1.Expert consensus on oral corticosteroid use and tapering in severe asthma management

Systemic corticosteroids play an essential role in the management of asthma. During acute exacerbation, the short-term use of systemic corticosteroids is recommended. For patients with uncontrolled asthma and severe asthma, long-term and low-dose oral corticosteroids (OCS) have frequently been advocated. However, both short-term and long-term use of systemic corticosteroids carry the risk of adverse events (AEs), including various morbidities and even mortality. Despite recent progress in adult severe asthma management and the availability of new treatment options, the current domestic guidelines for asthma do not provide specific recommendations for oral corticosteroid tapering in patients with severe asthma. Therefore, the task force team of the severe asthma working group in the Korean Academy of Allergy, Asthma, and Clinical Immunology has proposed a tapering protocol for systemic corticosteroid use in severe asthma. This includes practical recommendations for monitoring OCS-related AE, particularly for adrenal insufficiency and osteoporosis, which suggests corticosteroid-sparing strategies that include alternative therapies, modifying treatable traits, timely specialist assessment, and shared decision-making with patients. However, further real-world research and collaboration with doctors from primary and academic institutes, patients, and policymakers are necessary to establish an OCS stewardship approach. This should include realistic OCS-tapering strategies for patients with severe asthma using regular OCS, education, and campaigns for patients, the public, and healthcare providers about the burden of severe asthma, as well as improving timely access to specialized severe asthma services for optimal management.
2.Role of HIF-1α in the Responses of Tumors to Radiotherapy and Chemotherapy

Tumor microenvironment is intrinsically hypoxic with abundant hypoxia-inducible factors-1α (HIF-1α), a primary regulator of the cellular response to hypoxia and various stresses imposed on the tumor cells. HIF-1α increases radioresistance and chemoresistance by reducing DNA damage, increasing repair of DNA damage, enhancing glycolysis that increases antioxidant capacity of tumors cells, and promoting angiogenesis. In addition, HIF-1α markedly enhances drug efflux, leading to multidrug resistance. Radiotherapy and certain chemotherapy drugs evoke profound anti-tumor immunity by inducing immunologic cell death that release tumor-associated antigens together with numerous pro-immunological factors, leading to priming of cytotoxic CD8+ T cells and enhancing the cytotoxicity of macrophages and natural killer cells. Radiotherapy and chemotherapy of tumors significantly increase HIF-1α activity in tumor cells. Unfortunately, HIF-1α effectively promotes various immune suppressive pathways including secretion of immune suppressive cytokines, activation of myeloid-derived suppressor cells, activation of regulatory T cells, inhibition of T cells priming and activity, and upregulation of immune checkpoints. Consequently, the anti-tumor immunity elevated by radiotherapy and chemotherapy is counterbalanced or masked by the potent immune suppression promoted by HIF-1α. Effective inhibition of HIF-1α may significantly increase the efficacy of radiotherapy and chemotherapy by increasing radiosensitivity and chemosensitivity of tumor cells and also by upregulating anti-tumor immunity.
3.Safety and efficacy of HK-660S in patients with primary sclerosing cholangitis: A randomized double-blind phase 2a trial

Background/Aims:
A clinical unmet need persists for medications capable of modulating the progression of primary sclerosing cholangitis (PSC). This study aimed to assess the clinical feasibility of HK-660S (beta-lapachone) in PSC.
Methods:
In this multicenter, randomized, double-blind, placebo-controlled, parallel-group phase 2 trial, participants were assigned in a 2:1 ratio to receive either 100 mg of HK-660S or a placebo twice daily for 12 weeks. The primary outcomes were the reduction in serum alkaline phosphatase (ALP) levels and the percentage of participants showing improvements in PSC severity, as determined by magnetic resonance cholangiopancreatography with the Anali score. Secondary endpoints included changes in liver stiffness and adverse events.
Results:
The analysis included 21 patients, 15 receiving HK-660S, and six receiving a placebo. Improvements in the Anali score were observed in 13.3% of the HK-660S group, with no improvements in the placebo group. HK-660S treatment resulted in a 15.2% reduction in mean ALP levels, compared to a 6.6% reduction in the placebo group. A stratified ad-hoc analysis based on baseline ALP levels showed a statistically significant response in the HK-660S group among those with ALP levels greater than twice the upper limit of normal, with a 50% responder rate (p=0.05). Additionally, 26.7% of the HK-660S group showed improvements in the enhanced liver fibrosis score, with no improvements in the placebo group. HK-660S was generally well tolerated.
Conclusions
HK-660S is well tolerated among patients with PSC and may improve bile duct strictures, decrease serum ALP levels, and reduce liver fibrosis (cris.nih.go.kr, Number KCT0006590).

The sense of taste has a crucial role in maintaining good health, and this is why taste disturbance can negatively impact one’s quality of life. The purpose of this study was to investigate the etiologies, clinical characteristics, and effects of treatment in patients with taste disturbances.Subjects and Method A total of 160 patients with taste disorders, who visited our Smell and Taste Clinic from January 2021 to December 2022, were enrolled. All patients underwent chemical and electrical taste threshold tests, olfactory function tests, questionnaires including medical and dental history, and blood tests. The etiologies and clinical features of taste disorders were assessed and treatment was directed toward causative abnormalities. Factors affecting the improvement rates after treatment were evaluated.
Results:
Taste disorder due to olfactory disorder was the most frequent etiology, followed by laryngopharyngeal reflux, head trauma, and drug-induced. In many cases, there was a discrepancy in the severity of taste between the complaints reported by patients and the results of taste threshold tests. After treatment, 115 patients showed improvement in their taste function, and the improvement rates were significantly correlated with the age and etiologies of the taste disorders.
Conclusion
Careful history taking and accurate chemosensory testing were essential to establish the etiologies, nature, degree, and veracity of a patient’s complaint of taste disturbance. Appropriate treatments according to the etiologies allowed recovery of taste function in 71.9% of patients.

7.A Novel Approach for Estimating the Effective Atomic Number Using Dual Energy

Purpose:
This study aimed to present a novel method for estimating the effective atomic number(Zeff ) using dual-energy computed tomography (DECT) designed to improve accuracy andstreamline clinical workflows by reducing computational complexity.
Methods:
The proposed model leverages the DECT-derived mass attenuation coefficients without detailed compositional analysis. By incorporating additional parameters into the conventional Rutherford model, such as exponential and trigonometric functions, the model effectively capturescomplex variations in attenuation, enabling precise Zeff estimation. Model fitting was performedusing dual-energy data and evaluated using the percentage difference in error rates.
Results:
Compared with the Rutherford model, which recorded a maximum error rate of 0.55%, the proposed model demonstrated a significantly lower maximum error rate of 0.15%, highlightingits precision. Zeff estimates for various materials closely matched the reference values, confirmingthe improved accuracy of the model.
Conclusions
The proposed DECT-based model provides a practical and efficient approach to Zeff estimation, with potential applications in radiation oncology, particularly for accurate stopping power ratio calculations in proton and heavy ion therapies.
