1.Successful desensitization to contrast media in a patient with recurrent hypersensitivity to multiple iodinated contrast agents: A case report

Hypersensitivity reactions (HSRs) to iodinated contrast media (ICM) can range from mild cutaneous symptoms to life-threatening anaphylaxis. In patients with a history of ICM hypersensitivity, avoidance of the culprit agent is generally recommended. This case report describes a successful desensitization in a 56-year-old man with recurrent HSRs to multiple agents including ioversol, iohexol, iobitridol, and iopamidol. Intradermal testing was performed to identify potentially safe alternatives; however, all tested agents, including iohexol, ioversol, iobitridol, iopamidol, iodixanol, iomeprol, and iopromide, yielded positive results. Given the clinical necessity of transcatheter arterial chemoembolization, a 13-step rapid desensitization protocol with iodixanol was implemented. The procedure was completed without any breakthrough reactions. This case highlights desensitization as a feasible and effective strategy for patients with hypersensitivity to multiple ICM agents.
2.An adolescent case of post-ictal diffuse alveolar hemorrhage

Diffuse alveolar hemorrhage (DAH) is a rare but life-threatening condition characterized by bleeding into the alveolar spaces because of damage to the pulmonary microvasculature. While it is commonly associated with autoimmune or coagulation disorders, post-ictal DAH following a generalized tonic-clonic seizure is extremely rare, especially in pediatric populations. We report the case of a previously healthy 13-year-old boy with allergic rhinitis and buckwheat allergy who presented with his first generalized tonicclonic seizure. Following admission, he developed dyspnea and tachypnea (respiratory rate of 38/min); on hospital day 2, oxygen desaturation (SaO 2 88%) accompanied by hemoptysis. Laboratory tests showed no evidence of coagulopathy or autoimmune disease. Chest radiography and computed tomography revealed bilateral pulmonary infiltrates consistent with alveolar hemorrhage.Intravenous methylprednisolone (1 mg/kg for 3 days) was administered, resulting in rapid improvement of respiratory symptoms and imaging findings. This case highlights the importance of early clinical suspicion and imaging-based diagnosis of post-ictal DAH, and the necessity of prompt supportive management to achieve favorable outcomes.
3.Low-dose oral immunotherapy for severe persistent cow’s milk allergy in school-aged children and adolescents

Purpose:
This study aimed to evaluate the feasibility, safety, and immunological outcomes of low-dose cow’s milk oral immunotherapy (CM OIT) in school-aged children and adolescents with severe, persistent cow’s milk allergy (CMA), a group typically considered at a high risk for OIT.
Methods:
We conducted a retrospective study involving 12 patients (median age, 11.4 years) with immunoglobulin E (IgE)-mediated CMA, who underwent individualized low-dose CM OIT in the outpatient setting at a single tertiary hospital. Baseline and longitudinal clinical and immunological data, including casein-specific IgE and immunoglobulin G4 (IgG4) levels, were analyzed. Patients were classified as either desensitized or unsuccessful based on treatment outcomes.
Results:
Of the 12 patients with severe CMA, 6 achieved desensitization up to the target dose, while the remaining 6 discontinued treatment because of intolerance or poor adherence. The desensitized and unsuccessful groups showed no significant differences in baseline age, OIT starting dose, or serum casein-specific IgE level. Immunological evaluation revealed a significantly greater increase in serum casein-specific IgG4 (0.87 vs. -0.37 mg/L, P = 0.004) and a significantly lower natural log-transformed fold change in the IgE/IgG4 ratio (-1.38 vs. 0.44, P = 0.008) in the desensitized group. None of the patients required epinephrine during OIT, and adverse events were generally mild.
Conclusion
In selected high-risk pediatric populations with severe CMA, outpatient-based low-dose OIT using individualized flexible protocols may provide a relatively safe approach that improves tolerability and adherence. Increased casein-specific IgG4 levels and a reduced casein-specific IgE/IgG4 ratio may serve as valuable biomarkers for treatment response.
4.Clinical Characteristics and Outcomes of Life-sustaining Treatment Withdrawal in a Korean Neurocritical Care Unit: A Single-center Retrospective Study

Background:
The Act on Decisions on Life-Sustaining Treatment (LST) has been implemented in Korea since 2018, yet data on its application in neurocritical care units remain scarce. This study aimed to evaluate the clinical characteristics and outcomes of LST withdrawal or withholding in the neurocritical care unit.
Methods:
This study was a retrospective analysis conducted at a tertiary university hospital in Busan, South Korea. Among patients admitted to the neurocritical care unit between February 2018 and August 2023, those with documented decisions for LST withdrawal or withholding were enrolled. Demographic and clinical characteristics, underlying and combined conditions, reasons for LST decisions, measures taken, and time from LST withdrawal to death were extracted from medical records.
Results:
A total of 69 patients were included, with a median age of 67 years, and 38 (55%) were male. Cerebrovascular disease (62%) and traumatic brain injury (22%) were the most common underlying diagnoses. The primary reason for LST decisions was irreversible neurological damage (71%), followed by systemic complications (19%). Mechanical ventilation cessation (91%) and extubation (86%) were most frequently used measures for LST withdrawal. The median time from LST withdrawal to death was 22 minutes.
Conclusions
Our study demonstrates that LST decisions in the neurocritical care unit predominantly occur among patients with cerebrovascular disease or traumatic brain injury, mostly triggered by neurological deterioration. Most patients died shortly after withdrawal. These findings provide important insight into current LST withdrawal practices in neurocritical care and may assist clinical and ethical decision making in similar settings.
5.Clinical Outcomes Based on the Corneal Limbus–Scleral Tunnel Distance in Flanged Intrascleral Intraocular Lens Fixation

