1.Prevalence of HER2-ultralow breast cancer in South Korea: a multicenter study by reassessment of HER2-zero cases

This study aimed to determine the prevalence of human epidermal growth factor receptor 2 (HER2)–ultralow breast cancer among cases initially classified as HER2 immunohistochemistry (IHC) 0 and assess interobserver variability in interpreting low-level HER2 expression. Methods: In this multicenter retrospective study, all invasive breast cancer cases diagnosed between January and December 2022 across 10 Korean institutions were retrieved. Institutional pathologists reexamined HER2 IHC slides originally reported as IHC 0 according to the 2018 American Society of Clinical Oncology/College of American Pathologists guidelines and reclassified them as HER2-null (0), HER2-ultralow (0+), or HER2-low (1+). Slides from 10% of HER2-null and HER2-ultralow cases were digitized for central review and independently assessed by two pathologists, with discrepancies resolved by consensus. Results: Among 8,026 cases, 2,836 cases (35.5%) were initially reported as IHC 0. Upon re-review, 1,673 (59.0%), 1,139 (40.2%), and 24 (0.8%) cases were reclassified as HER2-null, HER2-ultralow, and HER2-low, respectively. The prevalence of HER2-ultralow breast cancer varied considerably across institutions (23.7%–78.1%). Central review of 268 digitized cases showed concordance in 193 cases (72.0%). Among the 75 discordant cases, 54 tumors (72.0%) were upgraded from HER2-null to HER2-ultralow, and 18 (24.0%) tumors were upgraded from HER2-ultralow to HER2-low. Furthermore, two tumors (2.7%) were downgraded from HER2-ultralow to HER2-null. Conclusions: Approximately 40% of cases initially categorized as IHC 0 were reclassified as HER2-ultralow. The substantial inter-institutional variability observed in interpreting low-level HER2 expression highlights the need for standardized training and quality assurance to ensure accurate identification of patients eligible for HER2-targeted antibody–drug conjugates.
2.High-protein diets for weight loss and their associations with bone status and diet quality in female college students

BACKGROUND/OBJECTIVES:
High-protein diets are increasingly used by young women for weight loss; however, concerns have been raised regarding their potential impact on bone health under insufficient calcium intake. This study investigated the associations between highprotein diet use for weight loss and bone status and diet quality among female college students.
SUBJECTS/METHODS:
In total, 260 female college students residing in Chungcheong, Korea, participated in this cross-sectional study. Data were collected using questionnaires, anthropometric measurements, 24-h dietary recall, and calcaneal ultrasound assessment.Participants were classified according to self-reported weight loss attempts during the past year into non-weight control (NWC; n = 108) and weight control (WC; n = 152) groups. The WC group was further subdivided into high-protein diet (HP-WC; n = 82) and non-high-protein diet (NHP-WC; n = 70) groups on the basis of high-protein diet practices.
RESULTS:
The mean body weight and body mass index (BMI) were significantly greater in the WC group than in the NWC group (P < 0.001 for both). Although calcium intake was significantly lower in the HP-WC group, protein intake exceeded the recommended level more frequently in this group (P < 0.05). A dietary diversity score less than 3 was more common in the HP-WC group (P < 0.05). Osteopenia prevalence was highest in the NWC group (64.8%), followed by the HP-WC group (50.0%) and the NHP-WC group (41.4%; P < 0.05). After adjustment for BMI and other confounding factors, the bone quality index and speed of sound values were significantly lower in the HP-WC group than in the NHP-WC group.
CONCLUSION
These findings suggest that high-protein weight-control practices may be associated with less favorable bone parameters among female college students during early adulthood, particularly in the context of lower calcium intake and reduced dietary diversity.
3.Preschool Teachers’ Perceptions, Teacher Efficacy, and Practices in Nutrition Education for Young Children According to Teachers’ Dietary Quality: A Focus on Chungnam and Sejong, Korea

This study examined preschool teachers’ attitudes, teacher efficacy, and practices related to nutrition education for young children according to their diet quality.Two hundred and thirty preschool teachers working in public and private early childhood education institutions in Chungnam and Sejong, Korea, participated in the study. The diet quality was assessed using the Nutrition Quotient for Adults (NQ-A) and classified into three groups based on NQ-A scores: high (n=60), medium (n=105), and low (n=65). The educational level showed a statistically significant difference among the NQ-A groups (P<0.05). All four subdomains of the NQ-A (balance, moderation, dietary behavior, and diversity) differed significantly among the three groups (all P<0.001). The teachers’ attitudes toward nutrition education, measured using an eight-item five-point Likert scale, were significantly more positive in the high NQ-A group than in the low NQ-A group (P<0.001). Nutrition education teacher efficacy, measured using a seven-item scale, was significantly lower in the low NQ-A group than in the other two groups (P<0.001). In addition, nutrition education practices, assessed using a six-item scale, were significantly more prevalent among teachers in the high NQ-A group (P<0.001). Teachers in the high NQ-A group were also more likely to have participated in nutrition training and to have conducted nutrition education. These findings highlight the importance of improving the dietary quality of preschool teachers to enhance their effectiveness in delivering nutrition education.
4.Impact of obesity on renal function in elderly Korean adults: a national population-based cohort study

