Despite widespread coronavirus disease 2019 (COVID-19) vaccine use, research on the association between vaccines and cerebrovascular venous sinus thrombosis (CVST) in diverse populations is limited. This study aimed to address this gap. Data from the World Health Organization pharmacovigilance database (1968–2024; total reports = 8,909,484) were used.Reporting odds ratios (RORs) and information components (ICs) were calculated to assess the association between each drug and CVST. In total, 851 cases were identified as vaccineassociated CVST, of which 527 (61.93%) occurred in female patients. Only Ad5-vectored COVID-19 vaccines had the highest ROR and IC value with CVST (ROR, 4.78; 95% confidence interval, 4.34–5.28; IC, 2.15). The risk of CVST increased with age, with the 45–64-years age group having an IC of 1.35, while the 65 years and older group had a higher IC of 2.08.The findings highlight the need for clinicians to recognize the potential risks of CVST and prioritize rigorous monitoring and research to ensure patient safety.

The influence of smoking habits, including the use of conventional cigarettes (CCs) and electronic cigarettes (ECs), on the prevalence and management of AR remains a subject of debate. This study aims to explore the association between smoking status (CC and EC use) and the prevalence and management of AR among Koreans by analyzing data from the Korea National Health and Nutrition Examination Survey (KNHANES) VII (2018) and VIII (2019–2021).
Methods:
. This cross-sectional study involved 22,290 participants aged 19 years and older from the KNHANES. Participants self-reported their smoking status, and urinary cotinine levels were measured to assess nicotine exposure. We employed statistical analyses, including logistic regression, to examine the relationships between smoking status, cotinine levels, and the prevalence and management of AR.
Results:
. In univariable logistic regression analysis, EC users exhibited a 35.8% increased risk of AR compared to non-smokers, whereas CC users experienced a 27.7% reduced risk. Multivariable logistic regression analysis showed a 20.3% lower risk of AR among CC users; however, no significant association was observed for EC users. Higher cotinine levels (>500 ng/mL) were associated with a lower prevalence of AR. Specifically, heavy CC users with high cotinine levels demonstrated a 35% reduced risk of AR. Nonetheless, after adjusting for confounders, this association was no longer significant, indicating that other variables might influence this relationship.
Conclusion
. Smoking status is associated with the prevalence of AR in Koreans. Notably, heavy use of CCs is negatively correlated with the prevalence of AR.

9.Contingency Plan for Anti-D Reagent Shortages for RhD Testing:Validation of Using Diluted Anti-D Reagents

Background:
In early 2024, there was an issue with the supply of the anti-D reagent for blood typing. This reduced the anti-D reagent in our laboratory below the minimum stock level. We validated the appropriateness of using diluted anti-D reagents as a contingency plan in the event of an anti-D reagent shortage.
Methods:
A total of eight dilutions, ranging from 2X to 256X, were prepared by serial dilution of the low-protein monoclonal anti-D reagent. The original anti-D reagent and the eight anti-D dilutions were used to perform RhD typing by the tube and plate methods. To further evaluate the reactivity and stability of the 8-fold diluted anti-D reagent, RhD typing was performed on internal quality control red blood cells and RhD-positive patient specimens for 30 days.
Results:
The maximum dilution that gave the same results as the original anti-D reagent in both the tube and plate methods was 8X. The 8X anti-D dilution was tested against internal quality control red blood cells and patient specimens. It showed the same result as the original anti-D reagent, with reactivity remaining constant over 30 days.
Conclusion
We have confirmed the appropriateness of using a diluted low-protein monoclonal anti-D reagent for RhD typing. Therefore, we suggest that the diluted anti-D method can be considered for priority use in emergencies when the anti-D reagent is in short supply. Although 8X is suggested as an appropriate dilution factor in this study, this may vary depending on the type of product used in each laboratory and the laboratory conditions.
10.Clinical Significance of Human Papillomavirus DNA Test and p16 Overexpression in Oropharyngeal Cancer

Background and Objectives:
Oropharyngeal squamous cell carcinoma (OPSCC) can be caused by human papilloma virus (HPV) infection or other factors like smoking. The 8th Edition of the AJCC Cancer Staging Manual recommends different staging and treatment approaches based on etiology. Despite criticisms of its low specificity, the current guidelines suggest using p16 immunohistochemistry (IHC) as a surrogate marker for the HPV-related OPSCC. This study assessed the reliability of p16 as a surrogate marker by correlating the survival rates of OPSCC patients with the results of p16 IHC and HPV-DNA testing.Subjects and Method A retrospective analysis was performed on patients treated for tonsil squamous cell carcinoma at a tertiary medical institution between 1994 and 2018. All patients underwent p16 immunostaining and HPV-DNA chip tests. Out of 88 patients, 17 were excluded due to insufficient data or secondary primary cancer, leaving 71 patients.
Results:
Among the 71 patients, 51 were p16 positive and 49 were HPV-DNA positive; both tests were associated with extended survival. However, discrepancies were noted in 18 patients: specifically, 11 patients were p16 positive but HPV-DNA negative, displaying a different survival pattern compared to HPV-associated and non-HPV-associated patients.
Conclusion
Both p16 immunostaining and HPV-DNA testing have their pros and cons. p16 immunostaining is cost-effective but has lower specificity. The study found discrepancies in 18 patients, suggesting that relying solely on p16 immunostaining may have limitations. It would be advisable to complement it with additional tests like the HPV-DNA chip test to predict the disease’s prognosis more accurately.
