Background:
s/Aims: Hepatocellular carcinoma (HCC) is the sixth most common cancer and second leading cause of cancer-related deaths in South Korea. This study evaluated the characteristics of Korean patients newly diagnosed with HCC in 2016-2018.
Methods:
Data from the Korean Primary Liver Cancer Registry (KPLCR), a representative database of patients newly diagnosed with HCC in South Korea, were analyzed. This study investigated 4,462 patients with HCC registered in the KPLCR in 2016-2018.
Results:
The median patient age was 63 years (interquartile range, 55-72). 79.7% of patients were male. Hepatitis B infection was the most common underlying liver disease (54.5%). The Barcelona Clinic Liver Cancer (BCLC) staging system classified patients as follows: stage 0 (14.9%), A (28.8%), B (7.5%), C (39.0%), and D (9.8%). The median overall survival was 3.72 years (95% confidence interval, 3.47-4.14), with 1-, 3-, and 5-year overall survival rates of 71.3%, 54.1%, and 44.3%, respectively. In 2016-2018, there was a significant shift toward BCLC stage 0-A and Child-Turcotte-Pugh liver function class A (P<0.05), although survival rates did not differ by diagnosis year. In the treatment group (n=4,389), the most common initial treatments were transarterial therapy (31.7%), surgical resection (24.9%), best supportive care (18.9%), and local ablation therapy (10.5%).
Conclusions
Between 2016 and 2018, HCC tended to be diagnosed at earlier stages, with better liver function in later years. However, since approximately half of the patients remained diagnosed at an advanced stage, more rigorous and optimized HCC screening strategies should be implemented.

7.Job Experiences, Stakeholder Expectations, and Policy Challenges of Early-Career Social Workers in Hospice and Palliative Care: A Qualitative Study

Purpose:
This study aimed to explore the job experiences of early-career social workerswith <2 years of experience in hospice and palliative care, examine the expectations of related stakeholders, and propose policy recommendations for the advancement of this field.
Methods:
Two focus group interviews were conducted with 10 early-career social workers working in hospice and palliative care settings, and the findings were analyzed using a constant comparative method.
Results:
The analysis revealed that while early-career social workers experienced personal growth and a sense of fulfillment in their roles, they also faced various challenges as they adapted to the unique environment and intervention approaches of hospice and palliative care teams. Hospice social workers are expected to serve as communication bridges between patients, caregivers, and medical staff while fulfilling their core responsibilities. However, a considerable gap was observed between these expectations and the realities of their work. For the advancement of hospice and palliative care, self-reflection and identity formation by social workers are necessary for personal transformation, while policy-level measures such as human resource management, job systematization, and organizational and systemic reform are also required.
Conclusion
This study provides valuablefoundational data and clinical preparation guidelines for early-career social workers enter-ing the hospice and palliative care field. Furthermore, it offers evidence supporting the policy and institutional changes essential for the continued development of hospice and palliative care.
8.Real‑world data on long‑term outcomes in patients with T‑cell lymphomas: a nationwide study of Korea

Background:
Peripheral T-cell lymphomas (PTCLs) are a rare and heterogeneous group of aggressive malignancies.This study aimed to comprehensively analyze patients diagnosed with PTCLs in Korea, evaluating treatment outcomes, including transplantation and long-term survival.Patients and methods In this retrospective study, clinical data from the National Health Insurance Service on patients with PTCL were investigated. Most patients diagnosed with mature T-cell lymphomas and natural killer (NK)/T-cell lymphomas between January 2005 and December 2022 in Korea were included. Incidence rates of each subtype and survival outcomes of both treated and untreated patients were analyzed.
Results:
A total of 12,573 patients were analyzed. PTCL not otherwise specified (PTCL-NOS) and extranodal NK/T-cell lymphoma were the most frequently diagnosed, followed by angioimmunoblastic T-cell lymphoma (AITL). Compared to the general population, the relative survival rate was highest in anaplastic lymphoma kinase (ALK)-positive anaplastic large cell lymphoma. With a median follow-up of 6.7 years, the 3-year and 5-year progression-free survival (PFS) rates among treated patients were 44.0% and 39.5%, while the overall survival (OS) rates were 48.6% and 43.5%, respectively. Kaplan–Meier survival curves indicated that patients who added etoposide to the CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisone) regimen showed improved PFS and OS. In addition, autologous stem cell transplantation significantly improved PFS and OS, particularly in the PTCL-NOS and AITL subtypes.
Conclusion
Patients who received etoposide-containing CHOP-based regimens had improved treatment outcomes.The survival benefits of consolidative autologous stem cell transplantation (auto-SCT) were evident in PTCL-NOS and AITL.

Background:
Recent diabetes management guidelines recommend that sodium-glucose cotransporter 2 inhibitors (SGLT2is) or glucagon-like peptide 1 receptor agonists (GLP-1RAs) with proven cardiovascular benefits should be prioritized for combination therapy in patients with type 2 diabetes mellitus (T2DM) and established cardiovascular disease (CVD). This study was aimed at evaluating SGLT2i or GLP-1RA usage rates and various related factors in patients with T2DM and established CVD.
Methods:
We enrolled adults with T2DM aged ≥30 years who were hospitalized due to established CVD from January 2019 to May 2020 at 13 secondary and tertiary hospitals in Korea in this retrospective observational study.
Results:
Overall, 2,050 patients were eligible for analysis among 2,107 enrolled patients. The mean patient age, diabetes duration, and glycosylated hemoglobin level were 70.0 years, 12.0 years, and 7.5%, respectively. During the mean follow-up duration of 9.7 months, 25.7% of the patients were prescribed SGLT2is after CVD events. However, only 1.8% were prescribed GLP-1RAs. Compared with SGLT2i non-users, SGLT2i users were more frequently male and obese. Furthermore, they had a shorter diabetes duration but showed worse glycemic control and better renal function at the time of the event. GLP-1RA users had a longer duration of diabetes and worse glycemic control at the time of the event than GLP-1RA non-users.
Conclusion
The SGLT2i or GLP-1RA prescription rates were suboptimal in patients with T2DM and established CVD. Sex, body mass index, diabetes duration, glycemic control, and renal function were associated with the use of these agents.

In the 2023–2024 season, the influenza epidemic in South Korea peaked earlier than in recent years. In this study, we aimed to estimate the interim vaccine effectiveness (VE) of the influenza vaccination to prevent influenza during the early season. From November 1, 2023, to December 31, 2023, we enrolled 2,632 subjects with influenza-like illness from eight hospitals participating in hospital-based influenza morbidity and mortality surveillance. A retrospective test-negative case-control study was conducted to estimate the VE. The results showed an adjusted VE of 22.5% (95% confidence interval [CI], 6.6 to 35.8) for the total population. The adjusted VE was 22.3% (95% CI, 6.1 to 35.7) for influenza A and 9.4% (95% CI, −51.3 to 45.7) for influenza A/H1N1. Full results of the analysis will be reported.
