1.Posterior Cerebral Artery Territory Infarction after Bilateral Carotid Artery Stenting

Carotid artery stenting (CAS) is considered for patients with symptomatic carotid stenosis of 50% or greater. Following CAS, hemodynamic changes can alter the cerebral perfusion status. Furthermore, hypotension and bradycardia may occur due to the stimulation of carotid sinus baroreceptors. A more meticulous approach to these hemodynamic changes is required, especially in patients with bilateral carotid artery stenosis and concurrent multiple cerebrovascular stenoses. This report presents a case of posterior cerebral artery territory infarction that occurred after bilateral CAS.
2.Left Atrial Time-to-Peak From Time-Resolved MR Angiography as a Noninvasive Indicator of Left Atrial Pressure Before Catheter Ablation of Atrial Fibrillation

Objective:
To evaluate the performance of the left atrial time-to-peak (TTP) derived from time-resolved magnetic resonance angiography (TR-MRA) as a noninvasive indicator of left atrial pressure (LAP) in patients with atrial fibrillation (AF).
Materials and Methods:
This retrospective study included 92 patients who underwent cardiac TR-MRA and catheter-based LAP measurements prior to catheter ablation for AF between January 2021 and December 2022. TR-MRA–derived TTP was measured in the left atrium using contrast-enhanced time-signal intensity curves. Catheter-based LAP measured during sinus rhythm served as the reference standard, with high LAP defined as ≥15 mmHg. Univariable and multivariable linear regression analyses were performed, with LAP as the dependent variable and echocardiographic parameters as covariates. The diagnostic performance of TTP for high LAP was evaluated using receiver operating characteristic (ROC) curve analysis.
Results:
Among the 92 patients (mean age ± standard deviation, 60 ± 10 years; 70 male), 70 (76.0%) had low LAP, and 22 (23.9%) had high LAP. Mean TTP was significantly longer in patients with high LAP than in those with low LAP (25.1 ± 6.9 s vs.16.9 ± 5.7 s, P < 0.001). In multivariable linear regression analysis, TTP remained independently associated with LAP (β = 0.46, P < 0.001). ROC analysis demonstrated good discriminative performance of TTP for identifying high LAP, with an area under the curve of 0.834 (95% confidence interval, 0.742–0.904). Using a TTP cutoff of >19.6 seconds, the sensitivity and specificity for identifying elevated LAP levels were 81.8% (18/22) and 75.7% (53/70), respectively.
Conclusion
TR-MRA–derived TTP showed a significant independent association with invasively measured LAP. It may serve as a reliable noninvasive imaging marker for identifying high LAP in patients with AF undergoing catheter ablation.
3.Risk Factors of Complications in Patients With Persistent or Long-Standing Persistent Atrial Fibrillation/Atrial Flutter Who Underwent Electrical Cardioversion:A Multicenter Cardioversion Registry

Background:
Identifying the risks related to the complications of electrical cardioversion (ECV) can alert the determinaton of rhythm control in patients with atrial fibrillation (AF).
Methods:
We retrospectively reviewed 1,058 patients who underwent ECV for persistent or long-standing persistent AF/atrial flutter (AFL) from multiple centers. Patients were classified into the no-complication (1,023 patients) and complication (35 patients) groups based on the following major complications: stroke and/or systemic embolism (SSE), myocardial infarction, major bleeding, implantation of cardiac implantable electronic devices, ventricular tachycardia/fibrillation, and death at 1 year follow-up after ECV.
Results:
Compared with the no-complication group, the complication group exhibited a higher proportion of female patients (37% vs. 22%), as well as a higher proportion of patients with older age (67 ± 11 vs. 61 ± 10 years), diabetes mellitus (DM) (49% vs. 24%), heart failure (HF) (49% vs. 30%), SSE (23% vs. 9%), high CHA 2 DS 2 -VASc (CV) score (3.6 ± 1.8 vs.2.2 ± 1.4), low left ventricular ejection fraction (LVEF) (50 ± 16% vs. 58 ± 21%), and high left atrial volume index (LAVI) (51 ± 26 vs. 40 ± 20 mL/m 2 ). Class I and III antiarrhythmics were less prescribed in the complication group than in the no-complication group (57% vs.76%). Univariate analysis for complications revealed age (≥ 65 years), female sex, DM, HF, SSE, LVEF (< 50%), LAVI (≥ 40 mL/m 2 ), CV score (≥ 3), bradycardia on Holter (< 60/min), and no antiarrhythmics as risk factors. Among these, multivariate analysis revealed clinical significance of female sex and SSE.
Conclusion
Female sex and a history of SSE were the most important risk factors of complications in patients with persistent or long-standing persistent AF/AFL who underwent ECV.
4.The relationship between apical hypertrophic cardiomyopathy and sudden cardiac death: benign indeed?

