Heart failure (HF) is a major cause of mortality and morbidity in South Korea, imposing substantial physical, emotional, and financial burdens on patients and society. Despite the high burden of symptom and complex care needs of HF patients, palliative care and hospice services remain underutilized in South Korea due to cultural, institutional, and knowledge-related barriers. This position statement from the Korean Society of Heart Failure emphasizes the need for integrating palliative and hospice care into HF management to improve quality of life and support holistic care for patients and their families. By clarifying the role of palliative care in HF and proposing practical referral criteria, this position statement aims to bridge the gap between HF and palliative care services in South Korea, ultimately improving patient-centered outcomes and aligning treatment with the goals and values of HF patients.
2.Evaluation of the Regulatory Required Post-Authorization Safety Study for Propacetamol:Nested Case-Control and Case-Time-Control Studies

Purpose:
Following the withdrawal of propacetamol in Europe owing to safety issues, the regulatory authority of South Korea requested a post-marketing surveillance study to investigate its safety profile.
Materials and Methods:
We conducted nested case-control and case-time-control (CTC) analyses of cases and controls identified for outcomes of interest, including anaphylaxis, thrombosis, and Stevens–Johnson syndrome (SJS), using the claims database of South Korea, 2010–2019. Risk-set sampling was used to match each case with up to 10 controls for age, sex, cohort entry date, and follow-up duration. Exposure to anaphylaxis, thrombosis, and SJS was assessed within 7, 90, and 30 days of the index date, respectively. We calculated odds ratios (OR) with 95% confidence intervals (CIs) using conditional logistic regression to assess the risk of outcomes associated with propacetamol.
Results:
We identified cases of anaphylaxis (n=61), thrombosis (n=95), and SJS (n=1) and matched them to controls (173, 268, and 4, respectively). In the nested case-control analysis, the ORs for anaphylaxis and SJS were inestimable given the small number of propacetamol users during the risk period; meanwhile, the OR for thrombosis was 1.60 (95% CI 0.71–3.62). In the CTC design, the effect estimate was only estimated for thrombosis (OR 0.56, 95% CI 0.09–3.47).
Conclusion
In both nested case-control and CTC analyses, propacetamol was not associated with an increased risk of anaphylaxis, thrombosis, or SJS. The findings from this study, which used routinely collected clinical data, provide reassuring real-world evidence regarding the safety of propacetamol in a nationwide population to support regulatory decision-making.
3.Effects of low-frequency positive square wave voltage on putative periodontal pathogen Aggregatibactor actinomycetemcomitans

The extremely low frequency-electromagnetic field (ELF-EMF) refers to the frequency range of 0-300 Hz. It has been reported that it causes biological effects on cell survival, growth, and function. Structural changes were observed in the cell membranes of bacteria exposed to a certain intensity of ELF-EMF, indicating that exposure of bacteria to ELF-EMF can directly affect the cell membrane and affect the survival and growth of bacteria. The purpose of this study was to investigate the effect of the application of low frequency square wave positive voltages on Aggregatibactor actinomycetemcomitans (A. actinomycetemcomitans), a putative pathogen of periodontal disease. A square wave positive voltage output of 20 V or less at low frequency (0-300 Hz) was applied to A. actinomycetemcomitans in a range of 60 minutes. Changes in the population of bacteria were observed by absorbance measurement, colony forming unit (CFU/ml) evaluation, and high-resolution field-emission scanning electron microscopy (HR FE-SEM). The results show that the most effective offset and frequency in inhibiting bacterial growth are 0.7V and 7.83 Hz (Schumann resonance). As the applied time increased and the voltage increased, it was effective in inhibiting bacterial growth. These results led to the conclusion that bacterial growth can be inhibited even at low frequencies below 10 Hz, and it was experimentally proven that the frequency, voltage setting, and exposure time of ELF-EMF have a significant effect on reducing the growth of A. actinomycetemcomitans.
4.Effect of platelet-rich plasma in Achilles tendon allograft in rabbits

Background:
Achilles tendon is composed of dense connective tissue and is one of the largest tendons in the body. In veterinary medicine, acute ruptures are associated with impact injury or sharp trauma. Healing of the ruptured tendon is challenging because of poor blood and nerve supply as well as the residual cell population. Platelet-rich plasma (PRP) contains numerous bioactive agents and growth factors and has been utilized to promote healing in bone, soft tissue, and tendons.
Objective:
The purpose of this study was to evaluate the healing effect of PRP injected into the surrounding fascia of the Achilles tendon after allograft in rabbits.
Methods:
Donor rabbits (n = 8) were anesthetized and 16 lateral gastrocnemius tendons were fully transected bilaterally. Transected tendons were decellularized and stored at −80°C prior to allograft. The allograft was placed on the partially transected medial gastrocnemius tendon in the left hindlimb of 16 rabbits. The allograft PRP group (n = 8) had 0.3 mL of PRP administered in the tendon and the allograft control group (n = 8) did not receive any treatment. After 8 weeks, rabbits were euthanatized and allograft tendons were transected for macroscopic, biomechanical, and histological assessment.
Results:
The allograft PRP group exhibited superior macroscopic assessment scores, greater tensile strength, and a histologically enhanced healing process compared to those in the allograft control group.
Conclusions
Our results suggest administration of PRP on an allograft tendon has a positive effect on the healing process in a ruptured Achilles tendon.
5.Asymptomatic Uterine Rupture Caused by Trauma in a Small-Sized Non-Gravid Dog

