest in rare diseases, with 40.00% suggesting specialized training as the most important factor for expanding pharmacists’ role.
Conclusions
Although community pharmacists show strong interest and potential in rare disease management, their current involvement is limited by in-sufficient specialized knowledge and systematic support. Implementing professional education programs and establishing special-ized pharmacy pilot projects could enhance pharmacists’ contribution to rare disease management, leveraging their accessibility and comprehensive healthcare capabilities.
