1.Diamond particle acoustic patch-mediated laser-induced shockwave for refractory facial fat graft overcorrection:a case report

The late complications of autologous fat grafting, including overcorrection and asymmetry, are difficult to treat non-invasively. This paper reports a novel approach using diamond particle acoustic patch-mediated laserinduced shockwave (LISW) therapy.A 38-year-old female with a 10-year history of bilateral malar overcorrection and asymmetry following an autologous fat graft was treated with a diamond particle acoustic patch (Belluti; Simple-Stick®) coupled with 1,064 nm Q-switched neodymium-doped yttrium aluminum garnet laser. The procedure successfully softened the firm tissue and reduced volume asymmetry without thermal epidermal injury. Transient wheals and petechiae resolved within five to seven days. Visible volume reduction began at two weeks, with sustained improvement and lifting effects observed at the two-month follow-up.Diamond particle acoustic patch-mediated LISW therapy provides a safe, non-thermal, purely mechanical approach to reduce the subcutaneous fat volume and remodel deep fibrosis, offering a promising potential noninvasive option for refractory fat graft overcorrection.
2.Immunophenotypic and Transcriptomic Analysis of Peripheral Blood Mononuclear Cells in Bullous Pemphigoid

Background:
Bullous pemphigoid (BP) is an autoimmune blistering disease driven by autoantibodies against BP180 and BP230. While type 2 inflammation plays a key role, the precise immune cell alterations and transcriptomic changes remain unclear.
Objective:
To characterize immune cell composition and transcriptomic changes in BP patients using fluorescence-activated cell sorting (FACS)-based immunophenotyping and RNA sequencing.
Methods:
A case-control study was conducted on 10 newly diagnosed, treatment-naive BP patients and six healthy controls. Disease activity was assessed using the Bullous Pemphigoid Disease Area Index (BPDAI). Peripheral blood mononuclear cells were isolated for FACS analysis to determine immune cell subsets. RNA sequencing was performed to identify differentially expressed genes (DEGs) and enriched pathways. Statistical analyses included t-tests, Mann-Whitney U tests, and correlation analysis.
Results:
FACS analysis revealed a reduction in CD4 + T cells, T helper 2 (Th2), and B cells in BP patients (p<0.05), alongside an increase in M2a-like monocytes (p<0.001). RNA sequencing identified 262 DEGs, with secretory leukocyte peptidase inhibitor (SLPI) and transmembrane protein 237 being the most significantly upregulated. Proline-serine-threonine phosphatase interacting protein 2 (PSTPIP2) and SAM domain, SH3 domain, and nuclear localization signals 1 (SAMSN1) positively correlated with BPDAI (p<0.001). Gene ontology analysis highlighted enrichment in inflammatory responses and neutrophil degranulation pathways.
Conclusion
BP patients show distinct immune dysregulation, including decreased CD4 + T cells, Th2, and B cells, increased M2a-like monocytes, altered gene expression profiles, and correlations between PSTPIP2, SAMSN1 and disease activity. These findings provide insights into pathogenesis and potential therapeutic targets of BP.

Cavernous lymphangiomas, characterized by the cystic enlargement of lymphatic vessels in the dermis and subcutaneous tissue, are rarely reported in the scalp. We present the case of a 6-month-old male infant with occipital scalp swelling, whose ultrasonographic examination identified a multiseptated hypoechoic mass in the subcutaneous layer. Histologically, the tumor exhibited irregular, dilated spaces lined with a single layer of endothelial cells, showing D2-40 positivity. This represents a rare case of pediatric cavernous lymphangioma of the scalp, accompanied by a literature review.

Gastric cancer is one of the most common cancers in both Korea and worldwide. Since 2004, the Korean Practice Guidelines for Gastric Cancer have been regularly updated, with the 4th edition published in 2022. The 4th edition was the result of a collaborative work by an interdisciplinary team, including experts in gastric surgery, gastroenterology, endoscopy, medical oncology, abdominal radiology, pathology, nuclear medicine, radiation oncology, and guideline development methodology. The current guideline is the 5th version, an updated version of the 4th edition. In this guideline, 6 key questions (KQs) were updated or proposed after a collaborative review by the working group, and 7 statements were developed, or revised, or discussed based on a systematic review using the MEDLINE, Embase, Cochrane Library, and KoreaMed database. Over the past 2 years, there have been significant changes in systemic treatment, leading to major updates and revisions focused on this area.Additionally, minor modifications have been made in other sections, incorporating recent research findings. The level of evidence and grading of recommendations were categorized according to the Grading of Recommendations, Assessment, Development and Evaluation system. Key factors for recommendation included the level of evidence, benefit, harm, and clinical applicability. The working group reviewed and discussed the recommendations to reach a consensus. The structure of this guideline remains similar to the 2022 version.Earlier sections cover general considerations, such as screening, diagnosis, and staging of endoscopy, pathology, radiology, and nuclear medicine. In the latter sections, statements are provided for each KQ based on clinical evidence, with flowcharts supporting these statements through meta-analysis and references. This multidisciplinary, evidence-based gastric cancer guideline aims to support clinicians in providing optimal care for gastric cancer patients.

10.Characteristics of Patients Administratively Admitted to a Single Psychiatric Hospital in 2019–2024 and Their Association With Length of Stay

Objectives:
Administrative hospitalization under Article 44 of the Mental Health Welfare Act serves as an important social response mechanism for mental health issues, but systematic research on the characteristics of administrative hospitalization patients remains limited. This study aims to analyze the demographic and clinical characteristics of patients administratively hospitalized at a single psychiatric hospital to provide foundational data for future mental health policy development.
Methods:
We conducted a retrospective cross-sectional study of 194 patients who were administratively hospitalized at Yangsan Hospital between January 2019 and December 2024. Eligibility was defined as hospitalization via administrative admission during the study period with medical records available for review; for multiple admissions, only the first admission record was included. We performed descriptive analyses of age, sex, insurance type, primary diagnosis, and additionally evaluated the associations between these variables and length of hospital stay. All analyses were conducted using IBM SPSS Statistics 26.0.
Results:
Among study subjects, 105 were male (54.1%) and 89 were female (45.9%), with a mean age of 50.0 years.Among all age groups, the 50-59 group was the largest, with 59 patients (30.4%). Among the insurance types, medical aid type 1 was the most common, with 92 patients (47.4%). Diagnostically, F2 (schizophrenia, schizotypal and delusional disorders) accounted for 74 patients (40.0%) and F1 (psychoactive substance use disorders) for 54 patients (29.2%).Mean length of stay was 79.4 days (range 9-364 days), with no statistically significant differences according to sex, insurance type, diagnosis, or age group.
Conclusions
Administrative hospitalization patients were characterized by a predominance of males in their 50s, medical aid recipients, and schizophrenia patients. Healthcare providers should develop individualized treatment plans and discharge strategies considering patients’ demographic and socioeconomic characteristics.
