1.Comparative Analysis of Tissue Injury Markers After Mini- Versus Conventional Open Deformity Corrections in Patients With Adolescent Idiopathic Scoliosis

Objective:
This study compared the impact of minimally invasive surgery (MIS) and open spinal deformity corrections on the paraspinal musculature and soft tissues in adolescent idiopathic scoliosis (AIS) patients by analyzing early postoperative serum tissue injury markers and later radiographic evidence of muscle atrophy within the paraspinal musculature until 6-month and 2-year postoperative follow-ups.
Methods:
Prospective data were collected at a single tertiary care center from January 1, 2015 to November 30, 2020. Demographic, clinical, laboratory, and radiographic data, including postoperative magnetic resonance imaging, were collected at various intervals.
Results:
Forty-four patients met the inclusion criteria. The mean estimated blood loss and mean operative time differed significantly between the groups. On postoperative day 1, the Open group had significantly higher serum aldolase concentrations (18.2±7.6 mU/mL vs. 12.9±4.2 mU/mL) and creatine kinase (CK) values (3,003.1±60.1 IU/L vs. 1,649.4±40.6 IU/L) than the MIS patients. In the Open group, serum aldolase and CK levels remained higher through postoperative day 4. The normalized difference in the loss of paraspinal muscle mass was higher in the Open group than in the MIS group. Patient-reported outcomes improved in both groups, but there were no significant between-group differences. Both groups reported similar complication rates.
Conclusion
In patients with AIS, MIS was associated with lower tissue injury markers and muscle atrophy than open surgical correction in the early postoperative period. While this difference may be associated with decreased blood loss and shorter hospital stays seen in MIS, it did not result in a significant difference in clinical outcomes versus open surgery.
2.Severe Preoperative Disability Is Associated With Greater Mental Health Improvements Following Surgery for Degenerative Spondylolisthesis: A Cohort Matched Analysis

Objective:
To evaluate preoperative disability’s influence on patient-reported outcomes (PROs) following surgery for degenerative spondylolisthesis (DS).
Methods:
DS patients who underwent surgical intervention were retrospectively identified from a single-surgeon spine registry. Cohorts based on Oswestry Disability Index (ODI) < 41 (milder disability) and ≥ 41 (severe disability) were created. Demographic differences were accounted for with 1:1 propensity score matching. For the matched sample, perioperative and PRO data were additionally collected. PROs assessed included mental health, physical function, pain, and disability. Pre- and up to 2-year postoperative PROs were utilized. Average time to final follow-up was 15.7 ± 8.8 months. Improvements in PROs and minimal clinically important difference (MCID) rates were calculated. Continuous variables were compared through Student t-test and categorical variables were compared through chi-square tests.
Results:
Altogether, 214 patients were included with 77 in the milder disability group. The severe disability group had worse postoperative day (POD) 1 pain scores and longer hospital stays (p ≤ 0.038, both). The severe disability group reported worse outcomes pre- and postoperatively (p < 0.011, all), but had greater average improvement in 12-item Short Form health survey mental composite score (SF-12 MCS), 9-Item Patient Health Questionnaire (PHQ-9), visual analogue scale (VAS)-back, and ODI by 6 weeks (p ≤ 0.037, all) and PHQ-9, VAS-back and ODI by final follow-up (p ≤ 0.015, all). The severe disability cohort was more likely to achieve MCID for SF-12 MCS, PHQ-9, and ODI (p ≤ 0.003, all).
Conclusion
Patients with greater baseline disability report higher POD 1 pain and discharge later than patients with milder disability. While these patients report inferior physical/mental health before and after surgery, they report greater improvements in mental health and disability postoperatively.

Retrospective data from a single-surgeon database created 2 cohorts: patients with VR-12 MCS ≥ 50 or VR-12 MCS < 50. Atlanto-occipital assimilation is an osseous embryological anomaly of the craniovertebral junction in which the atlas (C1) is fused to the occiput of skull. Embryologically, this assimilation may happen due to failure of the segmentation and separation of the caudal occipital and the cranial cervical sclerotome. The segmentation clock is maintained by NOTCH and WNT signalling pathways along with Hox genes and retinoic acid. This condition is likely to be a consequence of mutation in above mentioned genes. The knowledge of this assimilation may be crucial for the clinicians as it may lead to various neurovascular symptoms. The present case report involves the analysis of atlanto-occipital assimilation with its clinical significance and embryological basis.
