1.Coffee and the Risk of Hepatocellular Carcinoma: A Systematic Review and Meta-Analysis of Mendelian Randomization Studies

This systematic review and meta-analysis examined the potential causal link between coffee consumption and hepatocellular carcinoma (HCC) risk via Mendelian randomization (MR) studies.Five eligible MR studies that involved the use of analytical approaches such as inverse variance weighted (IVW), MR-Egger, and weighted median methods were included. While previous observational studies suggested a protective role of coffee, the MR-based analyses in this study did not demonstrate a statistically significant association across all methods. IVW analysis yielded an odds ratio of 0.92 (95% confidence interval, 0.58 to 1.47), indicating no significant effect. Moderate to substantial heterogeneity was observed, but no publication bias was detected. These findings suggest that the previously reported inverse association may have been overestimated due to methodological limitations in observational research. Our results emphasize the importance of using genetically informed methods to infer causality, and the results indicate that coffee consumption may not causally reduce the risk of HCC.
2.Development and Evaluation of an Antimicrobial Stewardship Education Program for Physician Assistant Nurses: A One-Group Pretest-Posttest Design

Purpose:
This study aimed to develop and implement an antimicrobial stewardship education program for physician assistant nurses and to evaluate its effects on their knowledge and clinical performance.
Methods:
A quasi-experimental, single-group pre-post design was conducted with 50 physician assistant nurses at a university hospital in Seoul, Republic of Korea. The antimicrobial stewardship education program, developed using the ADDIE model, consisted of 12 sessions including lectures and case-based learning (CBL)-based discussions.Knowledge was measured before and immediately after the intervention, while performance was assessed pre-intervention and four weeks post-program. Data were analyzed using paired t-tests, Wilcoxon signed-rank tests, and analysis of covariance (ANCOVA).
Results:
Knowledge scores significantly improved from 44.65±7.45 to 58.50±10.11 (p<.001), and all subdomains showed significant increases (p<.001). Performance scores increased from 3.68±0.77 to 4.28±0.68 (p<.001). Knowledge gain did not differ significantly between the medical and surgical departments (p=.710). Likewise, after adjusting for pre-test scores, no significant difference in performance improvement was observed between the two departments (ANCOVA, p=.170). These results indicate that the program was effective across both departments regardless of their characteristics.
Conclusion
The antimicrobial stewardship education program improved both knowledge and performance among physician assistant nurses. This program may contribute to the standardization of antimicrobial stewardship education and to appropriate antimicrobial use and the reduction of antimicrobial resistance.
3.An adolescent case of post-ictal diffuse alveolar hemorrhage

Diffuse alveolar hemorrhage (DAH) is a rare but life-threatening condition characterized by bleeding into the alveolar spaces because of damage to the pulmonary microvasculature. While it is commonly associated with autoimmune or coagulation disorders, post-ictal DAH following a generalized tonic-clonic seizure is extremely rare, especially in pediatric populations. We report the case of a previously healthy 13-year-old boy with allergic rhinitis and buckwheat allergy who presented with his first generalized tonicclonic seizure. Following admission, he developed dyspnea and tachypnea (respiratory rate of 38/min); on hospital day 2, oxygen desaturation (SaO 2 88%) accompanied by hemoptysis. Laboratory tests showed no evidence of coagulopathy or autoimmune disease. Chest radiography and computed tomography revealed bilateral pulmonary infiltrates consistent with alveolar hemorrhage.Intravenous methylprednisolone (1 mg/kg for 3 days) was administered, resulting in rapid improvement of respiratory symptoms and imaging findings. This case highlights the importance of early clinical suspicion and imaging-based diagnosis of post-ictal DAH, and the necessity of prompt supportive management to achieve favorable outcomes.
4.Factors associated with Hypertensive Retinopathy among People with Hypertension: A Systematic Review

Purpose:
The associated factors for hypertensive retinopathy (HTR) are rarely investigated. This study aimed to identify the associated factors for HTR using a systematic review.
Methods:
The review included cross-sectional, case-controlled, and cohort studies on HTR risk factors published in Korean and English with full texts available from PubMed, Embase, CINAHL, Web of Science, and Korean databases. Methodological quality was assessed using the Joanna Briggs Institute (JBI) checklist.
Results:
Eleven studies were finally selected, and three studies including patients with hypertension without diabetes mellitus, older age, male sex, alcohol consumption, the duration of hypertension, hyperglycemia, dyslipidemia, microalbuminuria, high creatinine levels, chronic kidney disease, and cardiovascular changes were identified as factors associated with HTR. Conversely, in the remaining eight studies, younger age, non-smoking status, and renal function indicators (albuminuria, high creatinine levels, chronic kidney disease, and uric acid) were identified as associated factors.
Conclusions
Regardless of the inclusion of patients with diabetes mellitus, impaired kidney functions were determined as significant factors associated with retinopathy in patients with HTR. However, considering a limited number of evidence and lack of evidence to confirm causality, we recommend further research on renal function and HTR.