Background and Objectives:
Until now, apical hypertrophic cardiomyopathy (HCM) has been considered to have a benign prognosis in terms of cardiovascular mortality. The prevalence of sudden cardiac arrest and the current status of implantable cardioverter-defibrillator (ICD) implantations in patients with apical HCM remain to be elucidated.
Methods:
Between July 2001 and May 2021, 96 HCM patients who had undergone ICD implantations from 3 tertiary hospitals were reviewed. We evaluated the prevalence of apical HCM and the indication for an ICD implantation.
Results:
Fifteen patients (15.6%) had apical HCM and 81 (84.4%) the non-apical type including septal (n = 56) and diffuse (n = 25) types, respectively. Sixty-four patients (66.7%) received ICD implantations for primary prevention and 32 (33.3%) for secondary prevention.Among those with apical HCM, 7 patients (46.7%) presented with an aborted sudden cardiac death or documented ventricular fibrillation, therefore, they received ICD implantations for secondary prevention. The proportion of secondary prevention ICD implantations did not significantly differ between apical and non-apical HCM subtypes (46.7% [7/15] vs. 30.9% [25/81], P = 0.233).
Conclusions
The proportion of secondary prevention ICD implantations did not differ significantly by HCM subtype. ICD implantations should be considered for high-risk patients with apical HCM.
5.Control of T-cell immunity by fatty acid metabolism

Fatty acids play critical roles in maintaining the cellular functions of T cells and regulating T-cell immunity. This review synthesizes current research on the influence of fatty acids on T-cell subsets, including CD8+ T cells, TH1, TH17, Treg (regulatory T cells), and TFH (T follicular helper) cells. Fatty acids impact T cells by modulating signaling pathways, inducing metabolic changes, altering cellular structures, and regulating gene expression epigenetically. These processes affect T-cell activation, differentiation, and function, with implications for diseases such as autoimmune disease and cancer. Based on these insights, fatty acid pathways can potentially be modulated by novel therapeutics, paving the way for novel treatment approaches for immune-mediated disorders and cancer immunotherapy.
6.Factors Affecting Life Satisfaction Among People With Physical Disabilities During COVID-19: Observational Evidence From a Korean Cohort Study

Objective:
To determine the factors influencing the life satisfaction of people with physical disabilities during the COVID-19 pandemic, considering demographics, disability-related characteristics, health behaviors, and psychosocial characteristics.
Methods:
We used cross-sectional data from 301 respondents of the 2021/2022 survey of the Korean Health Cohort Study for People with Physical Disabilities. Descriptive statistics were used to analyze the research subjects’ characteristics, and chi-square tests and multiple logistic regression were used to identify the determinants of life satisfaction.
Results:
Among socio demographic variables, occupation had a strong association with life satisfaction. Significant health behavior variables included daily regular meals, weight control effort, and chronic pain. All psychosocial characteristics (perceived stress, depression, suicidal ideation, cognitive function assessment, subjective health status, family satisfaction, income satisfaction) were strongly associated with life satisfaction. Results of the analysis of factors affecting life satisfaction showed that unemployment, lack of regular exercise, elevated stress, suicidal thoughts, and dissatisfaction with family contributed to increased life dissatisfaction.
Conclusion
Economic support through job creation that considers the specific characteristics of people with disabilities, and social access through community integration services that encourage participation in social activities, should be prioritized.
7.Extended trochanteric osteotomy for extraction of well-fixed fractured femoral stem after total hip replacement in a dog: a case report

A 5-year-old, 28 kg castrated male Border Collie was referred for surgical intervention following complications from a prior total hip replacement. A physical examination revealed reduced weight-bearing and metallic crepitus on the affected limb. A radiographic assessment confirmed a fractured femoral stem. Extended trochanteric osteotomy (ETO) was performed to extract the well-fixed stem, followed by the removal of the acetabular cup. ETO stabilization was achieved with cerclage wires and a locking plate. Six months postoperatively, the radiographs indicated successful bone healing, and the dog exhibited improved limb function. The ETO technique can provide enhanced surgical access to extract well-fixed implants.