Background:
Obesity is a well-known risk factor for chronic kidney disease and its progression. However, the impact of obesity on the renal function of the elderly population is uncertain. We investigated the association between obesity and renal outcomes in the elderly.
Methods:
We analyzed 130,504 participants from the Korean National Health Insurance Service-Senior cohort. Obesity was classified according to body mass index (BMI), sex-specific waist circumference (WC), and the presence of metabolic syndrome. The primary outcome was renal function decline, defined as a decline in the estimated glomerular filtration rate (eGFR) of at least 50% from baseline or new-onset end-stage renal disease.
Results:
During a follow-up period of 559,531.1 person-years (median, 4.3 years), 2,486 participants (19.0%; incidence rate of 4.44 per 1,000 person-years) showed renal function decline. A multivariate Cox proportional hazards model revealed that BMI/WC was not associated with renal function decline. However, the group with metabolic syndrome had a significantly increased risk of renal function decline compared to the group without metabolic syndrome (adjusted hazard ratio [HR], 1.24; 95% confidence interval [CI], 1.13–1.36). Compared with the non-metabolic syndrome group, the adjusted HRs (95% CI) for participants with one through five components were 0.96 (0.84–1.11), 1.10 (0.96–1.27), 1.24 (1.06–1.45), 1.37 (1.12–1.66), and 1.99 (1.42–2.79), respectively (p for trend < 0.001).
Conclusion
In elderly Korean adults, metabolic syndrome and the number of its components were associated with a higher risk of renal function decline, but BMI or WC was not significant.
5.Observer-Blind Randomized Control Trial for the Effectiveness of Intensive Case Management in Seoul: Clinical and Quality-of-Life Outcomes for Severe Mental Illness

Objective:
In South Korea, there is a significant gap in systematic, evidence-based research on intensive case management (ICM) for individuals with severe mental illness (SMI). This study aims to evaluate the effectiveness of ICM through a randomized controlled trial (RCT) comparing ICM with standard case management (non-ICM).
Methods:
An RCT was conducted to assess the effectiveness of Seoul-intensive case management (S-ICM) vs. non-ICM in individuals with SMI in Seoul. A total of 78 participants were randomly assigned to either the S-ICM group (n=41) or the control group (n=37). Various clinical assessments, including the Brief Psychiatric Rating Scale (BPRS), Montgomery–Åsberg Depression Rating Scale, Health of the Nation Outcome Scale, and Clinical Global Impression-Improvement (CGI-I), along with quality-of-life measures such as the WHO Disability Assessment Schedule, WHO Quality of Life scale, and Multidimensional Scale of Perceived Social Support (MSPSS) were evaluated over a 3-month period. Statistical analyses, including analysis of covariance and logistic regression, were used to determine the effectiveness of S-ICM.
Results:
The S-ICM group had significantly lower odds of self-harm or suicidal attempts compared to the control group (adjusted odds ratio [aOR]=0.30, 95% confidence interval [CI]: 0.21–1.38). Psychiatric symptoms measured by the BPRS and perceived social support measured by the MSPSS significantly improved in the S-ICM group. The S-ICM group also had significantly higher odds of CGI-I compared to the control group (aOR=8.20, 95% CI: 2.66–25.32).
Conclusion
This study provides inaugural evidence on the effectiveness of S-ICM services, supporting their standardization and potential nationwide expansion.
6.A Novel Histone Deacetylase 6 Inhibitor, 4-FHA, Improves Scopolamine-Induced Cognitive and Memory Impairment in Mice

Although histone deacetylase 6 (HDAC6) is considered a therapeutic target for Alzheimer’s disease (AD), its role in cholinergic dysfunction in AD patients remains unclear. This study investigated the effects of (E)-3-(2-(4-fluorostyryl)thiazol-4-yl)-N-hydroxypropanamide (4-FHA), a new synthetic HDAC6 inhibitor, on cognitive and memory impairments in a scopolamine-induced-AD mouse model. Behaviorally, 4-FHA improved scopolamine-induced memory impairments in the Y-maze, passive avoidance, and Morris water maze tests. In addition, 4-FHA ameliorated scopolamine-induced cognitive impairments in the novel object recognition and place recognition tests. Furthermore, 4-FHA increased acetylation of α-tubulin (a major HDAC6 substrate); the expression of BDNF; and the phosphorylation of ERK 1/2, CREB, and ChAT in the hippocampus of scopolamine-treated mice. In summary, according to our data 4-FHA, an HDAC6 inhibitor, improved the cognitive and memory deficits of the AD mouse model by normalizing BDNF signaling and synaptic transmission, suggesting that 4-FHA might be a potential therapeutic candidate for AD.