Background and Objectives:
Outcomes of deferring percutaneous coronary intervention (PCI) without invasive physiologic assessment for intermediate coronary lesions is uncertain.We sought to compare long-term outcomes between medical treatment and PCI of intermediate lesions without invasive physiologic assessment.
Methods:
A total of 899 patients with intermediate coronary lesions between 50% and 70% diameter-stenosis were randomized to the conservative group (n=449) or the aggressive group (n=450). For intermediate lesions, PCI was performed in the aggressive group, but was deferred in the conservative group. The primary endpoint was major adverse cardiac events (MACE, a composite of all-cause death, myocardial infarction [MI], or ischemia-driven any revascularization) at 3 years.
Results:
The number of treated lesions per patient was 0.8±0.9 in the conservative group and 1.7±0.9 in the aggressive group (p=0.001). At 3 years, the conservative group had a significantly higher incidence of MACE than the aggressive group (13.8% vs. 9.3%; hazard ratio [HR], 1.49; 95% confidence interval [CI], 1.00–2.21; p=0.049), mainly driven by revascularization of target intermediate lesion (6.5% vs. 1.1%; HR, 5.69; 95% CI, 2.20–14.73;p<0.001). Between 1 and 3 years after the index procedure, compared to the aggressive group, the conservative group had significantly higher incidence of cardiac death or MI (3.2% vs.0.7%; HR, 4.34; 95% CI, 1.24–15.22; p=0.022) and ischemia-driven any revascularization.
Conclusions
For intermediate lesions, medical therapy alone, guided only by angiography, was associated with a higher risk of MACE at 3 years compared with performing PCI, mainly due to increased revascularization.

Long coronavirus disease (COVID) is a condition in which coronavirus disease 2019 (COVID-19) symptoms persist for over 3 months, and currently poses a global public health challenge. Due to varying manifestations and lack of standardized definitions, diagnostic methods, and treatments, comprehensive clinical guidelines are required. This review article, summarizing research and expert consensus up to June 2023, provides recommendations for diagnosis and long-term management of long COVID symptoms. It emphasizes thorough patient evaluation, including medical history, physical examinations, and tests, and advocates vaccination and antiviral treatments to reduce risk. Guidelines for long COVID will be updated as new knowledge emerges.

10.Long-Term Outcomes in Patients With Non-Ampullary Duodenal Neuroendocrine Tumors

Objectives:
Although non-ampullary duodenal neuroendocrine tumors (NADNETs) are rare neoplasms, their incidence has been increasing. In this study, we aimed to analyze the long-term outcomes of patients with NADNETs who underwent endoscopic resection (ER) or surgery.
Methods:
This retrospective study included 55 adults (aged 26–78 years) diagnosed with NADNETs between 2009 and 2022 at Seoul National University Hospital. We categorized the patients into 3 groups: 21 underwent ER, 28 underwent surgical resection, and 6 had no detectable residual tumors after the initial biopsy during the follow-up period. Continuous data were examined using the Mann–Whitney U or Kruskal–Wallis tests, and categorical variables were analyzed using the χ2 or Fisher’s exact tests.
Results:
A total of 21 patients, with a mean tumor size of 9.2±5.3 mm, underwent successful ER. After ER, three patients experienced perforation (14% [3/21]); two underwent primary repair surgery, and one recovered with conservative treatment. The mean tumor size of 28 patients who underwent surgical resection was 13.9±6.7 mm. There were no cases of postoperative bleeding or perforation; however, four patients experienced ileus and required prolonged hospital stays. The median follow-up periods for patients who underwent ER, surgical resection, and removal after the initial biopsy were 42, 48, and 42.5 months, respectively. During the follow-up period, no recurrence was observed in any group.
Conclusions
Recurrence-free survival in patients undergoing ER for NADNETs, with an endoscopically measured size of approximately 10 mm, was comparable to that of patients undergoing surgical resection. However, ER carried a relatively high risk of perforation due to the challenging anatomical access and thin duodenal wall.