Uterine rupture is uncommon in small animals and can be caused by trauma, infection, dead fetus, or inappropriate obstetric techniques. A 4-yearold, intact female mixed-breed dog weighing 3 kg presented for elective ovariohysterectomy with a history of a motor vehicle accident two months previously. The patient showed no clinical signs of uterine abnormalities or evidence of pregnancy during physical examination. Clinical examinations, radiography, and ultrasonography revealed no significant findings. During ovariohysterectomy, a completely transected left uterine horn and dome-shaped ends were identified.The uterine layers were everted such that the endometrium could be seen outside the uterine horn. The dog recovered without post-operative complications and was discharged the following day. This report describes a rare case of an asymptomatic uterine rupture caused by trauma in a non-gravid dog.
6.Surgical Resection of Cardiac Hemangiosarcoma Using Articulated Surgical Stapler in a Dog

This case report discusses the successful surgical resection of cardiac hemangiosarcoma in a dog using an articulated surgical stapler. Cardiac tumors, particularly hemangiosarcomas, have a poor prognosis. Recommended treatment involves surgical removal of the primary tumor, along with chemotherapy. However, the use of staples for cardiac tumor resection has not yet been extensively documented in the veterinary literature. A 10-year-old dachshund with pericardial effusion underwent surgery to remove a right atrial mass. An articulated linear cutting stapler was used for tumor resection. The patient experienced chylothorax as a complication but recovered well. However, the patient later developed respiratory symptoms and died, most likely due to aspiration pneumonia. The surgical stapler provided stability, convenience, and a shorter surgical time during tumor resection. Surgical resection combined with chemotherapy improves the survival of dogs with cardiac hemangiosarcoma. The stability and adjustability of the stapling device make it advantageous for cardiac tumor resection. Although complications, such as chylothorax, can arise, appropriate management can lead to positive outcomes. This case report demonstrates the feasibility and safety of using an articulated surgical stapler for cardiac tumor resection in dogs. Insights from this case can guide future research and clinical practice.
7.Histological and Structural Evaluation of Canine Pulmonary Valves Following Cryopreservation

The objective of this study was to establish a cryopreservation protocol for canine pulmonary valves and assess the preservation quality of the tissue for transplantation. Pulmonary valves were harvested from six beagle dogs, with portions analyzed after antibiotic treatment, while the remaining sections were cryopreserved for six months. Following the thawing process, the cryopreserved valves were evaluated using histological and cellular analyses. The results indicated no significant structural damage in cryopreserved valves when compared to fresh valves. The trilaminar structure, consisting of the fibrosa, spongiosa, and ventricularis layers, was well-preserved, with the extracellular matrix (ECM) largely intact. The fibrosa layer, rich in collagen, exhibited minor disorganization in cryopreserved samples, which was statistically significant. The spongiosa layer, which contains proteoglycans, showed good preservation of its loose and hydrated matrix. Similarly, the ventricularis layer retained its elastic fiber network with minimal alterations. Cell density analysis revealed a mild decrease in cellularity within the fibrosa layer of cryopreserved tissues, but the overall difference in cell count between fresh and cryopreserved tissues was not statistically significant. Cellular viability was maintained, confirming the effectiveness of the cryopreservation protocol in preserving tissue quality. These findings suggest that long-term cryopreservation of canine pulmonary valves could be used for transplantation. This study provides important data for developing tissue banks in veterinary medicine and supports the potential use of cryopreserved valves in canine heart valve transplantation.
8.The predictive value of procalcitonin for septic shock in sepsis patients

Objective:
This study examined the difference in procalcitonin between sepsis and septic shock.
Methods:
The single-center retrospective cohort study was conducted from July 2017 to June 2018 at an emergency department (ED) of a university hospital. The inclusion criteria were patients over 18 years old who visited the ED with an infection. The exclusion criteria were the patients without organ failure by sepsis-3 definition, those with missing serum lactate data, and those discharged without workup. The sepsis patients were divided into those with and without septic shock, and the two groups were compared with biomarkers, including procalcitonin.
Results:
Of the 406 patients who visited the ED with an infection, 36 were excluded because they did not have sepsis or an unknown infection. Finally, 369 patients were enrolled, and 61.5% fitted the septic shock definition. A comparison of the septic shock and non-shock sepsis groups showed that a history of chronic liver disease, malignancy, pulse rate, prothrombin time, blood urea nitrogen, aspartate and alanine transaminase, troponin-I, Sequential Organ Failure Assessment score and procalcitonin levels were significantly higher in the septic shock group. In multivariate analysis, however, procalcitonin was an independent predictor for septic shock (adjusted odd ratio, 1.05; 95% confidential interval, 1.01-1.09). The area under the receiver operating characteristic curve was 0.729, and the cutoff value was 4.0 ng/mL.
Conclusion
The procalcitonin levels were higher in the septic shock group than in the non-shock sepsis group. This could help predict septic shock independently. Further prospective multicenter research is needed to determine if procalcitonin can predict the severity of sepsis.
9.Open-heart surgery using Del-Nido cardioplegia in two dogs: partial atrioventricular septal defect and mitral repair

Del-Nido cardioplegia (DNc) is a single-dose cardioplegia that is widely used in human medicine because of its long duration. In this report, we describe two cases of open-heart surgery with cardiopulmonary bypass (CPB) using DNc. One dog was diagnosed with partial atrioventricular septal defect, and the other dog was diagnosed with myxomatous mitral valve disease stage D. Both dogs were treated with open-heart surgery with DNc to induce temporary cardiac arrest. No complications from DNc were observed, and the patients were discharged. Veterinary heart surgeons should consider DNc as an option for temporary cardiac arrest during open-heart surgery with CPB.