8.Comparison of tissue-based and plasma-based testing for EGFR mutation in non–small cell lung cancer patients

Background:
Epidermal growth factor receptor (EGFR) gene mutation testing is crucial for the administration of tyrosine kinase inhibitors to treat non–small cell lung cancer. In addition to traditional tissue-based tests, liquid biopsies using plasma are increasingly utilized, particularly for detecting T790M mutations. This study compared tissue- and plasma-based EGFR testing methods.
Methods:
A total of 248 patients were tested for EGFR mutations using tissue and plasma samples from 2018 to 2023 at Pusan National University Yangsan Hospital. Tissue tests were performed using PANAmutyper, and plasma tests were performed using the Cobas EGFR Mutation Test v2.
Results:
All 248 patients underwent tissue-based EGFR testing, and 245 (98.8%) showed positive results. Of the 408 plasma tests, 237 (58.1%) were positive. For the T790M mutation, tissue biopsies were performed 87 times in 69 patients, and 30 positive cases (38.6%) were detected. Plasma testing for the T790M mutation was conducted 333 times in 207 patients, yielding 62 positive results (18.6%). Of these, 57 (27.5%) were confirmed to have the mutation via plasma testing. Combined tissue and plasma tests for the T790M mutation were positive in nine patients (13.4%), while 17 (25.4%) were positive in tissue only and 12 (17.9%) in plasma only. This mutation was not detected in 28 patients (43.3%).
Conclusions
Although the tissue- and plasma-based tests showed a sensitivity of 37.3% and 32.8%, respectively, combined testing increased the detection rate to 56.7%. Thus, neither test demonstrated superiority, rather, they were complementary.
9.Who Dies Alone? Demographics, Underlying Diseases, and Healthcare Utilization Patterns of Lonely Death Individuals in Korea

Objectives:
Lonely death is defined as “a person living in a state of social isolation, disconnected from family, relatives, and others, who dies from suicide, illness, or other causes”. This study investigated the characteristics of individuals who die alone in Korea.
Methods:
We constructed a database of lonely death cases by linking data from the Korea Crime Scene Investigation Unit of the Korea National Police Agency with National Health Insurance Service (NHIS) records. A descriptive analysis was performed to evaluate the demographics, underlying diseases, and healthcare utilization patterns among lonely death cases.
Results:
Among the 3122 individuals identified as lonely death cases, 2621 (84.0%) were male and 501 (16.0%) were female. The most common age group was 50-59 years (n=930, 29.8%). The NHIS covered 2161 individuals (69.2%), whereas 961 individuals (30.8%) were enrolled in Medical Aid (MA). The highest number of lonely deaths occurred in Seoul areas, with 1468 cases (47.0%). Mood disorders were diagnosed in 1020 individuals (32.7%), and various alcohol-related diseases, including alcoholic liver disease, were also observed. Outpatient visits increased leading up to death but declined in the final 3 months, while hospitalizations decreased and emergency room visits slightly increased.
Conclusions
Most lonely death cases involved male in their 50s, with a disproportionately high number of MA beneficiaries compared to the general population. Many of these individuals also experienced mental health issues or alcohol-related disorders. Preventing social isolation and strengthening social safety nets are critical to reducing the occurrence of lonely deaths.
10.A Study on Eating Habits and Dietary Quality according to Smartphone Overdependence of University Students

This study investigated the association between overdependence on smartphones and the dietary quality of university students. A total of 586 university students (212 men and 374 women) residing in Chungbuk participated in this questionnaire survey in December 2019. The men and women were divided into two groups, ‘normal’ (170 men and 258 women) and ‘danger’ (42 men and 116 women) according to the level of smartphone dependence. Eating habits and dietary quality were assessed using a nutrition quotient for adults (NQ-A). For men, a lower intake of vegetable dishes was significantly associated with a higher dependence on smartphones. For men, a higher frequency of consumption of sweet and greasy baked products, night snacks, and checking of nutrition labels was significantly associated with higher smartphone dependence. In both men and women with higher smartphone dependence, the NQ-A score was significantly lower, suggestive of low dietary quality. In summary, overdependence on smartphones among university students may be associated with lower dietary quality. These study results suggest that nutrition education programs and guidelines should be provided to university students who exhibit an overdependence on smartphones.