Purpose:
To compare the anatomical and clinical outcomes based on the distance from the corneal limbus to the scleral tunnel in flanged intrascleral intraocular lens (IOL) fixation.
Methods:
We retrospectively analyzed the medical records of patients who underwent scleral fixation of flanged IOLs. Group 1 (54 eyes) had a distance of 2.1 mm from the corneal limbus to the scleral tunnel, and Group 2 (48 eyes) had a distance of 2.8 mm. We evaluated the best corrected visual acuity (BCVA), postoperative complications, IOL tilt and decentration, refractive prediction error (RPE), effective lens position, and iris-IOL distance.
Results:
The BCVA, postoperative complications, IOL tilt, and IOL decentration did not differ between the two groups (p > 0.05). The RPE showed a hyperopic shift in Group 1 and a myopic shift in Group 2 (Group 1: +0.24 ± 0.68 D, Group 2: -0.03 ± 0.43 D, p = 0.03). The iris-IOL distance was statistically longer in Group 1 compared to Group 2 (Group 1: 1.02 ± 0.40 mm, Group 2: 0.57 ± 0.32 mm, p = 0.02). The incidence of pupillary optic capture was significantly higher in Group 2 compared to Group 1 (Group 1; 0%, Group 2; 8.3%, p = 0.03).
Conclusions
It should be considered that a shorter distance from the corneal limbus to the scleral tunnel results in a postoperative hyperopic shift and reduces the incidence of pupillary optic capture when performing flanged intrascleral IOLs fixation.
6.Current status of hepatitis C treatment and its barriers in Jeonbuk, Republic of Korea

Objectives:
In alignment with the World Health Organization’s goal of eliminating hepatitis C, this study assessed the current treatment status and reasons for non-treatment among patients with hepatitis C in Jeonbuk State, Republic of Korea, to inform strategies for improving care engagement.
Methods:
Among 311 individuals diagnosed with hepatitis C and reported through the NationalNotifiable Infectious Disease Surveillance system between January 2023 and June 2024, 208 patients were surveyed after excluding those who had died or could not be contacted.Statistical analyses included the chi-square test, the Cochran-Armitage test for trend, and logistic regression.
Results:
Overall, 116 participants (55.8%) reported having received antiviral therapy. Among the 92 untreated individuals, the most common reason for non-treatment was the absence of symptoms (n = 23; 25.0%), followed by the burden of drug costs (n = 21; 22.8%).
Conclusion
These findings highlight suboptimal treatment uptake and key barriers that may hinder progress toward hepatitis C elimination. Expanding screening and strengthening linkage-to-care strategies, while addressing financial barriers, will be essential to achievingnational elimination targets.
7.Secondary transmission of severe fever with thrombocytopenia syndrome in a hospital setting in Republic of Korea: a retrospective observational study of personal protective equipment use and infection risk factors

Objectives:
This study investigated a cluster of secondary human-to-human transmission of severe fever with thrombocytopenia syndrome (SFTS) in a hospital setting, focusing on infection risk factors and the role of personal protective equipment (PPE).
Methods:
A descriptive epidemiological investigation was conducted following the death of anindex patient with laboratory-confirmed SFTS. A total of 27 close contacts, including healthcareworkers and a funeral director, were monitored for symptoms. Suspected cases underwent real-time reverse transcription polymerase chain reaction testing. Clinical features, PPE use, and exposure histories were analyzed. The Fisher exact test was used to assess associations between PPE use and infection. Viral genotyping and sequence analyses were performed to evaluate transmission routes.
Results:
The index patient deteriorated rapidly and died after repeated cardiopulmonaryresuscitation (CPR), during which 8 secondary cases occurred. Most infections were identifiedamong individuals involved in CPR or postmortem care without adequate PPE. Although notstatistically significant, infection rates were higher among those who did not wear masks or who used low-filtration masks. Proper use of gloves, gowns, and goggles was associated withlower infection rates. Cycle threshold values in secondary cases (range, 34–39) were higherthan in the index case (14.07), suggesting lower viral loads. Sequence analysis demonstrated99.6%–100% homology between the index and secondary cases; all isolates were genotype B, indicating direct transmission.
Conclusion
This study provides molecular and epidemiological evidence of nosocomial SFTS transmission. Inadequate PPE use during aerosol-generating procedures likely facilitated infection, underscoring the importance of strict adherence to PPE protocols and reinforced infection control practices.
8.Crisis-driven innovation in the Republic of Korea's in vitro diagnostics industry: a pandemic case study

Objectives:
This study evaluated the effectiveness of government epidemic control policies centered on diagnostic testing and examined their impact on the in vitro diagnostics (IVD) industry. It also analyzed the complex interplay among policy interventions, epidemic dynamics, and the IVD industry’s value chain to identify key leverage points for managing future public health crises.
Methods:
A system dynamics (SD) model calibrated using national data from the Republic of Korea simulated the interactions between epidemic progression and the IVD value chain.We conducted a scenario analysis encompassing 6 policy interventions: research and development (R&D) investment, public–private collaboration, regulatory easing, diagnostic test performance, testing intensity, and social distancing.
Results:
Policies promoting investment, public–private collaboration, and regulatory easing accelerated the market entry of diagnostics, thereby reducing infections and deaths. However, these interventions were associated with lower overall industry revenue, attributable toincreased market competition and a reduced patient population. A critical trade-off was noted:although regulatory speed is advantageous, using low-sensitivity diagnostics substantially worsened public health outcomes. Aggressive testing strategies and stringent social distancingwere also confirmed to be effective in reducing both infections and mortality.
Conclusion
This study provides a strategic framework for understanding interactions between pandemic control policies and the IVD industry. Sustained pre-crisis investment in R&D,public–private networks, and public health infrastructure is essential for effective pandemicpreparedness. During a crisis, policymakers must carefully manage the critical trade-off between regulatory speed and diagnostic quality to ensure that rapid responses do not compromise public health outcomes.
9.A multi-city outbreak of Salmonella Enteritidis infections linked to bakery products, Republic of Korea

Objectives:
In May 2025, clusters of salmonellosis were identified in 7 cities in the Republic of Korea, all associated with consumption of identical bakery products. This investigation aimed to characterize the outbreak, identify potential contributing factors, and inform strategies for preventing similar multi-facility foodborne outbreaks.
Methods:
A case series study was conducted among individuals who consumed Manufacturer H’s Product I and Product II on May 15–16, 2025 at 7 facilities (n = 1,235). Clinical specimens from symptomatic individuals, retained food samples, and environmental samples were collected and tested. Food-exposure histories were assessed, and active case finding was implemented across all supplied facilities. Traceback investigations were conducted at the manufacturer, distributor, and egg farms. Human and food isolates underwent pulsed-field gel electrophoresis (PFGE) and whole-genome sequencing (WGS).
Results:
A total of 323 cases met the outbreak case definition (attack rate, 26.2%), of which 48 were laboratory-confirmed. Salmonella Enteritidis was isolated from both clinical specimens and retained bakery products. PFGE patterns were indistinguishable between human and food isolates, and WGS demonstrated high genetic relatedness. These findings confirmed a common-source outbreak linked to the implicated bakery products.
Conclusion
This outbreak underscores the value of integrating epidemiological investigation, active case finding, and molecular typing to identify common food vehicles in outbreaks involving widely distributed manufactured foods. Coordinated collaboration between public health and food safety authorities is essential for the effective detection, response, and prevention of multi-facility foodborne outbreaks.
10.Analysis of experiences on telemedicine pilot projects from the perspectives of doctors, pharmacists, and patients

Background/Aims:
This study is the first to analyze telemedicine pilot project experiences from doctors, pharmacists, and patients with different roles to support sustainable commercialization.
Methods:
An online survey targeted individuals (patients, doctors, and pharmacists) who participated in the telemedicine pilot project at least once between June 1, 2023, and July 17, 2024. The survey assessed satisfaction and usage conditions. The online survey conducted between May 2024 and July 2024 included 1,500 patients, 300 doctors, and 100 pharmacists.
Results:
Doctors, pharmacists, and patients all expressed their intention to participate actively in telemedicine in the future; however, pharmacists showed lower participation rates than doctors (84.7% vs. 67.0% vs. 91.7%, p < 0.001). The most common reason among doctors was “increasing demands from patients” (44.3%), while for pharmacists, it was “easy management of patients with chronic diseases” (67.0%). This showed a statistically significant difference between groups (p < 0.001). Among patients, 65.0% cited “lack of time and convenience.” Notably, both doctors and patients agreed that telemedicine requires more time than current practices, although their perceptions differed significantly (all p < 0.001). Additionally, 24.0% of patients who used telemedicine for "hair loss/beauty" purposes reported treatment times of “≤ 3 minutes” shorter than for other purposes. Regarding telemedicine platforms, 75.0% of doctors and 84.0% of pharmacists reported no prior experience using them.
Conclusions
Appropriate telemedicine systems require collaboration among doctors, pharmacists, and patients. While most groups expressed positive attitudes and future intentions, significant gaps in experience and understanding must be addressed to ensure successful implementation.
